BLK_STS_NEXUS is used for NVMe/SCSI reservation conflicts and DASD's
locking feature which works similar to NVMe/SCSI reservations where a
host can get a lock on a device and when the lock is taken it will get
failures.
This patch renames BLK_STS_NEXUS so it better reflects this type of
use.

The DASD driver does not kick the requeue list when requeuing IO requests
to the blocklayer. This might lead to hanging blockdevice when there is
no other trigger for this.
Fix by automatically kick the requeue list when requeuing DASD requests
to the blocklayer.

Add a sysfs attribute aq_timeouts that controls after how many
timeouts a autoquiesce event might be triggered.
The default value is 32768 which is the maximum number of retries
for the DASD device driver DASD_RETRIES_MAX. This means that the
timeout trigger will never happen.
The default value for DASD retries is 255.
Setting the value to below 255 will trigger the timeout autoquiesce
event before an IO error is generated.
Also add the check for the configured amount of timeouts and trigger
an autoquiesce event if exceeded.

Add a sysfs attribute to control if all IO requests will be requeued to
the blocklayer in case of an autoquiesce event or not.
A value of 1 means that in case of an autoquiesce event all IO requests
will be requeued to the blocklayer.
A value of 0 means that the device will only be stopped.

Add sysfs attribute that controls the DASD autoquiesce feature.
The autoquiesce is disabled when 0 is echoed to the attribute.
A value greater than 0 will enable the feature.
The aq_mask attribute will accept an unsigned integer and the value
will be interpreted as bitmask defining the trigger events that will
lead to an automatic quiesce.
The following autoquiesce triggers will currently be available:
DASD_EER_FATALERROR 1 - any final I/O error
DASD_EER_NOPATH 2 - no remaining paths for the device
DASD_EER_STATECHANGE 3 - a state change interrupt occurred
DASD_EER_PPRCSUSPEND 4 - the device is PPRC suspended
DASD_EER_NOSPC 5 - there is no space remaining on an ESE device
DASD_EER_TIMEOUT 6 - a certain amount of timeouts occurred
DASD_EER_STARTIO 7 - the IO start function encountered an error
The currently supported maximum value is 255.
Bit 31 is reserved for internal usage.
Bit 0 is not used.

Add the internal logic to check for autoquiesce triggers and handle
them.
Quiesce and resume are functions that tell Linux to stop/resume
issuing I/Os to a specific DASD.
The DASD driver allows a manual quiesce/resume via ioctl.
Autoquiesce will define an amount of triggers that will lead to
an automatic quiesce if a certain event occurs.
There is no automatic resume.
All events will be reported via DASD Extended Error Reporting (EER)
if configured.

For DASD devices in raw_track_access mode only full track images are
read and written.
For this purpose it is not necessary to do search operation in the
locate record extended function. The documentation even states that
this might fail if the searched record is not found on a track.
Currently the driver sets a value of 1 in the search field for the first
record after record zero. This is the default for disks not in
raw_track_access mode but record 1 might be missing on a completely
empty track.
There has not been any problem with this on IBM storage servers but it
might lead to errors with DASD devices on other vendors storage servers.
Fix this by setting the search field to 0. Record zero is always available
even on a completely empty track.

32 byte are to less for important data from prefix or
other commands.
Print up to 128 byte data. This is enough for the largest
CCW data we have.
Since printk can only print up to 1024 byte at once, print the
different parts of the CCW dumps separately.

After the rework from commit 1ebe2e5f9d ("block: remove
GENHD_FL_EXT_DEVT"), when calling device_add_disk(), dcssblk will end up
in disk_scan_partitions(), and not break out early w/o GENHD_FL_NO_PART.
This will trigger implicit open/release via blkdev_get/put_whole()
later. dcssblk_release() will then deadlock on dcssblk_devices_sem
semaphore, which is already held from dcssblk_add_store() when calling
device_add_disk().
dcssblk does not support partitions (DCSSBLK_MINORS_PER_DISK == 1), and
never scanned partitions before. Therefore restore the previous
behavior, and explicitly disallow partition scanning by setting the
GENHD_FL_NO_PART flag. This will also prevent this deadlock scenario.

Add a function to check if a device is accessible.
This makes mostly sense for copy pair secondary devices but it will work
for all devices.
The sysfs attribute ping is a write only attribute and will issue a NOP
CCW to the device.
In case of success it will return zero. If the device is not accessible
it will return an error code.

Suppress generic command reject messages and dump of sense data for
Peer-To-Peer-Remote-Copy (PPRC) secondary errors.

The newly defined ioctl BIODASDCOPYPAIRSWAP takes a structure that
specifies a copy pair that should be swapped. It will call the device
discipline function to perform the swap operation.
The structure looks as followed:

In case of errors or misbehaviour of the primary device a controlled
failover to one of the configured secondary devices needs to be
performed.
The swap processing stops I/O on the primary device, all requests are
re-queued to the blocklayer queue, the entries in the copy relation are
swapped and finally the link to the blockdevice is moved from primary to
secondary dasd device.
After this, the secondary becomes the new primary device and I/O is
restarted on that device.

A copy relation that is configured on the storage server side needs to be
enabled separately in the device driver. A sysfs interface is created
that allows userspace tooling to control such setup.
The following sysfs entries are added to store and read copy relation
information:
copy_pair
- Add/Delete a copy pair relation to the DASD device driver
- Query all previously added copy pair relations
copy_role
- Query the copy pair role of the device
To add a copy pair to the DASD device driver it has to be specified
through the sysfs attribute copy_pair. Only one secondary device can be
specified at a time together with the primary device. Both, secondary
and primary can be used equally to define the copy pair.
The secondary devices have to be offline when adding the copy relation.
The primary device needs to be specified first followed by the comma
separated secondary device.
Read from the copy_pair attribute to get the current setup and write
"clear" to the attribute to delete any existing setup.
Example:

$ cat /sys/bus/ccw/devices/0.0.9700/copy_pair

During device online processing the required data will be read from the
storage server and the information will be compared to the setup
requested through the copy_pair attribute. The registration of the
primary and secondary device will be handled accordingly.
A blockdevice is only allocated for copy relation primary devices.
To query the copy role of a device read from the copy_role sysfs
attribute. Possible values are primary, secondary, and none.

Fix Oops in dasd_alias_get_start_dev() function caused by the pavgroup
pointer being NULL.
The pavgroup pointer is checked on the entrance of the function but
without the lcu->lock being held. Therefore there is a race window
between dasd_alias_get_start_dev() and _lcu_update() which sets
pavgroup to NULL with the lcu->lock held.
Fix by checking the pavgroup pointer with lcu->lock held.

linux-next commit bf8d08532b ("iomap: add support for dma aligned
direct-io") changes the alignment requirement to come from the block
device rather than the block size, and the default alignment
requirement is 512-byte boundaries. Since DASD I/O has page
alignments for IDAW/TIDAW requests, let's override this value to
restore the expected behavior.
Make this change for both ECKD and DIAG disciplines, as they both
would fall into this category. Leave FBA alone, since it is always
comprised of 512-byte blocks.

The double indirect bio leads to somewhat suboptimal code generation.
Instead return the (original or split) bio, and make sure the
request_queue arguments to the lower level helpers is passed after the
bio to avoid constant reshuffling of the argument passing registers.
Also give it and the helpers used to implement it more descriptive names.

With new API blk_mq_is_reserved_rq() we can tell if a request is from
the reserved pool, so stop passing 'reserved' arg. There is actually
only a single user of that arg for all the callback implementations, which
can use blk_mq_is_reserved_rq() instead.
This will also allow us to stop passing the same 'reserved' around the
blk-mq iter functions next.

Set the queue dying flag and call blk_mq_exit_queue from del_gendisk for
all disks that do not have separately allocated queues, and thus remove
the need to call blk_cleanup_queue for them.
Rename blk_cleanup_disk to blk_mq_destroy_queue to make it clear that
this function is intended only for separately allocated blk-mq queues.
This saves an extra queue freeze for devices without a separately
allocated queue.

Up till now, dax_direct_access() is used implicitly for normal
access, but for the purpose of recovery write, dax range with
poison is requested. To make the interface clear, introduce

Read requests that return with NRF error are partially completed in
dasd_eckd_ese_read(). The function keeps track of the amount of
processed bytes and the driver will eventually return this information
back to the block layer for further processing via __dasd_cleanup_cqr()
when the request is in the final stage of processing (from the driver's
perspective).
For this, blk_update_request() is used which requires the number of
bytes to complete the request. As per documentation the nr_bytes
parameter is described as follows:
"number of bytes to complete for @req".
This was mistakenly interpreted as "number of bytes _left_ for @req"
leading to new requests with incorrect data length. The consequence are
inconsistent and completely wrong read requests as data from random
memory areas are read back.
Fix this by correctly specifying the amount of bytes that should be used
to complete the request.

When reading unformatted tracks on ESE devices, the corresponding memory
areas are simply set to zero for each segment. This is done incorrectly
for blocksizes < 4096.
There are two problems. First, the increment of dst is done using the
counter of the loop (off), which is increased by blksize every
iteration. This leads to a much bigger increment for dst as actually
intended. Second, the increment of dst is done before the memory area
is set to 0, skipping a significant amount of bytes of memory.
This leads to illegal overwriting of memory and ultimately to a kernel
panic.
This is not a problem with 4k blocksize because
blk_queue_max_segment_size is set to PAGE_SIZE, always resulting in a
single iteration for the inner segment loop (bv.bv_len == blksize). The
incorrectly used 'off' value to increment dst is 0 and the correct
memory area is used.
In order to fix this for blksize < 4k, increment dst correctly using the
blksize and only do it at the end of the loop.

For ESE devices we get an error for write operations on an unformatted
track. Afterwards the track will be formatted and the IO operation
restarted.
When using alias devices a track might be accessed by multiple requests
simultaneously and there is a race window that a track gets formatted
twice resulting in data loss.
Prevent this by remembering the amount of formatted tracks when starting
a request and comparing this number before actually formatting a track
on the fly. If the number has changed there is a chance that the current
track was finally formatted in between. As a result do not format the
track and restart the current IO to check.
The number of formatted tracks does not match the overall number of
formatted tracks on the device and it might wrap around but this is no
problem. It is only needed to recognize that a track has been formatted at
all in between.

For ESE devices we get an error when accessing an unformatted track.
The handling of this error will return zero data for read requests and
format the track on demand before writing to it. To do this the code needs
to distinguish between read and write requests. This is done with data from
the blocklayer request. A pointer to the blocklayer request is stored in
the CQR.
If there is an error on the device an ERP request is built to do error
recovery. While the ERP request is mostly a copy of the original CQR the
pointer to the blocklayer request is not copied to not accidentally pass
it back to the blocklayer without cleanup.
This leads to the error that during ESE handling after an ERP request was
built it is not possible to determine the IO direction. This leads to the
formatting of a track for read requests which might in turn lead to data
corruption.

The discard_alignment queue limit is named a bit misleading means the
offset into the block device at which the discard granularity starts.
Setting it to PAGE_SIZE while the discard granularity is the block size
that is smaller or the same as PAGE_SIZE as done by dasd is mostly
harmless but also useless.

Just use a non-zero max_discard_sectors as an indicator for discard
support, similar to what is done for write zeroes.
The only places where needs special attention is the RAID5 driver,
which must clear discard support for security reasons by default,

Follow arm64 and riscv and move the EX_TABLE define to asm-extable.h
which is a lot less generic than the current linkage.h.
Also make sure that all files which contain EX_TABLE usages actually
include the new header file. This should make sure that the files
always compile and there won't be any random compile breakage due to
other header file dependencies.

There are currently 2 ways to create a set of sysfs files for a
kobj_type, through the default_attrs field, and the default_groups
field. Move the s390 dasd sysfs code to use default_groups field
which has been the preferred way since commit aa30f47cf6 ("kobject:
Add support for default attribute groups to kobj_type") so that we can
soon get rid of the obsolete default_attrs field.

These methods indirect the actual DAX read/write path. In the end pmem
uses magic flush and mc safe variants and fuse and dcssblk use plain ones
while device mapper picks redirects to the underlying device.
Add set_dax_nocache() and set_dax_nomc() APIs to control which copy
routines are used to remove indirect call from the read/write fast path
as well as a lot of boilerplate code.

__dasd_device_check_path_events() calls the discipline path event handler.
This handler can leave the 'to be verified pathmask' populated for an
additional verification.
There is a race window where the worker has finished before
dasd_path_clear_all_verify() is called which resets the tbvpm.
Due to this there could be outstanding path verifications missed.
Fix by clearing the pathmasks before calling the handler and add them
again in case of an error.

dasd_eckd_path_available_action() does a memory allocation to store
the per path configuration data permanently.
In the unlikely case that this allocation fails there is no conf_data
stored for the corresponding path.
This is OK since this is not necessary for an operational path but some
features like control unit initiated reconfiguration (CUIR) do not work.
To fix this add the path to the 'to be verified pathmask' again and
schedule the handler again.

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.
Polling for the bio itself leads to a few advantages:
- the cookie construction can made entirely private in blk-mq.c
- the caller does not need to remember the request_queue and cookie
separately and thus sidesteps their lifetime issues
- keeping the device and the cookie inside the bio allows to trivially
support polling BIOs remapping by stacking drivers
- a lot of code to propagate the cookie back up the submission path can
be removed entirely.

Support for expanded storage was only available until z13 and z/VM 6.3
respectively. However there haven't been any use cases a long time
before for this device driver.

Here is the big set of driver core patches for 5.15-rc1.
These do change a number of different things across different
subsystems, and because of that, there were 2 stable tags created that
might have already come into your tree from different pulls that did the
following
- changed the bus remove callback to return void
- sysfs iomem_get_mapping rework
The latter one will cause a tiny merge issue with your tree, as there
was a last-minute fix for this in 5.14 in your tree, but the fixup
should be "obvious". If you want me to provide a fixed merge for this,
please let me know.
Other than those two things, there's only a few small things in here:
- kernfs performance improvements for huge numbers of sysfs
users at once
- tiny api cleanups
- other minor changes
All of these have been in linux-next for a while with no reported
problems, other than the before-mentioned merge issue.

Most drivers should use and have been converted to use blk_alloc_disk
and blk_mq_alloc_disk. Only the scsi ULPs and dasd still allocate
a disk separately from the request_queue, so don't bother with
convenience macros for something that should not see significant
new users and remove these wrappers.

When new configuration data is obtained after a path event it is stored
in the per path array. The old data needs to be freed.
The first valid configuration data is also referenced in the device
private structure to identify the device.
When the old per path configuration data was freed the device still
pointed to the already freed data leading to a use after free.
Fix by replacing also the device configuration data with the newly
obtained one before the old data gets freed.

The driver core ignores the return value of scmdev_remove()
(because there is only little it can do when a device disappears).
So make it impossible for future drivers to return an unused error code
by changing the remove prototype to return void.
The real motivation for this change is the quest to make struct
bus_type::remove return void, too.

In preparation for FORTIFY_SOURCE performing compile-time and run-time
field array bounds checking for memcpy(), memmove(), and memset(),
avoid intentionally reading across neighboring array fields.
Add a wrapping structure to serve as the memcpy() source, so the compiler
can do appropriate bounds checking, avoiding this future warning:

Using register asm statements has been proven to be very error prone,
especially when using code instrumentation where gcc may add function
calls, which clobbers register contents in an unexpected way.
Therefore get rid of register asm statement in dasd code, even though
there is currently nothing wrong with it. This way we know for sure
that the above mentioned bug class won't be introduced here.

Fix crash with illegal operation exception in dasd_device_tasklet.
Commit b729493288 ("s390/dasd: Prepare for additional path event handling")
renamed the verify_path function for ECKD but not for FBA and DIAG.
This leads to a panic when the path verification function is called for a
FBA or DIAG device.
Fix by defining a wrapper function for dasd_generic_verify_path().

Prevent that an IO request is build during device shutdown initiated by
a driver unbind. This request will never be able to be processed or
canceled and will hang forever. This will lead also to a hanging unbind.
Fix by checking not only if the device is in READY state but also check
that there is no device offline initiated before building a new IO request.

In case of an unbind of the DASD device driver the function
dasd_generic_remove() is called which shuts down the device.
Among others this functions removes the int_handler from the cdev.
During shutdown the device cancels all outstanding IO requests and waits
for completion of the clear request.
Unfortunately the clear interrupt will never be received when there is no
interrupt handler connected.
Fix by moving the int_handler removal after the call to the state machine
where no request or interrupt is outstanding.

Our intention was to only remove path kobjects whenever a device is
being set offline. However, one corner case was missing.
If a device is disabled and enabled (using the IOCTLs BIODASDDISABLE and
BIODASDENABLE respectively), the enabling process will call
dasd_eckd_reload_device() which itself calls dasd_eckd_read_conf() in
order to update path information. During that update,
dasd_eckd_clear_conf_data() clears all old data and also removes all
kobjects. This will leave us with an inconsistent state of path kobjects
and a subsequent path verification leads to a failing kobject creation.
Fix this by removing kobjects only in the context of offlining a device
as initially intended.

Replace the gendisk pointer in struct bio with a pointer to the newly
improved struct block device. From that the gendisk can be trivially
accessed with an extra indirection, but it also allows to directly
look up all information related to partition remapping.

In dasd_alias_disconnect_device_from_lcu the device is removed from any
list on the LCU. Afterwards the LCU is removed from the lcu list if it
does not contain devices any longer.
The lcu->lock protects the lcu from parallel updates. But to cancel all
workers and wait for completion the lcu->lock has to be unlocked.
If two devices are removed in parallel and both are removed from the LCU
the first device that takes the lcu->lock again will delete the LCU because
it is already empty but the second device also tries to free the LCU which
leads to a list corruption of the lcu list.
Fix by removing the device right before the lcu is checked without
unlocking the lcu->lock in between.

dasd_alias_add_device() moves devices to the active_devices list in case
of a scheduled LCU update regardless if they have previously been in a
pavgroup or not.
Example: device A and B are in the same pavgroup.
Device A has already been in a pavgroup and the private->pavgroup pointer
is set and points to a valid pavgroup. While going through dasd_add_device
it is moved from the pavgroup to the active_devices list.
In parallel device B might be removed from the same pavgroup in
remove_device_from_lcu() which in turn checks if the group is empty
and deletes it accordingly because device A has already been removed from
there.
When now device A enters remove_device_from_lcu() it is tried to remove it
from the pavgroup again because the pavgroup pointer is still set and again
the empty group will be cleaned up which leads to a list corruption.
Fix by setting private->pavgroup to NULL in dasd_add_device.
If the device has been the last device on the pavgroup an empty pavgroup
remains but this will be cleaned up by the scheduled lcu_update which
iterates over all existing pavgroups.

Prevent _lcu_update from adding a device to a pavgroup if the LCU still
requires an update. The data is not reliable any longer and in parallel
devices might have been moved on the lists already.
This might lead to list corruptions or invalid PAV grouping.
Only add devices to a pavgroup if the LCU is up to date. Additional steps
are taken by the scheduled lcu update.

For an LCU update a read unit address configuration IO is required.
This is started using sleep_on(), which has early exit paths in case the
device is not usable for IO. For example when it is in offline processing.
In those cases the LCU update should fail and not be retried.
Therefore lcu_update_work checks if EOPNOTSUPP is returned or not.
Commit 41995342b4 ("s390/dasd: fix endless loop after read unit address configuration")
accidentally removed the EOPNOTSUPP return code from
read_unit_address_configuration(), which in turn might lead to an endless
loop of the LCU update in offline processing.
Fix by returning EOPNOTSUPP again if the device is not able to perform the
request.

Now that the hd_struct always has a block device attached to it, there is
no need for having two size field that just get out of sync.
Additionally the field in hd_struct did not use proper serialization,
possibly allowing for torn writes. By only using the block_device field
this problem also gets fixed.

When requeueing all requests on the device request queue to the blocklayer
we might get to an ERP (error recovery) request that is a copy of an
original CQR.
Those requests do not have blocklayer request information or a pointer to
the dasd_queue set. When trying to access those data it will lead to a
null pointer dereference in dasd_requeue_all_requests().
Fix by checking if the request is an ERP request that can simply be
ignored. The blocklayer request will be requeued by the original CQR that
is on the device queue right behind the ERP request.

If the Fibre Channel Endpoint-Security status of a path changes, a
corresponding path event is received from the CIO layer.
Process this event by re-reading the FCES information.
As the information is retrieved for all paths on a single CU in one
call, the internal status can also be updated for all paths and no
processing per path is necessary.

Add a new sysfs attribute (fc_security) per device and per operational
channel path. The information of the current FC Endpoint Security state
is received through the CIO layer.
The state of the FC Endpoint Security can be either "Unsupported",
"Authentication", or "Encryption".
For example:
$ cat /sys/bus/ccw/devices/0.0.c600/fc_security
Encryption
If any of the operational paths is in a state different from all
others, the device sysfs attribute will display the additional state
"Inconsistent".
The sysfs attributes per paths are organised in a new directory called
"paths_info" with subdirectories for each path.

During online processing and setting up a DASD device, the configuration
data for operational paths is read and validated two times
(dasd_eckd_read_conf()). The first time to provide information that are
necessary for the LCU setup. A second time after the LCU setup as a
device might report different configuration data then.
When the configuration setup for each operational path is being
validated, an initial call to dasd_eckd_clear_conf_data() is issued.
This call wipes all previously available configuration data and path
information for each path.
However, the operational path mask is not updated during this process.
As a result, the stored operational path mask might no longer correspond
to the operational paths mask reported by the CIO layer, as several
paths might be gone between the two dasd_eckd_read_conf() calls.
This inconsistency leads to more severe issues in later path handling
changes. Fix this by removing the channel paths from the operational
path mask during the dasd_eckd_clear_conf_data() call.

Currently, the configuration data for a path is retrieved during a path
verification and used only temporarily. If a path is newly added to the
I/O setup after a boot, no configuration data will be stored for this
particular path.
However, this data is required for later use and should be present for
a valid I/O path anyway. Store this data during the path verification so
that newly added paths can provide all information necessary.

For storing retrieved path information both the if and else block in
dasd_eckd_read_conf() use the same code. To avoid duplicate code this
should be done after the if/else block. To further increase readability,
move the code to a new function, dasd_eckd_store_conf_data().

A discard request that writes zeros using the global kernel internal
ZERO_PAGE will fail for machines with more than 2GB of memory due to the
location of the ZERO_PAGE.
Fix this by using a driver owned global zero page allocated with GFP_DMA
flag set.

Two different callers use two different mutexes for updating the
block device size, which obviously doesn't help to actually protect
against concurrent updates from the different callers. In addition
one of the locks, bd_mutex is rather prone to deadlocks with other
parts of the block stack that use it for high level synchronization.
Switch to using a new spinlock protecting just the size updates, as
that is all we need, and make sure everyone does the update through
the proper helper.
This fixes a bug reported with the nvme revalidating disks during a
hot removal operation, which can currently deadlock on bd_mutex.

During initialization of the DASD DIAG driver a request is issued
that has a bio structure that resides on the stack. With virtually
mapped kernel stacks this bio address might be in virtual storage
which is unsuitable for usage with the diag250 call.
In this case the device can not be set online using the DIAG
discipline and fails with -EOPNOTSUP.
In the system journal the following error message is presented:
dasd: X.X.XXXX Setting the DASD online with discipline DIAG failed

Fix by allocating the bio structure instead of having it on the stack.

bd_block_size contains a value that matches the logic block size when
opening, so the statement is redundant. Even if it wasn't the dumb
assignment would cause a a mismatch with bd_inode->i_blkbits.

The make_request_fn is a little weird in that it sits directly in
struct request_queue instead of an operation vector. Replace it with
a block_device_operations method called submit_bio (which describes much
better what it does). Also remove the request_queue argument to it, as
the queue can be derived pretty trivially from the bio.

Move the call to blk_should_fake_timeout out of blk_mq_complete_request
and into the drivers, skipping call sites that are obvious error
handlers, and remove the now superflous blk_mq_force_complete_rq helper.
This ensures we don't keep injecting errors into completions that just
terminate the Linux request after the hardware has been reset or the
command has been aborted.

The IBM partition parser requires device type specific information only
available to the DASD driver to correctly register partitions. The
current approach of using ioctl_by_bdev with a fake user space pointer
is discouraged.
Fix this by replacing IOCTL calls with direct in-kernel function calls.

Use the blk_drop_partitions function instead of messing around with
ioctls that get kernel pointers. For this blk_drop_partitions needs
to be exported, which it normally shouldn't - make an exception for
s390 only.

and setting of the scheduler was removed with

fix compatibility across architectures and PowerPC page size
configurations.
- Introduce 'zero_page_range' as a dax operation. This facilitates
filesystem-dax operation without a block-device.
