9ce6c9875f nvme: always punt polled uring_cmd end_io work to task_work
Currently NVMe uring_cmd completions will complete locally, if they are
polled. This is done because those completions are always invoked from
task context. And while that is true, there's no guarantee that it's
invoked under the right ring context, or even task. If someone does
NVMe passthrough via multiple threads and with a limited number of
poll queues, then ringA may find completions from ringB. For that case,
completing the request may not be sound.

Always just punt the passthrough completions via task_work, which will
redirect the completion, if needed.

3c12a8939e nvme: enable vectored registered bufs for passthrough cmds
nvme already supports registered buffers for non-vectored io_uring
passthrough commands, enable it for the vectored mode as well. It takes
an iovec, each entry of which should contain a range within the same
registered buffer specificied in sqe->buf_index.

c4b680ac28 nvme: fix implicit bool to flags conversion
nvme_map_user_request() takes flags as the last argument, but
nvme_uring_cmd_io() shoves a bool "vec" into it. It behaves as
expected because bool is converted to 0/1 and NVME_IOCTL_VEC is
defined as 1, but it's better to pass flags explicitly.

38808af53c nvme/ioctl: move fixed buffer lookup to nvme_uring_cmd_io()
nvme_map_user_request() is called from both nvme_submit_user_cmd() and
nvme_uring_cmd_io(). But the ioucmd branch is only applicable to
nvme_uring_cmd_io(). Move it to nvme_uring_cmd_io() and just pass the
resulting iov_iter to nvme_map_user_request().

For NVMe passthru operations with fixed buffers, the fixed buffer lookup
happens in io_uring_cmd_import_fixed(). But nvme_uring_cmd_io() can
return -EAGAIN first from nvme_alloc_user_request() if all tags in the
tag set are in use. This ordering difference is observable when using
UBLK_U_IO_{,UN}REGISTER_IO_BUF SQEs to modify the fixed buffer table. If
the NVMe passthru operation is followed by UBLK_U_IO_UNREGISTER_IO_BUF
to unregister the fixed buffer and the NVMe passthru goes async, the
fixed buffer lookup will fail because it happens after the unregister.

Userspace should not depend on the order in which io_uring issues SQEs
submitted in parallel, but it may try submitting the SQEs together and
fall back on a slow path if the fixed buffer lookup fails. To make the
fast path more likely, do the import before nvme_alloc_user_request().

The callers of nvme_map_user_request() (nvme_submit_user_cmd() and
nvme_uring_cmd_io()) allocate the request, so have them free it if
nvme_map_user_request() fails.

eada75467f nvme/ioctl: don't warn on vectorized uring_cmd with fixed buffer
The vectorized io_uring NVMe passthru opcodes don't yet support fixed
buffers. But since userspace can trigger this condition based on the
io_uring SQE parameters, it shouldn't cause a kernel warning.

99fde895ff nvme: map uring_cmd data even if address is 0
When using kernel registered bvec fixed buffers, the "address" is
actually the offset into the bvec rather than userspace address.
Therefore it can be 0.

We can skip checking whether the address is NULL before mapping
uring_cmd data. Bad userspace address will be handled properly later when
the user buffer is imported.

With this patch, we will be able to use the kernel registered bvec fixed
buffers in io_uring NVMe passthru with ublk zero-copy support.

00817f0f1c nvme-ioctl: fix leaked requests on mapping error
All the callers assume nvme_map_user_request() frees the request on a
failure. This wasn't happening on invalid metadata or io_uring command
flags, so we've been leaking those requests.

487a3ea7b1 nvme/ioctl: add missing space in err message
nvme_validate_passthru_nsid() logs an err message whose format string is
split over 2 lines. There is a missing space between the two pieces,
resulting in log lines like "... does not match nsid (1)of namespace".
Add the missing space between ")" and "of". Also combine the format
string pieces onto a single line to make the err message easier to grep.

6fad84a4d6 nvme-pci: use sgls for all user requests if possible
If the device supports SGLs, use these for all user requests. This
format encodes the expected transfer length so it can catch short buffer
errors in a user command, whether it occurred accidently or maliciously.

For controllers that support SGL data mode, this is a viable mitigation
to CVE-2023-6238. For controllers that don't support SGLs, log a warning
in the passthrough path since not having the capability can corrupt
data if the interface is not used correctly.

7c2fd76048 nvme: fix metadata handling in nvme-passthrough
On an NVMe namespace that does not support metadata, it is possible to
send an IO command with metadata through io-passthru. This allows issues
like [1] to trigger in the completion code path.
nvme_map_user_request() doesn't check if the namespace supports metadata
before sending it forward. It also allows admin commands with metadata to
be processed as it ignores metadata when bdev == NULL and may report
success.

Reject an IO command with metadata when the NVMe namespace doesn't
support it and reject an admin command if it has metadata.

The nvme pci driver synchronizes with all the namespace queues during a
reset to ensure that there's no pending timeout work.

Meanwhile the timeout work potentially iterates those same namespaces to
freeze their queues.

Each of those namespace iterations use the same read lock. If a write
lock should somehow get between the synchronize and freeze steps, then
forward progress is deadlocked.

We had been relying on the nvme controller state machine to ensure the
reset work wouldn't conflict with timeout work. That guarantee may be a
bit fragile to rely on, so iterate the namespace lists without taking
potentially circular locks, as reported by lockdep.

NVMe is making up issue_flags, which is a no-no in general, and to make
matters worse, they are completely the wrong ones. For a pure polled
request, which it does check for, we're already inside the
ctx->uring_lock when the completions are run off io_do_iopoll(). Hence
the correct flag would be '0' rather than IO_URING_F_UNLOCKED.

Move the namesapce info to struct nvme_ns_head, because it's the same
for all associated namespaces.

Note: with multipathing enabled the PI information is shared between all
paths. If a path is using a different PI configuration it will overwrite
the previous settings. This is obviously not correct and such
configuration will be rejected in future. For the time being we expect
a correctly configured storage.

linux/io_uring.h is slowly becoming a rubbish bin where we put
anything exposed to other subsystems. For instance, the task exit
hooks and io_uring cmd infra are completely orthogonal and don't need
each other's definitions. Start cleaning it up by splitting out all
command bits into a new header file.

a7a7dabb5d nvme: core: don't hold rcu read lock in nvme_ns_chr_uring_cmd_iopoll
Now nvme_ns_chr_uring_cmd_iopoll() has switched to request based io
polling, and the associated NS is guaranteed to be live in case of
io polling, so request is guaranteed to be valid because blk-mq uses
pre-allocated request pool.

Remove the rcu read lock in nvme_ns_chr_uring_cmd_iopoll(), which
isn't needed any more after switching to request based io polling.

Fix "BUG: sleeping function called from invalid context" because
set_page_dirty_lock() from blk_rq_unmap_user() may sleep.

80814b8e35 bio-integrity: update the payload size in bio_integrity_add_page()
Previously, the bip's bi_size has been set before an integrity pages
were added. If a problem occurs in the process of adding pages for
bip, the bi_size mismatch problem must be dealt with.

When the page is successfully added to bvec, the bi_size is updated.

The parts affected by the change were also contained in this commit.

9408d8a37e nvme: improved uring polling
Drivers can poll requests directly, so use that. We just need to ensure
the driver's request was allocated from a polled hctx, so a special
driver flag is added to struct io_uring_cmd.

The allows unshared and multipath namespaces to use the same polling
callback, and multipath is guaranteed to get the same queue as the
command was submitted on. Previously multipath polling might check a
different path and poll the wrong info.

The other bonus is we don't need a bio payload in order to poll,
allowing commands like 'flush' and 'write zeroes' to be submitted on the
same high priority queue as read and write commands.

Finally, using the request based polling skips the unnecessary bio
overhead.

opcode is checked, without checking the command type (IO command or
Admin command). When we send a Dataset Management command (The opcode
of the Dataset Management command is the same as the Set Feature
command), kernel thinks it is a set feature command, then sets the
controller's keep alive interval, and calls nvme_keep_alive_work().

commands completion. It further delays the execution of task_work for

the waiting criteria, which reduces the number of wake ups we issue.

fd9b8547bc io_uring: Pass whole sqe to commands
Currently uring CMD operation relies on having large SQEs, but future
operations might want to use normal SQE.

The io_uring_cmd currently only saves the payload (cmd) part of the SQE,
but, for commands that use normal SQE size, it might be necessary to
access the initial SQE fields outside of the payload/cmd block.  So,
saves the whole SQE other than just the pdu.

This changes slightly how the io_uring_cmd works, since the cmd
structures and callbacks are not opaque to io_uring anymore. I.e, the
callbacks can look at the SQE entries, not only, in the cmd structure.

The main advantage is that we don't need to create custom structures for
simple commands.

Creates io_uring_sqe_cmd() that returns the cmd private data as a null
pointer and avoids casting in the callee side.
Also, make most of ublk_drv's sqe->cmd priv structure into const, and use
io_uring_sqe_cmd() to get the private structure, removing the unwanted
cast. (There is one case where the cast is still needed since the
header->{len,addr} is updated in the private structure)

io_uring_cmd_done() currently assumes that the uring_lock is held
when invoked, and while it generally is, this is not guaranteed.
Pass in the issue_flags associated with it, so that we have
IO_URING_F_UNLOCKED available to be able to lock the CQ ring
appropriately when completing events.
