380b75d307 Drivers: hv: Allow vmbus_sendpacket_mpb_desc() to create multiple ranges
vmbus_sendpacket_mpb_desc() is currently used only by the storvsc driver
and is hardcoded to create a single GPA range. To allow it to also be
used by the netvsc driver to create multiple GPA ranges, no longer
hardcode as having a single GPA range. Allow the calling driver to
specify the rangecount in the supplied descriptor.

Update the storvsc driver to reflect this new approach.

e8007fad54 scsi: sd_zbc: block: Respect bio vector limits for REPORT ZONES buffer
The REPORT ZONES buffer size is currently limited by the HBA's maximum
segment count to ensure the buffer can be mapped. However, the block
layer further limits the number of iovec entries to 1024 when allocating
a bio.

To avoid allocation of buffers too large to be mapped, further restrict
the maximum buffer size to BIO_MAX_INLINE_VECS.

Replace the UIO_MAXIOV symbolic name with the more contextually
appropriate BIO_MAX_INLINE_VECS.

cd6856d388 scsi: sd: Remove the stream_status member from scsi_stream_status_header
Having a variable length array at the end of scsi_stream_status_header
only causes problems.  Remove it and switch sd_is_perm_stream(), which is
the only place that currently uses it, to use the scsi_stream_status
directly following it in the local buf structure.

Besides being a much better data structure design, this also avoids a
-Wflex-array-member-not-at-end warning.

8c628207d6 scsi: scsi_debug: Reduce DEF_ATOMIC_WR_MAX_LENGTH
The default atomic write max length in DEF_ATOMIC_WR_MAX_LENGTH is
excessively large.

For 512B LBS, we would get a 4MB max, but due to block layer atomic write
restrictions this is limited to 512KB.

Reduce DEF_ATOMIC_WR_MAX_LENGTH to a value which would be more realistic
(for a real device supporting atomic writes), 64KB.

There are two main functional changes in this patch series:
smartpqi-take-drives-offline-when-controller-is-offline
smartpqi-fix-smp_processor_id-call-trace-for-preemptible-kernels

The other two patches add PCI-IDs for new controllers and change the
driver version.

This set of changes consists of:

* smartpqi-take-drives-offline-when-controller-is-offline
     On rare occasions, the controller can lock up and the driver was
     removing the controller instance from OS but leaving the
     drives exposed and their state was still 'running'.

     This patch sets the drive state as 'offline' to avoid confusion.
* smartpqi-add-new-pci_ids
     Add support for more PCI devices.

During a controller lockup, the physical and logical drives under the
locked up controller are still listed at the OS level. I.e. the
controller is offline but the status of each drive is 'running'.

When the controller is unexpectedly taken offline, show its drives as
offline.

8808c36b48 scsi: lpfc: Notify FC transport of rport disappearance during PCI fcn reset
A PCI function reset implies a temporary disappearance of a fc_rport.
So, call lpfc_scsi_dev_block(), which sets all mapped fc_rports into the
temporary FC_PORTSTATE_BLOCKED state.  Once PCI function reset completes
and link reinitialized, the fc_rport is rediscovered and
FC_PORTSTATE_BLOCKED state is removed.

In lpfc_check_sli_ndlp(), the get_job_els_rsp64_did remote_id assignment
does not apply for GEN_REQUEST64 commands as it only has meaning for a
ELS_REQUEST64 command.  So, if (iocb->ndlp == ndlp) is false, we could
erroneously return the wrong value.  Fix by replacing the fallthrough
statement with a break statement before the remote_id check.

8fe743b5eb PCI: Add CONFIG_MMU dependency
It turns out that there are no platforms that have PCI but don't have an
MMU, so adding a Kconfig dependency on CONFIG_PCI simplifies build testing
kernels for those platforms a lot, and avoids a lot of inadvertent build
regressions.

Add a dependency for CONFIG_PCI and remove all the ones for PCI specific
device drivers that are currently marked not having it.

There are a few platforms that have an optional MMU, but they usually
cannot have PCI at all. The one exception is Coldfire MCF54xx, but this is
mainly for historic reasons, and anyone using those chips should really use
the MMU these days.

14a3cc7558 scsi: Improve CDL control
With ATA devices supporting the CDL feature, using CDL requires that the
feature be enabled with a SET FEATURES command. This command is issued
as the translated command for the MODE SELECT command issued by
scsi_cdl_enable() when the user enables CDL through the device
cdl_enable sysfs attribute.

However, the implementation of scsi_cdl_enable() always issues a MODE
SELECT command for ATA devices when the enable argument is true, even if
CDL is already enabled on the device. While this does not cause any
issue with using CDL descriptors with read/write commands (the CDL
feature will be enabled on the drive), issuing the MODE SELECT command
even when the device CDL feature is already enabled will cause a reset
of the ATA device CDL statistics log page (as defined in ACS, any CDL
enable action must reset the device statistics).

Avoid this needless actions (and the implied statistics log page reset)
by modifying scsi_cdl_enable() to issue the MODE SELECT command to
enable CDL if and only if CDL is not reported as already enabled on the
device.

And while at it, simplify the initialization of the is_ata boolean
variable and move the declaration of the scsi mode data and sense header
variables to within the scope of ATA device handling.

1ca57644e2 scsi: hisi_sas: Wait until error handling is complete
SATA devices are lost when FLR is performed while the controller and disks
are in suspended state.

This is because the libata layer is called to initialize the SATA device
during controller resume. If FLR is executed at this time, the IDENTIFY
command fails. As a result, the revalidate fails, and the SATA device is
disabled by the libata layer.

Wait until error handling completes.

e4d953ca55 scsi: hisi_sas: Call I_T_nexus after soft reset for SATA disk
In commit 21c7e97247 ("scsi: hisi_sas: Disable SATA disk phy for severe
I_T nexus reset failure"), if the softreset fails upon certain
conditions, the PHY connected to the disk is disabled directly. Manual
recovery is required, which is inconvenient for users in actual use.

In addition, SATA disks do not support simultaneous connection of multiple
hosts. Therefore, when multiple controllers are connected to a SATA disk
at the same time, the controller which is connected later failed to issue
an ATA softreset to the SATA disk. As a result, the PHY associated with
the disk is disabled and cannot be automatically recovered.

Now that, we will not focus on the execution result of softreset. No
matter whether the execution is successful or not, we will directly carry
out I_T_nexus_reset.

54bebe4687 scsi: core: Clear flags for scsi_cmnd that did not complete
Commands that have not been completed with scsi_done() do not clear the
SCMD_INITIALIZED flag and therefore will not be properly reinitialized.
Thus, the next time the scsi_cmnd structure is used, the command may
fail in scsi_cmd_runtime_exceeded() due to the old jiffies_at_alloc
value:

When a character array without a terminating NUL character has a static
initializer, GCC 15's -Wunterminated-string-initialization will only
warn if the array lacks the "nonstring" attribute[1]. Mark the arrays

string" and thereby eliminate the warning.

ae82eaf4ae scsi: lpfc: Use memcpy() for BIOS version
The strlcat() with FORTIFY support is triggering a panic because it
thinks the target buffer will overflow although the correct target
buffer size is passed in.

Anyway, instead of memset() with 0 followed by a strlcat(), just use
memcpy() and ensure that the resulting buffer is NULL terminated.

BIOSVersion is only used for the lpfc_printf_log() which expects a
properly terminated string.

cdcb08d571 scsi: elx: sli4: Replace deprecated strncpy() with strscpy()
strncpy() is deprecated for NUL-terminated destination buffers; use
strscpy() instead.

Since sli_config_cmd_init() already zeroes out the destination buffers,
the potential NUL-padding by strncpy() is unnecessary. strscpy() copies
only the required characters and guarantees NUL-termination.

And since all three destination buffers have a fixed length, strscpy()
automatically determines their size using sizeof() when the argument is
omitted. This makes any explicit sizeof() calls unnecessary.

The source strings are also NUL-terminated and meet the __must_be_cstr()
requirement of strscpy().

No functional changes intended.

Devices of several types can be created for a single host. The module
device type should be used only when the devices are created.

Scsi_scan sets the device type initially to 0xff and sets the correct
type based in Inquiry results. This means that Inquiry must report
sdebug_ptype as long as scsi_device->type is not set (the limit 32
comes from the 5-bit length of the Peripheral Device Type in Inquiry).

A firmware bug was observed where ATA VPD inquiry commands with a
zero-length data payload were not handled and failed with a non-standard
status code of 0xf0.

Avoid sending ATA VPD inquiry commands without data payload by setting
the device no_vpd_size flag to 1. In addition, if the firmware returns a
status code of 0xf0, set scsi_cmnd->result to CHECK_CONDITION to
facilitate proper error handling.

a2d5a00722 scsi: smartpqi: Use is_kdump_kernel() to check for kdump
The smartpqi driver checks the reset_devices variable to determine
whether special adjustments need to be made for kdump. This has the
effect that after a regular kexec reboot, some driver parameters such as
max_transfer_size are much lower than usual. More importantly, kexec
reboot tests have revealed memory corruption caused by the driver log
being written to system memory after a kexec.

Fix this by testing is_kdump_kernel() rather than reset_devices where
appropriate.

hrtimers are initialized with hrtimer_init() and a subsequent store to
   the callback pointer. This turned out to be suboptimal for the upcoming
   Rust integration and is obviously a silly implementation to begin with.
 
   This cleanup replaces the hrtimer_init(T); T->function = cb; sequence
   with hrtimer_setup(T, cb);
 
   The conversion was done with Coccinelle and a few manual fixups.
 
   Once the conversion has completely landed in mainline, hrtimer_init()

daff37f00c scsi: hisi_sas: Fix I/O errors caused by hardware port ID changes
The hw port ID of phy may change when inserting disks in batches, causing
the port ID in hisi_sas_port and itct to be inconsistent with the hardware,
resulting in I/O errors. The solution is to set the device state to gone to
intercept I/O sent to the device, and then execute linkreset to discard and
find the disk to re-update its information.

8aa580cd92 scsi: hisi_sas: Enable force phy when SATA disk directly connected
when a SATA disk is directly connected the SAS controller determines the
disk to which I/Os are delivered based on the port ID in the DQ entry.

When many phys are disconnected and reconnect, the port ID of phys were
changed and used by other link, resulting in I/O being sent to incorrect
disk. Data inconsistency on the SATA disk may occur during I/O retries
using the old port ID. So enable force phy, then force the command to be
executed in a certain phy, and if the actual phy ID of the port does not
match the phy configured in the command, the chip will stop delivering the
I/O to disk.

8db816c6f1 scsi: st: Tighten the page format heuristics with MODE SELECT
In the days when SCSI-2 was emerging, some drives did claim SCSI-2 but did
not correctly implement it. The st driver first tries MODE SELECT with the
page format bit set to set the block descriptor.  If not successful, the
non-page format is tried.

The test only tests the sense code and this triggers also from illegal
parameter in the parameter list. The test is limited to "old" devices and
made more strict to remove false alarms.

040492ac25 scsi: lpfc: Restore clearing of NLP_UNREG_INP in ndlp->nlp_flag
Commit 32566a6f1a ("scsi: lpfc: Remove NLP_RELEASE_RPI flag from nodelist
structure") introduced a regression with SLI-3 adapters (e.g. LPe12000 8Gb)
where a Link Down / Link Up such as caused by disabling an host FC switch
port would result in the devices remaining in the transport-offline state
and multipath reporting them as failed.  This problem was not seen with
newer SLI-4 adapters.

The problem was caused by portions of the patch which removed the functions
__lpfc_sli_rpi_release() and lpfc_sli_rpi_release() and all their callers.
This was presumably because with the removal of the NLP_RELEASE_RPI flag
there was no need to free the rpi.

However, __lpfc_sli_rpi_release() and lpfc_sli_rpi_release() which calls it
reset the NLP_UNREG_INP flag. And, lpfc_sli_def_mbox_cmpl() has a path
where __lpfc_sli_rpi_release() was called in a particular case where
NLP_UNREG_INP was not otherwise cleared because of other conditions.

Restoring the else clause of this conditional and simply clearing the
NLP_UNREG_INP flag appears to resolve the problem with SLI-3 adapters.  It
should be noted that the code path in question is not specific to SLI-3,
but there are other SLI-4 code paths which may have masked the issue.

750d4fbe2c scsi: hisi_sas: Fixed failure to issue vendor specific commands
At present, we determine the protocol through the cmd type, but other cmd
types, such as vendor-specific commands, default to the PIO protocol.  This
strategy often causes the execution of different vendor-specific commands
to fail. In fact, for these commands, a better way is to use the protocol
configured by the command's tf to determine its protocol.

7dcbda8a1d scsi: storvsc: Don't report the host packet status as the hv status
The log statement reports the packet status code as the hv status code
which causes confusion when debugging as "hv" might refer to a hypervisor,
and sometimes to the host part of the Hyper-V virtualization stack.

Fix the name of the datum being logged to clearly indicate the component
reporting the error. Also log it in hexadecimal everywhere for consistency.

0711f1966a scsi: mpt3sas: Fix buffer overflow in mpt3sas_send_mctp_passthru_req()
The "sz" argument in mpt3sas_check_cmd_timeout() is the number of u32, not
the number of bytes.  We dump that many u32 values to dmesg.  Passing the
number of bytes will lead to a read overflow.  Divide by 4 to get the
correct value.

Commit b35108a51c ("jiffies: Define secs_to_jiffies()") introduced
secs_to_jiffies().  As the value here is a multiple of 1000, use
secs_to_jiffies() instead of msecs_to_jiffies() to avoid the multiplication

This is converted using scripts/coccinelle/misc/secs_to_jiffies.cocci with
the following Coccinelle rules:

ac0fb4a55b scsi: scsi_debug: Do not sleep in atomic sections
Function stop_qc_helper() is called while the debug_scsi_cmd lock is held,
and from here we may call cancel_work_sync(), which may sleep.

Sleeping in atomic sections is not allowed.

Hence change the cancel_work_sync() call into a cancel_work() call.

However now it is not possible to know if the work callback is running when
we return. This is relevant for eh_abort_handler handling, as the semantics
of that callback are that success means that we do not keep a reference to
the scsi_cmnd - now this is not possible. So return FAIL when we are unsure
if the callback still running.

Simplify command handling by moving struct sdebug_defer into the private
SCSI command data instead of allocating it separately. The only functional
change is that aborting a SCSI command now fails and is retried at a later
time if the completion handler can't be cancelled.

This structure is not used, so delete it.

It was originally intended for supporting checking for atomic writes
overlapping with ongoing reads and writes, but that support never got
added.

SBC-4 r22 section 4.29.3.2 "Performing operations during an atomic write
operation" describes two methods of handling overlapping atomic writes.
Currently the only method supported is for the ongoing read or write to
complete.

flag and start reply processing. When exiting the routine, the driver
resets the flag. However, there is a race condition when one thread (Thread
1) has processed replies and is about to reset the flag but in the meantime
few more replies are posted and another thread (Thread 2) is called to
process replies. Since the synchronization flag is still set, Thread 2 will
return without processing replies and those new replies will not be
flushed.

Make the watchdog thread monitor cases where admin ISR/poll call returns
due to another thread processing admin replies. If such an instance is
found, make driver call admin ISR to drain replies (if any).

The driver issues the time stamp update command periodically. Even if the
command fails with supervisor only IOC Status.

Instead check the Non-Supervisor capability bit reported by IOC as part of
IOC Facts.

Currently, the scsi_debug driver can create tape devices and the st
driver attaches to those. Nothing much can be done with the tape devices
because scsi_debug does not have support for the tape-specific commands
and features. These patches add some more tape support to the scsi_debug
driver. The end result is simulated drives with a tape having one or two
partitions (one partition is created initially).

The tape is implemented as fixed number (10 000) of 8-byte units.
The first four bytes of a unit contain the type of the unit (data
block, filemark or end-of-data mark). If the units is a data block,
the first four bytes also contain the block length and the remaining
four bytes the first bytes of written data. This allows the user
to use tags to see that the read block is what it was supposed to be.

aee1bf155d scsi: lpfc: rework lpfc_next_{online,present}_cpu()
lpfc_next_online_cpu() opencodes cpumask_next_and_wrap() by using
a for-loop. Use it and make the lpfc_next_online_cpu() a plain
one-liner.

While there, rework lpfc_next_present_cpu() similarly. Notice that
cpumask_next() followed by cpumask_first() in the worst case of an
empty mask may traverse the mask twice. Cpumask_next_wrap() takes
care of that correctly.

dc5bb9b769 cpumask: deprecate cpumask_next_wrap()
The next patch aligns implementation of cpumask_next_wrap() with the
find_next_bit_wrap(), and it changes function signature.

To make the transition smooth, this patch deprecates current
implementation by adding an _old suffix. The following patches switch
current users to the new implementation one by one.

No functional changes were intended.

23f4e82bb9 scsi: scsi_debug: Add support for partitioning the tape
This patch adds support for MEDIUM PARTITION PAGE in MODE SELECT and the
FORMAT MEDIUM command for tapes. After these additions, the virtual tape
can be partitioned containing either one or two partitions. The POFM flag
in the mode page is set, meaning that the FORMAT MEDIUM command must be
used to create the partitioning defined in the mode page.
