Add a ctxt file to spufs that shows spu context information that is used
in scheduling. This info can be used for debugging spufs scheduler
issues, and to isolate between application and spufs problems as it
shows a lot of state such as priorities and dispatch counts.
This file contains internal spufs state and is subject to change at any
time, and therefore no applications should depend on it. The file is
intended for the use of spufs kernel developers.

This updates the device tree manipulation routines so that memory
add/remove of lmbs represented under the
ibm,dynamic-reconfiguration-memory node of the device tree invokes the
hotplug notifier chain.
This change is needed because of the change in the way memory is
represented under the ibm,dynamic-reconfiguration-memory node. All lmbs
are described in the ibm,dynamic-memory property instead of having a
separate node for each lmb as in previous device tree layouts. This
requires the update_node() routine to check for updates to the
ibm,dynamic-memory property and invoke the hotplug notifier chain.
This also updates the pseries hotplug notifier to be able to gather information
for lmbs represented under the ibm,dynamic-reconfiguration-memory node and
have the lmbs added/removed.

Use the base address of the lmb to derive the starting page frame number
instead of trying to extract it from the drc index of the lmb. The drc
index should not be used for this as it will, and did, break.
Until this point, systems that have had memory represented in the device
tree with a node for each lmb the drc index would (luckily) closely
track the base address of the lmb. For example a lmb with a drc index
of 8000000a would have a base address of a0000000. This correlation
allowed the current code to derive the starting page frame number from
the drc inddex
Device tree layouts where lmbs are represented under the
ibm,dynamic-reconfiguration-memory node in the ibm,dynamic-memory
property do not have this correlation between the drc index and base
address of the lmb.

Allow the phandle passed to the /proc/ppc64/ofdt file to be specified
in formats other than decimal. This allows us to easily specify phandle
values in hex that would otherwise appear as negative integers.
This is an issue on systems where the value of
/proc/device-tree/ibm,dynamic-reconfiguration-memory.ibm,phandle is
fffffff9. Having to pass this to the ofdt file as a string results in
a large negative number, and simple_strtoul() does not handle negative
numbers.

Suspend/resume ("echo mem > /sys/power/state") does not work with
vanilla kernels -- the system does not suspend correctly and just
hangs. This patch fixes this so suspend/resume works:
1) of_iomap does not map the whole 0xC000 of the MPC5200 immr so
saving registers does not work.
2) PCI registers need to be saved and restored.

Currently create_branch() creates a branch instruction for you, and
patches it into the call site. In some circumstances it would be nice
to be able to create the instruction and patch it later, and also some
code might want to check for errors in the branch creation before
doing the patching. A future commit will change create_branch() to
check for errors.
For callers that don't care, replace create_branch() with
patch_branch(), which just creates the branch and patches it directly.
While we're touching all the callers, change to using unsigned int *,
as this seems to match usage better. That allows (and requires) us to
remove the volatile in the definition of vector in powermac/smp.c and
mpc86xx_smp.c, that's correct because now that we're passing vector as
an unsigned int * the compiler knows that it's value might change
across the patch_branch() call.

We currently have a few routines for patching code in asm/system.h, because
they didn't fit anywhere else. I'd like to clean them up a little and add
some more, so first move them into a dedicated C file - they don't need to
be inlined.
While we're moving the code, drop create_function_call(), it's intended
caller never got merged and will be replaced in future with something
different.

At present, if we have a kernel with a 64kB page size, and some
process maps something that has to be mapped with 4kB pages (such as a
cache-inhibited mapping on POWER5+, or the eHCA infiniband queue-pair
pages), we change the process to use 4kB pages everywhere. This hurts
the performance of HPC programs that access eHCA from userspace.
With this patch, the kernel will only demote the slice(s) containing
the eHCA or cache-inhibited mappings, leaving the remaining slices
able to use 64kB hardware pages.
This also changes the slice_get_unmapped_area code so that it is
willing to place a 64k-page mapping into (or across) a 4k-page slice
if there is no better alternative, i.e. if the program specified
MAP_FIXED or if there is not sufficient space available in slices that
are either empty or already have 64k-page mappings in them.

ppc loses the timeout functionality of smp_call_function_mask() with
this change, as the generic code does not provide that.

There is a delay in the transition to the stopped state for class 2
interrupts. In some cases, the controlling thread detects the state of
the spu as running, and goes back to sleep resulting in a hung
application as the event is missed.
This change detects the stop condition and re-generates the wakeup event
after a context save.

Time slicing can occur at the same time as spu exception handling
resulting in the wakeup of the wrong thread.
This change uses the the spu's register_lock to enforce synchronization
between bind/unbind and spu exception handling so that they are
mutually exclusive.

According to the CBEA, the SPU dsisr is not updated for class 0
exceptions.
spu_stopped() is testing the dsisr that was passed to it from the class
0 exception handler, so we return a false positive here.
This patch cleans up the interrupt handler and erroneous tests in
spu_stopped. It also removes the fields from the csa since it is not
needed to process class 0 events.

The "ndfc-chip" device doesn't need any resources. All resources
are handled by the "ndfc-nand" device. Registering the same memory
resource twice causes "cat /proc/iomem" to go into an infinite loop
displaying NDFC memory addresses.

The MPC85xx MDS board requires some board level tweaks of the PHYs that
either the eTSEC (gianfar) or UCC ethernet controllers are connected to.
Its possible to build the phylib as a module, however this breaks the
board level fix ups because phy_read and phy_write are not available
if we build as a module.
So we unconditionally select PHYLIB to ensure its built into the kernel
if we are building in MPC85xx MDS support. This was determined to be
the easiest soultion even though it prevents the user from removing
PHYLIB support if they decide they don't want it.

When I changed irq_alloc_host() to take an of_node
(52964f87c6: "Add an optional
device_node pointer to the irq_host"), I botched the reference
counting semantics.
Stephen pointed out that it's irq_alloc_host()'s business if
it needs to take an additional reference to the device_node,
the caller shouldn't need to care.

A change was made to walk_memory_resource() in commit
4b119e21d0 that added a
check of find_lmb(). Add the coresponding lmb_add()
call to ps3_mm_add_memory() so that that check will
succeed.
This fixes the condition where the PS3 boots up with
only the 128 MiB of boot memory, and doesn't see the
other 128MiB that is available.

This adds some debugging code to the Axon MSI driver. It creates a
file per MSIC in /sys/kernel/debug/powerpc, which allows the user to
trigger a fake MSI interrupt by writing to the file.
This can be used to test some of the MSI generation path. In
particular, that the MSIC recognises a write to the MSI address,
generates an interrupt and writes the MSI packet into the ring buffer.
All the code is inside #ifdef DEBUG so it causes no harm unless it's
enabled.

The following adds local bus, flash and MTD partition nodes for
sbc8548. As well, a compatible field for the soc node, so that
of_platform_bus_probe() will pick it up.

DIU platform code should not just write to the PIXIS' BRDCFG0 register,
it should set and clear its own bits only, otherwise it will break
firmware setup (in fact it breaks second uart).
Also get rid of magic numbers in the related code.

The 8568 MDS needs some configuration changes to the PHY in order to
work properly. These are done in the firmware, normally, but Linux
shouldn't need to rely on the firmware running such things (someone
could disable the PHY support in the firmware to save space, for instance).

We should not requeue the victim context in find_victim if the owner is
not in spu_run. It's first not needed because leaving the context on
the spu is an optimization and second is harmful because it means the
owner could re-enter spu_run when the context is on the runqueue and
trip the BUG_ON in __spu_update_sched_info.

Currently, page fault handlers don't issue a mfc restart if the context
switch pending flag is set, which can leave us with a hanging DMA after
a context restore.
This patch introduces fault pending flag that is set by the fault
handler and read by the context switch code, so that the latter can add
the restart bit at the right spot, after it has successfuly saved the
state of the mfc control register.

SPU class 0 & 1 exceptions may occur in parallel, so we may end up
overwriting csa.dsisr.
This change adds dedicated fields for each class to the spu and the spu
context so that fault data is not overwritten.

Currently, we re-route SPU interrupts to the current cpu, which may be
on a remote node. In the case of time slicing, all spu interrupts will
end up routed to the same cpu, where the spusched_tick occurs.
This change routes mfc interrupts to the cpu where the controlling
thread last ran, provided that cpu is on the same node as the spu
(otherwise don't reroute interrupts).
This should improve performance and provide a more predictable
environment for processing spu exceptions. In the past we have seen
concurrent delivery of spu exceptions to two cpus. This eliminates that
concern.

synchronize_irq() provides the serialization for
SPU_CONTEXT_SWITCH_PENDING which is read with a simple load. This
routine guarantees that the relevant interrupt handlers are not running,
so that the next time they do run they will see the update
memory value.
This must be done correctly so that exception handling code does not
restart the mfc in the middle of a context switch while we are trying
to atomically stop it and save state.

There's currently no way to tell if spu_process_callback has
returned with the state mutex held, as -EINTR may be returned
by either the syscall or the spu_acquire fail case.
Instead, just do a non-interruptible mutex_lock here.

Currently, we update the SPU master run control bit (ie,
spu_enable_spu) in spufs_run_spu before we grab the context mutex. This
can result in races with other processes accessing this context's
resources.
This change moves the spu_enable_spu to after we have acquired the
context lock.

We currently have two issues with the MFC save code:
* save_mfc_decr doesn't handle a transition of 1 -> 0 of the Ds bit
* The Q bit may be stale in the CSA
This change fixes the first issue by clearing the relevant bits from
the MFC_CNTL value in the CSA before or-ing in the updated status.
Also, we add the Q bit to the updated status.

Currently, we can introduce invalid entries into the MFC queues:
1) context starts a DMA
2) context gets scheduled out during a DMA
- kernel saves MFC queue to CSA
- kernel saves 0x0 in csa->mfc_control_RW
3) context gets scheduled in
- csa->mfc_control[Q] ('queues empty') isn't set, so DMA queues are
restored from the CSA
4) context's DMA is completed
5) context gets scheduled out again, no DMA occuring this time
- kernel sees that MFC_CNTL[Q] ('queues empty') is set, so doesn't
touch saved queue data in CSA
- kernel saves 0x0 in csa->mfc_control_RW
6) context gets scheduled in
- csa->mfc_control[Q] ('queues empty') isn't set (we saved is as 0!),
so DMA queues are restored from the CSA
In this last restore, we've restored the queue status from step 2,
which are now invalid.
This change makes save_mfc_cntl() closer to the save/restore sequence,
as specified in the CBE handbook.

When we issue a MFC purge request, we may inadvertantly clear the
suspended status.
This change adds the MFC_CNTL_SUSPEND_MASK when we issue a purge
request, so that the suspend bit is masked out.

We may currently lose interrupts during SPE context switch, as we alter
the INT_Route register. Because the IIC uses a per-thread priority
status, changing the interrupt routing to a different thread means that
the IRQ is no longer masked by the priority status, so we end up with
two fasteoi IRQ handlers executing for the one irq_desc. The fasteoi
handler doesn't handle multiple IRQs, so drops the second one.
Fix this by using our own flow handler. This is based on
handle_edge_irq, but issues an eoi after IRQs are handled, and doesn't
do any mask/unmasking.

There are userspace instrumentation tools that need to monitor spu
context switches. This patch adds a new file called 'switch_log' to
each spufs context directory that can be used to monitor the context
switches.
Context switch in/out and exit from spu_run are monitored after the
file was first opened and can be read from it.

The powerpc kernel maintains information about logical memory blocks
in the lmb.memory structure, which is initialized and updated at boot
time, but not when memory is added or removed while the kernel is
running.
This adds a hotplug memory notifier which updates lmb.memory when
memory is added or removed. This information is useful for eHEA
driver to find out the memory layout and holes.
NOTE: No special locking is needed for lmb_add() and lmb_remove().
Calls to these are serialized by caller. (pSeries_reconfig_chain).

Hotplug memory remove notifier for 64-bit powerpc. This gets invoked
by writing to /proc/ppc64/ofdt the string "remove_node " followed by
the firmware device tree pathname of the node that needs to be removed.
In response, this adjusts the sections and removes sysfs entries by
calling __remove_pages(). Then it calls arch-specific code to get rid
of the hardware MMU mappings for the section of memory.

This adds support for PCI Express port on Celleb. I/O space of this
PCI Express port is not mapped in memory space. So we use the
io-workaround mechanism to make accesses indirect.

Add a DEBUG config setting which turns on all (most) of the debugging
under platforms/pseries.
To have this take effect we need to remove all the #undef DEBUG's, in
various files. We leave the #undef DEBUG in platforms/pseries/lpar.c,
as this enables debugging printks from the low-level hash table routines,
and tends to make your system unusable. If you want those enabled you
still have to turn them on by hand.
Also some of the RAS code has a DEBUG block which causes a functional
change, so I've keyed this off a different (non-existant) debug #define.
This is only enabled if you have PPC_EARLY_DEBUG enabled also.

In pseries/lpar.c, fix some printf specifier mismatches, and add
a newline to one printk.
In pseries/rtasd.c add "rtasd" to some messages to make it clear
where they're coming from.
In pseries/scanlog.c remove the hand-rolled runtime debugging support
in there. This file has been largely unchanged for eons, if we need to
debug it in future we can recompile.

On pseries LPAR we can call the udbg routines, and the udbg console very
early. So mark the udbg console as safe to call early in boot, and register
the udbg console as soon as the udbg routines are hooked up.
This allows platforms/pseries code to use printk() and pr_debug() rather
than needing to call udbg_printf() directly for early debugging. This is
nice because a) it's standard, b) it goes via the printk buffer, and c)
you can get printk time stamps.

Currently all iSeries secondary CPUs spin directly on the cpu_start
field in their paca. Make them spin on the global
__secondary_hold_spinloop until after the pacas have been initialised.
As Stephen Rothwell points out, this works at the moment because
__secondary_hold_spinloop is being set already, but iSeries isn't
looking at it :)

This lets us check if the device is really added to the driver core or
not, which is what we need when walking some of the bus lists. The flag
is there in anticipation of getting rid of the other PCI device list,
which is what we used to check in this situation.

None of these files use any of the functionality promised by
asm/semaphore.h. It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have

The xics code currently has a direct and lpar variant of
xics_host_map, the only difference being which irq_chip they use. If
we remember which irq_chip we're using we can combine these two
routines. That also allows us to have a single irq_host_ops instead
of two.

pseries_mpic_init_IRQ() implements the same logic as the xics code did to
find the i8259 cascade irq. Now that we've pulled that logic out into
pseries_setup_i8259_cascade() we can use it in the mpic code.

Currently USB Host isn't functional on the MPC8315E boards, for two
reasons as described below.
MPC8315 Reference Manual says:
"The USB DR unit must have the same clock ratio as the encryption core
unit, unless one of them has its clock disabled."
The encryption core also drives I2C clock, so it is enabled and is equal
to 01. That means USBDRCM should be 01 here.
Plus, according to MPC8315E-RDB schematics, USB unit consumes CLK_IN
clock from the 24.00MHz oscillator, which means we must adjust REFSEL
bits as well.
p.s.
Idially we should rework whole 83xx/usb.c code, in two steps:
1. Move SCCR code to the U-Boot;
2. Implement fsl,usb-clock property in the device tree, so usb.c could
decide what clock exactly to use on per-board basis.
Though, today we're not in a hurry since there is just one 8315e board
out there.

As suggested by Timur Tabi, we match on the old compat node ID for one
version and warn accordingly. If we don't do this, we plunge people who
try to use an old DTB into silent boot death with no clear indication of
what the problem is.
This patch should be removed at the beginning of the 2.6.27 dev cycle.
It is only meant to ease the transition in the short term.

This adds support for the Wind River SBC8641D board, based

There is logic in platforms/peries/lpars.c which checks if the user has
specified a console on the command line, and refrains from adding a
preferred console entry for the hvc/hvsi console if they have.
This trips up if you use "netconsole=foo" on the command line, and has
the result that you get _only_ the netconsole, because the hvc device is
never added as a preferred console. Worse still if you get the netconsole
configuration wrong somehow, you end up with no console at all.
As it turns out we don't need to worry about checking the command line.
If the user has specified "console=foo", then foo will be set as the
preferred console when the command line is parsed in start_kernel(), much
later than the pseries code, and so the latter setting will take effect.

This changes the way we calculate how much space to reserve for the
pHyp dump. Currently we reserve 256MB only. With this change, the
code first checks to see if an amount has been specified on the boot
command line with the "phyp_dump_reserve_size" option, and if so, uses
that much.
Otherwise it computes 5% of total ram and rounds it down to a multiple
of 256MB, and uses the larger of that or 256MB.
This is for large systems with a lot of memory (10GB or more). The
aim is to have more space available for the kernel on reboot on
machines with more resources. Although the dump will be collected
pretty fast and the memory released really early on allowing the
machine to have the full memory available, this alleviates any issues
that can be caused by having way too little memory on very very large
systems during those few minutes.

This changes the cpu_idle loop for 44x platforms to utilize the Wait Enable
feature of the CPU. This helps virtulization solutions know when the guest
Linux kernel is in an idle state.
A command line option called "idle" is also added to allow people to change
the idle loop back to the original variation. This is done by setting
"idle=spin" on the kernel command line.

The iSeries HV only needs the first two fields of the paca statically
initialised, so create an alternate paca that contains only those and
switch to our real paca immediately after boot.
This is in order to make the 1024 cpu patches easier since they will no
longer have to statically initialise the pacas for iSeries.

With a multiplatform kernel, once built we always have warp_setup_nand_flash()
called and NDFC probed, no matter what machine we actually run on. This
potentially can cause problems (such as kernel crash), since NDFC is probed at
a warp-predefined address.
Using machine_device_initcall() NAND devices are registered if we run on a warp only.

This patch creates a common system reset routine for all 40x and 44x
systems. Previously only a 44x routine existed. But since this system
reset via the debug control register is common for 40x and 44x let's
share this code for all those platforms in ppc4xx_soc.c.
This patch also enables CONFIG_4xx_SOC for all 40x and 44x platforms.
Tested on Kilauea (405EX) and Canyonlands (440EX).

Split the device setup code in ps3_register_repository_device() in two
routines:
1. ps3_setup_static_device(), to handle the setup of static devices in the
PS3 repository, which can be __init,
2. ps3_setup_dynamic_device(), to handle the setup of storage devices that
may appear later in the PS3 repository.
This fixes a few section mismatch warnings.

At present, ppu-gdb can't trace spu infomation with coredump generated
by the kernel. While the core dumps notes have correct contents, they
have the wrong names, as the file descriptors used to generate the note
names are off-by-one. An application that opens a SPE context as fd 3,
the current core dump code will generate notes like:
SPU/4/mem
SPU/4/regs
etc.
This confuses GDB, which knows it is looking for SPE context 3 (from
parsing the spu_context_run system call arguments), and cannot find
any notes that match context 3.
This change corrects the file descriptor counting, to only increment
the fd until after we've written the note name.

During the context save process, we currently save the MFC command
channel after purging the MFC queues. This causes a systemsim warning,
as the command channel may be in an unknown state after the purge.
This change does the save before purging the MFC queues.

During spu_process callback, we release then acquire the SPU, but keep a
pointer to the local store memory. Since the context may have been
scheduled out during the callback, the ls pointer may become invalid.
This change reacquires the pointer to the context local store after
spu_acquire()-ing, so that it isn't invalidated by a context switch.

The KSI8560 is a single compact, mid-, or full-size Advanced Mezzanine Card
(AdvancedMC™) based on the Freescale™ Semiconductor MPC8560 PowerQUICC III™
microprocessor. This product will serve in data and signaling applications such
as signaling gateways (SGW) and softswitch signaling interface cards.
The board has altera maxii CPLD, that is used to obtain and manage board
configuration. Also there are two SCC UART serial consoles and FCC ethernet,
that is routed to the front panel, while other ethernet controlers (TSEC's) are
routed to the backplane.

This patch adds support for the 256k L2 cache found on some IBM/AMCC
4xx PPC's. It introduces a common 4xx SoC file (sysdev/ppc4xx_soc.c)
which currently "only" adds the L2 cache init code. Other common 4xx
stuff can be added later here.
The L2 cache handling code is a copy of Eugene's code in arch/ppc
with small modifications.
Tested on AMCC Taishan 440GX.

This adds a kernel command line option "phyp_dump", which takes a 0/1
value for disabling/ enabling phyp_dump at boot time. Kdump can use
this on cmdline (phyp_dump=0) to disable phyp-dump during boot when
enabling itself. This will ensure only one dumping mechanism is active
at any given time.

Check to see if there actually is data from a previously
crashed kernel waiting. If so, allow user-space tools to
grab the data (by reading /proc/kcore). When user-space
finishes dumping a section, it must release that memory
by writing to sysfs. For example,
echo "0x40000000 0x10000000" > /sys/kernel/release_region
will release 256MB starting at the 1GB. The released memory
becomes free for general use.

Initial patch for reserving memory in early boot, and freeing it
later. If the previous boot had ended with a crash, the reserved

The functions time_before, time_before_eq, time_after, and
time_after_eq are more robust for comparing jiffies against other
values.
This implements usage of the time_after() macro, defined at
linux/jiffies.h, which deals with wrapping correctly.

The hypervisor can look at the value in the wait_state_cycles field of
the VPA for an estimate of how busy dedicated processors are.
Currently, as the kernel never touches this field, we appear to be
100% busy. This records the duration the kernel is in powersave and
passes that to the HV to provide a reasonable indication of
utilisation.

Prevailing practice for define_machine() in powerpc is to use the
platform name when the platform has only one define_machine()
statement, but maple uses "maple_md". This caused me some
head-scratching when writing some new code that uses
machine_is(maple).
Use "maple" instead of "maple_md". There should not be any behavioral
change -- fixup_maple_ide() calls machine_is(maple) but the body of
the function is ifdef'd out.

The PCI bridge representing the PCIE root complex on Axon, contains
device BARs for a memory range and ROM that define inbound accesses.
This confuses the kernel resource management code -- the resources
need to be hidden when Axon is a host bridge.

The cell IOMMU code to parse the dma-ranges properties, used for the fixed
mapping, was broken in two ways for some devices.
Firstly it didn't cope with empty dma-ranges properties. An empty property
implies no translation so can be safely skipped.
The code also wrongly assumed it would be looking at PCI devices, and hard
coded the number of address and size cells.

At present, we can hit the BUG_ON in __spu_update_sched_info by reading
the regs file of a context between two calls to spu_run. The
spu_release_saved called by spufs_regs_read() is resulting in the (now
non-runnable) context being placed back on the run queue, so the next
call to spu_run ends up in the bug condition.
This change uses the SPU_SCHED_SPU_RUN flag to only reschedule a context
if it's still in spu_run().

commit 4ef11014 introduced a usage of SCHED_IDLE to detect when
a context is within spu_run.
Instead of SCHED_IDLE (which has other meaning), add a flag to
sched_flags to tell if a context should be running.

Not all e300 cores support the performance monitors, and the ones
that don't will be confused by the mf/mtpmr instructions. This
allows the support to be optional, so the 8349 can turn it off
while the 8379 can turn it on. Sadly, those aren't config options,
so it will be left to the defconfigs and the users to make that
determination.

The only tricky part is we need to adjust the PTE insertion loop to
cater for holes in the page table. The PTEs for each segment start on
a 4K boundary, so with 16M pages we have 16 PTEs per segment and then
a gap to the next 4K page boundary.
It might be possible to allocate the PTEs for each segment separately,
saving the memory currently filling the gaps. However we'd need to
check that's OK with the hardware, and that it actually saves memory.

We use n_pte_pages to calculate the stride through the page tables, but
we also use it to set the NPPT value in the segment table entry. That is
defined as the number of 4K pages per segment, so we should calculate
it as such regardless of the IOMMU page size.

Currently the cell IOMMU code allocates the entire IOMMU page table in a
contiguous chunk. This is nice and tidy, but for machines with larger
amounts of RAM the page table allocation can fail due to it simply being
too large.
So split the segment table and page table setup routine, and arrange to
have the dynamic and fixed page tables allocated separately.

The cell IOMMU tce build and free routines use pte_offset to convert
the index passed from the generic IOMMU code into a page table offset.
This takes into account the SPIDER_DMA_OFFSET which sets the top bit
of every DMA address.
However it doesn't cater for the IOMMU window starting at a non-zero
address, as the base of the window is not incorporated into pte_offset
at all.
As it turns out tbl->it_offset already contains the value we need, it
takes into account the base of the window and also pte_offset. So use
it instead!

The spu_runcntl_RW register is restored within spu_restore function.
So, at the end of spu_bind_context, the SPU context is not just loaded,
but running.
This change corrects the state switch to account the time as USER.

There is a potential race between flushes of the entire SLB in the MFC
and the point where new entries are being established. The problem is
that we might put a ESID entry into the MFC SLB when the VSID entry has
just been cleared by the global flush.
This can be circumvented by holding the register_lock throughout both
the flushing and the creation of SLB entries.

When we replace an SLB entry in the MFC after using up all the available
entries, there is a short window in which an incorrect entry is marked
as valid.
The problem is that the 'valid' bit is stored in the ESID, which is
always written after the VSID. Overwriting the VSID first will make the
original ESID entry point to the new VSID, which means that any
concurrent DMA accessing the old ESID ends up being redirected to the
new virtual address. A few cycles later, we write the new ESID and
everything is fine again.
That race can be closed by writing a zero entry to the ESID first, which
makes sure that the VSID is not accessed until we write the new ESID.
Note that we don't actually need to invalidate the SLB entry using the
invalidation register, which would also flush any ERAT entries for that
segment, because the segment translation does not become invalid but is
only removed from the SLB cache.

There is a small race between the context save procedure
and the SPU interrupt handling, where we expect all interrupt
processing to have finished after disabling them, while
an interrupt is still being processed on another CPU.
The obvious fix is to call synchronize_irq() after disabling
the interrupts at the start of the context save procedure
to make sure we never access the SPU any more during an
ongoing save or even after that.

Which leads me to believe that 160[67] is the current thread ID, and
1605 is the context backing the psmap.
However, the 'current' and 'owner' tids are reversed - the 'current'
tid is on the right. This change puts the current thread ID in the
left-hand column instead, and renames the right to 'ctxthread'.

At present, we have a situation where a context with no owner is
re-scheduled by spu_forget:
Thread 1: reading regs file Thread 2: context owner
spu_forget()
- ctx->owner = NULL
- set SPU_SCHED_WAS_ACTIVE
spu_acquire_saved()
- context is in saved state
spu_release_saved()
- SPU_SCHED_WAS_ACTIVE is set,
so spu_activate() the context,
which now has no owner
In spu_forget(), we shouldn't be requesting a re-schedule by setting
SPU_SCHED_WAS_ACTIVE. This change removes the set_bit in spu_forget(),
so that spu_release_saved() doesn't reinsert this destroyed context on
to the run queue.

We have a small window where a spu context may be destroyed while
we're servicing a page fault (from another thread) to the context's
problem state mapping.
After we up_read() the mmap_sem, it's possible that the context is
destroyed by its owning thread, and so the later references to ctx
are invalid. This can maifest as a deadlock on the (now free()-ed)
context state mutex.
This change adds a reference to the context before we release the
mmap_sem, so that the context cannot be destroyed.

m8xx_setup.c says:

And at the same time it is using bus-frequency for calculating
timebase. It is okay for most setups because bus-frequency is
equal to clock-frequency.
The problem emerges when cpu frequency is > 66MHz, quoting
u-boot/cpu/mpc8xx/speed.c:

Linux, we calculate timebase frequency as tb_freq = bus_clk / 16,
that is cpu_clk / 2 / 16, which is wrong.
This fixes the system time drifting problem on the EP885C board
running at 133MHz.

For memory remove, we need to clean up htab mappings for the
section of the memory we are removing.
This implements support for removing htab bolted mappings for pSeries
logical partitions. Other sub-archs may need to implement similar
functionality for hotplug memory remove to work on them.

At present, the __spufs_trap_data_map and __spu_trap_data_seq functions
exit if spu->flags has the SPU_CONTEXT_SWITCH_ACTIVE set. This was
resulting in suprious returns from these functions, as they may be
legitimately called when we have this bit set.
We only use it in these two sanity checks, so this change removes the
flag completely. This fixes hangs in the page-fault path of SPE apps.

2.6.25 has a regression where we can starve the scheduler by creating
(N_SPES+1) contexts, then running them one at a time.
The final context will never be run, as the other contexts are loaded on
the SPEs, none of which are repoted as free (ie, spu->alloc_state !=
SPU_FREE), so spu_get_idle() doesn't give us a spu to run on. Because
all of the contexts are stopped, none are descheduled by the scheduler
tick, as spusched_tick returns if spu_stopped(ctx).
This change replaces the spu_stopped() check with checking for SCHED_IDLE
in ctx->policy. We set a context's policy to SCHED_IDLE when we're not
in spu_run(). We also favour SCHED_IDLE contexts when looking for contexts
to unbind, but leave their timeslice intact for later resumption.
This patch fixes the following test in the spufs-testsuite:

This is the central patch of a cleanup series. In most cases there is no good
reason why someone would want to use a dentry for itself. This series reflects
that fact and embeds a struct path into nameidata.
Together with the other patches of this series
- it enforced the correct order of getting/releasing the reference count on
<dentry,vfsmount> pairs
- it prepares the VFS for stacking support since it is essential to have a
struct path in every place where the stack can be traversed
- it reduces the overall code size:

RCU style multiple probes support for the Linux Kernel Markers. Common case
(one probe) is still fast and does not require dynamic allocation or a
supplementary pointer dereference on the fast path.
- Move preempt disable from the marker site to the callback.
Since we now have an internal callback, move the preempt disable/enable to the
callback instead of the marker site.
Since the callback change is done asynchronously (passing from a handler that
supports arguments to a handler that does not setup the arguments is no
arguments are passed), we can safely update it even if it is outside the
preempt disable section.
- Move probe arm to probe connection. Now, a connected probe is automatically
armed.
Remove MARK_MAX_FORMAT_LEN, unused.
This patch modifies the Linux Kernel Markers API : it removes the probe
"arm/disarm" and changes the probe function prototype : it now expects a
va_list * instead of a "...".
If we want to have more than one probe connected to a marker at a given
time (LTTng, or blktrace, ssytemtap) then we need this patch. Without it,
connecting a second probe handler to a marker will fail.
It allow us, for instance, to do interesting combinations :
Do standard tracing with LTTng and, eventually, to compute statistics
with SystemTAP, or to have a special trigger on an event that would call
a systemtap script which would stop flight recorder tracing.

Sometimes simple attributes might need to return an error, e.g. for
acquiring a mutex interruptibly. In fact we have that situation in
spufs already which is the original user of the simple attributes. This
patch merged the temporarily forked attributes in spufs back into the
main ones and allows to return errors.

Currently the cell IOMMU fixed mapping just printks that it's been setup,
which is not particularly useful. Much more interesting is the address
ranges for the different windows. This adds one line to dmesg on a blade.

If we get a 64-bit dma mask we switch to the fixed ops and call
cell_dma_dev_setup(). If the driver then switches back to a 32-bit dma
mask for any reason we don't call cell_dma_dev_setup() again, which
has the potential to leave bogus data in dev->archdata.dma_data.

Stop bits are only valid when the running bit is not set. Status bits
carry over from one invocation of spufs_run_spu() to another, so the
RUNNING bit gets added to the previous state of the register which may
have been a remote library call. In this case, it looks like another
library routine should be invoked, but the spe is actually running.
This fixes a problem with a testcase that exercises the scheduler.

Currently, the kernel may fail to restart a SPE context which
has stopped and been swapped out.
This changes spu_backing_runcntl_write to emulate the real
SPU_Status register exactly. When the SPU Run Control register
is written with SPU_RunCntl[Run] set to '1', the physical SPU
automatically sets SPU_Status[R] and clears SPU_Status[CISHP].

Fix various state_mutex leaks. The worst one was introduced by the
interrutible state_mutex conversion but there've been a few before
too. Notably spufs_wait now returns without the state_mutex held
when returning an error, which actually cleans up some code.

It appears that with the U3 northbridge, if the processor is in NAP
mode the whole time while waiting for an SMU command to complete,
then the SMU will fail. It could be related to the weird backward
mechanism the SMU uses to get to system memory via i2c to the
northbridge that doesn't operate properly when the said bridge is
in napping along with the CPU. That is on U3 at least, U4 doesn't
seem to be affected.
This didn't show before NO_HZ as the timer wakeup was enough to make
it work it seems, but that is no longer the case.
This fixes it by disabling NAP mode on those machines while
an SMU command is in flight.

The xics code does update the default server information when the boot
cpu is removed. This patch recognizes when the boot cpu is being
removed and updates the appropriate information based on the new 'boot
cpu'.
Failure to update this information can causes us to leave irqs pinned
to cpus that are being removed, especially when removing the boot cpu.
The cpu is removed from the kernel, but cpu dlpar remove operations
fail since we cannot return the cpu to the hypervisor.

This splits off the kexec path bits of the xics_teardown_cpu() routine
into its own xics_kexec_teardown_cpu() routine. With the previous
combined routine the CPPR for a cpu that is being removed may have its
CPPR reset in the plpar_eoi() call (which explicitly sets the CPPR to
a non-zero value). Splitting of the kexec bits of the code prevents
this from happening in the cpu remove path.
Once again, this does not cause the cpu remove from the kernel to
fail, but it does cause cpu dlpar operations to not be able to return
the cpu to the hypervisor.

The affinity mask in the virq descriptor needs to be set before we
reset the affinity for the virq. Without doing this the call to get
the new irq server fails and we end up leaving the virq pinned to the
cpu we are removing.
This does not fail the cpu remove from the kernel, but it does prevent
cpu dlpar remove operations from returning the cpu to the hypervisor.

calibrate_delay() must be __cpuinit, not __{dev,}init.
I've verified that this is correct for all users.
While doing the latter, I also did the following cleanups:
- remove pointless additional prototypes in C files
- ensure all users #include <linux/delay.h>

1) xilinx->xlnx (recognized standard is to use the stock ticker)
2) In order to have the device tree focus on describing what the

I got this warning from gcc:
arch/powerpc/platforms/cell/axon_msi.c:118: warning: 'tmp' may be used uninitialized in this function
Which turns out to be a false positive, but pointed out that it was
possible for the error path in find_msi_translator() to do an extra
of_node_put on a node. This fixes it by localising the ref counting
a bit. As a side effect, the warning goes away.

There's a brown-paper-bag bug in axon_msi, we pass the address of our
FIFO directly to the hardware, without DMA mapping it. This leads to
DMA exceptions if you enable MSI & the IOMMU.
The fix is to correctly DMA map the fifo, dma_alloc_coherent() does
what we want - and we need to track the virt & phys addresses.

Currently cell publishes OF devices at device_initcall() time, which
means the earliest a driver can bind to a device is also device_initcall()
time. We have a driver we want to register before other devices, so
publish the devices at subsys_initcall() time.
This should not cause any behaviour change for existing drivers, as they
are still bound at device_initcall() time.

Reference count for the "neighbor" spu context was not
being correctly decremented after usage.
So, contexts used as reference during SPU affinity setup
were not being deallocated, leading to a memory leak.

Currently we only catch debug events through the 0x3fff status;
spufs_run_spu doesn't handle single-step SPE events.
This change adds a handler for conditions where the SPE is stopped due
to single-step-mode.

This adds markers two important points in the spufs code and a new
module (sputrace.ko) that allows reading these out through a proc file.
Long-term I'd rather see something like lttng extended to use the spufs
instrumentation, but for now I think this is a good enough quick
solution. We'll probably want to add various addition event in addition
to that ones I have already.

This patch converts PPC's IOMMU to use the IOMMU helper functions. The IOMMU
doesn't allocate a memory area spanning LLD's segment boundary anymore.
iseries_hv_alloc and iseries_hv_map don't have proper device
struct. 4GB boundary is used for them.

On ACPI systems the target state set by acpi_pm_set_target() is
reset by acpi_pm_finish(), but that need not be called if the
suspend fails. All platforms that use the .set_target() global
suspend callback are affected by analogous issues.
For this reason, we need an additional global suspend callback that
will reset the target state regardless of whether or not the suspend
is successful. Also, it is reasonable to rename the .set_target()
callback, since it will be used for a different purpose on ACPI
systems (due to ACPI 1.0x code ordering requirements).
Introduce the global suspend callback .end() to be executed at the
end of the suspend sequence and rename the .set_target() global
suspend callback to .begin().

More late-caught fallout from the mainline merge. Commit
35e4a6e26d ("[POWERPC] Use
archdata.dma_data in dma_direct_ops and add the offset") claimed
"Now that all platforms using dma_direct_offset setup the
archdata.dma_data correctly, ..."
..but nope -- the pasemi iommu setup code that disables translation on
the DMA pci device didn't set dma_data correctly.
This fixes it.

This patch adds support for setting up a fixed IOMMU mapping on certain
cell machines. For 64-bit devices this avoids the performance overhead of
mapping and unmapping pages at runtime. 32-bit devices are unable to use
the fixed mapping.
The fixed mapping is established at boot, and maps all of physical memory
1:1 into device space at some offset. On machines with < 30 GB of memory
we setup the fixed mapping immediately above the normal IOMMU window.
For example a machine with 4GB of memory would end up with the normal
IOMMU window from 0-2GB and the fixed mapping window from 2GB to 6GB. In
this case a 64-bit device wishing to DMA to 1GB would be told to DMA to
3GB, plus any offset required by firmware. The firmware offset is encoded
in the "dma-ranges" property.
On machines with 30GB or more of memory, we are unable to place the fixed
mapping above the normal IOMMU window as we would run out of address space.
Instead we move the normal IOMMU window to coincide with the hash page
table, this region does not need to be part of the fixed mapping as no
device should ever be DMA'ing to it. We then setup the fixed mapping
from 0 to 32GB.

Add support to cell_iommu_setup_page_tables() for handling two windows,
the dynamic window and the fixed window. A fixed window size of 0
indicates that there is no fixed window at all.
Currently there are no callers who pass a non-zero fixed window, but the
upcoming fixed IOMMU mapping patch will change that.

Introduce a DMA management library to manage the various DMA resources
on the PA Semi SoCs. Since several drivers need to allocate these shared

m8xx_pic_init calls both mpc8xx_pic_init and cpm_pic_init. Renaming the
function to use the same name space as the rest of the mpc8xx
specific funtions and to be more meaningful.
m8xx_pic_init is declared in ppc8xx_pic.h but defined nowhere in the ppc
tree. Remove it.

Currently there is no way to disable the CPM2 support. Some boards,
like the SBC8560 have their own external UART and don't have any direct
dependencies on the CPM for a serial console or anything else.

Replace all lock_cpu_hotplug/unlock_cpu_hotplug from the kernel and use
get_online_cpus and put_online_cpus instead as it highlights the
refcount semantics in these operations.
The new API guarantees protection against the cpu-hotplug operation, but
it doesn't guarantee serialized access to any of the local data
structures. Hence the changes needs to be reviewed.
In case of pseries_add_processor/pseries_remove_processor, use
cpu_maps_update_begin()/cpu_maps_update_done() as we're modifying the
cpu_present_map there.

This patch extends the Ebony and Walnut platform code to instantiate
the existing ds1742 RTC class driver for the DS1743 RTC/NVRAM chip
found on both those boards. The patch uses a helper function to scan
the device tree and instantiate the appropriate platform_device based
on it, so it should be easy to extend for other boards which have mmio
mapped RTC chips.
Along with this, the device tree binding for the ds1743 chips is
tweaked, based on the existing DS1385 OF binding found at:
http://playground.sun.com/1275/proposals/Closed/Remanded/Accepted/346-it.txt
Although that document covers the NVRAM portion of the chip, whereas
here we're interested in the RTC portion, so it's not entirely clear
if that's a good model.
This implements only RTC class driver support - that is /dev/rtc0, not
/dev/rtc, and the low-level get/set time callbacks remain
unimplemented. That means in order to get at the clock you will
either need a modified version of hwclock which will look at
/dev/rtc0, or you'll need to configure udev to symlink rtc0 to rtc.

Commit 6d98bda79b changed the init order
for chrp_pci_fixup_vt8231_ata().
It can not work anymore because either the irq is not yet set to 14 or
pci_get_device() returns nothing. At least the printk() in
chrp_pci_fixup_vt8231_ata() does not trigger anymore.

PS3: Add repository polling loop to work around timing bug
On some firmware versions (e.g. 1.90), the storage device may not show up
in the repository immediately after receiving the notification message.
Add a small polling loop to make sure we don't miss it.

The PS3 hypervisor has a storage device notification mechanism to wait
until a storage device is ready. Unfortunately the storage device
probing code used this mechanism in an incorrect way, needing a
polling loop and handling of devices that are not yet ready.
This change corrects this by:
- First waiting for the reception of an asynchronous notification
that a new storage device became ready,
- Then looking up the storage device in the device repository.
On shutdown, the storage probe thread is stopped and the storage
notification device is closed using a reboot notifier.

All kobjects require a dynamically allocated name now. We no longer
need to keep track if the name is statically assigned, we can just
unconditionally free() all kobject names on cleanup.

Dynamically create the kset instead of declaring it statically. We also
rename power_subsys to power_kset to catch all users of the variable and
we properly export it so that people don't have to guess that it really
is present in the system.
The pseries code is wierd, why is it createing /sys/power if CONFIG_PM
is disabled? Oh well, stupid big boxes ignoring config options...

We don't need a "default" ktype for a kset. We should set this
explicitly every time for each kset. This change is needed so that we
can make ksets dynamic, and cleans up one of the odd, undocumented
assumption that the kset/kobject/ktype model has.

Define the layout of a binary blob that contains a QE firmware and instructions
on how to upload it. Add function qe_upload_firmware() to parse the blob
and perform the actual upload. Fully define 'struct rsp' in immap_qe.h to
include the actual RISC Special Registers. Added description of a new
QE firmware node to booting-without-of.txt.

This patch adds support for 'mpc5200-simple-platform' compatible
boards which do not need a platform specific setup. Such boards
are supported assuming the following:
- GPIO pins are configured by the firmware,
- CDM configuration (clocking) is setup correctly by firmware,
- if the 'fsl,has-wdt' property is present in one of the
gpt nodes, then it is safe to use such gpt to reset the board,
- PCI is supported if enabled in the kernel configuration
and if there is a PCI bus node defined in the device tree.

xlate_iomm_address() really wants the ds_addr to pass to the HV, so store
that value (instead of the BAR number) when we allocate the device bars.
This is not a fast path, so we can look up the device_node property
there instead of using the bussubno field of the pci_dn.
The other user of iseries_ds_addr() was already scanning the device tree,
so looking up a property will not slow it down any more.

Similar to of_find_compatible_node(), of_find_matching_node() and
for_each_matching_node() allow you to iterate over the device tree
looking for specific nodes, except that they take of_device_id
tables instead of strings.
This also moves of_match_node() from driver/of/device.c to
driver/of/base.c to colocate it with the of_find_matching_node which
depends on it.

Commit 473980a993 added a call to clear
the SLB shadow buffer before registering it. Unfortunately this means
that we clear out the entries that slb_initialize has previously set in
there. On POWER6, the hypervisor uses the SLB shadow buffer when doing
partition switches, and that means that after the next partition switch,
each non-boot CPU has no SLB entries to map the kernel text and data,
which causes it to crash.
This fixes it by reverting most of 473980a9 and instead clearing the
3rd entry explicitly in slb_initialize. This fixes the problem that
473980a9 was trying to solve, but without breaking POWER6.

Before we register the SLB shadow buffer, we need to invalidate the
entries in the buffer, otherwise we can end up stale entries from when
we previously offlined the CPU.
This does this invalidate as well as unregistering the buffer with
PHYP before we offline the cpu. Tested and fixes crashes seen on
970MP (thanks to tonyb) and POWER5.

The logic that checks to see if a machine check is caused by an NMI will
always match when NMI hasn't been initialized, since the mpic routine
will return NO_IRQ (and that's what the nmi_virq value is as well).

Commit aed3a8c9bb introduced a
definition of notify_spus_active in .../cell/spu_syscalls.c, and
another definition under #ifndef MODULE in .../cell/spufs/sched.c.
The latter is not necessary and causes the build to fail when
CONFIG_SPU_FS=y, so this removes it. It also removes the export
of do_notify_spus_active, which is unnecessary.

By default the OpenPIC on PWRficient will bias to one core (since that
will improve changes of the other core being able to stay idle/powered
down). However, this conflicts with most irq load balancing schemes,
since setting an interrupt to be delivered to either core doesn't really
result in the load being shared. It also doesn't work well with the
soft irq disable feature of PPC, since EE will stay on until the first
interrupt is taken while soft disabled.
Set the gconf0 config bit that enables even distribution of interrupts
among the two cores.

Some PWRficient-based boards have a NMI button that's wired up to a GPIO
as interrupt source. By configuring the openpic accordingly, these get
delivered as a machine check with high priority, instead of as an external
interrupt.
The device tree contains a property "nmi-source" in the openpic node
for these systems, and it's the (hwirq) source for the input.
Also, for these interrupts, the IACK is read from another register than
the regular (MCACK instead), but they are EOI'd as usual. So implement
said function for the mpic driver.
Finally, move a couple of external function defines to include/ instead
of local under sysdev. Being able to mask/unmask and eoi directly saves
us from setting up a dummy irq handler that will never be called.

Fix a bug in the printing of the os-area magic numbers which assumed
that magic numbers were zero terminated strings. The magic numbers
are represented in memory as integers. If the os-area sections are
not initialized correctly they could contained random data that would
be printed to the display. Also unify the handling of header and db
magic numbers and make both of type array of u8.

This removes an OProfile dependency on the spufs module. This
dependency was causing a problem for multiplatform systems that are
built with support for Oprofile on Cell but try to load the oprofile
module on a non-Cell system.

This makes 4xx embedded platforms re-assign all PCI resources as we
pretty much never care about what the various firmwares have done on
these, it's generally not compatible with the way the kernel will map
the bridges.
We still need to also enable bus renumbering on some of them, but I
will do that from a separate patch after I've fixed 4xx PCIe to handle
all bus numbers.

Brings EP405 support to arch/powerpc. The IRQ routing for the CPLD
comes from a device-tree property, PCI is working to the point where
I can see the video card, USB device, and south bridge.
This should work with both EP405 and EP405PC.
I've not totally figured out how IRQs are wired on this hardware
though, thus at this stage, expect only USB interrupts working,
pretty much the same as what arch/ppc did.
Also, the flash, nvram, rtc and temp control still have to be wired.

This adds a cputable function pointer for the CPU-side machine
check handling. The semantic is still the same as the old one,
the one in ppc_md. overrides the one in cputable, though
ultimately we'll want to change that so the CPU gets first.
This removes CONFIG_440A which was a problem for multiplatform
kernels and instead fixes up the IVOR at runtime from a setup_cpu
function. The "A" version of the machine check also tweaks the
regs->trap value to differenciate the 2 versions at the C level.

Maple and pasemi both require PCI as does CONFIG_OF_PLATFORM_PCI.
The default setting of CONFIG_ISA_DMA_API is set to match the protection
around the relevant routines in asm/dma.h.
I also had to remove the PMAC platform from the combined build. The
precis is that to build a 64 bit kernel with no PCI, you can only include
pSeries and iSeries.

This fixes the behavior of spufs when a spu tries a DMA operation
based on a wrong / unavailable address.
Instead of just generating a SIGBUS signal, spufs now
generates a SIGSEGV signal and restarts the problematic DMA operation
after the execution of the application's signal handler. This allows
applications to employ user-level paging systems.
Although the restart_dma function is called before the application's
signal handler, the operation is not actually performed at this time,
since the spu context is already stopped. The operation only takes
place when spu_run is restarted (which happens automatically).

The original spusched_timer was designed to take effect only when
a context is waiting in the runqueue.
This change adds an additional lower-freq timer has been added to
purely handle the spu_load updates. The new timer will be triggered
per LOAD_FREQ ticks.

Make most places that use spu_acquire/spu_acquire_saved interruptible,
this allows getting out of the spufs code when e.g. pressing ctrl+c.
There are a few places where we get called e.g. from spufs teardown
routines were we can't simply err out so these are left with a comment.
For now I've also not touched the poll routines because it's open what
libspe would expect in terms of interrupted system calls.

handlers returning errors, which is required to make the state_mutex
interruptible. This adds a local copy that allows for an error
return from the get/set handlers.

This cleans up spu_run_init so that it does all of the spu
initialization for spufs_run_spu. It initializes the spu context as
much as possible before it activates the spu and writes the runcntl
register.

Currently, spu contexts need to be loaded to the SPU in order to take
class 0 and class 1 exceptions.
This change makes the actual interrupt-handlers much simpler (ie,
set the exception information in the context save area), and defers the
handling code to the spufs_handle_class[01] functions, called from
spufs_run_spu.
This should improve the concurrency of the spu scheduling leading to
greater SPU utilization when SPUs are overcommited.

Add a few #defines for the class 0, 1 and 2 interrupt status bits, and
use them instead of magic numbers when we're setting or checking for
these interrupts.

When doing a poll on the mbox stat file of a swapped-out context, we
clear the class 0 interrupt status, rather than the class 2 interrupt
status.
This change corrects the poll operation to clear the correct interrupt.

This change disables the logic that faults-in spu contexts under the
covers from the page fault handler. When a fault requires a runnable
context, the handler will block until the context is scheduled by
other means.

Currently, part of the spufs code (switch.o, lscsa_alloc.o and fault.o)
is compiled directly into the kernel.
This change moves these components of spufs into the kernel.
The lscsa and switch objects are fairly straightforward to move in.
For the fault.o module, we split the fault-handling code into two
parts: a/p/p/c/spu_fault.c and a/p/p/c/spufs/fault.c. The former is for
the in-kernel spu_handle_mm_fault function, and we move the rest of the
fault-handling code into spufs.

Add platform specific SPU run control routines to the spufs. The current
spufs implementation uses the SPU master run control bit (MFC_SR1[S]) to
control SPE execution, but the PS3 hypervisor does not support the use of
this feature.
This change adds the run control wrapper routies spu_enable_spu() and
spu_disable_spu(). The bare metal routines use the master run control
bit, and the PS3 specific routines use the priv2 run control register.
An outstanding enhancement for the PS3 would be to add a guard to check
for incorrect access to the spu problem state when the spu context is
disabled. This check could be implemented with a flag added to the spu
context that would inhibit mapping problem state pages, and a routine
to unmap spu problem state pages. When the spu is enabled with
ps3_enable_spu() the flag would be set allowing pages to be mapped,
and when the spu is disabled with ps3_disable_spu() the flag would be
cleared and mapped problem state pages would be unmapped.

The device node for the HT bridge on G5s doesn't contain useful ranges.
We used to give it a bunch of the known PCI space and then punch a "hole"
in it based on where the AGP or PCIe region was. This reworks it to
use the actual register in the bridge that controls the decoding instead.

Powermac's use of the pcibios_enable_device_hook() got slightly
broken by the recent PCI merge in that it won't be called for
the "initial" case of assigning resources to a previously
unassigned device. This was an abuse of that hook anyway, so
instead we now use a header quirk.
While at it, we move a #ifdef CONFIG_PPC32 to enclose more code
that is only ever used on 32 bits.

The way iSeries manages PCI IO and Memory resources is a bit strange
and is based on overriding the content of those resources with home
cooked ones afterward.
This changes it a bit to better integrate with the new resource handling
so that the "virtual" tokens that iSeries replaces resources with are
done from the proper per-device fixup hook, and bridge resources are
set to enclose that token space. This fixes various things such as
the output of /proc/iomem & ioports, among others. This also fixes up
various boot messages as well.

The 32 bits PCI code now uses the generic code for assigning unassigned
resources and an algorithm similar to x86 for claiming existing ones.
This works far better than the 64 bits code which basically can only
claim existing ones (pci_probe_only=1) or would fall apart completely.
This merges them so that the new 32 bits implementation is used for both.
64 bits now gets the new PCI flags for controlling the behaviour, though
the old pci_probe_only global is still there for now to be cleared if you
want to.
I kept a pcibios_claim_one_bus() function mostly based on the old 64
bits code for use by the DLPAR hotplug. This will have to be cleaned
up, thought I hope it will work in the meantime.

The PCI code in 32 and 64 bits fixes up resources differently.
32 bits uses a header quirk plus handles bridges in pcibios_fixup_bus()
while 64 bits does things in various places depending on whether you
are using OF probing, using PCI hotplug, etc...
This merges those by basically using the 32 bits approach for both,
with various tweaks to make 64 bits work with the new approach.

This adds to the 32 bits PCI code some flags, replacing the old
pci_assign_all_busses global, that allow us to control various
aspects of the PCI probing, such as whether to re-assign all
resources or not, or to not try to assign anything at all.
This also adds the flag x86 already has to avoid ISA alignment
on bridges that don't have ISA forwarding enabled (no legacy
devices on the top level bus) and sets it for PowerMacs.

The per-processor interrupt request register and current processor
priority register are only accessed on the current cpu. In fact the
hypervisor doesn't even let us choose which cpu's registers to access.
The only function to use cpu twice is xics_migrate_irqs_away, not a fast
path. But we can cache the result of get_hard_processor_id() instead of
calling get_hard_smp_processor_id(cpu) in a loop across the call to rtas.
Years ago the irq code passed smp_processor_id into get_irq, I thought
we might initialize the CPPR third party at boot as an extra measure of
saftey, and it made the code symmetric with the qirr (queued interrupt
for software generated interrupts), but now it is just extra and
sometimes unneeded work to pass it down.

Implement MSI support for PA Semi PWRficient platforms. MSI is done
through a special range of sources on the openpic controller, and they're
unfortunately breaking the usual concepts of how sources are programmed:
* The source is calculated as 512 + the value written into the MSI
register
* The vector for this source is added to the source and reported
through IACK
This means that for simplicity, it makes much more sense to just set the
vector to 0 for the source, since that's really the vector we expect to
see from IACK.
Also, the affinity/priority registers will affect 16 sources at a
time. To avoid most (simple) users from being limited by this, allocate
16 sources per device but use only one. This means that there's a total
of 32 sources.
If we get usage scenarions that need more sources, the allocator should
probably be revised to take an alignment argument and size, not just do
natural alignment.
Finally, since I'm already touching the MPIC names on pasemi, rename
the base one from the somewhat odd " PAS-OPIC " to "PASEMI-OPIC".

Currently, we have a possibilty that the SLBs setup during context
switch don't cover the entirety of the necessary lscsa and code
regions, if these regions cross a segment boundary.
This change checks the start and end of each region, and inserts a SLB
entry for each, if unique. We also remove the assumption that the
spu_save_code and spu_restore_code reside in the same segment, by using
the specific code array for save and restore.

Currently, the SPU context switch code (spufs/switch.c) sets up the
SPU's SLBs directly, which requires some low-level mm stuff.
This change moves the kernel SLB setup to spu_base.c, by exposing
a function spu_setup_kernel_slbs() to do this setup. This allows us
to remove the low-level mm code from switch.c, making it possible
to later move switch.c to the spufs module.
Also, add a struct spu_slb for the cases where we need to deal with
SLB entries.

vicinity property in spe device nodes.
The new implementation does not depend on having an initialized
cbe_spu_info[0].spus, and checks for presence of vicinity in all

The pm_interval register in the Cell PMU is read/write, but was implemented in
the kernel as write-only. Previously, the written value was saved in a "shadow"
copy so calls to cbe_read_pm() could return the value.
Perfmon2 needs to be able to read the current values of pm_interval, so change
cbe_read_pm() to read the actual register instead of the "shadow" copy. There
is currently no code in the kernel that tries to read the pm_interval register
with cbe_read_pm() (expecting to receive the "shadow" value), so this should
not break any existing code.

Currently it's impossible to build a ps3_defconfig which will reboot
without modules installed. This makes it all too easy to find yourself
with a PS3 that won't reboot.
This is because the system manager driver, which provides the reboot
mechanism, is only selectable if PS3_ADVANCED is set, else it defaults
to m. In ps3_defconfig PS3_ADVANCED is not set, therefore the system
manager is built as a module.
It would be desirable IMHO for the defconfig to produce a kernel that
boots and reboots, without needing modules to be installed.

Printk was observed to hang during module unload due to a limited
window of characters that may be sent to the hypervisor. The window
only reexpands when we receive an ack from the HV and the spinlock here
prevents us from ever processing that ack. This fixes it by dropping
the lock before doing the printk, then looping back to the top to
reacquire the lock.

Currently we hardwire the number of SLBs to 64, but PAPR says we
should use the ibm,slb-size property to obtain the number of SLB
entries. This uses this property instead of assuming 64. If no
property is found, we assume 64 entries as before.
This soft patches the SLB handler, so it shouldn't change performance
at all.

PowerMac and CHRP/BriQ platforms have quirks to switch some IDE
controllers from legacy mode to fully native mode. Those quirks
however will not work properly anymore due to a change to the
generic code to better handle legacy IDE resources.
This fixes it by moving those quirk to "early" quirks (so they
run before resources are probed for the devices) and clearing
all BARs after the conversion to force a reallocation of sane
values.

Do not wait for the pci slot status before reporting an error
to the device driver. Some systems may take many seconds to
report the slot status, and this can confuse unsuspecting

This cleans up the SMT thread handling, removing some hard coded
assumptions and providing a set of helpers to convert between linux
cpu numbers, thread numbers and cores.
This implementation requires the number of threads per core to be a
power of 2 and identical on all cores in the system, but it's an
implementation detail, not an API requirement and so this limitation
can be lifted in the future if anybody ever needs it.

It turns out that this change caused some machines to fail to come
back up when being rebooted, and generated an error in the hypervisor
error log on some machines. The platform architecture (PAPR) is a
little unclear on exactly when the RTAS ibm,os-term function should be
called. Until that is clarified I'm reverting this commit.

Use smp_send_stop() to move all cpus besides the one executing reboot
into a hold loop, to keep them from being in powersavings mode at the
time of reboot.

When the PWRficient cpus are entered into powersavings states, the
astate is automatically dropped down to 0. While we still restore it
when we come out of idle, it can still cause some weird effects with
respect to performance (especially since it takes a while to ramp up to
higher astates).
So, to avoid this, don't enter power savings when the cpufreq driver
(or user) has set higher astates than 0.
The restore is still required, since there's a chance the astate has
been raised from the other cpu while the idling one was asleep.

There will be more product numbers in the future than just PA6T-1682M,

references and compatibility checks with 1682M, and replace most of them
with the more generic term "PWRficient".

We can currently cause an oops by repeatedly creating and destroying
contexts, while doing getdents() calls on the "/spu" directory.
This is due to the context's top-level dentry remaining hashed while
the context is being destroyed.
Fix this by unhashing the context's dentry with the
dentry->d_inode->i_mutex held. This way, we'll hit the check for
d_unhashed in dentry_readdir, and won't be included in the
list of subdirs for /spu.

The rtas_os_term() routine was being called at the wrong time.
The actual rtas call "os-term" will not ever return, and so
calling it from the panic notifier is too early. Instead,
call it from the machine_reset() call.
This splits the rtas_os_term() routine into two: one part to capture
the kernel panic message, invoked during the panic notifier, and
another part that is invoked during machine_reset().
Prior to this patch, the os-term call was never being made,
because panic_timeout was always non-zero. Calling os-term
helps keep the hypervisor happy! We have to keep the hypervisor
happy to avoid service, dump and error reporting problems.

currently the board-level PHY reset code for the mpc832x MDS messes with
reset configuration words source settings which is plain wrong (it
looks like this board code was cut-n-pasted from the mpc8360 mds code,
which has the PHY reset bits in a different BCSR); this patch points
the PHY reset code to the proper mpc832x mds PHY reset bits in the BCSR.

There are plans afoot to use pci_restore_msi_state() to restore MSI
state after a device reset. In order for this to work for the RTAS MSI
backend, we need to read back the MSI message from config space after
it has been setup by firmware.
This should be sufficient for restoring the MSI state after a device
reset, however we will need to revisit this for suspend to disk if that
is ever implemented on pseries.

This patch enables the ibm_newemac driver for the Walnut board. It fixes the
device tree for the walnut board to order the MAL interrupts correctly and
adds the local-mac-address property to the EMAC node. The bootwrapper is also
updated to extract the MAC address from the OpenBIOS offset where it is stored.

Currently there's no way to enable early boot console on PowerPC 44x
not specifying uart's physical address in kernel config, which is used
for very early debug messages. This patch splits very early debug output
(which needs uart physical address in kernel config) and early boot console
(which searches for uarts in the device tree using find_legacy_serial_ports).
We enable early boot console for all 44x processors, while (dangerous)
early debug is user-selectable.

This adds RGMII support to Sequoia DTS and sets correct phy-mode
for EMACs. According to Sequoia datasheet, both ethernet ports
are connected to RGMII interface, while ZMII is used only for MDIO.

is_init() is an ambiguous name for the pid==1 check. Split it into
is_global_init() and is_container_init().
A cgroup init has it's tsk->pid == 1.
A global init also has it's tsk->pid == 1 and it's active pid namespace
is the init_pid_ns. But rather than check the active pid namespace,
compare the task structure with 'init_pid_ns.child_reaper', which is
initialized during boot to the /sbin/init process and never changes.

There is no reason why the .prepare() and .finish() methods in 'struct
platform_suspend_ops' should take any arguments, since architectures don't use
these methods' argument in any practically meaningful way (ie. either the
target system sleep state is conveyed to the platform by .set_target(), or
there is only one suspend state supported and it is indicated to the PM core
by .valid(), or .prepare() and .finish() aren't defined at all). There also
is no reason why .finish() should return any result.

The name of 'struct pm_ops' suggests that it is related to the power
management in general, but in fact it is only related to suspend. Moreover,
its name should indicate what this structure is used for, so it seems
reasonable to change it to 'struct platform_suspend_ops'. In that case, the
name of the global variable of this type used by the PM core and the names of
related functions should be changed accordingly.

Move the definition of 'struct pm_ops' and related functions from <linux/pm.h>
to <linux/suspend.h> .
There are, at least, the following reasons to do that:
* 'struct pm_ops' is specifically related to suspend and not to the power
management in general.
* As long as 'struct pm_ops' is defined in <linux/pm.h>, any modification of it
causes the entire kernel to be recompiled, which is unnecessary and annoying.
* Some suspend-related features are already defined in <linux/suspend.h>, so it
is logical to move the definition of 'struct pm_ops' into there.
* 'struct hibernation_ops', being the hibernation-related counterpart of
'struct pm_ops', is defined in <linux/suspend.h> .

Driver for the CompactFlash slot on the PA Semi Electra eval board. It's
a simple device sitting on localbus, with interrupts and detect/voltage
control over GPIO.
The driver is implemented as an of_platform driver, and adds localbus
as a bus being probed by the of_platform framework.

This patch adds support for the core of the BestComm API
for the Freescale MPC5200(b). The BestComm engine is a
microcode-controlled / tasks-based DMA used by several
of the onchip devices.
Setting up the tasks / memory allocation and all common
low level functions are handled by this patch.
The specifics details of each tasks and their microcode
are split-out in separate patches.
This is not the official API, but a much cleaner one.

Fix up conflicts in arch/um/sys-{x86_64,i386}/Makefile manually.
Convert cpu_sibling_map from a static array sized by NR_CPUS to a per_cpu
variable. This saves sizeof(cpumask_t) * NR unused cpus. Access is mostly
from startup and CPU HOTPLUG functions.

Now that all users of dcr_read()/dcr_write() add the dcr_host_t.base, we
can save them the trouble and do it in dcr_read()/dcr_write().
As some background to why we just went through all this jiggery-pokery,
benh sayeth:
Initially the goal of the dcr_read/dcr_write routines was to operate like
mfdcr/mtdcr which take absolute DCR numbers. The reason is that on 4xx
hardware, indirect DCR access is a pain (goes through a table of
instructions) and it's useful to have the compiler resolve an absolute DCR
inline.
We decided that wasn't worth the API bastardisation since most places
where absolute DCR values are used are low level 4xx-only code which may
as well continue using mfdcr/mtdcr, while the new API is designed for
device "instances" that can exist on 4xx and Axon type platforms and may

This changes the uevent buffer functions to use a struct instead of a
long list of parameters. It does no longer require the caller to do the
proper buffer termination and size accounting, which is currently wrong
in some places. It fixes a known bug where parts of the uevent
environment are overwritten because of wrong index calculations.

Fixed up conflict in Documentation/powerpc/booting-without-of.txt manually.
This makes the kernel use 1TB segments for all kernel mappings and for
user addresses of 1TB and above, on machines which support them
(currently POWER5+, POWER6 and PA6T).
We detect that the machine supports 1TB segments by looking at the
ibm,processor-segment-sizes property in the device tree.
We don't currently use 1TB segments for user addresses < 1T, since
that would effectively prevent 32-bit processes from using huge pages
unless we also had a way to revert to using 256MB segments. That
would be possible but would involve extra complications (such as
keeping track of which segment size was used when HPTEs were inserted)
and is not addressed here.

asm-powerpc/mpc85xx.h was really a hold over from arch/ppc. Now that
more decoupling has occurred we can remove <asm/mpc85xx.h> and some of
its legacy.
As part of this we moved the definition of CPM_MAP_ADDR into cpm2.h
for 85xx platforms. This is a stop gap until drivers stop using
CPM_MAP_ADDR.

Don't allow cpu hotplug on systems lacking XICS interrupt controller
(i.e. with an MPIC interrupt controller), since the current pSeries
platform code is hardcoded for XICS.
This works around the bug reported by Paul Mackerras where the
disable_nonboot_cpus() call recently added to the shutdown path will
cause an oops on older pSeries machines.

pasemi_mac: enable iommu support
Enable IOMMU support for pasemi_mac, but avoid using it on non-partitioned
systems for performance reasons.
The user can override this by selecting the PPC_PASEMI_IOMMU_DMA_FORCE
configuration option.

Based on BenH's earlier work, this is a new version of the EMAC driver
for the built-in ethernet found on PowerPC 4xx embedded CPUs. The
same ASIC is also found in the Axon bridge chip. This new version is
designed to work in the arch/powerpc tree, using the device tree to
probe the device, rather than the old and ugly arch/ppc OCP layer.
This driver is designed to sit alongside the old driver (that lies in
drivers/net/ibm_emac and this one in drivers/net/ibm_newemac). The
old driver is left in place to support arch/ppc until arch/ppc itself
reaches its final demise (not too long now, with luck).
This driver still has a number of things that could do with cleaning
up, but I think they can be fixed up after merging. Specifically:
- Should be adjusted to properly use the dma mapping API.
Axon needs this.
- Probe logic needs reworking, in conjuction with the general
probing code for of_platform devices. The dependencies here between
EMAC, MAL, ZMII etc. make this complicated. At present, it usually
works, because we initialize and register the sub-drivers before the
EMAC driver itself, and (being in driver code) runs after the devices
themselves have been instantiated from the device tree.

The Lite5200 u-boot image doesn't entirely configure the processor
correctly and so Linux needs to fixup the cpu setup in setup_arch. Fixing
the CPU setup is good, but making it into common code is not a good idea.
New board ports should be encouraged not to take the lead of the lite5200
and instead get their firmware to setup the CPU the right way.

Add support for a simple tagged database in the PS3 flash rom
os-area. The database allows the flash rom os-area to be shared
between a bootloader and installed operating systems. The
application ps3-flash-util or the library libps3-utils from the
ps3-utils package can be used for userspace database operations.

path tricks to get to it, it's time to move it to asm-powerpc. While
at it, rename it to be consistent with cell-pmu.h (and dashes look

Since bootdevice never gets initialized, it's always NULL, and hence a
whole pile of code in arch/powerpc/platforms/setup.c never gets used.
(This was the code that originally was there so that the automatic
root partition selection mechanism would prefer a rootish-looking
partition on the device that OF loaded the kernel from over a similar
partition on other devices.)
This removes the unused code.

set_irq_chained_handler overwrites MPIC's handle_irq function
(handle_fasteoi_irq) thus MPIC never gets eoi event from the
cascaded IRQ. This situation hangs MPIC on MPC8568E.
To solve this problem efficiently, QEIC needs pluggable handlers,
specific to the underlaying interrupt controller.
Patch extends qe_ic_init() function to accept low and high interrupt
handlers. To avoid #ifdefs, stack of interrupt handlers specified in
the header file and functions are marked 'static inline', thus
handlers are compiled-in only if actually used (in the board file).
Another option would be to lookup for parent controller and
automatically detect handlers (will waste text size because of
never used handlers, so this option abolished).
qe_ic_init() also changed in regard to support multiplexed high/low
lines as found in MPC8568E-MDS, plus qe_ic_cascade_muxed_mpic()
handler implemented appropriately.

On the majority of 85xx & 86xx we have a register that's ability to
assert HRESET_REQ to reset the board. We refactored that code so it
can be shared between both platforms into fsl_soc.c and removed all
the duplication in each platform directory.

On the Freescale embedded (83xx, 85xx, 86xx) and a few of the discrete
bridges (mpc10x, tsi108) use the new for_each_compatible_node() or
for_each_node_by_type() to provide more exact matching when looking for
PHBs in the device tree.
With the previous code it was possible to match on pci bridges since
we were only matching on device_type.

1. PCI and reset are factored out into pq2.c. I renamed them from m82xx
to pq2 because they won't work on the Integrated Host Processor line of
82xx chips (i.e. 8240, 8245, and such).
2. The PCI PIC, which is nominally board-specific, is used on multiple
boards, and thus is used into pq2ads-pci-pic.c.
3. The new CPM binding is used.
4. General cleanup.

The CPU15 erratum on MPC8xx chips can cause incorrect code execution
under certain circumstances, where there is a conditional or indirect
branch in the last word of a page, with a target in the last cache line
of the next page. This patch implements one of the suggested
workarounds, by forcing a TLB miss whenever execution crosses a page
boundary. This is done by invalidating the pages before and after the
one being loaded into the TLB in the ITLB miss handler.

1. Keep a global mpc8xx_immr mapping, rather than constantly
creating temporary mappings.
2. Look for new fsl,cpm1 and fsl,cpm1-pic names.

this is required in case the firmware initialized a device

This introduces a new device binding for the CPM and other devices on
these boards. Some of the changes include:
1. Proper namespace scoping for Freescale compatibles and properties.
2. Use compatible rather than things like device_type and model
to determine which particular variant of a device is present.
3. Give the drivers the relevant CPM command word directly, rather than
requiring it to have a lookup table based on device-id, SCC v. SMC, and
CPM version.
4. Specify the CPCR and the usable DPRAM region in the CPM's reg property.
Boards that do not require the legacy bindings should select
CONFIG_PPC_CPM_NEW_BINDING to enable the of_platform CPM devices. Once
all existing boards are converted and tested, the config option can
become default y to prevent new boards from using the old model. Once
arch/ppc is gone, the config option can be removed altogether.

Some older pSeries machines were panicking in pSeries_log_error
because it was getting called before it was ready. This is a result
of commit "[POWERPC] pseries: Fix jumbled no_logging flag."
(79c0108d1b).
This fixes it by explicitly enabling RTAS error logging when it has
been initialized, and also makes the code clearer by renaming the
"no_more_logging" variable to "logging_enabled".

This supports Power/Reset buttons on Beat on Celleb.
On Beat, we have an event from Beat if Power button or Reset button
is pressed. This patch catches the event and convert it to a signal
to INIT process by calling ctrl_alt_del() function.
/sbin/inittab have no entry to turn the machine power off so we have
to detect if power button is pressed or not internally in our driver.
This idea is taken from PS3's event handling subsystem.

This provides an implementation of the <linux/clk.h> interface for
arch/powerpc using a set of function pointers in clk_functions.
Platforms that want to support this interface should fill
clk_functions and select CONFIG_PPC_CLOCK in Kconfig.

Currently rtas_change_msi() returns either the error code from RTAS, or if
the RTAS call succeeded the number of irqs that were configured by RTAS.
This makes checking the return value more complicated than it needs to be.
Instead, have rtas_change_msi() check that the number of irqs configured by
RTAS is equal to what we requested - and return an error otherwise. This makes
the return semantics match the usual 0 for success, something else for error.

rtas_setup_msi_irqs() doesn't need to call teardown() itself, the
generic code will do this for us as long as we return a non-zero
value.

The various embedded 6xx systems can easily coexist in one kernel
together with the other 6xx based systems, so there is no strict
reason to keep them separate.

It seems that some versions of firmware will report a device
node status as the string "okay". As we are not expecting this
string, the device node will be ignored by the EEH subsystem.
Which means EEH will not be enabled.
When EEH is not enabled, PCI errors will be converted into
Machine Check exceptions, and we'll have a very unhappy system.

On a POWER6 machine running 2.6.23-rc8 I sometimes see the following error:
xics_set_affinity: No online cpus in the mask 00000000,00000000,00000000,00000001 for irq 20
In a desperate attempt to get a changelog entry in 2.6.23, I took a look
into it.
It turns out we are passing a real and not a virtual irq into
get_irq_server. This works for the case where hwirq < NR_IRQS and we
set virq = hwirq. In my case however hwirq = 590082 and we try and
access irq_desc[590082], slightly past the end at 512 entries.
Lucky we ship lots of memory with our machines.

The commit 8b6f50ef1d seems to have
been affected by a mismerge of a duplicate patch
(d054b36ffd) - both the
spufs_dir_contents and spufs_dir_nosched_contents have been given
write-only signal notification files.
This change reverts the spufs_dir_contents array to use the
readable signal notification file implementation.

Since the PPE on cell is an in-order core, it suffers significantly
from wrong instruction scheduling. This adds a Kconfig option that
enables passing -mtune=cell to gcc in order to generate object
code that runs well on cell.

Commit 69331af, "Fixes and cleanups for earlyprintk aka boot console",
resulted in printk output prior to the initialization of the mpsc
console driver not being printed. That commit causes the mpsc's
CON_PRINTBUFFER flag to be cleared since udbg should have printed
the previous output.
I guess we can no longer ignore udbg. :)
This patch provides udbg_putc() and udbg_getc() functions for the
Marvell mv64x60 chips. These functions are enabled if an mv64x60
port is to be used as the console as determined from the device tree.

find_victim can dereference a NULL pointer when iterating over the list
of victim spus because list_mutex only guarantees spu->ct to be stable,
but of course not to be non-NULL.
Also fix find_victim to not call spu_unbind_context without list_mutex
because that violates the above guarantee.
