With the ATA error model, an NCQ command failure always triggers an abort
(termination) of all NCQ commands queued on the device. In such case, the
SAT or the host must handle the failed command according to the command
sense data and immediately retry all other NCQ commands that were aborted
due to the failed NCQ command.
For SAS HBAs controlled by the mpt3sas driver, NCQ command aborts are not
handled by the HBA SAT and sent back to the host, with an ioc log
information equal to 0x31080000 (IOC_LOGINFO_PREFIX_PL with the PL code
PL_LOGINFO_CODE_SATA_NCQ_FAIL_ALL_CMDS_AFTR_ERR). The function
_scsih_io_done() always forces a retry of commands terminated with the
status MPI2_IOCSTATUS_SCSI_IOC_TERMINATED using the SCSI result
DID_SOFT_ERROR, regardless of the log_info for the command. This
correctly forces the retry of collateral NCQ abort commands, but with the
retry counter for the command being incremented. If a command to an ATA
device is subject to too many retries due to other NCQ commands failing
(e.g. read commands trying to access unreadable sectors), the collateral
NCQ abort commands may be terminated with an error as they run out of
retries. This violates the SAT specification and causes hard-to-debug
command errors.
Solve this issue by modifying the handling of the
MPI2_IOCSTATUS_SCSI_IOC_TERMINATED status to check if a command is for an
ATA device and if the command loginfo indicates an NCQ collateral
abort. If that is the case, force the command retry using the SCSI result
DID_IMM_RETRY to avoid incrementing the command retry count.

The "sz" argument in mpt3sas_check_cmd_timeout() is the number of u32, not
the number of bytes. We dump that many u32 values to dmesg. Passing the
number of bytes will lead to a read overflow. Divide by 4 to get the
correct value.

The MPI specification defines support for sending MCTP management commands
as a passthrough function to the IOC. Add support for driver to discover
the IOC capability to support MCTP passthrough function. Driver will
support applications and kernel modules to send MPT commands containing the
MCTP passthrough request to firmware through an MPI request.

Rename .slave_alloc() into .sdev_init() and .slave_destroy() into
.sdev_destroy(). The new names make it clear that these are actions on
SCSI devices. Make this change in the SCSI core, SCSI drivers and also
in the ATA drivers. No functionality has been changed.
This patch has been created as follows:
* Change the text "slave_alloc" into "sdev_init" in all source files
except those in drivers/net/ and Documentation/.
* Change the text "slave_destroy" into "sdev_destroy" in all source
files except those in drivers/net/ and Documentation/.
* Rename lpfc_no_slave() into lpfc_no_sdev().
* Manually adjust whitespace where necessary to restore vertical
alignment (dc395x driver and include/linux/libata.h).

The workqueue maintainer wants to remove the create*_workqueue() macros
because these macros always set the WQ_MEM_RECLAIM flag and because these
only support literal workqueue names. Hence this patch that replaces the
create*_workqueue() invocations with the definition of this macro. The
WQ_MEM_RECLAIM flag has been retained because I think that flag is necessary
for workqueues created by storage drivers. This patch has been generated by
running spatch and git clang-format. spatch has been invoked as follows:

Some firmware versions of the 9600 series SAS HBA byte-swap the REPORT
ZONES command reply buffer from ATA-ZAC devices by directly accessing the
buffer in the host memory. This does not respect the default command DMA
direction and causes IOMMU page faults on architectures with an IOMMU
enforcing write-only mappings for DMA_FROM_DEVICE DMA driection (e.g. AMD
hosts).

Setting QUEUE_FLAG_NOMERGES was added in commit d1b01d14b7 ("scsi:
mpt3sas: Set NVMe device queue depth as 128") without any explanation.
Drivers should second guess the block layer merge decisions, so remove

Three obvious driver fixes. The two core fixes are to disable Command
Duration Limits by default to fix an inconsistency in SATA and some
USB devices. The other is to change the default read size for block
zero to follow the device preference (some USB bridges preferring 16
byte commands don't have a translation for READ(10) and thus don't
scan properly).

The function mpi3mr_qcmd() of the mpi3mr driver is able to indicate to
the HBA if a read or write command directed at an ATA device should be
translated to an NCQ read/write command with the high prioiryt bit set
when the request uses the RT priority class and the user has enabled NCQ
priority through sysfs.
However, unlike the mpt3sas driver, the mpi3mr driver does not define
the sas_ncq_prio_supported and sas_ncq_prio_enable sysfs attributes, so
the ncq_prio_enable field of struct mpi3mr_sdev_priv_data is never
actually set and NCQ Priority cannot ever be used.
Fix this by defining these missing atributes to allow a user to check if
an ATA device supports NCQ priority and to enable/disable the use of NCQ
priority. To do this, lift the function scsih_ncq_prio_supp() out of the
mpt3sas driver and make it the generic SCSI SAS transport function
sas_ata_ncq_prio_supported(). Nothing in that function is hardware
specific, so this function can be used in both the mpt3sas driver and
the mpi3mr driver.

(caused by device manufacturers having an endiannes issue) and reject
them rather than trying to parse a huge non-existent array. The
remaining fixes are in drivers the most user visible of which is the
ALUA state transition recognition (leads to intermittent I/O errors in
some situations otherwise).

There is a potential out-of-bounds access when using test_bit() on a single
word. The test_bit() and set_bit() functions operate on long values, and
when testing or setting a single word, they can exceed the word

The prior strscpy() replacement of strncpy() here expected the
manufacture_reply strings to be NUL-terminated, but it is possible
they are not, as the code pattern here shows, e.g., edev->vendor_id
being exactly 1 character larger than manufacture_reply->vendor_id,
and the replaced strncpy() was copying only up to the size of the
source character array. Replace this with memtostr(), which is the
unambiguous way to convert a maybe not-NUL-terminated character array
into a NUL-terminated string.

This series contains multiple replacements of strncpy throughout the
scsi subsystem.
strncpy() is deprecated for use on NUL-terminated destination strings
[1] and as such we should prefer more robust and less ambiguous string
interfaces. The details of each replacement will be in their
respective patch.

set of patches moving retries out of the drivers and into the core.

The replacement in mpt3sas_base.c is a trivial one because desc is already
zero-initialized meaning there is no functional change here.
For mpt3sas_transport.c, we know edev is zero-initialized as well while
manufacture_reply comes from dma_alloc_coherent(). No functional change
here either.
For all cases, use the more idiomatic strscpy() usage of: strscpy(dest,
src, sizeof(dest))
