Fix stuck sessions in get port database. When a thread is in the process of
re-establishing a session, a flag is set to prevent multiple threads /
triggers from doing the same task. This flag was left on, where any attempt
to relogin was locked out. Clear this flag, if the attempt has failed.

The timeout handler and the done function are racing. When
qla2x00_async_iocb_timeout() starts to run it can be preempted by the
normal response path (via the firmware?). qla24xx_async_gpsc_sp_done()
releases the SRB unconditionally. When scheduling back to
qla2x00_async_iocb_timeout() qla24xx_async_abort_cmd() will access an freed
sp->qpair pointer:

Obvious solution to this is to introduce a reference counter. One reference
is taken for the normal code path (the 'good' case) and one for the timeout
path. As we always race between the normal good case and the timeout/abort
handler we need to serialize it. Also we cannot assume any order between
the handlers. Since this is slow path we can use proper synchronization via
locks.
When we are able to cancel a timer (del_timer returns 1) we know there
can't be any error handling in progress because the timeout handler hasn't
expired yet, thus we can safely decrement the refcounter by one.
If we are not able to cancel the timer, we know an abort handler is
running. We have to make sure we call sp->done() in the abort handlers
before calling kref_put().

Currently, the dev_loss_tmo setting is only ever used for SCSI
devices. This patch reshuffles initialisation such that the SCSI remote
ports are registered before the NVMe ones, allowing the dev_loss_tmo
setting to be synchronized between SCSI and NVMe.

On ipsec start by remote port, target port may use RSCN to trigger
initiator to relogin. If driver is already in the process of a relogin,
then ignore the RSCN and allow the current relogin to continue. This
reduces thrashing of the connection.

Current driver does unnecessary pause for each session to get to certain
state before allowing the app start call to return. In larger environment,
this introduces a long delay. Originally the delay was meant to
synchronize app and driver. However, the with current implementation the
two sides use various events to synchronize their state.
The same is applied to the authentication failure call.

When user uses issue_lip to do link bounce, driver sends additional target
reset to remote device before resetting the link. The target reset would
affect other paths with active I/Os. This patch will remove the unnecessary
target reset.

For RSCN of type "Area, Domain, or Fabric", which indicate a portion or
entire fabric was disturbed, current driver does not set the scan_need flag
to indicate a session was affected by the disturbance. This in turn can
lead to I/O timeout and delay of relogin. Hence initiate relogin in the
event of fabric disturbance.

instance, qlt_rdy_to_xfer() maps sgl and exits when session is being
deleted keeping the sgl mapped.
This patch removes use-after-free of the sgl and ensures that the sgl is
unmapped for any command that was not sent to firmware.

DPC thread gets restricted due to a no-op mailbox, which is a blocking call
and has a high execution frequency. To free up the DPC thread we move no-op
handling to the workqueue. Also, modified qla_do_heartbeat() to send no-op
MBC if we don’t have any active interrupts, but there are still I/Os
outstanding with firmware.

Authentication application may be running and in the past tried to probe
driver (app_start) but was unsuccessful. This could be due to the bsg layer
not being ready to service the request. On a successful link up, driver
will use the netlink Link Up event to notify the app to retry the app_start
call.
In another case, app does not poll for new NPIV host. This link up event
would notify app of the presence of a new SCSI host.

When Target port transitions personality from one to another (NVMe <-->
FCP), there could be some overlap of the two where one layer is going down
while the other layer is coming up. This overlap can cause temporary I/O
error. Detect those errors/transitions and recover from them. Triggers
session tear down and allow relogin to re-drive the connection under the
following conditions:
- NVMe command error
- On PRLO + N2N (rida format 2)

For target port that register itself as both FCP + NVMe, initiator driver
will try to login one mode at a time. If the last mode did not succeed,
then driver will try the other mode.
When error is encountered, current code only flip to other mode one time
(NVMe->FCP) and remain on the last mode. Driver wrongly assumed target
port does not support PRLI NVMe, instead it was not ready to receive PRLI.
This patch will alternate back and forth on every PRLI failure until login
retry count has depleted or it is succeeded.

The abort callback gets called only when it gets posted to firmware. The
refcounting is done properly in the callback. On internal errors, the
callback is not invoked leading to a hung I/O. Fix this by having separate
error code when command gets returned from firmware.

Currently driver saves the personality type (FCP|NVMe) at the start of
first discovery of the remote device. If the remote device personality do
change over time, then qla driver needs to present that to user to decide.

For EDIF + N2N to work, firmware 9.8 or later is required. The driver will
pause after PLOGI to allow app to authenticate. Once authentication
completes, app will tell driver to do PRLI.

Abort threads were getting out without processing due to the "deleted"
flag check. The delete thread, meanwhile, could not proceed with a
logout (that would have cleared out pending requests) as the logout IOCB
work was not progressing. It appears like the hung qlt_free_session_done()
thread is causing the ha->wq works on hold. The qlt_free_session_done()
was hung waiting for nvme_fc_unregister_remoteport() + localport_delete cb
to be complete, which would only happen when all I/Os are released.
Fix this by allowing abort to progress until device delete is completely
done. This should make the qlt_free_session_done() proceed without hang and
thus clear up the deadlock.

When user creates multiple NPIVs, the switch capabilities field is checked
before a vport is allowed to be created. This field is being toggled if a

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
After the completion of PLOGI, both sides have authenticated and PRLI
completed, encrypted I/Os are allowed to proceed.

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
During runtime, driver and authentication application need to stay in sync
in terms of: Session being down|up, arrival of new authentication
message (AUTH ELS) and SADB update completion.
These events are queued up as doorbell to the authentication
application. Application would read this doorbell on regular basis to stay
up to date. Each SCSI host would have a separate doorbell queue.
The doorbell interface can daisy chain a list of events for each read. Each
event contains an event code + hint to help application steer the next
course of action.

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
There is no FC switch scan service that can indicate whether a device is
secure or non-secure.
In order to detect whether the remote port supports encrypted operation,
driver must first do a PLOGI with the remote device. On completion of the
PLOGI, driver will query firmware to see if the device supports secure
login. To do that, driver + firmware must advertise the security bit via

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
On completion of the authentication process, the authentication application
will notify driver on whether it is successful or not.
In case of success, application will use the QL_VND_SC_AUTH_OK BSG call to
tell driver to proceed to the PRLI phase.
In case of failure, application will use the QL_VND_SC_AUTH_FAIL bsg call
to tell driver to tear down the connection and retry. In the case where an
existing session is active, the re-key process can fail. The session tear
down ensures data is not further compromised.

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
As part of the authentication process, the authentication application will
generate a SADB entry (Security Association/SA, key, SPI value, etc). This
SADB is then passed to driver to be programmed into hardware. There will be
a pair of SADB's (Tx and Rx) for each connection.
After some period, the application can choose to change the key. At that
time, a new set of SADB pair is given to driver. The old set of SADB will
be deleted.
Add a new bsg call (QL_VND_SC_SA_UPDATE) to allow application to allow
adding or deleting SADB entries. Driver will not keep the key in
memory. It will pass it to HW.
It is assumed that application will assign a unique SPI value to this SADB
(SA + key). Driver + hardware will assign a handle to track this unique
SPI/SADB.

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
Once authentication messages sent from a remote device have arrived, each
message is extracted and placed in a buffer for application to retrieve.
The FC frame header will be stripped, leaving behind the AUTH ELS payload.
It is up to the application to strip the AUTH ELS header to get to the
actual authentication message.

Some FC adapters from Marvell offer the ability to encrypt data in flight
(EDIF). This feature requires an application to act as an authenticator.
Add the ability for authentication application to send and retrieve
messages as part of the authentication process via existing
FC_BSG_HST_ELS_NOLOGIN BSG interface.
To send a message, application is expected to format the data in the AUTH
ELS format. Refer to FC-SP2 for details.
If a message was received, application is required to reply with either a
LS_ACC or LS_RJT complete the exchange using the same interface. Otherwise,
remote device will treat it as a timeout.

Target de-configuration panics at high CPU load because TPGT and WWPN can
be removed on separate threads.
TPGT removal requests a reset HBA on a separate thread and waits for reset
complete (phase1). Due to high CPU load that HBA reset can be delayed for
some time.
WWPN removal does qlt_stop_phase2(). There it is believed that phase1 has
already completed and thus tgt.tgt_ops is subsequently cleared. However,
tgt.tgt_ops is needed to process incoming traffic and therefore this will
cause one of the following panics:

In a case when the initiator in P2P mode by some circumstances does not
send PRLI, the target, in a case when the target port's WWPN is less than
initiator's, changes the discovery state in DSC_GNL. When gnl completes it
sends PRLI to the initiator.
Usually the initiator in P2P mode always sends PRLI. We caught this issue
on Linux stable v5.4.6 https://www.spinics.net/lists/stable/msg458515.html.
Fix this particular corner case in the behaviour of the P2P mod target
login state machine.

Commit a6dcfe0848 ("scsi: qla2xxx: Limit interrupt vectors to number of
CPUs") lowers the number of allocated MSI-X vectors to the number of CPUs.
That breaks vector allocation assumptions in qla83xx_iospace_config(),
qla24xx_enable_msix() and qla2x00_iospace_config(). Either of the functions
computes maximum number of qpairs as:
ha->max_qpairs = ha->msix_count - 1 (MB interrupt) - 1 (default
response queue) - 1 (ATIO, in dual or pure target mode)
max_qpairs is set to zero in case of two CPUs and initiator mode. The
number is then used to allocate ha->queue_pair_map inside
qla2x00_alloc_queues(). No allocation happens and ha->queue_pair_map is
left NULL but the driver thinks there are queue pairs available.
qla2xxx_queuecommand() tries to find a qpair in the map and crashes:

The driver should allocate enough vectors to provide every CPU it's own HW
queue and still handle reserved (MB, RSP, ATIO) interrupts.
The change fixes the crash on dual core VM and prevents unbalanced QP
allocation where nr_hw_queues is two less than the number of CPUs.

For the mailbox thread that encounters a PCIe error, pause that thread
until PCIe link reset/recovery has completed to prevent the thread from
possibly unmapping any type of DMA resource that might be in progress.

On bsg command completion, bsg_job_done() was called while qla driver
continued to access the bsg_job buffer. bsg_job_done() would free up
resources that ended up being reused by other task while the driver
continued to access the buffers. As a result, driver was reading garbage
data.

The host:channel:scsi_target_id information is helpful in matching an FC
port with a SCSI device, so add it. For initiator FC ports, a -1 would be
displayed for "target" part.

Calling vha->hw->tgt.tgt_ops->free_cmd() from qlt_xmit_response() is wrong
since the command for which a response is sent must remain valid until the
SCSI target core calls .release_cmd(). It has been observed that the
following scenario triggers a kernel crash:
- qlt_xmit_response() calls qlt_check_reserve_free_req()
- qlt_check_reserve_free_req() returns -EAGAIN
- qlt_xmit_response() calls vha->hw->tgt.tgt_ops->free_cmd(cmd)
- transport_handle_queue_full() tries to retransmit the response
Fix this crash by reverting the patch that introduced it.

tcm_loop could be used like a normal block device, so we can't use
GFP_KERNEL and should use GFP_NOIO. This adds a gfp_t arg to
target_cmd_init_cdb() and converts the users. For every driver but loop
GFP_KERNEL is kept.
This will also be useful in subsequent patches where loop needs to do
target_submit_prep() from interrupt context to get a ref to the se_device,
and so it will need to use GFP_ATOMIC.

target_submit_cmd() is now only for simple drivers that do their
own sync during shutdown and do not use target_stop_session().
tcm_qla2xxx uses target_stop_session() to sync session shutdown with LIO
core, so we use target_init_cmd()/target_submit_prep()/target_submit(),
because target_init_cmd() will detect the target_stop_session() call and
return an error.

Due to a bug in the older scan logic, when a once lost device re-appeared,
it was not discovered. Fix this by resetting login_retry counter upon
device discovery.
This is applicable only for 4G and older HBAs.

FC-NVMe target discovery failed when initiator wwpn < target wwpn in an N2N
(Direct Attach) config, where the driver was stuck on FCP PRLI mode and

When the fcport is about to be deleted we should return EBUSY instead of
ENODEV. Only for EBUSY will the request be requeued in a multipath setup.
Also return EBUSY when the firmware has not yet started to avoid dropping
the request.

qla83xx_wait_logic() is used to control the frequency of device IDC lock
retries. If in_interrupt() is true, it does 20 loops of cpu_relax().
Otherwise, it sleeps for 100ms and yields the CPU.
While in_interrupt() is ill-defined and does not provide what the name
suggests, it is not needed here: that qla83xx_wait_logic() is exclusively
called by qla83xx_idc_lock() / unlock(), and they always run from process
context. Below is an analysis of all the idc lock/unlock callers, in order
of appearance:
- qla_os.c:
qla83xx_nic_core_unrecoverable_work(),
qla83xx_idc_state_handler_work(),
qla83xx_nic_core_reset_work(),

tcm_qla2xxx_free_session() has a BUG_ON(in_interrupt()).
While in_interrupt() is ill-defined and does not provide what the name
suggests, it is not needed here: the function is always invoked from
workqueue context through "struct qla_tgt_func_tmpl" ->free_session() hook
it is bound to.
The function also calls wait_event_timeout() down the chain, which already
has a might_sleep().
Remove the in_interrupt() check.

the end of each spin, if in_interrupt() is true, it does 20 loops of
cpu_relax(). Otherwise, it yields the CPU.

suggests, it is not needed here: qla82xx_idc_lock() is always called from
process context. Below is an analysis of its callers, in order of

hook. That hook is only called once, in a mutex locked context, from
qla2x00_beacon_store().
- qla_nx.c: qla82xx_beacon_off(), bound to isp_operations ->beacon_off()
hook. Like ->beacon_on(), it's only called once, in a mutex locked
context, from qla2x00_beacon_store().
- qla_nx.c: qla82xx_fw_dump(), calls qla2x00_wait_for_chip_reset(), which
has msleep() in a loop. It is bound to isp_operations ->fw_dump()
hook. That hook *is* called from atomic context at qla_isr.c by
multiple interrupt handlers. Nonetheless, it's other controllers
interrupt handlers, and not the qla82xx.
- qla82xx_msix_default() and qla82xx_msix_rsp_q() call
qla24xx_process_response_queue() which doesn't implement the firmware
dumping.
- qla_attr.c: qla2x00_sysfs_write_fw_dump(), and
qla2x00_sysfs_write_reset(), process-context sysfs ->write() hooks.
- qla_os.c: qla2x00_probe_one(). PCI ->probe(), process context.
- qla_os.c: qla2x00_clear_drv_active(), called solely from
qla2x00_remove_one(), which is PCI ->remove() hook, process context.
- qla_os.c: qla2x00_do_dpc(), kthread function, process context.
Remove the in_interrupt() check. Change qla82xx_idc_lock() specification to
a purely process-context function. Mark it with "Context: task, might
sleep".
Change qla82xx_idc_lock() implementation to sleep 100ms, instead of a
schedule(), for each spin. This is more deterministic, and it matches the
other qla models idc_lock() functions.

Drop the sess_cmd_lock by:
- Removing the sess_cmd_list use from LIO core, because it's been
moved to qla2xxx.
- Removing sess_tearing_down check in the I/O path. Instead of using that
bit and the sess_cmd_lock, we rely on the cmd_count percpu ref. To do
this we switch to percpu_ref_kill_and_confirm/percpu_ref_tryget_live.

Except for debug output in the shutdown path, tcm_qla2xxx is the only

Only the __qlt_24xx_handle_abts() code path does not know the LUN for an
abort and it uses the TARGET_SCF_LOOKUP_LUN_FROM_TAG flag to have LIO core
look it up. LIO uses target_lookup_lun_from_tag to go from cmd tag to LUN
for the driver. However, qla2xxx has a tcm_qla2xxx_find_cmd_by_tag() which
does almost the same thing as the LIO helper (it finds the cmd but does not
return the LUN). This patch has qla2xxx use its internal helper.

The code to try to shut up sparse warnings about questionable locking
didn't shut up sparse: it made the result not parse as valid C at all,
since the end result now has a label with no statement.
The proper fix is to just always lock the hardware, the same way Bart
did in commit 8ae178760b ("scsi: qla2xxx: Simplify the functions for
dumping firmware"). That avoids the whole problem with having locking
that is not statically obvious.
But in the meantime, just remove the incorrect attempt at trying to
avoid a sparse warning that just made things worse.
This was exposed by commit 3e6efab865 ("scsi: qla2xxx: Fix reset of
MPI firmware"), very similarly to how commit cbb01c2f2f ("scsi:
qla2xxx: Fix MPI failure AEN (8200) handling") exposed the same problem
in another place, and caused that commit 8ae178760b.
Please don't add code to just shut up sparse without actually fixing
what sparse complains about.

qla2xx_process_get_sp_from_handle() will clear the slot in which the
current srb is stored. As a result it can't be used in
qla24xx_process_mbx_iocb_response() to check for consistency and later
again in qla24xx_mbx_iocb_entry().
Move the consistency check directly into qla24xx_mbx_iocb_entry() and avoid
the double call or any open coding of the
qla2xx_process_get_sp_from_handle() functionality.

Driver was using a shorter timeout waiting for PLOGI from the peer in
point-to-point configurations. Some devices takes some time (~4 seconds) to
initiate the PLOGI. This peer initiating PLOGI is when the peer has a
higher P-WWN.
Increase the wait time based on N2N R_A_TOV.

Normally, the MPI firmware is reset when an MPI dump is collected. If an
unsaved MPI dump exists in the driver, though, an alternate mechanism is
used. This mechanism, which was not fully correct, is not recommended and
instead an MPI dump template walk is suggested to perform the MPI reset.
To allow for the MPI dump template walk, extra space is reserved in the MPI
dump buffer which gets used only when there is already an MPI dump in
place.

BIT_13 of extended FW attribute informs about NVMe-2 support. Set BIT_15
of special feature control block for enabling SLER in FW. Set bit 8 (SLER
supported) to 1 for the service parameter information when sending NVMe
PRLI request. Set BIT_14 of special feature control block for enabling PI
Control in FW. Driver should set bit 9 (PI Control supported) to 1 for the
service parameter information when sending NVMe PRLI request. Set BIT_13
for NVMe Async events.

This patch tracks number of IOCB resources used in the I/O fast path. If
the number of used IOCBs reach a high water limit, driver would return the
I/O as busy and let upper layer retry. This prevents over subscription of
IOCB resources where any future error recovery command is unable to cut
through. Enable IOCB throttling by default.

FCP-4 (referred FCP-4 rev-2b) identifies the earlier known "retry delay
timer" field as "status qualifier", which is described in SAM-5 and later
specs. This fix makes appropriate driver side modifications to honor the

Driver was using a lower value for dev_loss_tmo making it more prone to I/O
failures during remote port toggle testing. Set dev_loss_tmo to zero during
remote port registration to allow nvme-fc default dev_loss_tmo to be used,
which is higher than what driver was using.

It was observed on an ISP8324 16Gb HBA with fw=8.08.203 (d0d5) in a
PowerPC64 machine that pkt->entry_type was MBX_IOCB_TYPE/0x39 with an
sp->type SRB_SCSI_CMD which is invalid and should not be possible.
Reading the entry_type from the crash dump shows the expected value of
STATUS_TYPE/0x03 but the call trace shows that qla24xx_mbx_iocb_entry() is
used.
Add a check to verify for consistency and reset the HBA if an invalid state
is reached. Obviously, this is only a workaround until the real problem is
solved.

Emit a warning when ->done or ->free are called on an already freed
srb. There is a hidden use-after-free bug in the driver which corrupts
the srb memory pool which originates from the cleanup callbacks.
An extensive search didn't bring any lights on the real problem. The
initial fix was to set both pointers to NULL and try to catch invalid
accesses. But instead the memory corruption was gone and the driver
didn't crash. Since not all calling places check for NULL pointer, add
explicitly default handlers. With this we workaround the memory
corruption and add a debug help.

Eleven fixes, mostly in drivers or minor fixes in driver related
infrastructure libraries (target, libfc and libsas). Most of the bugs
fixed only show up under rare circumstances, the exception being the
endianness problem in qla2xxx which is used as a device on some sparc
systems.

Commit 98aee70d19 ("qla2xxx: Add endianizer to max_payload_size
modifier.") in 2014 broke qla2xxx on sparc64, e.g. as in the Sun Blade 1000
/ 2000. Unbreak by partial revert to fix endianness in nvram firmware
default initialization. Also mark the second frame_payload_size in nvram_t
__le16 to avoid new sparse warnings.

In the case of a failed retry, a positive value EIO is returned here. I
think this is a typo error. It is necessary to return an error value.

OS boot during Boot from SAN was stuck at dracut emergency shell after
enabling NVMe driver parameter. For non-MQ support the driver was enabling
MQ. Add a check to confirm if FW supports MQ.

qla_nvme_register_hba() puts out a warning when there are not enough queue
pairs available for FC-NVME. Just fail the NVME registration rather than a
WARNING + call Trace.

Multipath errors were seen during failback due to login timeout. The
remote device sent LOGO, the local host tore down the session and did
relogin. The RSCN arrived indicates remote device is going through failover
after which the relogin is in a 20s timeout phase. At this point the
driver is stuck in the relogin process. Add a fix to delete the session as
part of abort/flush the login.

On Zone Disable, certain switches would ignore all commands. This causes
timeout for both switch scan command and abort of that command. On
detection of this condition, all sessions will be shutdown.

The request_t 'handle' member is 32-bits wide, hence use wrt_reg_dword().
Change the cast in the wrt_reg_byte() call to make it clear that a regular
pointer is casted to an __iomem pointer.
Note: 'pkt' points to I/O memory for the qlafx00 adapter family and to
coherent memory for all other adapter families.
This patch fixes the following Coverity complaint:
CID 358864 (#1 of 1): Reliance on integer endianness (INCOMPATIBLE_CAST)
incompatible_cast: Pointer &pkt->handle points to an object whose effective
type is unsigned int (32 bits, unsigned) but is dereferenced as a narrower
unsigned short (16 bits, unsigned). This may lead to unexpected results
depending on machine endianness.

The driver performs SCR (state change registration) in all modes including
pure target mode.
For each RSCN, scan_needed flag is set in qla2x00_handle_rscn() for the
port mentioned in the RSCN and fabric rescan is scheduled. During the
rescan, GNN_FT handler, qla24xx_async_gnnft_done() deletes session of the
port that caused the RSCN.
In target mode, the session deletion has an impact on ATIO handler,
qlt_24xx_atio_pkt(). Target responds with SAM STATUS BUSY to I/O incoming
from the deleted session. qlt_handle_cmd_for_atio() and
qlt_handle_task_mgmt() return -EFAULT if they are not able to find session
of the command/TMF, and that results in invocation of qlt_send_busy():

qla_target(0): Unable to send command to target, sending BUSY status
Such response causes command timeout on the initiator. Error handler thread
on the initiator will be spawned to abort the commands:
scsi 23:0:0:0: tag#0 abort scheduled
scsi 23:0:0:0: tag#0 aborting command
qla2xxx [0000:af:00.0]-188c:23: Entered qla24xx_abort_command.
qla2xxx [0000:af:00.0]-801c:23: Abort command issued nexus=23:0:0 -- 0 2003.
Command abort is rejected by target and fails (2003), error handler then
tries to perform DEVICE RESET and TARGET RESET but they're also doomed to
fail because TMFs are ignored for the deleted sessions.
Then initiator makes BUS RESET that resets the link via
qla2x00_full_login_lip(). BUS RESET succeeds and brings initiator port up,
SAN switch detects that and sends RSCN to the target port and it fails
again the same way as described above. It never goes out of the loop.
The change breaks the RSCN loop by keeping initiator sessions mentioned in
RSCN payload in all modes, including dual and pure target mode.

The qla2xxx driver knows when request was processed successfully or
not. But it always sets the NVMe status code to 0/NVME_SC_SUCCESS. The
upper layer needs to figure out from the rcv_rsplen and transferred_length
variables if the request was transferred successfully. This is not always
possible, e.g. when the request data length is 0, the transferred_length is
also set 0 which is interpreted as success in nvme_fc_fcpio_done(). Let's
inform the upper layer (nvme_fc_fcpio_done()) when something went wrong.
nvme_fc_fcpio_done() maps all non-NVME_SC_SUCCESS status codes to
NVME_SC_HOST_PATH_ERROR. There isn't any benefit to map the QLA status code
to the NVMe status code. Therefore, use NVME_SC_INTERNAL to indicate an
error which aligns it with the lpfc driver.

'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.
This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.

The function always returns QLA_SUCCESS and the caller qla2x00_start_sp()
doesn't even evalute the return value. So there is no point in returning a

Annotate members of FC protocol and firmware dump data structures as big
endian. Annotate members of RISC control structures as little endian.
Annotate mailbox registers as little endian. Annotate the mb[] arrays as
CPU-endian because communication of the mb[] values with the hardware
happens through the readw() and writew() functions. readw() converts from
__le16 to u16 and writew() converts from u16 to __le16. Annotate 'handles'
as CPU-endian because for the firmware these are opaque values.

Casting a pointer to void * and relying on an implicit cast from void *
to uint16_t or uint32_t suppresses sparse warnings about endianness. Hence
cast explicitly to uint16_t and uint32_t. Additionally, remove superfluous

Make the MMIO accessors strongly typed such that the compiler checks
whether the accessor function is used that matches the register width. Fix

Calling ql_log() inside qla2x00_port_speed_show() is causing messages to be
output to the console for no particularly good reason. The sysfs read
routine should just return the information to userspace. The only reason
to log a message is when the port speed actually changes, and this already
occurs elsewhere.

The reason of this message is the QLA_FUNCTION_FAILED code that
qla2x00_wait_for_hba_online() returns. However, qlt_disable_vha() expects
that adapter is offlined and QLA_FUNCTION_FAILED informs about the offline
state of the adapter.
The qla2x00_abort_isp() function finishes the execution at the point of
checking the adapter's mode (for example, qla_tgt_mode_enabled()) because
of the qlt_disable_vha() calls qlt_clear_mode() method. It means that
qla2x00_abort_isp() keeps vha->flags.online is equal to zero. Finally,
qla2x00_wait_for_hba_online() checks the state of this flag and returns
QLA_FUNCTION_FAILED error code.
This patch changes the failure message which informs about adapter's
offline state.

The bitfields mpi_fw_dump_reading and mpi_fw_dumped are currently signed
which is not recommended as the representation is an implementation defined

In NPIV environment, a NPIV host may use a queue pair created by base host
or other NPIVs, so the check for a queue pair created by this NPIV is not
correct, and can cause an abort to fail, which in turn means the NVME
command not returned. This leads to hang in nvme_fc layer in
nvme_fc_delete_association() which waits for all I/Os to be returned, which
is seen as hang in the application.

Today, upon an MPI failure AEN, on top of collecting an MPI dump, a regular
firmware dump is also taken and then chip reset. This is disruptive to IOs
and not required. Make the firmware dump collection, followed by chip
reset, optional (not done by default).
Firmware dump buffer and MPI dump buffer are independent of each
other with this change and each can have dump that was taken at two
different times for two different issues. The MPI dump is saved in a
separate buffer and is retrieved differently from firmware dump.
To collect full dump on MPI failure AEN, a module parameter is
introduced:

qlt_free_session_done() tries to post async PRLO / LOGO, and waits for the
completion of these async commands. If UNLOADING is set, this is doomed to
timeout, because the async logout command will never complete.
The only way to avoid waiting pointlessly is to fail posting these commands
in the first place if the driver is in UNLOADING state. In general,
posting any command should be avoided when the driver is UNLOADING.
With this patch, "rmmod qla2xxx" completes without noticeable delay.

The purpose of the UNLOADING flag is to avoid port login procedures to
continue when a controller is in the process of shutting down. It makes
sense to set this flag before starting session teardown.
Furthermore, use atomic test_and_set_bit() to avoid the shutdown being run
multiple times in parallel. In qla2x00_disable_board_on_pci_error(), the
test for UNLOADING is postponed until after the check for an already
disabled PCI board.

The original patch was to resolve the lldd being able to be unloaded
while being used to talk to the boot device of the system. However, the
end result of the original patch is that any driver unload while a nvme
controller is live via the lldd is now being prohibited. Given the module
reference, the module teardown routine can't be called, thus there's no
way, other than manual actions to terminate the controllers.

Having this log in a ring buffer helps to diagnose qla2xxx driver and
firmware issues instead of having to reproduce the problem with
extended_logging enabled. This saves cycles and helps when it is hard
to reproduce problem.

When transitioning from loop to N2N, stale NPort ID is not

For N2N, fc_port struct is created during report id acquisition. At
later time, the loop resync (fabric, n2n, loop) would trigger the rest
of the login using the created fc_port struct. The loop resync logic
can trigger another fc_port allocation if the 1st allocation was not
able to execute. This patch prevents the 2nd allocation trigger.

NPIV re-enable code was blocked after FW had been initialized. The

For single port 28XX adapter, the second core can still run in the
background. The flash semaphore can be held by the non-active core.
This patch tell MPI FW to check for this case and clear the semaphore

commit e4e3a2ce95 ("scsi: qla2xxx: Add ability to autodetect SFP
type") takes a heavy handed approach to BPM (Buffer Plus Management)
enablement:
1) During hardware initialization, if an LR-capable transceiver is
recognized, the driver schedules a disruptive post-initialization
chip-reset (ISP-ABORT) to allow the BPM settings to be sent to the
firmware. This chip-reset will result in (short-term) path-loss to
all fc-rports and their attached SCSI devices.
2) LR-detection is triggered during any link-up event, resulting in a
refresh and potential chip-reset
Based on firmware-team guidance, upon LR-capable transceiver
recognition, the driver's hardware initialization code will now
re-execute firmware with the new BPM settings, then continue on with
driver initialization. To address the second issue, the driver
performs LR-capable detection upon the driver receiving a
transceiver-insertion asynchronous event from firmware. No short-term
path loss is needed with this new semantic.

Since the SCSI core does not reuse the tag of the SCSI command that is
being aborted by .eh_abort() before .eh_abort() has finished it is not
necessary to check from inside that callback whether or not the SCSI
command has already completed. Instead, rely on the firmware to return an
error code when attempting to abort a command that has already
completed. Additionally, rely on the firmware to return an error code when
attempting to abort an already aborted command.
In qla2x00_abort_srb(), use blk_mq_request_started() instead of
sp->completed and sp->aborted.

This patch reduces RDP response payload length, if requesting port is a
domain controller (sid 0xfffc01) and fw is earlier than 8.09.00 and fw is
not 8.05.65 then limit the RDP response payload length to maximum of 256
bytes by terminating the response just before the optical element
descriptor.

- Rename field port_id to d_id in ELS and PUREX iocb structs to match FW
spec.
- Remove redundant comments from ELS and PUREX iocb structs.
- Refactor fields in ELS iocb struct for error subcode common access.
- Properly use error subcode fields in rdp processing routine.
- Add print logs for alloc failure in purex rdp processing routine.

This patch provides separate module parameter ql2xrdpenable to turn on/off
RDP capability in the driver. However, if ql2xsmartsan parameter is
enabled, it will also turn on ql2xfdmienable parameter since it is required
for RDP to work.

This patch adds RDP command support in the driver. With the help of new
ql2xsmartsan parameter, driver will use PUREX IOCB mode to send RDP command
to switch and will be able to receive various diagnostic data.

This patch provides an interface to do the following (using MBC 0x3B):
- Displays (in hex) the LED config words for all three LEDs.
- Programs the config words for one LED or for all three LEDs.
The sysfs node defined is named beacon_config.
First, to allow driver to gain LED control, do this:
# echo 1 > /sys/class/scsi_host/host#/beacon
Then, to display config words for all three LEDs do this:
# cat /sys/class/scsi_host/host#/beacon_config
To set config words for all three LEDs do this:
# echo 3 xxxx yyyy zzzz > /sys/class/scsi_host/host#/beacon_config
Or, to set config word for a specific single LED n do this:
# echo n xxxx > /sys/class/scsi_host/host#/beacon_config
where n is the LED number (0, 1, 2)
Finally, to restore LED control back to firmware, do this:
# echo 0 > /sys/class/scsi_host/host#/beacon

Reduce code duplication by introducing the tcm_qla2xxx_{get,rel}_cmd()
functions. Introduce these functions in the tcm_qla2xxx.c source files such
that the qla_target.c source file becomes independent of the SCSI target
core header files. This patch fixes a bug, namely by ensuring that
qlt_alloc_qfull_cmd() sets cmd->se_cmd.map_tag.
