17883dfaaf test: avoid usage of mixed IPV6 addresses
The test case fails in AIX due to the mixed-use of unspecified
and loopback addresses. This is not a problem in most platforms
but fails in AIX. (In Windows too, but does not manifest as the
test is omitted in Windows for a different reason).

There exists no documented evidence which supports the mixed use
of unspecified and loopback addresses.

While AIX strictly follows the IPV6 specification with respect to
unspecified address ('::') and loopback address ('::1'), the test
case latches on to the behavior exhibited by other platforms,
and hence it fails in AIX.

The proposed fix is to make it work in all platforms including
AIX by using the loopback address for the client to connect,
as that is the address at which the server listens.

c5c28c3d50 debugger: make listen address configurable
`--debug=1.2.3.4:5678` and `--debug=example.com:5678` are now accepted,
likewise the `--debug-brk` and `--debug-port` switch.  The latter is
now something of a misnomer but it's undocumented and for internal use
only so it shouldn't matter too much.

`--inspect=1.2.3.4:5678` and `--inspect=example.com:5678` are also
accepted but don't use the host name yet; they still bind to the
default address.

a9492f503f test: verify cluster worker exit
test-cluster-disconnect-handles already includes logic that
tries to cleanup any child processes when the test fails. This
commit adds additional checks to verify that the child exited
normally, and fails the test if that is not the case.

49b5445cbc test: improve cluster-disconnect-handles test
This commit fixes two issues in test-cluster-disconnect-handles:

1. If the master's TCP connection to the worker fails, the worker
process stays alive and causes many other tests that use the same
common port number to also fail (with EADDRINUSE).

2. One particular problem that can cause the master's TCP connection
to fail is attempting an IPv6 connection to the worker when no IPv6
network interfaces are available.

8677627054 test: fix cluster-disconnect-handles flakiness
Sometimes the test was timing out because the worker process remained
stuck in the breakpoint and didn't exit. This could happen because the
continue was sent before the breakpoint was set. If that's the case,
with this change, a new continue command is sent so the worker process
can end.

33827e3f68 cluster: remove handles when disconnecting worker
Due to the race window between the master's "disconnect" message and the
worker's "handle received" message, connections sometimes got stuck in
the pending handles queue when calling `worker.disconnect()` in the
master process.

The observable effect from the client's perspective was a TCP or HTTP
connection that simply stalled.  This commit fixes that by closing open
handles in the master when the "disconnect" message is sent.
