reformat most existing 'text' as either `text` or ``text''
reformat most existing "text" as either ``text'' or `text`
reformat 'x' as `x`
harmonize bullet list syntax to use '*' instead of '-'
Rename to 'start failure policy' as else it may be confused with
recovery after fencing, also derscribe its intend.
