Enable running tests inside workers by passing `--worker`
to `tools/test.py`. A number of tests are marked as skipped,
or have been slightly altered to fit the different environment.

Rename the `parentId` property on the PromiseWrap object to a
`isChainedPromise` property. The former wasn't quite useful as it was
always defined to be the same value as the trigger id available in the
init hook. Instead rename the property to be closer to the information
it communicates: whether the promise is a chained promise or not.

Use `PromiseWrap` resource objects whose lifetimes are tied to
the `Promise` instances themselves to track promises, and have
a `.promise` getter that points to the `Promise` and a `.parent`
property that points to the parent Promise’s resource object,
if there is any.
The properties are implemented as getters for internal fields
rather than normal properties in the hope that it helps keep
performance for the common case that async_hooks users will
often not inspect them.
