This patch does two things for SCM eUSCSI USB-SCSI converters:
1. SCM eUSCSI bridge devices are hard-wired to use SCSI ID 7. On connecting
the converter, access to that ID is attempted during the bus scan. Asking
the converter to issue INQUIRY commands to itself isn't very polite and
wastes time. Set this_id to 7 so __scsi_scan_target() skips it in the scan.
2. Enable multi-LUN support. eUSCSI devices don't support Get Max LUN
requests, returning an error (-32). [Different targets could have different
numbers of LUNs, so it wouldn't make sense to return a particular value in
response to Get Max LUN.]
usb_stor_scan_dwork() does this:

[If the user doesn't want multi-LUN support (perhaps there are SCSI devices
which respond to commands on all LUNs?), the US_FL_SINGLE_LUN quirk can be
specified on the kernel command line.]

usb_stor_euscsi_init() enables multi-target mode for SCM eUSB SCSI bridge
devices. The control message it sends has wLength = 1 and the byte sent is
0x01. While that works, the SCM Windows driver does it with wLength = 0. We
may as well match what the SCM driver does.

When system is being suspended, if host device is not allowed to do wakeup,
xhci_suspend() needs to clear all root port wake on bits. Otherwise, some
platforms may generate spurious wakeup, even if PCI PME# is disabled.
The initial commit ff8cbf250b ("xhci: clear root port wake on bits"),
which also got into stable, turned out to not work correctly and had to
be reverted, and is now rewritten.

If a device is halted and reuturns a STALL, then the halted endpoint
needs to be cleared both on the host and device side. The host
side halt is cleared by issueing a xhci reset endpoint command. The device side
is cleared with a ClearFeature(ENDPOINT_HALT) request, which should
be issued by the device driver if a URB reruen -EPIPE.
Previously we cleared the host side halt after the device side was cleared.
To make sure the host side halt is cleared in time we want to issue the
reset endpoint command immedialtely when a STALL status is encountered.
Otherwise we end up not following the specs and not returning -EPIPE
several times in a row when trying to transfer data to a halted endpoint.

A halted endpoint ring must first be reset, then move the ring
dequeue pointer past the problematic TRB. If we start the ring too
early after reset, but before moving the dequeue pointer we
will end up executing the same problematic TRB again.
As we always issue a set transfer dequeue command after a reset
endpoint command we can skip starting endpoint rings at reset endpoint
command completion.
Without this fix we end up trying to handle the same faulty TD for
contol endpoints. causing timeout, and failing testusb ctrl_out write
tests.

On some platforms a PHY may need to be handled also in the
host controller driver. Exynos5420 SoC requires some "PHY
tuning" based on the USB speed. This patch delivers dwc3's
PHYs to the xhci platform device when it's created.

Suspend/resume code assumed that the gadget was always started and
enabled to connect to usb bus. This means that the actual state of the
gadget (started/stopped or connected/disconnected) was not correctly
preserved on suspend/resume cycle. This patch fixes this issue.

This patch adds a call to s3c_hsotg_disconnect() from 'end session'
interrupt (GOTGINT_SES_END_DET) to correctly notify gadget subsystem
about unplugged usb cable. DISCONNINT interrupt cannot be used for this
purpose, because it is asserted only in host mode.
To avoid reporting disconnect event more than once, a disconnect call has
been moved from USB_REQ_SET_ADDRESS handling function to SESSREQINT
interrupt. This way driver ensures that disconnect event is reported
either when usb cable is unplugged or every time the host starts a new
session. To handle devices which has been synthesized without
SRP support, connected state is set in ENUMDONE interrupt.

prepare clk before calling enable) added clock preparation in interrupt
context. This is not allowed as it might sleep. Also setting the clock
rate is unsafe to call from there for the same reason. Move clock

There was another instance where we were
holding pointers which could be long gone.
Fix that by caching only values pointed to
by such pointer.
Because no crash has been observed, this patch

Disable the MUSB interrupts till MUSB is recovered fully from BABBLE
condition. There are chances that we could get multiple interrupts
till the time the babble recover work gets scheduled. Sometimes
this could even end up in an endless loop making MUSB itself unusable.

musb registers can be dumped using the file regdump
which is created in debugfs. Up to now hard coded
register addresses are used for that. Different glue
layers however have different register addresses. The
patch addresses this issue by substituting bare register
addresses with defines.

The reason for this backtrace is the attempt of the USB code to resume
the HUB twice and thus enqueue the status URB twice.
Alan Stern was a great help by explaining how the USB code supposed to
work and what is most likely the problem. The root problem is that after
resume the musb runtime-suspend state remains RPM_SUSPENDED.
According to git log it RPM was added for the omap2430 platform. If I
understand it correct the omap2430 invokes a get on musb once a cable is
connected and a put once the cable is gone. In between the device could
go auto-idle/off. Not sure what happens when the device goes into suspend
but then I guess it was gadget only.
On DSPS I see only a get in probe and put in remove function. This would
forbid RPM from working but then the devices enterns suspended state
anyway :)
To get rid of this warning, I set the device state to RPM_ACTIVE which
the expected state.

There were a two issues here.
1) We returned PTR_ERR(NULL) which means success if class_create()
failed.
2) If alloc_chrdev_region() failed then we should clean up before
returning.
Also kernel style is to have "error handling" as opposed to "success
handling". In the original code checking for "if (!status) " is
confusing and this bad style is what lead to bug #2.

Update DWC2 kconfig and makefile to support dual-role mode. The platform
file will always get compiled for the case where the controller is directly
connected to the CPU. So for loadable modules, dwc2.ko is built for host,
peripheral, and dual-role mode. The PCI bus interface will be called
dwc2_pci.ko and the platform interface module will be called dwc2_platform.ko.

Since platform.c will get built for both Host and Gadget, if we leave the
usb_disabled() call in platform.c, it results in the following build error
when (!USB && USB_GADGET) condition is met.
ERROR: "usb_disabled" [drivers/usb/dwc2/dwc2_platform.ko] undefined!
Since usb_disabled() is mostly used to disable USB host functionality, move
the call the host portion for the DWC2 driver.

Since the dwc2 hcd driver is currently not looking for a clock node during
init, we should not completely fail if there isn't a clock provided.
By assigning clk = NULL, this allows the driver, when configured for dual-role
mode, to be able to continue loading the host portion of the driver when
a clock node is not specified.

Make dwc2_handle_common_intr call the gadget interrupt function when operating
in peripheral mode. Remove the spinlock functions in s3c_hsotg_irq as
dwc2_handle_common_intr() already has the spinlocks.
Move the registeration of the IRQ to common code for platform and PCI.
Remove duplicate interrupt conditions that was in gadget, as those are handled
by dwc2 common interrupt handler.

This patch will aggregate the probing of gadget/hcd driver into platform.c.
The gadget probe funtion is converted into gadget_init that is now only
responsible for gadget only initialization. All the gadget resources are now
handled by platform.c
Since the host workqueue will not get initialized if the driver is configured
for peripheral mode only. Thus we need to check for wq_otg before calling
queue_work().
Also, we move spin_lock_init to common location for both host and gadget that
is either in platform.c or pci.c.
We also move suspend/resume code to common platform code.
Lastly, move the "samsung,s3c6400-hsotg" binding into dwc2_of_match_table.

Adds the gadget data structure and appropriate data structure pointers
to the common dwc2_hsotg data structure. To keep the driver data
dereference code looking clean, the gadget variable declares are only available
for peripheral and dual-role mode. This is needed so that the dwc2_hsotg data
structure can be used by the hcd and gadget drivers.
Updates gadget.c to use the dwc2_hsotg data structure and gadget pointers
that have been moved into the common dwc2_hsotg structure.

Hcd controller needs bus_suspend/resume, dwc2 controller make
root hub generate suspend/resume signal with hprt0 register
when work in host mode.
After the root hub enter suspend, we can make controller enter
low power state with PCGCTL register.
We also update the lx_state for hsotg state.
This patch has tested on rk3288 with suspend/resume.

This patch expands USB-DMAC channels for R-Car Gen2 SoCs. The SoCs

the USB PHY to the OTG structure) moved the
OTG state from struct usb_phy to struct usb_otg.
Unfortunately, even though I fixed quite a few
build regressions with that patch already, this
one was still missing.
Note that this driver still has other randconfig
build problems which I'll leave for driver author
to fix, as that's less trivial.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq. This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Currently scsi piggy backs on the block layer to define the concept
of a tagged command. But we want to be able to have block-level host-wide
tags assigned even for untagged commands like the initial INQUIRY, so add
a new SCSI-level flag for commands that are tagged at the scsi level, so
that even commands without that set can have tags assigned to them. Note
that this alredy is the case for the blk-mq code path, and this just lets
the old path catch up with it.
We also set this flag based upon sdev->simple_tags instead of the block
queue flag, so that it is entirely independent of the block layer tagging,
and thus always correct even if a driver doesn't use block level tagging
yet.
Also remove the old blk_rq_tagged; it was only used by SCSI drivers, and
removing it forces them to look for the proper replacement.

Replace the calls to __skb_alloc_page that are passed NULL with calls to
__dev_alloc_page.
In addition remove __GFP_COLD flag from allocations as we only want it for
the Rx buffer which is taken care of by __dev_alloc_skb, not for any
secondary allocations such as the queue element transmit descriptors.

SET_FEATURE request with DEBUG_MODE only worked the first time after module
initialisation. Per the USB 2.0 debug device specification, said request
is to be treated as if it were a SET_CONFIGURATION request, i.e. endpoint
must be re-configured.
As configure_endpoints() may now get called multiple times, move it outside
__init and move serial_alloc_tty() call into __init.
Code has assumption that endpoint mapping remains unchanged with consecutive
calls of configure_endpoints().

commit 6856d30 (usb: dwc3: ep0: return early
on NULL requests) tried to fix a minor corner
case where we could dereference a NULL pointer
but it also ended up introducing some dead code.
Unfortunately, that dead code, if reached, could
end up starving the endpoint request list because
a request would never be given back when it should.
Fix this by moving the check for empty request list
before its first use.

The fotg210_dbg_port struct is a copy of the ehci_dbg_port definition
in the <linux/usb/ehci_def.h> header. Embedded in this definition are
a number of macros which came along for the ride. These macros are not
used in the fotg210 driver and will conflict those in the new
<linux/usb/ehci-dbgp.h> header.

If CONFIG_XEN_DOM0 is enabled, the ehci-dbgp driver notifies Xen of
controller reset events via xen_dbgp_reset_prep() and
xen_dbgp_external_startup() (via calls to xen_dbgp_op().) Otherwise
<linux/usb/ehci_def.h> defines them as no-ops to disable this logic.
The fotg210 driver copies much of the dbgp code from ehci_def.h, but it
unconditionally defines the Xen hooks as no-ops, effectively disabling
these notifications when CONFIG_EARLY_PRINTK_DBGP is disabled. When
enabled, though, notifying Xen is dependent on CONFIG_XEN_DOM0 due to
fotg210 leveraging the ehci-dbgp driver.
The following table compares the implementations of xen_dbgp_reset_prep()
and xen_dbgp_external_startup() in the ehci-dbgp and fotg210 drivers
under the relevant configurations:

The fusbh200_dbg_port struct is a copy of the ehci_dbg_port definition
in the <linux/usb/ehci_def.h> header. Embedded in this definition are
a number of macros which came along for the ride. These macros are not
used in the fusbh200 driver and will conflict those in the new
<linux/usb/ehci-dbgp.h> header.

If CONFIG_XEN_DOM0 is enabled, the ehci-dbgp driver notifies Xen of
controller reset events via xen_dbgp_reset_prep() and
xen_dbgp_external_startup() (via calls to xen_dbgp_op().) Otherwise
<linux/usb/ehci_def.h> defines them as no-ops to disable this logic.
The fusbh200 driver copies much of the dbgp code from ehci_def.h, but it
unconditionally defines the Xen hooks as no-ops, effectively disabling
these notifications when CONFIG_EARLY_PRINTK_DBGP is disabled. When
enabled, though, notifying Xen is dependent on CONFIG_XEN_DOM0 due to
fusbh200 leveraging the ehci-dbgp driver.
The following table compares the implementations of xen_dbgp_reset_prep()
and xen_dbgp_external_startup() in the ehci-dbgp and fusbh200 drivers
under the relevant configurations:

Some mass storage devices return a bogus value in response to a Get Max LUN
request. The Iomega Jaz USB Adapter responds with 0x10, hence my recent
patch to use the US_FL_SINGLE_LUN quirk for it.
The USB MSC Bulk Only Transport document says "The device shall return one
byte of data that contains the maximum LUN supported by the device."
Since the LUN field in the command block wrapper is only 4 bits wide, it
might be helpful to report too-large LUN values in the kernel log, and
assume max LUN is actually 0. That could get some devices which currently
need the US_FL_SINGLE_LUN quirk to work.

Converting hid to the new function interface requires converting
the USB hid's function code and its users.
This patch converts the f_hid.c to the new function interface.
The file can now be compiled into a separate usb_f_hid.ko module.
The old function interface is provided by means of a preprocessor
conditional directives. After all users are converted, the old interface
can be removed.

So far platform driver has been unregistered first,
so just after that the usb composite is still available,
but there is no hid data any more.
Reverse the order so that first the usb composite becomes
unavailable and second the hid data goes away.

Add new quirk for devices that cannot handle control-line state
requests.
Note that we currently send these requests to all devices, regardless of
whether they claim to support it, but that errors are only logged if
support is claimed.
Since commit 0943d8ead3 ("USB: cdc-acm: use tty-port dtr_rts"), which
only changed the timings for these requests slightly, this has been
reported to cause occasional firmware crashes on Simtec Electronics
Entropy Key devices after re-enumeration. Enable the quirk for this
device.

The soundcard index to use for the ALSA device creation is passed as a
parameter to f_midi_bind_config(), but is assigned to midi->index only
after the call to f_midi_register_card(midi). So no matter what is passed
to f_midi_bind_config(), the actual index for snd_card_new() is always 0.
This probably works ok if at the moment of f_midi's bind there are no
other snd_cards, but if there are, it is not possible to bind f_midi.
This patch moves the assignment to a place before the call to
f_midi_register_card(midi).

It seems that since we got woken up via MUSB_INTR_RESUME the
musb_host_finish_resume() callback is executed before the
resume-callbacks of the PHY and glue layer are invoked. If I delay it
until the glue layer resumed then I don't see this problem.
I also move musb_host_resume_root_hub() into that callback since I don't
see any reason in doing anything resume-link if there are still pieces
not restored.

The am335x-evmsk support two kinds of suspend:
- standby
the USB device remains powered while the system goes into suspend
- mem
the USB device becomes powerless while the system goes into suspend.
In the "standby" case the device resumes quickly. In the "mem" case the
system hangs for a few seconds. It seems to me that the USB-device has
no address (it was disconnected) and the USB stack thinks that it is
fully operational and GetPortStatus returns the status from before the
suspend so it is not a big help here.
This adds a check in the resume path to see if the device mode (A or B)
and the speed is the same. If the device went missing between
suspend/resume (VBUS went down) then MUSB seems to go into B mode and
HS/FS bits are cleared. In that case we clear the port1_status bits and
assume a disconnect. Once the stack learns this it does a "logical
disconnect" and removes the USB-device quickly. Should the device remain
connected during the suspend then MUSB will receives a "CONNECT" interrupt.

DWC3 Exynos Specific Glue layer can be used only for Exynos SoCs.
In addition, non-DT for EXYNOS SoCs is not supported from v3.11;
thus, there is no need to support non-DT for DWC3 Exynos Specific
Glue layer.
The 'linux/platform_data/dwc3-exynos.h' file has been used for
non-DT support. Thus, the 'dwc3-exynos.h' file is removed, because
it is not used anymore.

Since the DCPCTR doesn't have the ACLRM bit, the usbus_pipe_clear()
should not call the usbhsp_pipectrl_set() with ACLRM.
So, this patch fixes this issue to add the usbhs_fifo_clear_dcp()
in fifo.c because the controller needs the CFIFO to clear the
the DCP PIPE.

According to the datasheet, this driver should clear the INTSTS0.CTRT
bit before this controller detects the next stage transition. Otherwise,
the driver may not be able to clear the bit after the controller went to
the next stage transition. After that, the driver will not be able to
clear the INTSTS0.VALID, and a usb control transfer will not finish
finally.
If we use the testusb tool, it is easy to reproduce this issue:
# testusb -a -t 10
Since the previous code handled a data stage and a status stage in
the usbhsf_pio_try_push(), it may not clear the INTSTS0.CTRT at the
right timing.
So, this patch change the timing of usbhs_dcp_control_transfer_done()
to the usbhsg_irq_ctrl_stage().

This patch fixes an issue that this driver always enable the D+ pullup
after it detected the VBUS connection even though this usb controller
can control the D+ pullup timing by software. So, this driver should
enable the D+ pullup after a gadget driver called usb_gadget_connect().

This patch fixes an issue that the NULL pointer dereference happens
when we uses g_audio driver. Since the g_audio driver will call
usb_ep_disable() in afunc_set_alt() before it calls usb_ep_enable(),
the uep->pipe of renesas usbhs driver will be NULL. So, this patch
adds a condition to avoid the oops.

It is safe to call notify disconnect when the usb core
thinks the device is disconnected.
This commit also fixes one bug found at below situation:
we have not enabled usb wakeup, we do system suspend when
there is an usb device at the port, after suspend, we plug out
the usb device, then plug in device again. At that time,
the nofity disconnect was not called at current code, as
the controller doesn't know the usb device was disconnected
during the suspend, but USB core knows the port has changed
during that periods.
So to fix this problem, and let the usb core call notify disconnect.

Since we notify disconnecting based on the usb device is existed
(port_dev->child, the child device at roothub is not NULL), we
need to notify connect after device has been registered.
This fixes a bug that do fast plug in/out test, and the notify_disconnect
is not called due to roothub child is NULL and the enumeration has failed.

ehci/ohci-platform just define .suspend/.resume functions for dev_pm_ops,
but in order to support both STR(suspend-to-ram) and hibernation, other
callbacks such as .freeze/.thaw are also required.
Registering all required callbacks for both STR and hibernation can
be done by SIMPLE_DEV_PM_OPS macro function.

The bcm63xx and bcm3384 platforms need to set OHCI_QUIRK_FRAME_NO, but
they are non-PPC platforms and don't enable CONFIG_PPC_MPC52xx.
Therefore this patch changes the code that uses OHCI_QUIRK_FRAME_NO,
making it not depend on CONFIG_PPC_MPC52xx.
Also, rephrase the comments describing OHCI_QUIRK_FRAME_NO and the
related PSW endian swap.

The current EHCI implementation is prepared to toggle the
PORT_POWER bit to enable or disable a USB-Port. In some
cases this port power can not be just toggled by the PORT_POWER
bit, and the gpio-regulator is needed to be toggled too.
This patch defines a port power control interface ehci_port_power for
ehci core use, it toggles PORT_POWER bit as well as calls platform
defined .port_power if it is defined.

With uas over usb-3 the tags inside the uas iu-s must match the usb-3 stream
ids, and those go from 1 - qdepth.
Before blk-mq calling scsi_activate_tcq(sdev, qdepth) guaranteed that we would
only get cmnd->request->tag from 0 - (qdepth - 1), and we used those as
uas-tags / stream-ids.
With blk-mq however we are guaranteed to never get more then qdepth commands
queued at the same time, but the cmnd->request->tag values may be much larger,
which breaks uas.
This commit fixes this by generating uas tags in the 1 - qdepth range ourselves
instead of using cmnd->request->tag.
While touching all involved code anyways also rename the uas_cmd_info stream
field to uas_tag, because when using uas over usb-2 streams are not used.

This patch modifies the usb_authorize_device() function such as that it does
not reload the device descriptor for wired devices. The reasons for this
are as follows:
* Some devices dislike the master requesting the descriptor from them twice,
failing on the usb_get_device_descriptor() call with -ETIMEOUT. Observed this
on my Pretec 16GB flash drive (4146:ba65).
* Malicious device could send two different descriptors - one before
authorization, used by userspace to determine whether to authorize it and
second to be actually used by the kernel when determining which drivers to
bind.

On enabling CONFIG_OF_SELFTEST which enables CONFIG_OF_DYNAMIC,
we found out that while getting PHYs for the controller we were
doing an extra of_node_put on the child node in our routines -
exynos_e/ohci_get_phy().
This child is however already put by of_get_next_available_child()
which does a of_node_put() on the "prev" node. So there's no point
in putting the same node again in our routine.

Streams seem to be broken on the Asmedia 1042. An uas capable Seagate disk
which is known to work fine with other controllers causes the system to freeze
when connected over usb-3 with this controller, where as it works fine with
uas in usb-2 ports, indicating a problem with streams.
This is a bit bigger hammer then I would like to use for this, but for now it
will have to make do. I've ordered a pci-e usb controller card with an Asmedia
1042, once that arrives I'll try to get streams to work (with a quirk flag if
necessary) and then we can re-enable them. For now this at least makes uas
capable disk enclosures work again by forcing fallback to the usb-storage
driver.

Sometimes mass-storage devices using the Bulk-only transport will
mistakenly skip the data phase of a command. Rather than sending the
data expected by the host or sending a zero-length packet, they go
directly to the status phase and send the CSW.
This causes problems for usb-storage, for obvious reasons. The driver
will interpret the CSW as a short data transfer and will wait to
receive a CSW. The device won't have anything left to send, so the
command eventually times out.
The SCSI layer doesn't retry commands after they time out (this is a
relatively recent change). Therefore we should do our best to detect
a skipped data phase and handle it promptly.
This patch adds code to do that. If usb-storage receives a short
13-byte data transfer from the device, and if the first four bytes of
the data match the CSW signature, the driver will set the residue to
the full transfer length and interpret the data as a CSW.
This fixes Bugzilla #86611.

From DesignWare databook:
When HIRD_Threshold[4] is set to 1b1 and HIRD value is greater than or
equal to the value in HIRD_Threshold[3:0], dwc3 asserts output signals
utmi_l1_suspend_n to put PHY into Deep Low-Power mode in L1.
When HIRD_Threshold[4] is set to 1b0 or the HIRD value is less than
HIRD_Threshold[3:0], dwc3 asserts output signals utmi_sleep_n on L1.

When parameter DWC_USB3_LPM_ERRATA_ENABLE is enabled in Andvanced
Configuration of coreConsultant, it supports of xHCI BESL Errata Dated
10/19/2011 is enabled in host mode. In device mode it adds the capability
to send NYET response threshold based on the BESL value received in the LPM
token, and the threhold is configurable for each soc platform.
This patch adds an entry that soc platform is able to define the lpm
capacity with their own device tree or bus glue layer.

Some chip vendor is on pre-silicon phase, which needs to use the simulation
board. It should have the same product and vendor id with the true soc, but
might have some minor different configurations.

In Andvanced Configuration of coreConsultant, there is the parameter of
DWC_USB_EN_FPGA. This bit has the function we need. And it would response as 7
bit of GHWPARAMS6 register. So it's able to check this functional bit to confirm
if works on FPGA board.

This patch modify the generic code handling PHYs to allow them to be
supplied from the drivers. This adds checks to ensure no PHY was already
there when looking for one in the generic code. This also makes sure we
do not modify its state in the generic HCD functions, it was provided by

This patch prepares the introduction of the generic PHY support in the
USB OTG common functions. The USB PHY member of the OTG structure is
renamed to 'usb_phy' and modifications are done in all drivers accessing
it. Renaming this pointer will allow to keep the compatibility for USB
PHY drivers.

Before using the PHY framework instead of the USB PHY one, we need to
move the OTG state into another place, since it won't be available when
USB PHY isn't used. This patch moves the OTG state into the OTG
structure, and makes all the needed modifications in the drivers
using the OTG state.

In commit 0ec8fd70fb ("USB: musb: fix possible panic while resuming")
musb_resume() became musb_resume_early() in order to enable the clocks
early on resume. This piece of the resume code was removed later in
commit 034917612 ("usb: musb: move clock handling to glue layer").
In between the function was renamed from musb_resume_early() to
musb_resume_noirq() by commit commit 48fea9659e ("USB: Rework musb
suspend()/resume_early()").
Now I see that first musb_core is resumed followed by phy and glue
layer and I ask myself is this really what we intend to do? This kind
of revoked the purpose of the first commit (0ec8fd70fb).
Because of this and because it looks wrong to resume (core) before the
glue layer I push it to the ->resume callback.

This patch moves calls to phy enable/disable out of spinlock protected
blocks in device suspend/resume to fix incorrect caller context. Phy
related functions must not be called from atomic context. To protect
device internal state from a race during suspend, a call to
s3c_hsotg_core_disconnect() is added under a spinlock, what prevents any
further activity on the usb bus.

This patch moves udc initialization from pullup() method to
s3c_hsotg_udc_start(), so that method ends with hardware fully
initialized and left in soft-disconnected state. After this change, the
pullup() method simply clears soft-disconnect start() when called with
is_on=1. For completeness, a call to s3c_hsotg_core_disconnect() has
been added when pullup() method is called with is_on=0, what puts the
udc hardware back to soft-disconnected state.

This patch moves phy enable/disable calls from pullup() method to
udc_start/stop functions. This solves the issue related to limited caller
context for PHY functions, because they cannot be called from non-sleeping
context. This is also a preparation for using soft-disconnect feature of
udc controller in pullup() method.

This patch changes s3c_hsotg_core_init function to leave hardware in
soft disconnect mode, so the moment of coupling the hardware to the usb
bus can be later controlled by the separate functions for enabling and
disabling soft disconnect mode. This patch is a preparation to rework
pullup() method.

As the udc clock controls both the output signals and the internal IP,
it must be enabled before any UDC register is touched.
The bug is revealed when the clock framework disables the clock for a
couple of milliseconds during the boot sequence, and the endpoint
configuration is lost. The bug is hidden when clock framework is not
used, because no "unused clocks disable" occurs.
This patch fixes the wrong behaviour by ensuring that :
- whenever a UDC register is read or written, the clock is enabled
- reworks the endpoints programming to have it done under running clock
- reworks suspend/resume to ensure the same thing

now that no UDC driver relies on the extra
'driver' argument to ->udc_stop(), we can
safely remove it.
This commit is based on previous work by
Robert Baldyga <r.baldyga@samsung.com> which
can be found at [1]; however that patch turned
out to have a high probability of regressing
many UDC drivers because of a blind search & replace
s/driver/$udc->driver/ which caused the 'driver'
argument to stop_activity() to be a valid non-NULL
pointer when it should be NULL, thus causing UDCs
to mistakenly call gadget driver's ->disconnect()
callback.

Some gadget/function drivers might want to do
improper request recycling by allocating a single
request from one particular endpoint and queueing
it to another completely unrelated endpoint.
One such case was found with f_loopback.c.
To prevent such cases from happening again, let's
WARN() so we get a loud enough failure and persuade
users to report errors.

when disconnecting from host using our soft_connect
sysfs interface, also let the gadget driver know about
it by calling the gadget driver's ->disconnect()
method.
No problems have been found while not calling ->disconnect()
so far, it's just good convention.

Using ->prepare()/->complete() to mask/unmask
IRQs is wrong at least for dwc3. We need to
make sure that by the end of ->resume(), IRQs
are working and ready to fire because a child
device may need working IRQs for its own ->resume()
method.

Enabling the core interrupts in complete is too late for
XHCI, and stops it from proper operation. The root of the
problem is due to a disagreement between dwc3-omap and XHCI
about when IRQs should be enabled.
As it turns out, ->resume's documentation states that:
"... generally the driver is expected to start working
again, responding to hardware events and software requests
(the device itself may be left in a low-power state, waiting
for a runtime resume to occur) ..."
From that we infer that IRQs must be unmasked by the end of
->resume().
Due to that, we will remove ->prepare() and ->complete() and
disable/enable interrupts in ->suspend()/->resume().

Convert the mach info, and store the udc_command in the pxa_udc control
structure.
It is to be noticed that the udc_is_connected() in mach info is not
transfered. This was not used, as mioa701 machine doesn't need it,
balloon3 doesn't really use it, and most importantly the current driver
never uses it.

In case we unload a gadget driver while any of
os_desc_req or req are still pending, we need
to make sure to dequeue them.
By using our setup_pending and os_desc_pending
flags we achieve that in a way that doesn't
cause any regressions because we won't dequeue
a request which was already completed.
The original idea came from Li Jun's commit
f2267089ea
(usb: gadget: composite: dequeue cdev->req
before free it in composite_dev_cleanup) which,
unfortunately, caused two regressions (kfree()
being called before usb_ep_dequeue() and calling
usb_ep_dequeue() when the request was already
completed). That commit also didn't take care
of os_desc_req which can fall into the same
situation so we must care for that one too.
Note that in order to make code slightly easier
to read, we introduce composite_ep_queue() which
hides details about how to fiddle with our pending
flags.

DWC3's GUID register is supposed to be used to write
any sort of version we might want. It helps when getting
bug reports for platforms you don't have HW to know
which kernel version of the driver was running on the
platform.
Because we don't really track driver version, but we _do_
track the kernel version, let's write LINUX_VERSION_CODE to
that register and use it for debugging.

Excessive debug messages might cause timing issues that prevent correct
usb enumeration. This patch hides information about USB bus reset to let
driver enumerate fast enough to avoid making host angry. This fixes
endless enumeration and usb reset loop observed with some Linux hosts.

On ISOC endpoints the last trb_pool entry used as a
LINK TRB is not getting zeroed out correctly due to
memset being called incorrectly and in the wrong place.
If pool allocated from DMA was not zero-initialized
to begin with this will result in the size and ctrl
values being random garbage. Call memset correctly after
assignment of the trb_link pointer.

udc_stop() should clear ->driver pointer unconditionally to let the UDC
framework to work correctly with both registering/unregistering gadgets
and enabling/disabling gadgets by writing to
/sys/class/udc/*hsotg/soft_connect interface.

So testing managed to configure musb in DMA mode but not load the
matching cppi41 driver for DMA. This results in

A request allocated from e.g. ep1out cannot
be queued to any other endpoint. This bug has
been in f_loopback at least since mid-2011 and
since nobody has really screamed about it, we're
not backporting to any stable kernels.
Debugged with MUSB since that's the only controller
which complains about this case.

The commit '2e4c7553cd usb: gadget: f_fs: add aio support' broke the
quirk implemented to align buffer size to maxpacketsize on out endpoint.
As result, functionfs does not work on Intel platforms using dwc3 driver
(i.e. Bay Trail and Merrifield). This patch fixes the issue.
This code is based on a previous Qiuxu's patch.

During FunctionFS bind, ffs_data_get() function was called twice
(in functionfs_bind() and in ffs_do_functionfs_bind()), while on unbind
ffs_data_put() was called once (in functionfs_unbind() function).
In result refcount never reached value 0, and ffs memory resources has
been never released.
Since ffs_data_get() call in ffs_do_functionfs_bind() is redundant
and not neccessary, we remove it to have equal number of gets ans puts,
and free allocated memory after refcount reach 0.

That commit is bogus in two ways:
1) There's no way dwc3-omap's ->suspend() can cause any effect
on xhci's ->suspend(). Linux device driver model guarantees
that a parent's ->suspend() will only be called after all
children are suspended. dwc3-omap is the parent of the
parent of xhci.
2) When implementing Deep Sleep states where context is lost,
USBOTGSS_IRQ0 register, well, looses context so we
_must_ rewrite it otherwise core IRQs will never be
reenabled and USB will appear to be dead.

commit c58d80f52 ("usb: musb: Ensure that cppi41 timer gets armed on
premature DMA TX irq") fixed hrtimer scheduling bug. There is one left
which does not trigger that often.
The following scenario is still possible:

not yet queued.

According to Section 8.5.3.2 of the USB 2.0 specification,
a USB device must terminate a Data Phase with either a
short packet or a ZLP (if the previous transfer was
a multiple of wMaxPacketSize).
For reference, here's what the USB 2.0 specification, section
8.5.3.2 says:

8.5.3.2 Variable-length Data Stage
A control pipe may have a variable-length data phase
in which the host requests more data than is contained
in the specified data structure. When all of the data
structure is returned to the host, the function should
indicate that the Data stage is ended by returning a
packet that is shorter than the MaxPacketSize for the
pipe. If the data structure is an exact multiple of
wMaxPacketSize for the pipe, the function will return
a zero-length packet to indicate the end of the Data
stage.

An official recent Windows driver from FTDI detects counterfeit devices
and reprograms the internal EEPROM containing the USB PID to 0, effectively
bricking the device.
Add support for this VID/PID pair to correctly bind the driver on these
devices.

On USB20CV's Interface Descriptor Test, a series
of SetInterface/GetInterface requests are issued
and gadget driver is required to always return
correct alternate setting.
In one step of the test, g_serial with f_obex
was returning the wrong value (1 instead of 0).
In order to fix this, we will now hold currently
selected alternate setting inside our struct f_obex
and just return that from our ->get_alt()
implementation.
Note that his also simplifies the code a bit.

During Halt Endpoint Test, our interrupt endpoint
will be disabled, which will clear out ep->desc
to NULL. Unless we call config_ep_by_speed() again,
we will not be able to enable this endpoint which
will make us fail that test.

If a set_alt() to the same alternate setting that's
already selected is received, functions are required
to reset the interface state, this means we must disable
all endpoints and reenable them again.
This is also documented on our kdoc for struct usb_function
* @set_alt: (REQUIRED) Reconfigures altsettings; function drivers may
* initialize usb_ep.driver data at this time (when it is used).
* Note that setting an interface to its current altsetting resets
* interface state, and that all interfaces have a disabled state.

According to our Gadget Framework API documentation,
->set_halt() *must* return -EAGAIN if we have pending
transfers (on either direction) or FIFO isn't empty (on
TX endpoints).
Fix this bug so that the mass storage gadget can be used
without stall=0 parameter.
This patch should be backported to all kernels since v3.2.

"Most notable changes in here:
1) By far the biggest accomplishment, thanks to a large range of
contributors, is the addition of multi-send for transmit. This is
the result of discussions back in Chicago, and the hard work of
several individuals.
Now, when the ->ndo_start_xmit() method of a driver sees
skb->xmit_more as true, it can choose to defer the doorbell
telling the driver to start processing the new TX queue entires.
skb->xmit_more means that the generic networking is guaranteed to
call the driver immediately with another SKB to send.
There is logic added to the qdisc layer to dequeue multiple
packets at a time, and the handling mis-predicted offloads in
software is now done with no locks held.
Finally, pktgen is extended to have a "burst" parameter that can
be used to test a multi-send implementation.
Several drivers have xmit_more support: i40e, igb, ixgbe, mlx4,
virtio_net
Adding support is almost trivial, so export more drivers to
support this optimization soon.

Instead of building all of the xHCI code into a single module, separate
it out into the core (xhci-hcd), PCI (xhci-pci, now selected by the new
config option CONFIG_USB_XHCI_PCI), and platform (xhci-plat) drivers.
Also update the PCI/platform drivers with module descriptions/licenses
and have them register their respective drivers in their initcalls.

Since the struct hc_driver is mostly the same across the xhci-pci,
xhci-plat, and the upcoming xhci-tegra driver, introduce the function
xhci_init_driver() which will populate the hc_driver with the default
xHCI operations. The caller must supply a setup function which will
be used as the hc_driver's reset callback.
Note that xhci-plat also overrides the default ->start() callback so
that it can do rcar-specific initialization.

The uas driver uses the block layer tag for USB3 stream IDs. With
blk-mq we can get larger tag numbers that the queue depth, which breaks
this assumption. A fix is under way for 3.18, but sits on top of
large changes so can't easily be backported. Set the disable_blk_mq
path so that a uas device can't easily crash the system when using
blk-mq for SCSI.

The newly added sti ehci and ohci drivers come with a single
Kconfig entry that does not depend on either of the base drivers,
which leads to a link error when they are disabled:

To fix that, this patch introduces two separate Kconfig options
with proper dependencies, which avoids the problem and is also

Now that we have completely moved from older USB-PHY drivers
to newer GENERIC-PHY drivers for PHYs available with USB controllers
on Exynos series of SoCs, we can remove the support for the same
in our host drivers too.
We also defer the probe for our host in case we end up getting
EPROBE_DEFER error when getting PHYs.

With this patch, USB activity can be signaled by blinking a LED. There
are two triggers, one for activity on USB host and one for USB gadget.
Both triggers should work with all host/device controllers. Tested only
with musb.
Performace: I measured performance overheads on ARM Cortex-A8 (TI
AM335x) running on 600 MHz.
Duration of usb_led_activity():
- with no LED attached to the trigger: 2 ± 1 µs
- with one GPIO LED attached to the trigger: 2 ± 1 µs or 8 ± 2 µs (two peaks in histogram)
Duration of functions calling usb_led_activity() (with this patch
applied and no LED attached to the trigger):
- __usb_hcd_giveback_urb(): 10 - 25 µs
- usb_gadget_giveback_request(): 2 - 6 µs

In the next commit, we will want the usb-common module to be composed of
two object files. Since Kbuild cannot "append" another object to an
existing one, we need to rename usb-common.c to something
else (common.c) and create usb-common.o by linking the wanted objects
together. Currently, usb-common.o comprises only common.o.

Use the recently introduced usb_gadget_giveback_request() in favor of
direct invocation of the completion routine.
All places in drivers/usb/ matching "[-.]complete(" were replaced with a
call to usb_gadget_giveback_request(). This was compile-tested with all
ARM drivers enabled and runtime-tested for musb.

It seems that only choose_devnum() was not ready to process more hub
events at the same time.

There is no need to have separate kthread for handling USB hub events.
It is more elegant to use the workqueue framework.
The workqueue is allocated as freezable because the original thread was
freezable as well.
Also it is allocated as ordered because the code is not ready for parallel
processing of hub events, see choose_devnum().
struct usb_hub is passed via the work item. Therefore we do not need
hub_event_list.
Also hub_thread() is not longer needed. It would call only hub_event().
The rest of the code did manipulate the kthread and it is handled by the
workqueue framework now.
kick_khubd is renamed to kick_hub_wq() to make the function clear. And the
protection against races is done another way, see below.
hub_event_lock has been removed. It cannot longer be used to protect struct
usb_hub between hub_event() and hub_disconnect(). Instead we need to get
hub->kref already in kick_hub_wq().
The lock is not really needed for the other scenarios as well. queue_work()
returns whether it succeeded. We could revert the needed operations
accordingly. This is enough to avoid duplicity and inconsistencies.
Yes, the removed lock causes that there is not longer such a strong
synchronization between scheduling the work and manipulating
hub->disconnected.
But kick_hub_wq() must never be called together with hub_disconnect()
otherwise even the original code would have failed. Any callers are
responsible for this.
Therefore the only problem is that hub_disconnect() could be called in parallel
with hub_event(). But this was possible even in the past. struct usb_hub is

Note that the source file is still full of the obsolete "khubd" strings.
Let's remove them in a follow up patch. This patch already is complex enough.

We would like to convert khubd kthread to a workqueue. As a result hub_events()
will handle only one event per call.
In fact, we could do this already now because there is another cycle in
hub_thread(). It calls hub_events() until hub_event_list is empty.
This patch renames the function to hub_event(), removes the while cycle, and
renames the goto targets from loop* to out*.
When touching the code, it fixes also formatting of dev_err() and dev_dbg()
calls to make checkpatch.pl happy :-)

This is just a small optimization of the fix from the commit c605f3cdff
("usb: hub: take hub->hdev reference when processing from eventlist).
We do not need to take the reference for each event. Instead we could get it
when struct usb_hub is allocated and put it when it is released. By other words,
we could handle it the same way as the reference for hub->intfdev.
The motivation is that it will make the life easier when switching from khubd
kthread to a workqueue.

There is apparently another SCM USB-SCSI converter with ID 04E6:000F. It
is listed along with 04E6:000B in the Windows INF file for the Startech
ICUSBSCSI2 as "eUSB SCSI Adapter (Bus Powered)". The quirk allows
devices with SCSI ID other than 0 to be accessed.
Also make a couple of existing SCM product IDs lower case to be
consistent with other entries.

Castlewood Systems supplied various models of USB-SCSI converter with their
ORB external removable-media drive. The ORB Windows and Macintosh drivers
support six USB IDs:

[Apparently the official VID for Double-H Technology is 0x07EB = 2027
decimal. That's another hex/decimal mix-up with these SCM-based products
(in addition to the Ariston and Entrega ones). Perhaps the USB-IF informed
companies of their allocated VID in decimal, but they assumed it was hex?
It seems all Entrega products used VID 0x1645, not just the USB-SCSI
converter.]
Double-H Technology Co., Ltd. produced a USB-SCSI converter, model
DH-2000SC, which is probably the one supported by the ORB drivers. Perhaps
the Castlewood-bundled product had a different label or PID though?
Castlewood mentioned Conmate as being one type of USB-SCSI converter.
Conmate and Double-H seem related somehow; both company addresses in the
same road, and at one point the Conmate web site mentioned DH-2000H4,
DH-200D4/DH-2000C4 as models of USB hub (DH short for Double-H presumably).
Conmate did show a USB-SCSI converter model CM-660 on their web site at one
point. My guess is that was identical to the DH-2000SC.

usb_stor_euscsi_init() calls usb_stor_control_msg() with timeout
argument 5000. USB_CTRL_SET_TIMEOUT is defined to be 5000 in usb.h, so
would it make sense to use that instead? Patch below if it would.

This full-speed USB device generates spurious remote wakeup event
as soon as USB_DEVICE_REMOTE_WAKEUP feature is set. As the result,
Linux can't enter system suspend and S0ix power saving modes once
this keyboard is used.
This patch tries to introduce USB_QUIRK_IGNORE_REMOTE_WAKEUP quirk.
With this quirk set, wakeup capability will be ignored during
device configure.
This patch could be back-ported to kernels as old as 2.6.39.

If three or more wireless devices are connected and two of them
disconnect between 1-3 seconds apart, it can cause the HWA to disconnect
the remaining devices due to failing to see a DN_Alive message from
them. This happens because when the HWA detects that the first device
is gone, it will attempt to rekey the remaining devices. If one of the
devices is not responding because it has also been disconnected but not
yet timed out, the synchronous rekey operation running on the wusbd
workqueue can block for up to 5 seconds. This will prevent the
KEEPALIVE timer from running and DN_Alive messages from being processed
because they are processed by the same workqueue. This patch moves the
rekey operation to a separate workqueue since it is the only wusb work
item that needs to communicate directly with wireless devices. The rest
of the WUSB work items either perform no device IO or communicate
directly with the host controller and should not be blocked out by a
non-responding wireless device.

When completing an aborted transfer, skip done segs before calling
wa_complete_remaining_xfer_segs to avoid a runtime warning. The warning
is harmless in this case but avoiding it prevents false error reports.

Now that we have completely moved from older USB-PHY drivers
to newer GENERIC-PHY drivers for PHYs available with USB controllers
on Exynos series of SoCs, we can remove the support for the same
in our host drivers too.
We also defer the probe for our host in case we end up getting
EPROBE_DEFER error when getting PHYs.

Even though a Set TR deq ptr command operates on a ring, and an endpoint
can have multiple rings, we can have only one Set TR deq ptr command pending.
When an endpoint with streams halts or is stopped to unlink urbs, there
will only be at most one ring active / one td being executed (the td
stopped_td points to).
So when we reset the endpoint (for a halt), or the stop command completes, we
will queue one Set TR deq ptr command at most, cancelled urbs on other stream
rings then the one being executed will have there trbs turned to nops, and
once the hcd gets around to execute that stream ring they will be simply
skipped.
So the SET_DEQ_PENDING flag in the endpoint is sufficient protection against
starting the endpoing before all stream rings are cleaned up.

xhci_queue_new_dequeue_state is the only caller of queue_set_tr_deq
and queue_set_tr_deq checks for SET_DEQ_PENDING, where as
xhci_queue_new_dequeue_state sets it which is inconsistent.
Simply fold the 2 into one is a nice cleanup and fixes the inconsistency.

V2 - Restart polling (which will restart the timer) for the shared
HCD in xhci_resume().
xhci_suspend() will stop the primary HCD's root hub timer, but leaves
the shared HCD's timer running. This change adds stopping of the
shared HCD timer.

1) This fixes a missing check for xhci_alloc_command failing in
xhci_handle_cmd_stop_ep()
2) This adds a warning when we cannot set the new dequeue state because of
xhci_alloc_command failing
3) It puts the allocation of the command after the sanity checks in
queue_set_tr_deq(), avoiding leaking the command if those fail
4) Since queue_set_tr_deq now owns the command it can free it if queue_command
fails
5) It reduces code duplication

If something goes wrong in our communication with an uas device we may get
a response iu in reaction to a cmnd, rather then a status iu. In this case
propagate an error upwards, rather then logging a bogus iu message.

I've access to a number of different uas devices now, and none of them use
old style sense urbs. The only case where these code-paths trigger is with
the asm1051 and there they do the wrong thing, as the asm1051 sends 8 bytes
status iu-s when it does not have any sense data, but uses new style
sense iu-s regardless, as can be seen for scsi cmnds where there is sense
data.

The purpose of uas_pre_reset is to:
1) Stop any new commands from being submitted while an externally triggered
usb-device-reset is running
2) Wait for any pending commands to finish before allowing the usb-device-reset
to continue
The purpose of uas_suspend is to:
2) Wait for any pending commands to finish before suspending
This commit fixes races in both paths:
1) For 1) we use scsi_block_requests, but the scsi midlayer calls queuecommand
without holding any locks, so a queuecommand may already past the midlayer
scsi_block_requests checks when we call it, add a check to uas_queuecommand
to fix this
2) For 2) we were waiting for all sense-urbs to complete, there are 2 problems
with this approach:
a) data-urbs may complete after the sense urb, so we need to check for those
too
b) if a sense-urb completes with a iu id of READ/WRITE_READY a command is not
yet done. We submit a new sense-urb immediately in this case, but that
submit may fail (in which case it will get retried by uas_do_work), if this
happens the sense_urbs anchor may become empty while the cmnd is not yet
done
Also unblock requests on timeout, to avoid things getting stuck in that case.

Not all urbs we've allocated are necessarily also submitted, non-submitted
urbs will not be free-ed by their completion handler. So we need to free
them manually.
There are 2 scenarios where this can happen:
1) We have failed to submit some urbs at abort / disconnect
2) When running over usb-2 we may have never tried to submit the data urbs
when completing the scsi cmnd, because we never got a READ/WRITE_READY iu

The data urbs are all killed before calling zap_pending, and their completion
handler should have cleared their inflight flag.
Do not 0 the data inflight flags, and add a check for try_complete succeeding,
as it should always succeed when called from zap_pending.

Drop the whole dance with first moving cmnds to a dead-list. The resetting
flag ensures that no new cmds / urbs will be submitted, and that any urb
completions are short-circuited without trying to complete the scsi cmnd.

There is no need for all the trickery with dropping the lock, we can
simply reference the urbs while we hold the lock to ensure the urbs don't
disappear beneath us, and do the actual unlink (+ unreference) after we've
dropped the lock.
This also fixes a race where we may loose of cmnd ownership to the scsi
midlayer without holding the lock due to the midlayer re-claiming ownership
through an abort (which will be handled by a future patch in this series).

should we get a second status urb for the same tag after a IU_ID_STATUS.
Data urbs should not complete before the command has been submitted, but may

Using scsi_host_find_tag with tags returned by the device is unsafe for
multiple reasons:
1) It returns tags->rqs[tag], which may be non NULL even when the cmnd is
not owned by us
2) It returns tags->rqs[tag], without holding any locks protecting it
3) It returns tags->rqs[tag], without doing any boundary checking
Instead keep our own list which maps tags -> inflight cmnds.

There are various bug reports about oopses / hangs with the uas driver,
which all point to the abort-command and logical-unit-reset (task-management)
error handling paths.
Getting these right is very hard, there are quite a few corner cases, and
testing is almost impossible since under normal operation these code paths
are not used at all.
Another problem is that there are also some cases where it simply is not clear
what to do at all. E.g. over usb-2 multiple outstanding commands share the same
endpoint. What if a command gets aborted while its sense urb is half way
through completing (so some data has been transfered but not all). Since the
urb is not yet complete we don't know if the sense urb is actually for this
command, or for one of the other oustanding commands. If it is for one of the
other commands and we cancel it, then we end up in an undefined state. But if
it is actually for the command we're aborting, and the abort succeeds, then it
may never complete...
This exact same problem applies to logical unit resets too, if there are
multiple luns, then commands outstanding on both luns share the sense
endpoint. If there is only a single lun, then doing a logical unit reset is
little better then doing a full usb device reset.
So summarizing because:
1) abort / lun-reset is very tricky to get right
2) Not being able to test the tricky code, which means it will have bugs
3) This being a code path which under normal operation will never happen,
so being slow / sub-optimal here is not really an issue
4) Under error conditions we will still be able to recover through usb
device resets.
5) This may be a bit slower in some cases, but this is actually faster in
cases where the bridge ship has locked up, which seems to be the most
common error case sofar.
This commit removes the abort / lun-reset error handling paths, and also the
taks-mgmt code since those are the only 2 task-mgmt users. Leaving only the
(tested and testable) usb-device-reset error handling path in place.
Note I realize that this is somewhat of a big hammer, but currently people
are seeing very hard to debug oopses with uas. First let focus on making uas
work reliable, then we can later look into adding more fine grained error
handling.

the JMicron JMS567 also needs it to work properly with uas (usb-storage always
sets it). Since some of the scsi devs were not to keen on the idea to
outrightly set sdev->no_report_opcodes = 1 for all uas devices, so add a quirk
for this, and set it for the JMS567.

We have only needed to enable otg initialization when both of
below conditions are satisfied:
- The controller is otg capable
- The gadget function is enabled
If the controller is otg capable, but is host-only configuration, we do
not need to access register otgsc and do any otg operations (eg, create
otg workqueue).

This adds Vybrid VF610 SoC support. The IP is very similar to i.MX6,
however, the non-core registers are spread in two different register
areas. Hence we support multiple instances of the USB misc driver
and add the driver instance to the imx_usbmisc_data structure.

According to On-The-Go and Embedded Host Supplement to the USB Revision
2.0 Specification, the targeted hosts (non-PC hosts) include both
embedded hosts and otg, and each targeted host product defines the
set of supported peripherals on a TPL (Targeted Peripheral List). So,
TPL should apply for both OTG and embedded host, and the otg support is
not a must for embedded host.
The TPL support feature will only be effect when CONFIG_USB_OTG_WHITELIST
has been chosen and hcd->tpl_support flag is set, it can avoid the enumeration
fails problem for the user who chooses CONFIG_USB_OTG_WHITELIST wrongly.

Enable device-qualifier quirk for Elan Touchscreen, which often fails to
handle requests for the device_descriptor.
Note that the device sometimes do respond properly with a Request Error
(three times as USB core retries), but usually fails to respond at all.
When this happens any further descriptor requests also fails, for
example:

Add new quirk for devices that cannot handle requests for the
device_qualifier descriptor.
A USB-2.0 compliant device must respond to requests for the
device_qualifier descriptor (even if it's with a request error), but at
least one device is known to misbehave after such a request.

That commit causes more problem than fixes. Firstly, kfree()
should be called after usb_ep_dequeue() and secondly, the way
things are, we will try to dequeue a request that has already
completed much more frequently than one which is pending.

The driver's handling of DMA buffers for non-aligned transfers
was kind of nuts. For IN transfers, it left the URB DMA buffer
mapped until the transfer completed, then synced it, copied the
data from the bounce buffer, then synced it again.
Instead of that, just call usb_hcd_unmap_urb_for_dma() to unmap
the buffer before starting the transfer. Then no syncing is
required when doing the copy. This should also allow handling of
other types of mappings besides just dma_map_single() ones.
Also reduce the size of the bounce buffer allocation for Isoc
endpoints to 3K, since that's the largest possible transfer size.
Tested on Raspberry Pi and Altera SOCFPGA.

Clip max_transfer_size to 65535 for host. dwc2_hc_setup_align_buf()
allocates coherent buffers with this size, and if it's too large we
can exhaust the coherent DMA pool.

Because we have not enough memory to have each TX FIFO of size at least
3072 bytes (the maximum single packet size with 3 transactions per
microframe), we create four FIFOs of lenght 1024, and four of length
3072 bytes, and assing them to endpoints dynamically according to
maxpacket size value of given endpoint.
Up to now there were initialized 16 TX FIFOs, but we use only 8 IN
endpoints, so we can split available memory for 8 FIFOs to have more
memory for each one.
It needed to do some small modifications in few places in code, because
there was assumption that TX FIFO numbers assigned to endpoints are the
same as the endpoint numbers, which is not true since we have dynamic
FIFO assigning.
