Use all data disks for sequential read operations.
Testing discovered inconsistent performance on RAID 10 volumes when
performing 256K sequential reads. The driver was only using a single
tracker to determine which physical drive to send a request to for AIO
requests.
Change the single tracker (next_bypass_group) to an array of trackers based
on the number of data disks in a row of the RAID map.

Avoid dropping into shell if the controller is in locked up state.
Driver issues SIS soft reset to bring back the controller to SIS mode while
OS boots into kdump mode.
If the controller is in lockup state, SIS soft reset does not work.
Since the controller lockup code has not been cleared, driver considers the
firmware is no longer up and running. Driver returns back an error code to
OS and the kdump fails.

After modifying logical volume size, lsblk command still shows previous
size of logical volume.
When the driver gets any event from firmware it schedules rescan worker
with delay of 10 seconds. If array expansion is so quick that it gets
completed in a second, the driver could not catch logical volume expansion
due to worker delay.
Since driver does not detect volume expansion, driver would not call
rescan device to update new size to the OS.

On certain systems (based on PCI IDs), when the OS transitions the system
into the suspend (S3) state, the BMIC flush cache command will indicate a
system RESTART instead of SUSPEND.
This avoids drive spin-down.

Change PQI_HZ to HZ.
PQI_HZ macro was set to 1000 when HZ value is less than 1000. By default,
PQI_HZ will result into a delay of 10 seconds(for kernel, which has HZ =
100). So in this case when firmware raises an event, rescan worker will be
scheduled after a delay of (10 x PQI_HZ) = 100 seconds instead of 10
seconds.
Also driver uses PQI_HZ at many instances, which might result in some other
issues with respect to delay.

Use correct pqi_aio_path_request structure to calculate the offset to
sg_descriptors.
The function pqi_aio_submit_io() uses the pqi_raid_path_request structure
to calculate the offset of the structure member sg_descriptors. This is
incorrect. It should be using the pqi_aio_path_request structure instead.
This typo is benign because the offsets are the same in both structures.

Return DID_NO_CONNECT when a path failure is detected.
When a path fails during I/O and AIO path gets disabled for a multipath
device, the I/O was retried in the RAID path slowing down path fail
detection. Returning DID_NO_CONNECT allows multipath to switch paths more
quickly.

Add device attribute 'sas_ncq_prio_enable' to enable SATA NCQ priority
support and provide I/O priority in SCSI command and pass priority
information to controller firmware.
This device attribute works only when device has NCQ priority support and
controller firmware can handle I/Os with NCQ priority attribute.

Stop the OS from re-discovering multiple LUNs for tape drive and medium
changer.
Duplicate device nodes for Ultrium tape drive and medium changer are being
created.
The Ultrium tape drive is a multi-LUN SCSI target. It presents a LUN for
the tape drive and a 2nd LUN for the medium changer. Our controller FW
lists both LUNs in the RPL results.
As a result, the smartpqi driver exposes both devices to the OS. Then the
OS does its normal device discovery via the SCSI REPORT LUNS command, which
causes it to re-discover both devices a 2nd time, which results in the
duplicate device nodes.

Prevent kernel crash by failing outstanding I/O request when the OS takes
device offline.
When posted I/Os to the controller's inbound queue are not picked by the
controller, the driver will halt the controller and take the controller
offline.
When the driver takes the controller offline, the driver will fail all the
outstanding requests which can sometimes lead to an OS crash.

Send a TEST UNIT READY to HBA disks and do not present them to the OS if
0x02/0x04/0x1b (SANITIZE IN PROGRESS) is returned.
During boot-up, some OSes appear to hang when there are one or more disks
undergoing a sanitize operation.
According to SCSI SBC4 specification section 4.11.2 "Commands allowed
during SANITIZE", some SCSI commands are permitted, but read/write
operations are not.
When the OS attempts to read the disk partition table a CHECK CONDITION ASC
0x04 ASCQ 0x1b is returned which causes the OS to retry the read until
SANITIZE has completed. This can take hours.
According to document HPE Smart Storage Administrator User Guide, during
the sanitize erase operation, the drive is unusable. I.e. the expected
behavior for SANITIZE is the that disk remains offline even after SANITIZE
has completed. The customer is expected to re-enable the disk using the
management utility.

Enhance check for commands queued to the controller. Add new function
pqi_nonempty_inbound_queue_count() that will wait for all I/O queued for
submission to controller across all queue groups to drain. Add helper
functions to obtain queue command counts for each queue group. These
queues should drain quickly as they are already staged to be submitted down
to the controller's IB queue.
Enhance check for outstanding command completion. Update the count of
outstanding commands while waiting. This value was not re-obtained and was
potentially causing infinite wait for all completions.

describing why the controller was halted. Store this reason code in a
controller register to aid in debugging the issue.

Correct kdump hangs when controller is locked up.
There are occasions when a controller reboot (controller soft reset) is
issued when a controller firmware crash dump is in progress.
This leads to incomplete controller firmware crash dump:
- When the controller crash dump is in progress, and a kdump is initiated,
the driver issues inbound doorbell reset to bring back the controller in
SIS mode.
- If the controller is in locked up state, the inbound doorbell reset does
not work causing controller initialization failures. This results in the
driver hanging waiting for SIS mode.
To avoid an incomplete controller crash dump, add in a controller crash
dump handshake:
- Controller will indicate start and end of the controller crash dump by
setting some register bits.
- Driver will look these bits when a kdump is initiated. If a controller
crash dump is in progress, the driver will wait for the controller crash
dump to complete before issuing the controller soft reset then complete
driver initialization.

Correct driver's ISR accessing a data structure member that has not been
fully initialized during driver initialization.
The pqi queue groups can have uninitialized members when an interrupt
fires. This has not resulted in any driver crashes. This was found during
our own internal testing. No bugs were ever filed.

Dan Carpenter found a possible divide by 0 issue in the smartpqi driver in
functions pci_get_aio_common_raid_map_values() and pqi_calc_aio_r5_or_r6().
The variable rmd->blocks_per_row is used as a divisor and could be 0.
Using rmd->blocks_per_row as a divisor without checking
it for 0 first.
Correct these possible divide by 0 conditions by insuring that
rmd->blocks_per_row is not zero before usage. The check for non-0 was too
late to prevent a divide by 0 condition. Add in a comment to explain why
the check for non-zero is necessary. If the member is 0, return
PQI_RAID_BYPASS_INELIGIBLE before any division is performed.

hibernation/resume cycle. Rare condition involving OFA.

Update enclosure identifier field corresponding to physical devices in
lsscsi/sysfs.
During device add the SCSI devtype is filled in during slave_configure().
However, when pqi_scsi_update_device() runs (REGNEWD) the firmware returns
zero for the SCSI devtype field, and valid devtype is overwritten by
zero. Due to this, lsscsi output shows wrong values.

LUN resets can take longer to complete. Adding in more driver logging helps
show where the driver is in the reset process.
Add a timeout in pqi_device_wait_for_pending_io() to cap how long the
driver will wait for outstanding commands.

- Synchronize OFA and controller offline events. Prevent I/O during the
above conditions.
- Cleanup pqi_device_wait_for_pending_io() by checking the
device->scsi_cmds_outstanding instead of walking the device's list of
commands.
- Stop failing all I/O for all devices. This was causing OS to retry them,
delaying OFA.
- Clean up cache flush. The controller is checked for offline status in
lower level functions.

Change return type from EINPROGRESS to EBUSY to signal applications to
retry a REGNEWD if the driver cannot process the REGNEWD. Events such as
OFA, suspend, and shutdown return EINPROGRESS if a scan is currently
running. This prevents applications from immediately retrying REGNEWD.
Schedule a new REGNEWD if system low on memory.

Simplify AIO retry management by removing retry list and list
management. Need to retry is already set in the response status. Also
remove the bypass worker thread.
Accelerated I/O requests bypass the RAID engine and go directly to either
an HBA disk or to a physical component of a RAID volume.

Cleanup soft reset code for Online Firmware Activation (OFA). OFA allows
controller firmware updates without a reboot.
OFA updates require an on-line controller reset to activate the updated
firmware. There were some missing actions for some of the reset cases. The
controller is first set back to sis mode before returning to pqi mode.
Check to ensure the controller is in sis mode.
Release QRM memory (OFA buffer) on OFA error conditions. Clean up
controller state which can cause a kernel panic upon reboot after an
unsuccessful OFA.

Remove timeouts for driver-initiated commands. Responses to internal
requests can take longer than hard coded timeout values and the driver will
still have an outstanding request that may complete in the future with no
context.

Enhance performance by adding sequential stream detection for RAID5/RAID6
sequential write requests. Reduce stripe lock contention with full-stripe
write operations.
There is one common stripe lock for each RAID volume that can be set by
either the RAID engine or the AIO engine. The AIO path has I/O request
sizes well below the stripe size resulting in many Read-Modify-Write
operations.
Sending the request to the RAID engine allows for coalescing requests into
full stripe operations resulting in reduced Read-Modify-Write operations.

While failing queued I/Os in TMF path, there was a request leak and hence
stale entries in request pool with ref count being non-zero. In shutdown
path we have a BUG_ON to catch stuck I/O either in firmware or in the
driver. The stale requests caused a system crash. The I/O request pool
leakage also lead to a significant performance drop.

MODULE_SUPPORTED_DEVICE was added in pre-git era and never was
implemented. We can safely remove it, because the kernel has grown
to have many more reliable mechanisms to determine if device is
supported or not.

Correct rmmod hangs when using HBA disks with write cache enabled.
Do not set controller flag "in_shutdown" during rmmod. SCSI SYNCHRONIZE
CACHE(10) and SCSI SYNCHRONIZE CACHE(16) requests were blocked with
SCSI_MLQUEUE_HOST_BUSY.

The PCI config accessors (pci_read_config_word(), et al) return
PCIBIOS_SUCCESSFUL (zero) or positive error values like
PCIBIOS_FUNC_NOT_SUPPORTED.
The PCIe capability accessors (pcie_capability_read_word(), et al)
similarly return PCIBIOS errors, but some callers assume they return
generic errno values like -EINVAL.
For example, the Myri-10G probe function returns a positive PCIBIOS error
if the pcie_capability_clear_and_set_word() in pcie_set_readrq() fails:

A positive return from a PCI driver probe function would cause a "Driver
probe function unexpectedly returned" warning from local_pci_probe()
instead of the desired probe failure.

Since commit 84af7a6194 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.
This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.
There are a variety of indentation styles found.

c) 8 spaces + '---help---'

When each ld is deleted, a rescan event is triggered in the driver. These
can stack up waiting on mutex_lock.
Change to mutex_try_lock and schedule a rescan for later.

Basically, the fact that the coherent DMA mask value wasn't set caused the
driver to fall back to SWIOTLB when SME is active.
For correct operation, lets call the dma_set_mask_and_coherent() to
properly set the mask for both streaming and coherent, in order to inform
the kernel about the devices DMA addressing capabilities.

When a HARDWARE_ERROR is triggered for ASC=0x3e, the existing code is only
considering the case where ASCQ=0x1.
According to the http://www.t10.org/lists/asc-num.htm#ASC_3E specification,
other values may occur like a timeout (ASCQ=0x2).
This patch prints an error message when a non-handled message is received.
This can help diagnose a possible misbehavior of the controller or a
missing implementation in the Linux kernel.
This patch keeps the exact same error handling but prints a message if an
ASCQ != 1 is reported.

When the HARDWARE_ERROR/0x3e/0x1 case is triggered, the logical volume
is offlined. When reading the kernel log, the reason why the device
got offlined isn't reported to the user. This situation makes it
difficult for admins to root cause.
Log a message when this condition occurs.

The root cause is that the _IOC macro can generate really large numbers,
which don't fit into type 'int', which is used for the cmd parameter in
the ioctls in scsi_host_template. My research into how GCC and Clang are
handling this at a low level didn't prove fruitful. However, looking at
the rest of the kernel tree, all ioctls use an 'unsigned int' for the
cmd parameter, which will fit all of the _IOC values in the scsi/ata
subsystems.
Make that change because none of the ioctls expect a negative value for
any command, it brings the ioctls inline with the reset of the kernel,
and it removes ambiguity, which is never good when dealing with compilers.

I was able to build without these extra header search paths.
Especially, the header search path -I. in kernel Makefiles is always
suspicious; it allows the compiler to search for headers in the top of
$(srctree), where obviously no header file exists.

- when OFA event occurs, driver will stop traffic to RAID/HBA path. Driver
waits for all the outstanding requests to complete.
- Driver sends OFA event acknowledgment to firmware.
- Driver will wait until the new firmware is up and running.
- Driver will free up the resources.
- Driver calls SIS/PQI initialization and rescans the device list.
- Driver will resume the traffic to RAID/HBA path.

Problem:
- during the driver initialization, driver will poll fw
for KERNEL_UP in a 30 seconds timeout.
- if the firmware is not ready after 30 seconds,
driver will not be loaded.
Fix:
- change timeout from 30 seconds to 3 minutes.

Problem:
The Linux kernel takes a logical volume offline after a LUN reset. This is
generally accompanied by this message in the dmesg output:
Device offlined - not ready after error recovery
Root Cause:
The root cause is a "quirk" in the timeout handling in the Linux SCSI
layer. The Linux kernel places a 30-second timeout on most media access
commands (reads and writes) that it send to device drivers. When a media
access command times out, the Linux kernel goes into error recovery mode
for the LUN that was the target of the command that timed out. Every
command that timed out is kept on a list inside of the Linux kernel to be
retried later. The kernel attempts to recover the command(s) that timed out
by issuing a LUN reset followed by a TEST UNIT READY. If the LUN reset and
TEST UNIT READY commands are successful, the kernel retries the command(s)
that timed out.
Each SCSI command issued by the kernel has a result field associated with
it. This field indicates the final result of the command (success or
error). When a command times out, the kernel places a value in this result
field indicating that the command timed out.
The "quirk" is that after the LUN reset and TEST UNIT READY commands are
completed, the kernel checks each command on the timed-out command list
before retrying it. If the result field is still "timed out", the kernel
treats that command as not having been successfully recovered for a
retry. If the number of commands that are in this state are greater than
two, the kernel takes the LUN offline.
Fix:
When our RAIDStack receives a LUN reset, it simply waits until all
outstanding commands complete. Generally, all of these outstanding commands
complete successfully. Therefore, the fix in the smartpqi driver is to
always set the command result field to indicate success when a request
completes successfully. This normally isn’t necessary because the result
field is always initialized to success when the command is submitted to the
driver. So when the command completes successfully, the result field is
left untouched. But in this case, the kernel changes the result field
behind the driver’s back and then expects the field to be changed by the
driver as the commands that timed-out complete.

- fix race condition when a unit is deleted after an RLL,
and before we have gotten the LV_STATUS page of the unit.
- In this case we will get a standard inquiry, rather than
the desired page. This will result in a unit presented
which no longer exists.
- If we ask for LV_STATUS, insure we get LV_STATUS

- set allow_restart option during scsi_device init.
