6bbb9ebf8d test: rework to remove flakiness, and be parallel
Let .end() propogate from client, to server, and back, before
considering the test complete. Also, remove the test vector and exit
handling in favour of running all the tests in parallel and using
common.must/mustNotCall().

lib,src: remove openssl feature conditionals
Remove compile-time and run-time conditionals for features that
OpenSSL 1.0.0 and 1.0.1 didn't support: ALPN, OCSP and/or SNI.
They are no longer necessary since our baseline is OpenSSL 1.0.2.

tls: better error message for socket disconnect
The error emitted when a connection is closed before the
TLS handshake completes seemed rather unspefic by just saying
`socket hang up`.

Use a more verbose message, that also indicates that this is
a purely client-side error, and remove a misleading comment.

7535a94c8a test: begin normalizing fixtures use
Adds a new `../common/fixtures' module to begin normalizing
`test/fixtures` use. Our test code is a bit inconsistent with
regards to use of the fixtures directory. Some code uses
`path.join()`, some code uses string concats, some other
code uses template strings, etc. In mnay cases, significant
duplication of code is seen when accessing fixture files, etc.

This updates many (but by no means all) of the tests in the
test suite to use the new consistent API. There are still
many more to update, which would make an excelent Code-n-Learn
exercise.

using the legacy indentation rules in the test directory. This commit
changes the indentation of files to comply with the stricter 4.x linting
and enable stricter linting in the test directory.

The tests which didn't make it in #2109, are included in this patch.
The skip messages are supposed to follow the format

1..0 # Skipped: [Actual reason why the test is skipped]

and the tests should be skipped with the return statement.
