This is used by TTM to communicate the physical address
which should be used with ioremap(), ioremap_wc(). We don't
need to separate the base and offset in any way here.

drivers to use, while also making the debug callback optional
(don't need to test for system as it won't init it).

The original intention was to avoid CPU page table unmaps
when BOs move between the GTT and SYSTEM domain.
The problem is that this never correctly handled changes
in the caching attributes or backing pages.
Just drop this for now and simply unmap the CPU page
tables in all cases.

Support huge (PMD-size and PUD-size) page-table entries by providing a
huge_fault() callback.
We still support private mappings and write-notify by splitting the huge
page-table entries on write-access.
Note that for huge page-faults to occur, either the kernel needs to be
compiled with trans-huge-pages always enabled, or the kernel needs to be
compiled with trans-huge-pages enabled using madvise, and the user-space
app needs to call madvise() to enable trans-huge pages on a per-mapping
basis.
Furthermore huge page-faults will not succeed unless buffer objects and
user-space addresses are aligned on huge page size boundaries.

This patch reworks the whole delayed deletion of BOs which aren't idle.
Instead of having two counters for the BO structure we resurrect the BO
when we find that a deleted BO is not idle yet.
This has many advantages, especially that we don't need to
increment/decrement the BOs reference counter any more when it
moves on the LRUs.

Graphics APIs like OpenGL 4.4 and Vulkan require the graphics driver
to provide coherent graphics memory, meaning that the GPU sees any
content written to the coherent memory on the next GPU operation that
touches that memory, and the CPU sees any content written by the GPU
to that memory immediately after any fence object trailing the GPU
operation is signaled.
Paravirtual drivers that otherwise require explicit synchronization
needs to do this by hooking up dirty tracking to pagefault handlers
and buffer object validation.
Provide mm helpers needed for this and that also allow for huge pmd-
and pud entries (patch 1-3), and the associated vmwgfx code (patch 4-7).
The code has been tested and exercised by a tailored version of mesa
where we disable all explicit synchronization and assume graphics memory
is coherent. The performance loss varies of course; a typical number is
around 5%.

Drivers like vmwgfx may want to test whether the dma page pool is present

The default TTM fault handler may not be completely sufficient
(vmwgfx needs to do some bookkeeping, control the write protectionand also
needs to restrict the number of prefaults).
Also make it possible replicate ttm_bo_vm_reserve() functionality for,
for example, mkwrite handlers.
So turn the TTM vm code into helpers: ttm_bo_vm_fault_reserved(),
ttm_bo_vm_open(), ttm_bo_vm_close() and ttm_bo_vm_reserve(). Also provide
a default TTM fault handler for other drivers to use.

With nouveau fixed all ttm-using drives have the correct nesting of

The default TTM fault handler may not be completely sufficient
(vmwgfx needs to do some bookkeeping, control the write protectionand also
needs to restrict the number of prefaults).
Also make it possible replicate ttm_bo_vm_reserve() functionality for,
for example, mkwrite handlers.
So turn the TTM vm code into helpers: ttm_bo_vm_fault_reserved(),
ttm_bo_vm_open(), ttm_bo_vm_close() and ttm_bo_vm_reserve(). Also provide
a default TTM fault handler for other drivers to use.

This allows blocking for BOs to become available
in the memory management.
Amdgpu is doing this for quite a while now during CS. Now
apply the new behavior to all drivers using TTM.

Rename ttm_fbdev_mmap to ttm_bo_mmap_obj. Move the vm_pgoff sanity
check to amdgpu_bo_fbdev_mmap (only ttm_fbdev_mmap user in tree).
The ttm_bo_mmap_obj function can now be used to map any buffer object.
This allows to implement &drm_gem_object_funcs.mmap in gem ttm helpers.

Rename the embedded struct vma_offset_manager, new name is _vma_manager.
ttm_bo_device.vma_manager changed to a pointer.
The ttm_bo_device_init() function gets an additional vma_manager
argument which allows to initialize ttm with a different vma manager.
When passing NULL the embedded _vma_manager is used.
All callers are updated to pass NULL, so the behavior doesn't change.

This notifies the driver that a BO is about to be released.
Releasing a BO also invokes the move_notify callback from
ttm_bo_cleanup_memtype_use, but that happens too late for anything
that would add fences to the BO and require a delayed delete.

With the vmwgfx dirty tracking, the default TTM fault handler is not
completely sufficient (vmwgfx need to modify the vma->vm_flags member,
and also needs to restrict the number of prefaults).
We also want to replicate the new ttm_bo_vm_reserve() functionality
So start turning the TTM vm code into helpers: ttm_bo_vm_fault_reserved()
and ttm_bo_vm_reserve(), and provide a default TTM fault handler for other
drivers to use.

Add a pointer to the struct vm_operations_struct in the bo_device, and
assign that pointer to the default value currently used.
The driver can then optionally modify that pointer and the new value
can be used for each new vma created.

We are already doing this for DMA-buf imports and also for
amdgpu VM BOs for quite a while now.

When a driver unloads without unloading TTM we don't correctly
clear the global structures leading to errors on re-init.
Next step should probably be to remove the global structures and
kobjs all together, but this is tricky since we need to maintain
backward compatibility.

Most TTM drivers define the constant DRM_FILE_PAGE_OFFSET of the same
value. The only exception is vboxvideo, which is being converted to the
new offset by this patch. Unifying the constants in a single place
simplifies the driver code.

So far, struct ttm_bo_global_ref was the only way of initializing a struct
ttm_bo_global. Providing separate initializer and release functions for
struct ttm_bo_global gives drivers the option of implementing their own
init and release callbacks for drm_global_references of type
DRM_GLOBAL_TTM_BO.
The original functions for initializing and releasing via struct
ttm_bo_global_ref are wrappers around the new interfaces.

This function allow us to bulk move a group of BOs to the tail of their LRU.
The positions of group of BOs are stored on the (first, last) bulk_move_pos
structure.

When move a BO to the end of LRU, it need remember the BO positions.
Make sure all moved bo in between "first" and "last". And they will be bulk
moving together.

The TTM buffer-object interface provides ttm_bo_reference() and
ttm_bo_unref() for managing reference counts. Replacing them with
ttm_bo_get() and ttm_bo_put() aligns the API with conventions used
throughout the Linux kernel.
The implementation of ttm_bo_unref() clears the supplied pointer
to NULL. This leads to workarounds where the caller saves the
pointer's value before de-referencing the BO. ttm_bo_put() does
not clear the supplied pointer.

the free mem space and the lower limit both include two parts:
system memory and swap space.
For the OOM triggered by TTM, that is the case as below:
first swap space is full of swapped out pages and soon
system memory also is filled up with ttm pages. and then
any memory allocation request will run into OOM.
to cover two cases:
a. if no swap disk at all or free swap space is under swap mem
limit but available system mem is bigger than sys mem limit,
allow TTM allocation;
b. if the available system mem is less than sys mem limit but
free swap space is bigger than swap mem limit, allow TTM
allocation.

Allows gdb to access contents of user mode mapped BOs. System memory
is handled by TTM using kmap. Other memory pools require a new driver
callback in ttm_bo_driver.

Previously, when a surface was opened using a legacy (non prime) handle,
it was verified to have been created by a client in the same master realm.
Relax this so that opening is also allowed recursively if the client
already has the surface open.
This works around a regression in svga mesa where opening of a shared
surface is used recursively to obtain surface information.

Ensure that the driver can listen to evictions even when they don't take the
path through ttm_bo_driver::move.
This is crucial for amdgpu, which relies on an eviction counter to skip

Patch 3d50d4dcb0 exposed the CPU address of DMA-allocated pages as
returned by dma_alloc_coherent because Nouveau on Tegra needed it.
This is not required anymore - as there were no other users for it,
remove it and save some memory for everyone.

It tries to do fancy things with excluding agp support if ttm is
built-in, but agp isn't. Instead just express this depency like drm
does and use CONFIG_AGP everywhere.
Also use the neat Makefile magic to make the entire ttm_agp_backend
file optional.

We need to store device offsets in 64 bit as the device
address space may be larger than the CPU's.
Fixes GPU init failures on radeons with 4GB or more of
vram on 32 bit kernels. We put vram at the start of the
GPU's address space so the gart aperture starts at 4 GB
causing all GPU addresses in the gart aperture to get
truncated.

Clients like i915 need to segregate cache domains within the GTT which
can lead to small amounts of fragmentation. By allocating the uncached
buffers from the bottom and the cacheable buffers from the top, we can
reduce the amount of wasted space and also optimize allocation of the
mappable portion of the GTT to only those buffers that require CPU
access through the GTT.
For other drivers, allocating small bos from one end and large ones
from the other helps improve the quality of fragmentation.

With dev->anon_inode we have a global address_space ready for operation
right from the beginning. Therefore, there is no need to do a delayed
setup with TTM. Instead, set dev_mapping during initialization in
ttm_bo_device_init() and remove any "if (dev_mapping)" conditions.

Anyway, nothing big here, Three more code cleanup patches from Rashika
Kheria, and one TTM/vmwgfx patch from me that tightens security around TTM
objects enough for them to opened using prime objects from render nodes:
Previously any client could access a shared buffer using the "name", also
without actually opening it. Now a reference is required, and for render nodes
such a reference is intended to only be obtainable using a prime fd.

When a client looks up a ttm object, don't look it up through the device hash
table, but rather from the file hash table. That makes sure that the client
has indeed put a reference on the object, or in gem terms, has opened
the object; either using prime or using the global "name".
To avoid a performance loss, make sure the file hash table entries can be
looked up from under an RCU lock, and as a consequence, replace the rwlock
with a spinlock, since we never need to take it in read mode only anymore.
Finally add a ttm object lookup function for the device hash table, that is
intended to be used when we put a ref object on a base object or, in gem terms,
when we open the object.

"[BUG] completely bonkers use of set_need_resched + VM_FAULT_NOPAGE".
In the first occurence it was used to try to be nice while releasing the
mmap_sem and retrying the fault to work around a locking inversion.
The second occurence was never used.
There has been some discussion whether we should change the locking order to
mmap_sem -> bo_reserve. This patch doesn't address that issue, and leaves
that locking order undefined. The solution that we release the mmap_sem if
tryreserve fails and wait for the buffer to become unreserved is something
we want in any case, and follows how the core vm system waits for pages
to be come unlocked while releasing the mmap_sem.
The code also outlines what needs to be changed if we want to establish the
locking order as mmap_sem -> bo::reserve.
One slight issue that remains with this code is that the fault handler might
be prone to starvation if another thread countinously reserves the buffer.
IMO that usage pattern is highly unlikely.

If no reservation ticket is given to the execbuf reservation utilities,
try reservation with non-blocking semantics.
This is intended for eviction paths that use the execbuf reservation
utilities for convenience rather than for deadlock avoidance.

Use the new vma-manager infrastructure. This doesn't change any
implementation details as the vma-offset-manager is nearly copied 1-to-1
from TTM.
The vm_lock is moved into the offset manager so we can drop it from TTM.
During lookup, we use the vma locking helpers to take a reference to the
found object.
In all other scenarios, locking stays the same as before. We always
guarantee that drm_vma_offset_remove() is called only during destruction.
Hence, helpers like drm_vma_node_offset_addr() are always safe as long as
the node has a valid offset.
This also drops the addr_space_offset member as it is a copy of vm_start
in vma_node objects. Use the accessor functions instead.

Now that the code is compatible in semantics, flip the switch.
Use ww_mutex instead of the homegrown implementation.
ww_mutex uses -EDEADLK to signal that the caller has to back off,
and -EALREADY to indicate this buffer is already held by the caller.
ttm used -EAGAIN and -EDEADLK for those, respectively. So some changes
were needed to handle this correctly.

and trying over, a better strategy would be to do a blocking wait.
This can be mapped a lot better to a mutex_lock-like call.

There should no longer be assumptions that reserve will always succeed
with the lru lock held, so we can safely break the whole atomic
reserve/lru thing. As a bonus this fixes most lockdep annotations for
reservations.

All items on the lru list are always reservable, so this is a stupid
thing to keep. Not only that, it is used in a way which would
guarantee deadlocks if it were ever to be set to block on reserve.
This is a lot of churn, but mostly because of the removal of the
argument which can be nested arbitrarily deeply in many places.
No change of code in this patch except removal of the no_wait_reserve
argument, the previous patch removed the use of no_wait_reserve.

The mostly used lookup+get put+potential_destroy path of TTM objects
is converted to use RCU locks. This will substantially decrease the amount
of locked bus cycles during normal operation.
Since we use kfree_rcu to free the objects, no rcu synchronization is needed
at module unload time.

This adds the ability for ttm common code to take an SG table
and use it as the backing for a slave TTM object.
The drivers can then populate their GTT tables using the SG object.

If a header file is making use of BUG, BUG_ON, BUILD_BUG_ON, or any
other BUG variant in a static inline (i.e. not in a #define) then
that header really should be including <linux/bug.h> and not just
expecting it to be implicitly present.
We can make this change risk-free, since if the files using these
headers didn't have exposure to linux/bug.h already, they would have
been causing compile failures/warnings.

ttm tt rework modified the way we allocate and populate the
ttm_tt structure, the AGP side was missing some bit to properly
work. Fix those and fix radeon and nouveau AGP support.

Provide helper function to compute the kernel memory size needed
for each buffer object. Move all the accounting inside ttm, simplifying
driver and avoiding code duplication accross them.

In TTM world the pages for the graphic drivers are kept in three different
pools: write combined, uncached, and cached (write-back). When the pages
are used by the graphic driver the graphic adapter via its built in MMU
(or AGP) programs these pages in. The programming requires the virtual address
(from the graphic adapter perspective) and the physical address (either System RAM
or the memory on the card) which is obtained using the pci_map_* calls (which does the
virtual to physical - or bus address translation). During the graphic application's
"life" those pages can be shuffled around, swapped out to disk, moved from the
VRAM to System RAM or vice-versa. This all works with the existing TTM pool code
- except when we want to use the software IOTLB (SWIOTLB) code to "map" the physical
addresses to the graphic adapter MMU. We end up programming the bounce buffer's
physical address instead of the TTM pool memory's and get a non-worky driver.
There are two solutions:
1) using the DMA API to allocate pages that are screened by the DMA API, or
2) using the pci_sync_* calls to copy the pages from the bounce-buffer and back.
This patch fixes the issue by allocating pages using the DMA API. The second
is a viable option - but it has performance drawbacks and potential correctness
issues - think of the write cache page being bounced (SWIOTLB->TTM), the
WC is set on the TTM page and the copy from SWIOTLB not making it to the TTM
page until the page has been recycled in the pool (and used by another application).
The bounce buffer does not get activated often - only in cases where we have
a 32-bit capable card and we want to use a page that is allocated above the
4GB limit. The bounce buffer offers the solution of copying the contents
of that 4GB page to an location below 4GB and then back when the operation has been
completed (or vice-versa). This is done by using the 'pci_sync_*' calls.
Note: If you look carefully enough in the existing TTM page pool code you will
notice the GFP_DMA32 flag is used - which should guarantee that the provided page
is under 4GB. It certainly is the case, except this gets ignored in two cases:
- If user specifies 'swiotlb=force' which bounces _every_ page.
- If user is using a Xen's PV Linux guest (which uses the SWIOTLB and the
underlaying PFN's aren't necessarily under 4GB).
To not have this extra copying done the other option is to allocate the pages
using the DMA API so that there is not need to map the page and perform the
expensive 'pci_sync_*' calls.
This DMA API capable TTM pool requires for this the 'struct device' to
properly call the DMA API. It also has to track the virtual and bus address of
the page being handed out in case it ends up being swapped out or de-allocated -
to make sure it is de-allocated using the proper's 'struct device'.
Implementation wise the code keeps two lists: one that is attached to the
'struct device' (via the dev->dma_pools list) and a global one to be used when
the 'struct device' is unavailable (think shrinker code). The global list can
iterate over all of the 'struct device' and its associated dma_pool. The list
in dev->dma_pools can only iterate the device's dma_pool.

The maximum amount of dma pools a device can have is six: write-combined,
uncached, and cached; then there are the DMA32 variants which are:
write-combined dma32, uncached dma32, and cached dma32.
Currently this code only gets activated when any variant of the SWIOTLB IOMMU
code is running (Intel without VT-d, AMD without GART, IBM Calgary and Xen PV
with PCI devices).

Move the page allocation and freeing to driver callback and
provide ttm code helper function for those.
Most intrusive change, is the fact that we now only fully
populate an object this simplify some of code designed around
the page fault design.

ttm_backend will only exist with a ttm_tt, and ttm_tt
will only be of interest when bound to a backend. Merge them
to avoid code and data duplication.

Split btw highmem and lowmem page was rendered useless by the
pool code. Remove it. Note further cleanup would change the
ttm page allocation helper to actualy take an array instead
of relying on list this could drasticly reduce the number of
function call in the common case of allocation whole buffer.
