The TUSB8044 is similar to the TUSB8041. This adds the PID/VID values
and allows to specify the reset GPIO signal on the board.

In some cases, there is a small-time gap in which CMD_RING_BUSY can be
cleared by controller but adding command completion event to event ring
will be delayed. As the result driver will return error code.
This behavior has been detected on usbtest driver (test 9) with
configuration including ep1in/ep1out bulk and ep2in/ep2out isoc
endpoint.
Probably this gap occurred because controller was busy with adding some
other events to event ring.
The CMD_RING_BUSY is cleared to '0' when the Command Descriptor has been
executed and not when command completion event has been added to event
ring.
To fix this issue for this test the small delay is sufficient less than
10us) but to make sure the problem doesn't happen again in the future
the patch introduces 10 retries to check with delay about 20us before
returning error code.

usb core avoids sending a Set-Interface altsetting 0 request after device
reset, and instead relies on calling usb_disable_interface() and
usb_enable_interface() to flush and reset host-side of those endpoints.
xHCI hosts allocate and set up endpoint ring buffers and host_ep->hcpriv
during usb_hcd_alloc_bandwidth() callback, which in this case is called
before flushing the endpoint in usb_disable_interface().
Call usb_disable_interface() before usb_hcd_alloc_bandwidth() to ensure
URBs are flushed before new ring buffers for the endpoints are allocated.
Otherwise host driver will attempt to find and remove old stale URBs
from a freshly allocated new ringbuffer.

The Qualcomm DWC3 glue driver duplicates the logic of the bulk clock
API to acquire, prepare, and unprepare the controller's clocks. It also
manages the life cycle of these handled explicitly.
Transition to the bulk clock API and manage the resources using devres,
to clean up the code. The resource acquisition is moved above the
initial reset pulse, to handle resource issues before the state is
touched - other than this, this no functional change.

Avoid unlocking serial_port_lock twice in gserial_suspend(), this can
occur if gserial_wakeup_host() fails. And since wakeup is performed
outside spinlock, check if the port is valid before proceeding again.

Initialize negotiated_rev and negotiated_rev_prime based on the port's
configured PD revision (rev_major) rather than always defaulting to
PD_MAX_REV. This ensures ports start PD communication using their
appropriate revision level.
This allows proper communication with devices that require specific
PD revision levels, especially for the hardware designed for PD 1.0
or 2.0 specifications.

A state check was previously added to tcpm_queue_vdm_unlocked to
prevent a deadlock where the DisplayPort Alt Mode driver would be
executing work and attempting to grab the tcpm_lock while the TCPM
was holding the lock and attempting to unregister the altmode, blocking
on the altmode driver's cancel_work_sync call.
Because the state check isn't protected, there is a small window
where the Alt Mode driver could determine that the TCPM is
in a ready state and attempt to grab the lock while the
TCPM grabs the lock and changes the TCPM state to one that

Register read of TCPC_RX_BYTE_CNT returns the total size consisting of:
PD message (pending read) size + 1 Byte for Frame Type (SOP*)
This is validated against the max PD message (`struct pd_message`) size
without accounting for the extra byte for the frame type. Note that the
struct pd_message does not contain a field for the frame_type. This
results in false negatives when the "PD message (pending read)" is equal
to the max PD message size.

The Interrupt Register Set contains Interrupt Moderation register (IMOD).
The IMOD register contains the following fields:
- Bits 15:0: Interrupt Moderation Interval (IMODI)
- Bits 31:16: Interrupt Moderation Counter (IMODC)
In the xHCI driver, the pointer currently named 'irq_control' refers to the
IMOD register. However, the name 'irq_control' does not accurately describe
the register or its contents, and the xHCI specification does not use the
term "irq control" or "interrupt control" for this register. To improve
clarity and better align with the xHCI specification, the pointer is
renamed to 'imod'.
Additionally, the IMOD register fields IMODI & IMODC have their own masks,
which are also renamed for consistency:
* 'ER_IRQ_INTERVAL_MASK' -> 'IMODI_MASK'
* 'ER_IRQ_COUNTER_MASK' -> 'IMODC_MASK'

Currently, an inverted RsvdP mask (ERST_BASE_RSVDP) is used to extract
bits 63:6. Replaces the inverted mask with a non-inverted mask,

xHCI specification 1.2, section 5.5.2.1.
Interrupt Pending bit is RW1C (Write-1-to-clear), which means that
writing '0' to is has no effect and is removed.
The Interrupt Pending (IP) bit is cleared at the start of interrupt
handling; xhci_clear_interrupt_pending(). This could theoretically
cause a new interrupt to be issued before the xhci driver reaches
the interrupter disable functions.
To address this, the IP bit is read after Interrupt Enable is
disabled, and a debug message is issued if the IP bit is still set.

The Interrupt Management register (IMAN), contains three fields:
- Bit 0: Interrupt Pending (IP)
- Bit 1: Interrupt Enable (IE)
- Bits 31:2: RsvdP (Reserved and Preserved)
Currently, there are multiple macros for both the IP and IE fields.
Consolidates them into single mask macros for better clarity and
maintainability.
Comment "THIS IS BUGGY - FIXME - IP IS WRITE 1 TO CLEAR" refers to the
fact that both macros 'ER_IRQ_ENABLE' and 'ER_IRQ_DISABLE' clear the IP bit
by writing '0' before modifying the IE bit. However, the IP bit is actually
cleared by writing '1'. To prevent any regression, this behavior has not
been altered. Instead, when the IE bit is modified, the IP macro is used
explicitly to highlight this "quirk".

The function configures the Interrupt Moderation Interval (IMODI) via bits
15:0 in the Interrupt Moderation Register. The IMODI value is specified in
increments of 250 nanoseconds. For instance, an IMODI register value of 16
corresponds to 4000 nanoseconds, resulting in an interrupt every ~1ms.
Currently, the function fails when a requested IMODI value is too large,
only logging a warning message for secondary interrupters. Prevent this by
automatically adjusting the IMODI value to the nearest supported value.

Move the primary interrupter (0) initialization from xhci_mem_init() to
xhci_init(). This change requires us to save the allocated interrupter
somewhere before initialization. Therefore, store it in the 'interrupters'
array and rework xhci_add_interrupter() to retrieve the interrupter from
the array.
This is part of the ongoing effort to separate allocation and
initialization.

Remove redundant error handling from xhci_add_interrupter() instead of
trying to accommodate them in future changes.
======== Reasoning for the removal ========
Function xhci_add_interrupter() is invoked in two scenarios:
Primary Interrupter Setup (ID 0):
The maximum number of interrupters is always greater than zero, and the
primary interrupter is always allocated as part of the driver's
initialization process. In case of failure, the xHCI driver errors and
exits.
Secondary Interrupter Creation (ID >= 1):
The interrupter is pre-allocated, and an empty slot is identified before
invoking xhci_add_interrupter().
In both cases, the existing error handling within xhci_add_interrupter() is
redundant and unnecessary.
Upcoming Changes:
In the subsequent commit, interrupter initialization will move from
xhci_mem_init() to xhci_init(). This change is necessary to facilitate
the ability to restart the xHCI driver without re-allocating memory.
As a result, the allocated interrupter must be stored in the interrupters
pointer array before initialization.
Consequently, xhci_create_secondary_interrupter() would need to handle
pointer removal for allocated 'interrupters' array upon failure, although
xhci_add_interrupter() will never fail.

Move the assignment of the doorbell array pointer from xhci_mem_init()
to xhci_init(). The assignment now utilizes the newly introduced
xhci_set_doorbell_ptr() function.
Doorbell Array Offset mask (DBOFF_MASK) is updated to directly specify its
bit range as 31:2, rather than using inverted reserved bits 1:0.
This change simplifies the mask representation, making it more intuitive
and easier to understand.
Remove the "// " prefix from trace messages, as it is unnecessary and
distracting.

Refactor xhci_set_cmd_ring_deq() making the code more understandable by
using more descriptive constants and separating operations logically.
- Remove 'CMD_RING_RSVD_BITS' the macro is misleading, the reserved bits
are 5:4, yet the mask is for bits 5:0.
- Introduce masks 'CMD_RING_PTR_MASK' and 'CMD_RING_CYCLE' to clearly
define the bits for the Command Ring pointer and Command Ring Cycle.
- Simplifying the process of setting the command ring address by separating
the DMA address calculation and the Command Ring Control register (crcr)
updates.
- Remove the "// " prefix from trace messages, as it is unnecessary and
distracting.
Note: In the current implementation, the cycle bit is not cleared before
applying the OR operation. Although this hasn't caused issues so far
because the bit is '0' before reaching this function, the bit is now
cleared before being set to prevent potential future problems and simplify
the process.

Move command ring pointer write from xhci_mem_init() to xhci_init(),
and utilize the xhci_set_cmd_ring_deq() function.
The xhci_set_cmd_ring_deq() function is nearly identical to the Command
Ring Control register code in xhci_mem_init(). The only notable change is
the use of:
xhci_trb_virt_to_dma(xhci->cmd_ring->deq_seg, xhci->cmd_ring->dequeue)
instead of:
xhci->cmd_ring->first_seg->dma
but they are effectively the same in this context. The former represents
the exact position of the dequeue pointer, while the latter is the first
DMA in the first segment. Before use, the dequeue pointer is at the first
DMA in the first segment.
The xhci_set_cmd_ring_deq() function is moved without modification, except
for (long unsigned long) -> (unsigned long long) due to checkpatch.pl.

Move pre-allocation initialization from xhci_mem_init() to xhci_init().
This change is part of an ongoing effort to separate initialization from
allocation within the xhci driver. By doing so, it will enable future
patches to re-initialize xhci driver memory without the necessity of fully
recreating it.
Additionally, compliance mode recovery initialization has been adjusted to
only occur after successful memory allocation.

Commit aa7a9275ab ("PM: sleep: Suspend async parents after suspending
children") had triggered a suspend issue on Tegra boards because it had
reordered the syspend of devices with async suspend enabled with respect
to some other devices. Specifically, the devices with async suspend
enabled that have no children are now suspended before any other devices
unless there are device links pointing to them as suppliers.
The investigation that followed the failure report uncovered that async
suspend was enabled for the cypd4226 device that was a Type-C controller
with a dependency on USB PHY and it turned out that disabling async
suspend for that device made the issue go away. Since async suspend
takes dependencies between parents and children into account as well
as other dependencies between devices represented by device links, this
means that the cypd4226 has a dependency on another device that is
not represented in any form in the kernel (a "hidden" dependency), in
which case async suspend should not be enabled for it.
Accordingly, make ucsi_ccg_probe() disable async suspend for the
devices handled by, which covers the cypd4226 device on the Tegra
boards as well as other devices likely to have similar "hidden"
dependencies.

usb-storage is the last user of the block layer bounce buffering now,
and only uses it for HCDs that do not support DMA on highmem configs.
Remove this support and fail the probe so that the block layer bounce
buffering can go away.

Event polling delay is set to 0 if there are any pending requests in
either rx or tx requests lists. Checking for pending requests does
not work well for "IN" transfers as the tty driver always queues
requests to the list and TRBs to the ring, preparing to receive data
from the host.
This causes unnecessary busylooping and cpu hogging.
Only set the event polling delay to 0 if there are pending tx "write"
transfers, or if it was less than 10ms since last active data transfer
in any direction.

VIA VL805 doesn't bother updating the EP Context cycle bit when the
endpoint halts. This is seen by patching xhci_move_dequeue_past_td()
to print the cycle bits of the EP Context and the TRB at hw_dequeue
and then disconnecting a flash drive while reading it. Actual cycle
state is random as expected, but the EP Context bit is always 1.
This means that the cycle state produced by this function is wrong
half the time, and then the endpoint stops working.
Work around it by looking at the cycle bit of TD's end_trb instead
of believing the Endpoint or Stream Context. Specifically:
- rename cycle_found to hw_dequeue_found to avoid confusion
- initialize new_cycle from td->end_trb instead of hw_dequeue
- switch new_cycle toggling to happen after end_trb is found
Now a workload which regularly stalls the device works normally for
a few hours and clearly demonstrates the HW bug - the EP Context bit
is not updated in a new cycle until Set TR Dequeue overwrites it:

Also tested on the buggy ASM1042 which moves EP Context dequeue to
the next TRB after errors, one problem case addressed by the rework
that implemented this loop. In this case hw_dequeue can be enqueue,
so simply picking the cycle bit of TRB at hw_dequeue wouldn't work.
Commit 5255660b20 ("xhci: add quirk for host controllers that
don't update endpoint DCS") tried to solve the stale cycle problem,
but it was more complex and got reverted due to a reported issue.

This patch fixes Type-C Compliance Test TD 4.7.6 - Try.SNK DRP Connect
SNKAS.
The compliance tester moves into SNK_UNATTACHED during toggling and
expects the PUT to apply Rp after tPDDebounce of detection. If the port
is in SNK_TRY_WAIT_DEBOUNCE, it will move into SRC_TRYWAIT immediately
and apply Rp. This violates TD 4.7.5.V.3, where the tester confirms that
the PUT attaches Rp after the transitions to Unattached.SNK for
tPDDebounce.
Change the tcpm_set_state delay between SNK_TRY_WAIT_DEBOUNCE and
SRC_TRYWAIT to tPDDebounce.

usbtmc_read() calls usbtmc_generic_read()
which uses interruptible sleep, but usbtmc_read()
itself uses uninterruptble sleep for mutual exclusion
between threads. That makes no sense.
Both should use interruptible sleep.

The controllers with rtl version larger than
RTL_REVISION_NEW_LPM (0x00002700) has bug which causes that controller
doesn't resume from L1 state. It happens if after receiving LPM packet
controller starts transitioning to L1 and in this moment the driver force
resuming by write operation to PORTSC.PLS.
It's corner case and happens when write operation to PORTSC occurs during
device delay before transitioning to L1 after transmitting ACK
time (TL1TokenRetry).
Forcing transition from L1->L0 by driver for revision larger than
RTL_REVISION_NEW_LPM is not needed, so driver can simply fix this issue
through block call of cdnsp_force_l0_go function.

This patch introduces the ucsi_con_mutex_lock / ucsi_con_mutex_unlock
functions to the UCSI driver. ucsi_con_mutex_lock ensures the connector
mutex is only locked if a connection is established and the partner pointer
is valid. This resolves a deadlock scenario where
ucsi_displayport_remove_partner holds con->mutex waiting for
dp_altmode_work to complete while dp_altmode_work attempts to acquire it.

The Cypress HX3 USB3.0 hubs use different PID values depending
on the product variant. The comment in compatibles table is
misleading, as the currently used PIDs (0x6504 and 0x6506 for
USB 3.0 and USB 2.0, respectively) are defaults for the CYUSB331x,
while CYUSB330x and CYUSB332x variants use different values.
Based on the datasheet [1], update the compatible usb devices table
to handle different types of the hub.
The change also includes vendor mode PIDs, which are used by the
hub in I2C Master boot mode, if connected EEPROM contains invalid
signature or is blank. This allows to correctly boot the hub even
if the EEPROM will have broken content.
Number of vcc supplies and timing requirements are the same for all
HX variants, so the platform driver's match table does not have to
be extended.

Device tree bindings state that the clock is optional for UHCI platform
controllers, and some existing device trees don't provide those - such
as those for VIA/WonderMedia devices.
The driver however fails to probe now if no clock is provided, because
devm_clk_get returns an error pointer in such case.
Switch to devm_clk_get_optional instead, so that it could probe again
on those platforms where no clocks are given.

Currently gadget_wakeup() waits for U0 synchronously if it was
called from func_wakeup(), this is because we need to send the
function wakeup command soon after the link is active. And the
call is made synchronous by polling DSTS continuosly for 20000
times in __dwc3_gadget_wakeup(). But it observed that sometimes
the link is not active even after polling 20K times, leading to
remote wakeup failures. Adding a small delay between each poll
helps, but that won't guarantee resolution in future. Hence make
the gadget_wakeup completely asynchronous.
Since multiple interfaces can issue a function wakeup at once,
add a new variable wakeup_pending_funcs which will indicate the
functions that has issued func_wakup, this is represented in a
bitmap format. If the link is in U3, dwc3_gadget_func_wakeup()
will set the bit corresponding to interface_id and bail out.
Once link comes back to U0, linksts_change irq is triggered,
where the function wakeup command is sent based on bitmap.

Currently when the host sends GET_STATUS request for an interface,
we use get_status callbacks to set/clear remote wakeup capability
of that interface. And if get_status callback isn't present for
that interface, then we assume its remote wakeup capability based
on bmAttributes.
Now consider a scenario, where we have a USB configuration with
multiple interfaces (say ECM + ADB), here ECM is remote wakeup
capable and as of now ADB isn't. And bmAttributes will indicate
the device as wakeup capable. With the current implementation,
when host sends GET_STATUS request for both interfaces, we will
set FUNC_RW_CAP for both. This results in USB3 CV Chapter 9.15
(Function Remote Wakeup Test) failures as host expects remote
wakeup from both interfaces.
The above scenario is just an example, and the failure can be
observed if we use configuration with any interface except ECM.
Hence avoid configuring remote wakeup capability from composite
driver based on bmAttributes, instead use get_status callbacks
and let the function drivers decide this.

When a USB device is connected to the OTG port, the tegra_xhci_id_work()
routine transitions the PHY to host mode and calls xhci_hub_control()
with the SetPortFeature command to enable port power.
In certain cases, the XHCI controller may be in a low-power state
when this operation occurs. If xhci_hub_control() is invoked while
the controller is suspended, the PORTSC register may return 0xFFFFFFFF,
indicating a read failure. This causes xhci_hc_died() to be triggered,
leading to host controller shutdown.

In very rare cases after resuming controller from L1 to L0 it reads
registers before the clock UTMI have been enabled and as the result
driver reads incorrect value.
Most of registers are in APB domain clock but some of them (e.g. PORTSC)
are in UTMI domain clock.
After entering to L1 state the UTMI clock can be disabled.
When controller transition from L1 to L0 the port status change event is
reported and in interrupt runtime function driver reads PORTSC.
During this read operation controller synchronize UTMI and APB domain
but UTMI clock is still disabled and in result it reads 0xFFFFFFFF value.
To fix this issue driver increases APB timeout value.
The issue is platform specific and if the default value of APB timeout
is not sufficient then this time should be set Individually for each
platform.

We identified a bug where the ST_RC bit in the status register was not
being acknowledged after clearing the CTRL_RUN bit in the control
register. This could lead to unexpected behavior in the USB gadget
drivers.
This patch resolves the issue by adding the necessary code to explicitly
acknowledge ST_RC after clearing CTRL_RUN based on the programming
sequence, ensuring proper state transition.

This patch enhances the HID gadget driver to support dynamic configuration
of the interrupt polling interval (bInterval) via configfs. A new
‘interval’ attribute is exposed under each HID function’s configfs
directory, and any write to it will adjust the poll rate for all endpoints
without requiring a rebuild.
When the attribute has never been written, legacy defaults are preserved:
• Full-Speed (FS) endpoints (IN & OUT) poll every 10 ms
• High-Speed (HS) endpoints (IN & OUT) poll every 4 micro-frames
(~1 ms)
To implement this cleanly:
• Add two new fields to f_hid_opts and f_hidg:
– unsigned char interval
– bool interval_user_set
• Introduce dedicated f_hid_opts_interval_show/store functions.
The store routine parses into an unsigned int, bounds‐checks,
assigns to opts->interval, and sets opts->interval_user_set = true.
• Initialize opts->interval = 4 and opts->interval_user_set = false in
hidg_alloc_inst(), then copy both into the live f_hidg instance in
hidg_alloc().
• In hidg_bind(), set each endpoint’s bInterval based on whether the
user has written the attribute:
– If interval_user_set == false, use FS=10 / HS=4
– If interval_user_set == true, use the user’s value for both FS
& HS

In preparation for making the kmalloc family of allocators type aware,
we need to make sure that the returned type from the allocation matches
the type of the variable being assigned. (Before, the allocator would
always return "void *", which can be implicitly cast to any pointer type.)
The assigned type is "struct usb_function **" but the returned type
will be "struct usb_function ***". These are the same size allocation
(pointer size), but different types. Adjust the allocation type to match
the assignment.

As demonstrated by the fix for update_port_device_state,
commit 12783c0b9e ("usb: core: Prevent null pointer dereference in update_port_device_state"),
usb_hub_to_struct_hub() can return NULL in certain scenarios,
such as during hub driver unbind or teardown race conditions,
even if the underlying usb_device structure exists.
Plus, all other places that call usb_hub_to_struct_hub() in the same file
do check for NULL return values.
If usb_hub_to_struct_hub() returns NULL, the subsequent access to
hub->ports[udev->portnum - 1] will cause a null pointer dereference.

It is possible that the gadget will be disabled, while the udc is
suspended. When enabling the udc in that case, the clock gating
will not be enabled again. Leaving the phy unclocked. Even when the
udc is not enabled, connecting this powered but not clocked phy leads
to enumeration errors on the host side.
To ensure that the clock gating will be in an valid state, we ensure
that the clock gating will be enabled before stopping the udc.

Update struct hid_descriptor to better reflect the mandatory and
optional parts of the HID Descriptor as per USB HID 1.11 specification.
Note: the kernel currently does not parse any optional HID class
descriptors, only the mandatory report descriptor.
Update all references to member element desc[0] to rpt_desc.
Add test to verify bLength and bNumDescriptors values are valid.
Replace the for loop with direct access to the mandatory HID class
descriptor member for the report descriptor. This eliminates the
possibility of getting an out-of-bounds fault.
Add a warning message if the HID descriptor contains any unsupported
optional HID class descriptors.

Here's a new simple driver for Owon oscilloscopes and a couple of new

All have been in linux-next with no reported issues.

Add serial support for OWON HDS200 series oscilloscopes and likely
many other pieces of OWON test equipment.
OWON HDS200 series devices host two USB endpoints, designed to
facilitate bidirectional SCPI. SCPI is a predominately ASCII text
protocol for test/measurement equipment. Having a serial/tty interface
for these devices lowers the barrier to entry for anyone trying to
write programs to communicate with them.

OWON appears to be using the same USB Vendor and Product ID for many
of their oscilloscopes. Looking at the discussion about the USB
vendor/product ID, in the link bellow, suggests that this VID/PID is
shared with VDS, SDS, PDS, and now the HDS series oscilloscopes.
Available documentation for these devices seems to indicate that all
use a similar SCPI protocol, some with RS232 options. It is likely that
this same simple serial setup would work correctly for them all.

Abacus Electrics makes optical probes for interacting with smart meters
over an optical interface.
At least one version uses an FT232B chip (as detected by ftdi_sio) with
a custom USB PID, which needs to be added to the list to make the device
work in a plug-and-play fashion.

The variable tpgt in usbg_make_tpg() is defined as unsigned long and is
assigned to tpgt->tport_tpgt, which is defined as u16. This may cause an
integer overflow when tpgt is greater than USHRT_MAX (65535). I
haven't tried to trigger it myself, but it is possible to trigger it
by calling usbg_make_tpg() with a large value for tpgt.
I modified the type of tpgt to match tpgt->tport_tpgt and adjusted the
relevant code accordingly.
This patch is similar to commit 59c816c1f2 ("vhost/scsi: potential
memory corruption").

The USB IP-block found in most Qualcomm platforms is modelled in the
Linux kernel as 3 different independent device drivers, but as shown by
the already existing layering violations in the Qualcomm glue driver
they can not be operated independently.
With the current implementation, the glue driver registers the core and
has no way to know when this is done. As a result, e.g. the suspend
callbacks needs to guard against NULL pointer dereferences when trying
to peek into the struct dwc3 found in the drvdata of the child.
Even with these checks, there are no way to fully protect ourselves from
the race conditions that occur if the DWC3 is unbound.
Missing from the upstream Qualcomm USB support is handling of role
switching, in which the glue needs to be notified upon DRD mode changes.
Several attempts has been made through the years to register callbacks
etc, but they always fall short when it comes to handling of the core's
probe deferral on resources etc.
Moving to a model where the DWC3 core is instantiated in a synchronous
fashion avoids above described race conditions.
It is however not feasible to do so without also flattening the
DeviceTree binding, as assumptions are made in the DWC3 core and
frameworks used that the device's associated of_node will the that of
the core. Furthermore, the DeviceTree binding is a direct
representation of the Linux driver model, and doesn't necessarily
describe "the USB IP-block".
The Qualcomm DWC3 glue driver is therefor transitioned to initialize and
operate the DWC3 within the one device context, in synchronous fashion.
To provide a limited time backwards compatibility, a snapshot of the
driver is retained in a previous commit. As such no care is taken in the
dwc3-qcom driver for the qcom,dwc3 backwards compatibility.

With the upcoming transition to a model where DWC3 core and glue operate
on a single struct device the drvdata datatype will change to be owned
by the core.
The drvdata is however used by the Qualcomm DWC3 glue to pass the qcom
glue context around before the core is allocated.
Remove this problem, and clean up the code, by passing the dwc3_qcom
struct around during probe, instead of acquiring it from the drvdata.

When the core is integrated with glue, it's reasonable to assume that
the glue driver will have to touch the IP before/after the core takes
the hardware out and into reset. As such the glue must own these
resources and be allowed to turn them on/off outside the core's
handling.
Allow the platform or glue layer to indicate if the core logic for
clocks and resets should be skipped to deal with this.

The DWC3 IP block is handled by three distinct device drivers: XHCI,
DWC3 core and a platform specific (optional) DWC3 glue driver.
This has resulted in, at least in the case of the Qualcomm glue, the
presence of a number of layering violations, where the glue code either
can't handle, or has to work around, the fact that core might not probe
deterministically.
An example of this is that the suspend path should operate slightly
different depending on the device operating in host or peripheral mode,
and the only way to determine the operating state is to peek into the
core's drvdata.
The Qualcomm glue driver is expected to make updates in the qscratch
register region (the "glue" region) during role switch events, but with
the glue and core split using the driver model, there is no reasonable
way to introduce listeners for mode changes.
Split the dwc3 core platform_driver callbacks and their implementation
and export the implementation, to make it possible to deterministically
instantiate the dwc3 core as part of the dwc3 glue drivers and to
allow flattening of the DeviceTree representation.

In order to more tightly integrate the Qualcomm glue driver with the
dwc3 core the driver is redesigned to avoid splitting the implementation
using the driver model. But due to the strong coupling to the Devicetree
binding needs to be updated as well.
Various ways to provide backwards compatibility with existing Devicetree
blobs has been explored, but migrating the Devicetree information
between the old and the new binding is non-trivial.
For the vast majority of boards out there, the kernel and Devicetree are
generated and handled together, which in practice means that backwards
compatibility needs to be managed across about 1 kernel release.
For some though, such as the various Snapdragon laptops, the Devicetree
blobs live a life separate of the kernel. In each one of these, with the
continued extension of new features, it's recommended that users would
upgrade their Devicetree somewhat frequently.
With this in mind, simply carrying a snapshot/copy of the current driver
is simpler than creating and maintaining the migration code.
The driver is kept under the same Kconfig option, to ensure that Linux
distributions doesn't drop USB support on these platforms.
The driver, which is going to be refactored to handle the newly
introduced qcom,snps-dwc3 compatible, is updated to temporarily not
match against any compatible.
This driver should be removed after the next LTS release.

The OHCI controller (rev 0x02) under LS7A PCI host has a hardware flaw.
MMIO register with offset 0x60/0x64 is treated as legacy PS2-compatible
keyboard/mouse interface, which confuse the OHCI controller. Since OHCI
only use a 4KB BAR resource indeed, the LS7A OHCI controller's 32KB BAR
is wrapped around (the second 4KB BAR space is the same as the first 4KB
internally). So we can add an 4KB offset (0x1000) to the OHCI registers
(from the PCI BAR resource) as a quirk.

The "reset" GPIO controls the RESET signal to an external, usually
ULPI PHY, chip. The original code path acquires the signal in LOW
state, and then immediately asserts it HIGH again, if the reset
signal defaulted to asserted, there'd be a short "spike" before the
reset.
Here is what happens depending on the pre-existing state of the reset
signal:

At point A, the low going transition is because the reset line is
requested using GPIOD_OUT_LOW. If the line is successfully requested,
the first thing we do is set it high _without_ any delay. This is
point B. So, a glitch occurs between A and B.
Requesting the line using GPIOD_OUT_HIGH eliminates the A and B
transitions. Instead we get:

Where A and C are the points described above in the code. Point B
has been eliminated.
The issue was found during code inspection.

The cdns3 driver has the same NCM deadlock as fixed in cdnsp by commit
58f2fcb3a8 ("usb: cdnsp: Fix deadlock issue during using NCM gadget").
Under PREEMPT_RT the deadlock can be readily triggered by heavy network
traffic, for example using "iperf --bidir" over NCM ethernet link.
The deadlock occurs because the threaded interrupt handler gets
preempted by a softirq, but both are protected by the same spinlock.
Prevent deadlock by disabling softirq during threaded irq handler.

These drivers have no way to probe as there are no match tables nor
devices created with a matching name in the kernel tree.
Marvell UDC was only ever supported by board files which were removed
in 2022.
For Marvell U3D, which was added in 2012, the PXA2128 aka MMP3 support
was never upstreamed with board files and only revived in 2019 with DT
support. No U3D DT support has been added since then.
The PLX net2272 driver was formerly used on blackfin. It also has PCI
support, but that appears to be only for a development board which is
likely unused given this device dates back to 2006.

The current implementation of the usb-conn-gpio driver uses a fixed
"usb-charger" name for all USB connector devices. This causes conflicts
in the power supply subsystem when multiple USB connectors are present,
as duplicate names are not allowed.
Use IDA to manage unique IDs for naming usb connectors (e.g.,
usb-charger-0, usb-charger-1).

Enabling LPM is done in hub workqueue, often in paths handling possible
link issues. So fail immediately on USB3 LPM issues and avoid hub wq
from unnecessary blocking, thus allowing it to handle other port events
faster.
Detect errors when enabling U1/U2 link states, and return immediately
if there is an issue.

Several usb requests are needed to allow a USB3 link to enter U1/U2
hardware link power management LPM states. Reorder these requests
and send the more significant and likely to succeed first.
This is similar to the change done for disabling LPM
Enable LPM by first sending requests to the upstream hub of the device
SetPortFeature(U1_TIMEOUT)
SetPortFeature(U2_TIMEOUT)
These are more likely to succeed due to the shorter path, and LPM can
be considered enabled as link may go to U1/U2 LPM states after those.
Send the requests to the device after this, they allow the device
to initialte U1/U2 link transitions. Hub can already initiate U1/U2
SetFeature(U1_ENABLE)
SetFeature(U2_ENABLE)
Fail fast and bail out if a requests to the device fails.
This changes device initated LPM policy a bit. Device is no longer
able to initiate U2 if it failed or is not allowed to initiate
U1.
Enabling and disabling Link power management is done as part of
hub work. Avoid trying to send additional USB requests to a device
when there are known issues. It just causes hub work to block for
even longer.

Enabling device initiated USB3 link power management (LPM) may fail for
various reasons such as too long system exit latency, or link issues.
These are not good reason to disable hub initiated LPM U1/U2
states, especially as it requires sending more requests over a
possibly broken link, causing the hub work to block for even longer.

Several usb requests are needed to allow or forbid a USB3 link from
going into U1 or U2 hardware link power management (LPM) states.
Fail fast on issues in LPM disabling path. LPM disabling is done in hub
workqueue paths that are often already handling possible link issues.
Enabling and disabling LPM involves four usb requests.
Two requests sent to the upstream hub of the connected device:
SetPortFeature(U1_TIMEOUT)
SetPortFeature(U2_TIMEOUT)
And two to the device itself:
SetFeature(U1_ENABLE)
SetFeature(U2_ENABLE)
The requests to the hub sets the inactivity timeout used by the hub to
know when to initiate U1 and U2 LPM link state transitions.
These requests are also used prevent U1/U2 LPM transitions completely
by passing zero timeout value.
The requsts sent to the device only controls if device is allowed to
initiate U1/U2 transitions. If not enabled then only hub initiates U1/U2
transitions. Hub may block these device initiated attempts.
Reorder and send the hub requests first, these are more likely to succeed
due to shorter path, and we can consider LPM disabled if these succeed
as U1/U2 link state can not be entered after that.
Fail immediately if a request fails, and don't try to enable back LPM
after a failed request, that will just send more LPM requests over a
bad link.
If a device request controlling device initiateed LPM fails then exit
immediately, but consider LPM disabled at this stage.

When creating a device path in the driver the snprintf() takes
up to 16 characters long argument along with the additional up to
12 characters for the signed integer (as it can't see the actual limits)
and tries to pack this into 16 bytes array. GCC complains about that
when build with `make W=1`:
drivers/usb/core/usb.c:705:25: note: ‘snprintf’ output between 3 and 28 bytes into a destination of size 16
Since everything works until now, let's just check for the potential
buffer overflow and bail out. It is most likely a never happen situation,
but at least it makes GCC happy.

On i.MX95 platform, USB wakeup setting is controlled by HSIO Block
Control:
HSIO Block Control Overview:
- The HSIO block control include configuration and status registers that
provide miscellaneous top-level controls for clocking, beat limiter
enables, wakeup signal enables and interrupt status for the PCIe and USB
interfaces.
The wakeup function of HSIO blkctl is basically same as non-core, except
improvements about power lost cases. This will add the wakeup setting for
HSIO blkctl on i.MX95. It will firstly ioremap hsio blkctl memory, then do
wakeup setting as needs.

In previous imx platform, normal USB controller interrupt and wakeup
interrupt are bound to one irq line. However, it changes on latest
i.MX95 platform since it has a dedicated irq line for wakeup interrupt.
This will add wakeup interrupt handling for i.MX95 to support various
wakeup events.

In the course of fixing up the usages of flexible arrays, Gustavo
submitted a patch updating the ehci-fsl driver. However, the patch
was wrong because the driver was using the .priv member of the
ehci_hcd structure incorrectly. The private data is not supposed to
be a wrapper containing the ehci_hcd structure; it is supposed to be a
sub-structure stored in the .priv member.
Fix the problem by replacing the ehci_fsl structure with
ehci_fsl_priv, containing only the private data, along with a suitable
conversion macro for accessing it. This removes the problem of having
data follow a flexible array member.

Don't set WDM_READ flag in wdm_in_callback() for ZLP-s, otherwise when
userspace tries to poll for available data, it might - incorrectly -
believe there is something available, and when it tries to non-blocking
read it, it might get stuck in the read loop.
For example this is what glib does for non-blocking read (briefly):
1. poll()
2. if poll returns with non-zero, starts a read data loop:
a. loop on poll() (EINTR disabled)
b. if revents was set, reads data
I. if read returns with EINTR or EAGAIN, goto 2.a.
II. otherwise return with data
So if ZLP sets WDM_READ (#1), we expect data, and try to read it (#2).
But as that was a ZLP, and we are doing non-blocking read, wdm_read()
returns with EAGAIN (#2.b.I), so loop again, and try to read again
(#2.a.).
With glib, we might stuck in this loop forever, as EINTR is disabled
(#2.a).

loops to ensure no readers get stuck when the function is disabled/unbound,
protecting against corruption when the waitq and spinlocks are reinitialized
in hidg_bind().

strncpy() is deprecated for NUL-terminated destination buffers; use
strscpy() instead. Since kzalloc() already zeroes out the destination
buffer, the potential NUL-padding by strncpy() is unnecessary. strscpy()
copies only the required characters and guarantees NUL-termination.
Since the destination buffer has a fixed length, strscpy() automatically
determines its size using sizeof() when the argument is omitted. This
makes an explicit sizeof() call unnecessary.
The source string is also NUL-terminated and meets the __must_be_cstr()
requirement of strscpy().
No functional changes intended.

dwc3 device suspend/resume callbacks were being triggered during system
suspend and resume even if the device was already runtime-suspended.
This is redundant for device mode because the suspend and resume routines
are essentially identical for system PM and runtime PM.
To prevent these unnecessary callbacks, indicate to the PM core that it
can safely leave the device in runtime suspend if it's already
runtime-suspended in device mode by returning a positive value in
prepare() callback. This optimization only applies to devices without
pinctrl, as pinctrl has distinct logic tied to system suspend/resume.

Although some Type-C DRD devices that do not support the DP Sink
function (such as Huawei Mate 40Pro), the Source Port initiates
Enter Mode CMD, but the device responds to Enter Mode ACK, the
Source port then initiates DP Status Update CMD, and the device
responds to DP Status Update NAK.
As PD2.0 spec ("6.4.4.3.4 Enter Mode Command"),A DR_Swap Message
Shall Not be sent during Modal Operation between the Port Partners.
At this time, the source port initiates DR_Swap message through the
"echo device > /sys/class/typec/port0/data_role" command to switch
the data role from host to device. The device will initiate a Hard
Reset for recovery, resulting in the failure of data role swap.
Therefore, when DP Status Update NAK is received, Exit Mode CMD is
initiated to exit the currently entered DP altmode.

The funciton tcpm_acc_attach is not setting the proper state when
calling tcpm_set_role. The function tcpm_set_role is currently only
handling TYPEC_STATE_USB. For the tcpm_acc_attach to switch into other
modal states tcpm_set_role needs to be extended by an extra state
parameter. This patch is handling the proper state change when calling
tcpm_acc_attach.

also handles the transition to access the DEBUG_ACC_ATTACHED state in
the sink case. It also handles the debounce case in which the cc
pins are not immediately valid after the plug event.

Since the typec connectors can have many muxes or switches for different
lanes (sbu, usb2, usb3) going into different modal states (usb2, usb3,
audio, debug) all of them will be called on typec_switch_set and
typec_mux_set. But not all of them will be handling the expected mode.
If one of the mux or switch will come back with EOPTNOSUPP this is no
reason to stop running through the next ones. Therefor we skip this
particular error value and continue calling the next.

Concurrent calls to typec_partner_unlink_device can lead to a NULL pointer
dereference. This patch adds a mutex to protect USB device pointers and
prevent this issue. The same mutex protects both the device pointers and
the partner device registration.

Current xhci bus resume implementation prevents xHC host from generating
interrupts during high-speed USB 2 and super-speed USB 3 bus resume.
Only reason to disable interrupts during bus resume would be to prevent
the interrupt handler from interfering with the resume process of USB 2
ports.
Host initiated resume of USB 2 ports is done in two stages.
The xhci driver first transitions the port from 'U3' to 'Resume' state,
then wait in Resume for 20ms, and finally moves port to U0 state.
xhci driver can't prevent interrupts by keeping the xhci spinlock
due to this 20ms sleep.
Limit interrupt disabling to the USB 2 port resume case only.
resuming USB 2 ports in bus resume is only done in special cases where
USB 2 ports had to be forced to suspend during bus suspend.
The current way of preventing interrupts by clearing the 'Interrupt
Enable' (INTE) bit in USBCMD register won't prevent the Interrupter
registers 'Interrupt Pending' (IP), 'Event Handler Busy' (EHB) and
USBSTS register Event Interrupt (EINT) bits from being set.
New interrupts can't be issued before those bits are properly clered.
Disable interrupts by clearing the interrupter register 'Interrupt
Enable' (IE) bit instead. This way IP, EHB and INTE won't be set
before IE is enabled again and a new interrupt is triggered.

This check is performed before prepare_transfer() and prepare_ring(), so
enqueue can already point at the final link TRB of a segment. And indeed
it will, some 0.4% of times this code is called.
Then enqueue + 1 is an invalid pointer. It will crash the kernel right
away or load some junk which may look like a link TRB and cause the real
link TRB to be replaced with a NOOP. This wouldn't end well.
Use a functionally equivalent test which doesn't dereference the pointer
and always gives correct result.
Something has crashed my machine twice in recent days while playing with
an Etron HC, and a control transfer stress test ran for confirmation has
just crashed it again. The same test passes with this patch applied.

A Short Packet event before the last TRB of a TD is followed by another
event on the final TRB on spec-compliant HCs, which is most of them.
A 'last_td_was_short' flag was added to know if a TD has just completed
as Short Packet and another event is to come. The flag was cleared after
seeing the event (unless no TDs are pending, but that's a separate bug)
or seeing a new TD complete as something other than Short Packet.
A rework replaced the flag with an 'old_trb_comp_code' variable. When
an event doesn't match the pending TD and the previous event was Short
Packet, the new event is silently ignored.
To preserve old behavior, 'old_trb_comp_code' should be cleared at this
point, but instead it is being set to current comp code, which is often
Short Packet again. This can cause more events to be silently ignored,
even though they are no longer connected with the old TD that completed
short and indicate a serious problem with the driver or the xHC.
Common device classes like UAC in async mode, UVC, serial or the UAS
status pipe complete as Short Packet routinely and could be affected.
Clear 'old_trb_comp_code' to zero, which is an invalid completion code
and the same value the variable starts with. This restores original
behavior on Short Packet and also works for illegal Etron events, which
the code has been extended to cover too.

In the case of handling a USB bus reset, the xhci_discover_or_reset_device
can run without first notifying the xHCI sideband client driver to stop or
prevent the use of the transfer ring. It was seen that when a bus reset
situation happened, the USB offload driver was attempting to fetch the xHCI
transfer ring information, which was already freed.

As part of xHCI bus suspend, the xHCI is halted. However, if there are
pending events in the secondary event ring, it is observed that the xHCI
controller stops responding to further commands upon host or device
initiated bus resume. Iterate through all pending events and update the
dequeue pointer to the beginning of the event ring.

Introduce XHCI sideband, which manages the USB endpoints being requested by
a client driver. This is used for when client drivers are attempting to
offload USB endpoints to another entity for handling USB transfers. XHCI
sec intr will allow for drivers to fetch the required information about the
transfer ring, so the user can submit transfers independently. Expose the
required APIs for drivers to register and request for a USB endpoint and to
manage XHCI secondary interrupters.
Driver renaming, multiple ring segment page linking, proper endpoint clean
up, and allowing module compilation added by Wesley Cheng to complete
original concept code by Mathias Nyman.

Previous signature was:

Internally, it extracted two information out of the message:
- whether we are after hibernation: msg.event == PM_EVENT_RESTORE,
- whether this is an auto resume: msg.event == PM_EVENT_AUTO_RESUME.
First bulletpoint is somewhat wrong: driver wants to know if the device
did lose power, it doesn't care about hibernation per se. Knowing that,
refactor to ask upper layers the right questions: (1) "did we lose
power?" and, (2) "is this an auto resume?". Change the signature to:

bool is_auto_resume);
The goal is to allow some upper layers (cdns3-plat) to tell us when
power was lost after system-wise suspend.
Note that lost_power is ORed at the start of xhci_resume() to
xhci->quirks & XHCI_RESET_ON_RESUME || xhci->broken_suspend. It is
simpler to keep those checks inside of xhci_resume() instead of doing
them at each caller of xhci_resume().

At runtime_resume(), read the W1 (Wrapper Register 1) register to detect
if an hardware reset occurred. If it did, run the hardware init sequence.
This callback will be called at system-wide resume. Previously, if a
reset occurred during suspend, we would crash. The wrapper config had
not been written, leading to invalid register accesses inside cdns3.

The device probe function mixes management code and hardware
initialisation code. Extract the latter into an explicitly named
cdns_ti_reset_and_init_hw() function to clarify intent. It also will
allow easier transition to using runtime PM for triggering HW init.

The cdns_role_driver->resume() callback takes a second boolean argument
named `hibernated` in its implementations. This is mistaken; the only
potential caller is:

This is a revert of
commit 04fb365c45 ("usb: dwc3: replace %p with %pK")
When the formatting was changed from %p to %pK that was a security
improvement, as %p would leak raw pointer values to the kernel log.
Since commit ad67b74d24 ("printk: hash addresses printed with %p")
the regular %p has been improved to avoid this issue.
On the other hand, restricted pointers ("%pK") were never meant to be used
through printk(). They can unintentionally still leak raw pointers or
acquire sleeping looks in atomic contexts.
Switch back to regular %p again.

commit 2f964780c0 ("USB: core: replace %p with %pK").
When the formatting was changed from %p to %pK that was a security
improvement, as %p would leak raw pointer values to the kernel log.
Since commit ad67b74d24 ("printk: hash addresses printed with %p")
the regular %p has been improved to avoid this issue.
On the other hand, restricted pointers ("%pK") were never meant to be used
through printk(). They can unintentionally still leak raw pointers or
acquire sleeping looks in atomic contexts.
Switch back to regular %p again.

The variable d->name, returned by devm_kasprintf(), could be NULL.
A pointer check is added to prevent potential NULL pointer dereference.
This is similar to the fix in commit 3027e7b15b

A subtle error got introduced while manually fixing merge conflict in
tcpm.c for commit 85c4efbe60 ("Merge v6.12-rc6 into usb-next"). As a
result of this error, the next state is unconditionally set to
SNK_WAIT_CAPABILITIES_TIMEOUT while handling SNK_WAIT_CAPABILITIES state
in run_state_machine(...).
Fix this by setting new state of TCPM state machine to `upcoming_state`
(that is set to different values based on conditions).

The Altera USB Blaster 3, available as both a cable and an on-board
solution, is primarily used for programming and debugging FPGAs.
It interfaces with host software such as Quartus Programmer,
System Console, SignalTap, and Nios Debugger. The device utilizes
either an FT2232 or FT4232 chip.
Enabling the support for various configurations of the on-board
USB Blaster 3 by including the appropriate VID/PID pairs,
allowing it to function as a serial device via ftdi_sio.
Note that this check-in does not include support for the
cable solution, as it does not support UART functionality.
The supported configurations are determined by the
hardware design and include:
1) PID 0x6022, FT2232, 1 JTAG port (Port A) + Port B as UART

Nothing prevents a broken HC from claiming that an endpoint is Running
and repeatedly rejecting Stop Endpoint with Context State Error.
Avoid infinite retries and give back cancelled TDs.
No such cases known so far, but HCs have bugs.

When the device stalls an endpoint, current TD is assigned -EPIPE
status and Reset Endpoint is queued. If a Stop Endpoint is pending
at the time, it will run before Reset Endpoint and fail due to the
stall. Its handler will change TD's status to -EPROTO before Reset
Endpoint handler runs and initiates giveback.
Check if the stall has already been handled and don't try to do it
again. Since xhci_handle_halted_endpoint() performs this check too,
not overwriting td->status is the only difference.
I haven't seen this case yet, but I have seen a related one where
the xHC has already executed Reset Endpoint, EP Context state is
now Stopped and EP_HALTED is set. If the xHC took a bit longer to
execute Reset Endpoint, said case would become this one.

If EP_STALLED flag is set in xhci_urb_dequeue(), without EP_HALTED or
SET_DEQ_PENDING flags, then the endpoint is in stopped state and the
cancelled URB can be given back immediately withouth queueing
a 'stop endpoint' command.
Without this change the cancelled URB would eventually be given back
in the 'context state error' completion path of the 'stop endpoint'
command. This is not optimal.
For this improvement to work the EP_STALLED flag must be cleared with
xhci lock held.

When used on Huawei hisi platforms, Prolific Mass Storage Card Reader
which the VID:PID is in 067b:2731 might fail to enumerate at boot time
and doesn't work well with LPM enabled, combination quirks:
USB_QUIRK_DELAY_INIT + USB_QUIRK_NO_LPM

Etron xHC generates two transfer events for the TRB if an error is
detected while processing the last TRB of an isoc TD.
The first event can be any sort of error (like USB Transaction or
Babble Detected, etc), and the final event is Success.
The xHCI driver will handle the TD after the first event and remove it
from its internal list, and then print an "Transfer event TRB DMA ptr
not part of current TD" error message after the final event.
Commit 5372c65e13 ("xhci: process isoc TD properly when there was a
transaction error mid TD.") is designed to address isoc transaction
errors, but unfortunately it doesn't account for this scenario.
This issue is similar to the XHCI_SPURIOUS_SUCCESS case where a success
event follows a 'short transfer' event, but the TD the event points to
is already given back.
Expand the spurious success 'short transfer' event handling to cover
the spurious success after error on Etron hosts.
Kuangyi Chiang reported this issue and submitted a different solution
based on using error_mid_td. This commit message is mostly taken
from that patch.

Two clearly different specimens of NEC uPD720200 (one with start/stop
bug, one without) were seen to cause IOMMU faults after some Missed
Service Errors. Faulting address is immediately after a transfer ring
segment and patched dynamic debug messages revealed that the MSE was
received when waiting for a TD near the end of that segment:

It's possible that data from the isochronous device were written to
random buffers of pending TDs on other endpoints (either IN or OUT),
other devices or even other HCs in the same IOMMU domain.
Lastly, an error from a different USB device on another HC. Was it
caused by the above? I don't know, but it may have been. The disk
was working without any other issues and generated PCIe traffic to
starve the NEC of upstream BW and trigger those MSEs. The two HCs
shared one x1 slot by means of a commercial "PCIe splitter" board.

Fortunately, it appears that this ridiculous bug is avoided by setting
the chain bit of Link TRBs on isochronous rings. Other ancient HCs are
known which also expect the bit to be set and they ignore Link TRBs if
it's not. Reportedly, 0.95 spec guaranteed that the bit is set.
The bandwidth-starved NEC HC running a 32KB/uframe UVC endpoint reports
tens of MSEs per second and runs into the bug within seconds. Chaining
Link TRBs allows the same workload to run for many minutes, many times.
No negative side effects seen in UVC recording and UAC playback with a
few devices at full speed, high speed and SuperSpeed.
The problem doesn't reproduce on the newer Renesas uPD720201/uPD720202
and on old Etron EJ168 and VIA VL805 (but the VL805 has other bug).

Ensure that an endpoint halted due to device STALL is not
restarted before a Clear_Feature(ENDPOINT_HALT) request is sent to
the device.
The host side of the endpoint may otherwise be started early by the
'Set TR Deq' command completion handler which is called if dequeue
is moved past a cancelled or halted TD.
Prevent this with a new flag set for bulk and interrupt endpoints
when a Stall Error is received. Clear it in hcd->endpoint_reset()
which is called after Clear_Feature(ENDPOINT_HALT) is sent.
Also add a debug message if a class driver queues a new URB after the
STALL. Note that class driver might not be aware of the STALL
yet when it submits the URB as URBs are given back in BH.

Function trb_in_td() currently includes debug capabilities that are
triggered when its debug argument is set to true. The only consumer of
these debug capabilities is handle_tx_event(), which calls trb_in_td()
twice, once for its primary functionality and a second time solely for
debugging purposes if the first call returns 'NULL'.
This approach is inefficient and can lead to confusion, as trb_in_td()
executes the same code with identical arguments twice, differing only in
the debug output during the second execution.
To enhance clarity and efficiency, move the debug capabilities out of
trb_in_td() and integrates them directly into handle_tx_event().
This change reduces the argument count of trb_in_td() and ensures that
debug steps are executed only when necessary, streamlining the function's
operation.

Relocate trb_in_td() and marks it as static, as it's exclusively utilized
in xhci-ring.c. This adjustment lays the groundwork for future rework of
the function.
The function's logic remains unchanged; only its access specifier is
altered to static and a redundant "else" is removed on line 325
(due to checkpatch.pl complaining).

The current xHCI driver does not validate whether a page size of 4096
bytes is supported. Address the issue by setting the page size to the
value supported by the xHCI controller, as read from the Page Size
register. In the event of an unexpected value; default to a 4K page size.
Additionally, this commit removes unnecessary debug messages and instead
prints the supported and used page size once.
The xHCI controller supports page sizes of (2^{(n+12)}) bytes, where 'n'
is the Page Size Bit. Only one page size is supported, with a maximum
page size of 128 KB.

The ffs() function returns the index of the first set bit, starting from 1.
If no bits are set, it returns zero. This behavior causes an off-by-one
page size in the debug message, as the page size calculation [1]
is zero-based, while ffs() is one-based.
Fix this by subtracting one from the result of ffs(). Note that since
variable 'val' is unsigned, subtracting one from zero will result in the
maximum unsigned integer value. Consequently, the condition 'if (val < 16)'
will still function correctly.

If skipping is deferred to events other than Missed Service Error itsef,
it means we are running on an xHCI 1.0 host and don't know how many TDs
were missed until we reach some ordinary transfer completion event.
And in case of ring xrun, we can't know where the xrun happened either.
If we skip all pending TDs, we may prematurely give back TDs added after
the xrun had occurred, risking data loss or buffer UAF by the xHC.
If we skip none, a driver may become confused and stop working when all
its URBs are missed and appear to be "in flight" forever.
Skip exactly one TD on each xrun event - the first one that was missed,
as we can now be sure that the HC has finished processing it. Provided
that one more TD is queued before any subsequent doorbell ring, it will
become safe to skip another TD by the time we get an xrun again.

xHCI spec rev. 1.0 allowed the TRB pointer of Missed Service events
to be NULL. Having no idea which of the queued TDs were missed and
which are waiting, we can only set a flag to skip missed TDs later.
But HCs are also allowed to give us pointer to the last missed TRB,
and this became mandatory in spec rev. 1.1 and later.
Use this pointer, if available, to immediately skip all missed TDs.
This reduces latency and risk of skipping-related bugs, because we
can now leave the skip flag cleared for future events.
Handle Missed Service Error events as 'error mid TD', if applicable,
because rev. 1.0 spec excplicitly says so in notes to 4.10.3.2 and
later revs in 4.10.3.2 and 4.11.2.5.2. Notes to 4.9.1 seem to apply.
Tested on ASM1142 and ASM3142 v1.1 xHCs which provide TRB pointers.
Tested on AMD, Etron, Renesas v1.0 xHCs which provide TRB pointers.
Tested on NEC v0.96 and VIA v1.0 xHCs which send a NULL pointer.

The TRB pointer of these events points at enqueue at the time of error
occurrence on xHCI 1.1+ HCs or it's NULL on older ones. By the time we
are handling the event, a new TD may be queued at this ring position.
I can trigger this race by rising interrupt moderation to increase IRQ
handling delay. Similar delay may occur naturally due to system load.
If this ever happens after a Missed Service Error, missed TDs will be
skipped and the new TD processed as if it matched the event. It could
be given back prematurely, risking data loss or buffer UAF by the xHC.
Don't complete TDs on xrun events and don't warn if queued TDs don't
match the event's TRB pointer, which can be NULL or a link/no-op TRB.
Don't warn if there are no queued TDs at all.
Now that it's safe, also handle xrun events if the skip flag is clear.
This ensures completion of any TD stuck in 'error mid TD' state right
before the xrun event, which could happen if a driver submits a finite
number of URBs to a buggy HC and then an error occurs on the last TD.

Missed Service Error after an error mid TD means that the failed TD has
already been passed by the xHC without acknowledgment of the final TRB,
a known hardware bug. So don't wait any more and give back the TD.
Reproduced on NEC uPD720200 under conditions of ludicrously bad USB link
quality, confirmed to behave as expected using dynamic debug.

Up until commit d56b0b2ab1 ("usb: xhci: ensure skipped isoc TDs are
returned when isoc ring is stopped") in v6.11, the driver didn't skip
missed isochronous TDs when handling Stoppend and Stopped - Length
Invalid events. Instead, it erroneously cleared the skip flag, which
would cause the ring to get stuck, as future events won't match the
missed TD which is never removed from the queue until it's cancelled.
This buggy logic seems to have been in place substantially unchanged
since the 3.x series over 10 years ago, which probably speaks first
and foremost about relative rarity of this case in normal usage, but
by the spec I see no reason why it shouldn't be possible.
After d56b0b2ab1, TDs are immediately skipped when handling those
Stopped events. This poses a potential problem in case of Stopped -
Length Invalid, which occurs either on completed TDs (likely already
given back) or Link and No-Op TRBs. Such event won't be recognized
as matching any TD (unless it's the rare Link TRB inside a TD) and
will result in skipping all pending TDs, giving them back possibly
before they are done, risking isoc data loss and maybe UAF by HW.
As a compromise, don't skip and don't clear the skip flag on this
kind of event. Then the next event will skip missed TDs. A downside
of not handling Stopped - Length Invalid on a Link inside a TD is
that if the TD is cancelled, its actual length will not be updated
to account for TRBs (silently) completed before the TD was stopped.
I had no luck producing this sequence of completion events so there
is no compelling demonstration of any resulting disaster. It may be
a very rare, obscure condition. The sole motivation for this patch
is that if such unlikely event does occur, I'd rather risk reporting
a cancelled partially done isoc frame as empty than gamble with UAF.
This will be fixed more properly by looking at Stopped event's TRB
pointer when making skipping decisions, but such rework is unlikely
to be backported to v6.12, which will stay around for a few years.

The function is a remnant from a previous implementation and is now
redundant. There is no longer a need to search for the dequeue pointer,
as both the TRB and segment dequeue pointers are saved within
'queued_deq_seg' and 'queued_deq_ptr'.

The device id entries for Telit FN990B ended up matching only on the
interface protocol. While this works, the protocol is qualified by the
interface class (and subclass) which should have been included.
Switch to matching using USB_DEVICE_AND_INTERFACE_INFO() while keeping
the entries sorted also by protocol for consistency.
