lib,src: remove openssl feature conditionals
Remove compile-time and run-time conditionals for features that
OpenSSL 1.0.0 and 1.0.1 didn't support: ALPN, OCSP and/or SNI.
They are no longer necessary since our baseline is OpenSSL 1.0.2.

5bfbe5ceae tls: drop NPN (next protocol negotiation) support
NPN has been superseded by ALPN. Chrome and Firefox removed support for

7535a94c8a test: begin normalizing fixtures use
Adds a new `../common/fixtures' module to begin normalizing
`test/fixtures` use. Our test code is a bit inconsistent with
regards to use of the fixtures directory. Some code uses
`path.join()`, some code uses string concats, some other
code uses template strings, etc. In mnay cases, significant
duplication of code is seen when accessing fixture files, etc.

This updates many (but by no means all) of the tests in the
test suite to use the new consistent API. There are still
many more to update, which would make an excelent Code-n-Learn
exercise.

4e6dc00401 tools: lint for object literal spacing
There has been occasional nits for spacing in object literals in PRs but
the project does not lint for it and it is not always handled
consistently in the existing code, even on adjacent lines of a file.

This change enables a linting rule requiring no space between the key
and the colon, and requiring at least one space (but allowing for more
so property values can be lined up if desired) between the colon and the
value. This appears to be the most common style used in the current code
base.

Example code the complies with lint rule:

    myObj = { foo: 'bar' };

Examples that do not comply with the lint rule:

    myObj = { foo : 'bar' };
    myObj = { foo:'bar' };

802a2e79e1 tls, crypto: add ALPN Support
ALPN is added to tls according to RFC7301, which supersedes NPN.
When the server receives both NPN and ALPN extensions from the client,
ALPN takes precedence over NPN and the server does not send NPN
extension to the client. alpnProtocol in TLSSocket always returns
false when no selected protocol exists by ALPN.
In https server, http/1.1 token is always set when no
options.ALPNProtocols exists.
