18dad454cd bcachefs: Replace rcu_read_lock() with guards
The new guard(), scoped_guard() allow for more natural code.

Some of the uses with creative flow control have been left.

cca2c0d224 bcachefs: bch_dev.io_ref -> enumerated_ref
Convert device IO refs to enumerated_refs, for easier debugging of

040c762152 bcachefs: bch2_dev_missing_bkey()
Part of the ongoing project to kill off bch2_(fs|trans)_inconsistent
calls - they generally need to be replaced with either

- a fsck_err() call that can repair the error, or

- logging an error of the appropriate type in the superblock, and
  flagging the appropriate recovery pass to repair the error

dcffc3b1ae bcachefs: Split up bch_dev.io_ref
We now have separate per device io_refs for read and write access.

This fixes a device removal bug where the discard workers were still
running while we're removing alloc info for that device.

It's also a bit of hardening; we no longer allow writes to devices that
are read-only.

d71e023376 bcachefs: Change BCH_MEMBER_STATE_failed semantics
Previously, we woudn't try to read at all from a failed device - that
doesn't make much sense, the device may be unhealthy (perhaps taking
longer than it should to service reads), but if it's our only option we
should still try to read from it.

Now, bch2_bkey_pick_read_device() will pick failed devices only if there
are no non-failed replicas to read from.

cf164a9106 bcachefs: bch2_dev_get_ioref() may now sleep
The next patch implementing freezing will change bch2_dev_get_ioref() to
sleep if a device is currently frozen.

Add an annotation and fix the journal code accordingly.

f269ae55d2 bcachefs: Scrub
Add a new data op to walk all data and metadata in a filesystem,
checking if it can be read successfully, and on error repairing from
another copy if possible.

- New helper: bch2_dev_idx_is_online(), so that we can bail out and
  report to userspace when we're unable to scrub because the device is
  offline

- data_update_opts, which controls the data move path, now understands
  scrub: data is only read, not written. The read path is responsible
  for rewriting on read error, as with other reads.

- scrub_pred skips data extents that don't have checksums

- bch_ioctl_data has a new scrub member, which has a data_types field
  for data types to check - i.e. all data types, or only metadata.

- Add new entries to bch_move_stats so that we can report numbers for
  corrected and uncorrected errors

- Add a new enum to bch_ioctl_data_event for explicitly reporting
  completion and return code (i.e. device offline)

b07eb8252f bcachefs: bch2_dev_tryget()
Most uses of bch2_dev_bkey_exists() are going away, where we assume that
because a key references a device the device most exist - instead, we'll
be explicitly checking if the device exists and getting a reference to
it.

This adds the new helpers.

18b4abcead bcachefs: Fix lifetime issue in device iterator helpers
bch2_get_next_dev() and bch2_get_next_online_dev() iterate over devices,
dropping and taking refs as they go; we can't access the previous device
(for ca->dev_idx) after we've dropped our ref to it, unless we take
rcu_read_lock() first.

27c15ed297 bcachefs: bch_member.btree_allocated_bitmap
This adds a small (64 bit) per-device bitmap that tracks ranges that
have btree nodes, for accelerating btree node scan if it is ever needed.

- New helpers, bch2_dev_btree_bitmap_marked() and
  bch2_dev_bitmap_mark(), for checking and updating the bitmap

- Interior btree update path updates the bitmaps when required

- The check_allocations pass has a new fsck_err check,
  btree_bitmap_not_marked

- New on disk format version, mi_btree_mitmap, which indicates the new
  bitmap is present

- Upgrade table lists the required recovery pass and expected fsck error

- Btree node scan uses the bitmap to skip ranges if we're on the new
  version

f5d26fa31e bcachefs: bch_sb_field_errors
Add a new superblock section to keep counts of errors seen since
filesystem creation: we'll be addingcounters for every distinct fsck
error.

The new superblock section has entries of the for [ id, count,
time_of_last_error ]; this is intended to let us see what errors are
occuring - and getting fixed - via show-super output.

3f7b9713da bcachefs: New superblock section members_v2
members_v2 has dynamically resizable entries so that we can extend
bch_member. The members can no longer be accessed with simple array
indexing Instead members_v2_get is used to find a member's exact
location within the array and returns a copy of that member.
Alternatively member_v2_get_mut retrieves a mutable point to a member.
