It is reported that on some systems pciehp binds to an Upstream Port and
attempts to operate it which causes devices below the Port to disappear
from the bus.
This happens because acpiphp sets dev->is_hotplug_bridge for that Port
(after receiving a Device Check notification on it from the platform
firmware via ACPI) during the enumeration of PCI devices.
get_port_device_capability() sees that dev->is_hotplug_bridge is set and
adds PCIE_PORT_SERVICE_HP to Port services, which allows pciehp to bind to
the Port in question.
Even though this particular problem can be addressed by making the
portdrv_core checks more robust, it also causes power management to work
differently on the affected systems which generally is not desirable (PCIe
Ports with dev->is_hotplug_bridge set have to pass additional tests to be
allowed to go into the D3hot/cold power states which affects runtime PM of
devices below these Ports).
For this reason, amend check_hotplug_bridge() with a PCIe type check to
prevent it from setting dev->is_hotplug_bridge for Upstream Ports.

The Qualcomm PCI bridge device (Device ID 0x0110) found in chipsets such as
SM8450 does not set the Command Completed bit unless writes to the Slot
Command register change "Control" bits.
This results in timeouts like below:

Writes to a Downstream Port's Slot Control register are PCIe hotplug
"commands." If the Port supports Command Completed events, software must
wait for a command to complete before writing to Slot Control again.
pcie_do_write_cmd() sets ctrl->cmd_busy when it writes to Slot Control. If
software notification is enabled, i.e., PCI_EXP_SLTCTL_HPIE and
PCI_EXP_SLTCTL_CCIE are set, ctrl->cmd_busy is cleared by pciehp_isr().
But when software notification is disabled, as it is when pcie_init()
powers off an empty slot, pcie_wait_cmd() uses pcie_poll_cmd() to poll for
command completion, and it neglects to clear ctrl->cmd_busy, which leads to
spurious timeouts:

This lockdep warning is triggered because with Thunderbolt, hotplug ports
are nested. When removing multiple devices in a daisy-chain, each hotplug
port's reset_lock may be acquired recursively. It's never the same lock, so
the lockdep splat is a false positive.
Because locks at the same hierarchy level are never acquired recursively, a
per-level lockdep class is sufficient to fix the lockdep warning.
The choice to use one lockdep subclass per pcie-hotplug controller in the
path to the root-bus was made to conserve class keys because their number
is limited and the complexity grows quadratically with number of keys
according to Documentation/locking/lockdep-design.rst.

The Power Fault Detected bit in the Slot Status register differs from
all other hotplug events in that it is sticky: It can only be cleared
after turning off slot power. Per PCIe r5.0, sec. 6.7.1.8:
If a power controller detects a main power fault on the hot-plug slot,
it must automatically set its internal main power fault latch [...].
The main power fault latch is cleared when software turns off power to
the hot-plug slot.
The stickiness used to cause interrupt storms and infinite loops which
were fixed in 2009 by commits 5651c48cfa ("PCI pciehp: fix power fault
interrupt storm problem") and 99f0169c17 ("PCI: pciehp: enable
software notification on empty slots").
Unfortunately in 2020 the infinite loop issue was inadvertently
reintroduced by commit 8edf5332c3 ("PCI: pciehp: Fix MSI interrupt
race"): The hardirq handler pciehp_isr() clears the PFD bit until
pciehp's power_fault_detected flag is set. That happens in the IRQ
thread pciehp_ist(), which never learns of the event because the hardirq
handler is stuck in an infinite loop. Fix by setting the
power_fault_detected flag already in the hardirq handler.

returned to the CPU is PCI_ERROR_RESPONSE (~0 or 0xffffffff).
Obviously a successful PCI config read may *also* return that data if a
config register happens to contain ~0, so it doesn't definitively indicate
an error unless we know the register cannot contain ~0.
Use PCI_POSSIBLE_ERROR() to check the response we get when we read data
from hardware. This unifies PCI error response checking and makes error
checks consistent and easier to find.

This is done by adding a zpci_hot_reset_device() call which does a low
level reset of the PCI function without changing its higher level
function state. This way it can be used while the zPCI function is bound
to a driver and with DMA tables being controlled either through the
IOMMU or DMA APIs which is prohibited when using zpci_disable_device()
as that drop existing DMA translations.
As this reset, unlike a normal FLR, also calls zpci_clear_irq() we need
to implement arch_restore_msi_irqs() and make sure we re-enable IRQs for
the PCI function if they were previously disabled.

Stuart Hayes reports that an error handled by DPC at a Root Port results
in pciehp gratuitously bringing down a subordinate hotplug port:
RP -- UP -- DP -- UP -- DP (hotplug) -- EP
pciehp brings the slot down because the Link to the Endpoint goes down.
That is caused by a Hot Reset being propagated as a result of DPC.
Per PCIe Base Spec 5.0, section 6.6.1 "Conventional Reset":
For a Switch, the following must cause a hot reset to be sent on all
Downstream Ports: [...]
* The Data Link Layer of the Upstream Port reporting DL_Down status.
In Switches that support Link speeds greater than 5.0 GT/s, the
Upstream Port must direct the LTSSM of each Downstream Port to the
Hot Reset state, but not hold the LTSSMs in that state. This permits
each Downstream Port to begin Link training immediately after its
hot reset completes. This behavior is recommended for all Switches.
* Receiving a hot reset on the Upstream Port.
Once DPC recovers, pcie_do_recovery() walks down the hierarchy and
invokes pcie_portdrv_slot_reset() to restore each port's config space.
At that point, a hotplug interrupt is signaled per PCIe Base Spec r5.0,
section 6.7.3.4 "Software Notification of Hot-Plug Events":
If the Port is enabled for edge-triggered interrupt signaling using
MSI or MSI-X, an interrupt message must be sent every time the logical
AND of the following conditions transitions from FALSE to TRUE: [...]
* The Hot-Plug Interrupt Enable bit in the Slot Control register is
set to 1b.
* At least one hot-plug event status bit in the Slot Status register
and its associated enable bit in the Slot Control register are both
set to 1b.
Prevent pciehp from gratuitously bringing down the slot by clearing the
error-induced Data Link Layer State Changed event before restoring
config space. Afterwards, check whether the link has unexpectedly
failed to retrain and synthesize a DLLSC event if so.
Allow each pcie_port_service_driver (one of them being pciehp) to define
a slot_reset callback and re-use the existing pm_iter() function to
iterate over the callbacks.
Thereby, the Endpoint driver remains bound throughout error recovery and
may restore the device to working state.
Surprise removal during error recovery is detected through a Presence
Detect Changed event. The hotplug port is expected to not signal that
event as a result of a Hot Reset.
The issue isn't DPC-specific, it also occurs when an error is handled by
AER through aer_root_reset(). So while the issue was noticed only now,
it's been around since 2006 when AER support was first introduced.

Since commit 2a671f77ee ("s390/pci: fix use after free of zpci_dev")
the reference count of a zpci_dev is incremented between
pcibios_add_device() and pcibios_release_device() which was supposed to
prevent the zpci_dev from being freed while the common PCI code has
access to it. It was missed however that the handling of zPCI
availability events assumed that once zpci_zdev_put() was called no
later availability event would still see the device. With the previously
mentioned commit however this assumption no longer holds and we must
make sure that we only drop the initial long-lived reference the zPCI
subsystem holds exactly once.
Do so by introducing a zpci_device_reserved() function that handles when
a device is reserved. Here we make sure the zpci_dev will not be
considered for further events by removing it from the zpci_list.
This also means that the device actually stays in the
ZPCI_FN_STATE_RESERVED state between the time we know it has been
reserved and the final reference going away. We thus need to consider it
a real state instead of just a conceptual state after the removal. The
final cleanup of PCI resources, removal from zbus, and destruction of
the IOMMU stays in zpci_release_device() to make sure holders of the
reference do see valid data until the release.

Downstream Port Containment (PCIe r5.0, sec. 6.2.10) disables the link upon
an error and attempts to re-enable it when instructed by the DPC driver.
A slot which is both DPC- and hotplug-capable is currently powered off by
pciehp once DPC is triggered (due to the link change) and powered back up
on successful recovery. That's undesirable, the slot should remain powered
so the hotplugged device remains bound to its driver. DPC notifies the
driver of the error and of successful recovery in pcie_do_recovery() and
the driver may then restore the device to working state.
Moreover, Sinan points out that turning off slot power by pciehp may foil
recovery by DPC: Power off/on is a cold reset concurrently to DPC's warm
reset. Sathyanarayanan reports extended delays or failure in link
retraining by DPC if pciehp brings down the slot.
Fix by detecting whether a Link Down event is caused by DPC and awaiting
recovery if so. On successful recovery, ignore both the Link Down and the
subsequent Link Up event.
Afterwards, check whether the link is down to detect surprise-removal or
another DPC event immediately after DPC recovery. Ensure that the
corresponding DLLSC event is not ignored by synthesizing it and invoking
irq_wake_thread() to trigger a re-run of pciehp_ist().
The IRQ threads of the hotplug and DPC drivers, pciehp_ist() and
dpc_handler(), race against each other. If pciehp is faster than DPC, it
will wait until DPC recovery completes.
Recovery consists of two steps: The first step (waiting for link
disablement) is recognizable by pciehp through a set DPC Trigger Status
bit. The second step (waiting for link retraining) is recognizable through
a newly introduced PCI_DPC_RECOVERING flag.
If DPC is faster than pciehp, neither of the two flags will be set and
pciehp may glean the recovery status from the new PCI_DPC_RECOVERED flag.
The flag is zero if DPC didn't occur at all, hence DLLSC events are not
ignored by default.
pciehp waits up to 4 seconds before assuming that DPC recovery failed and
bringing down the slot. This timeout is not taken from the spec (it
doesn't mandate one) but based on a report from Yicong Yang that DPC may
take a bit more than 3 seconds on HiSilicon's Kunpeng platform.
The timeout is necessary because the DPC Trigger Status bit may never
clear: On Root Ports which support RP Extensions for DPC, the DPC driver
polls the DPC RP Busy bit for up to 1 second before giving up on DPC
recovery. Without the timeout, pciehp would then wait indefinitely for DPC
to complete.

The sysfs_emit() and sysfs_emit_at() functions were introduced to make
it less ambiguous which function is preferred when writing to the output
buffer in a device attribute's "show" callback [1].
Convert the PCI sysfs object "show" functions from sprintf(), snprintf()
and scnprintf() to sysfs_emit() and sysfs_emit_at() accordingly, as the
latter is aware of the PAGE_SIZE buffer and correctly returns the number
of bytes written into the buffer.
No functional change intended.

With zpci_configure_device() now always called on a device that has
already been configured on the platform level its name has become
misleading. Rename it to zpci_scan_configured_device() to signify that
the function now only handles the correct scanning of a newly configured
PCI function taking care of the special handling necessary for function
0 and functions parked waiting for a PCI bus that can't be created
without first seeing function 0.

Currently zpci_configure_device() can be called on a zPCI function in
two completely different states. Either the underlying zPCI function has
already been configured by the platform and we are only doing the
scanning to get it usable by Linux drivers. Or the underlying function
is in Standby and we first do an SCLP to get it configured. This makes
zpci_configure_device() harder to reason about. Since calling
zpci_configure_device() on a function in Standby only happens in
enable_slot() simply pull out the SCLP call and setting of zdev->state
and thus call zpci_configure_device() under the same circumstances as
in the event handling code.

In enable_slot(), if pci_get_slot() returns NULL, we clear the SLOT_ENABLED
flag. When pci_get_slot() finds a device, it increments the device's
reference count. In this case, we did not call pci_dev_put() to decrement
the reference count, so the memory of the device (struct pci_dev type) will
eventually leak.
Call pci_dev_put() to decrement its reference count when pci_get_slot()
returns a PCI device.

A zPCI event with PEC 0x0301 for an existing zPCI device goes through
the same actions as enable_slot(). Similarly a zPCI event with PEC
0x0303 does the same steps as disable_slot().
We can thus unify both actions as zpci_configure_device() respectively
zpci_deconfigure_device().

The current zdev->state mixes the configuration states supported by CLP
with an additional Online state which is used inconsistently to include
enabled zPCI functions which are not yet visible to the common PCI
subsytem. In preparation for a clean separation between architected
configuration states and fine grained function states remove the Online
function state.
Where we previously checked for Online it is more accurate to check if
the function is enabled to avoid an edge case where a disabled device
was still treated as Online. This also simplifies checks whether
a function is configured as this is now directly reflected by its
function state.

Both add_slot_store() and remove_slot_store() try to fix up the
drc_name copied from the store buffer by placing a NUL terminator at
nbyte + 1 or in place of a '\n' if present. However, the static buffer
that we copy the drc_name data into is not zeroed and can contain
anything past the n-th byte.
This is problematic if a '\n' byte appears in that buffer after nbytes
and the string copied into the store buffer was not NUL terminated to
start with as the strchr() search for a '\n' byte will mark this
incorrectly as the end of the drc_name string resulting in a drc_name
string that contains garbage data after the n-th byte.
Additionally it will cause us to overwrite that '\n' byte on the stack
with NUL, potentially corrupting data on the stack.
The following debugging shows an example of the drmgr utility writing
"PHB 4543" to the add_slot sysfs attribute, but add_slot_store()
logging a corrupted string value.

Fix this by using strscpy() instead of memcpy() to ensure the string
is NUL terminated when copied into the static drc_name buffer.
Further, since the string is now NUL terminated the code only needs to
change '\n' to '\0' when present.

In commit 05bc1be6db ("s390/pci: create zPCI bus") we removed the
pci_dev_put() call matching the earlier pci_get_slot() done as part of
__zpci_event_availability(). This was based on the wrong understanding
that the device_put() done as part of pci_destroy_device() would counter
the pci_get_slot() when it only counters the initial reference. This
same understanding and existing bad example also lead to not doing
a pci_dev_put() in zpci_remove_device().
Since releasing the PCI devices, unlike releasing the PCI slot, does not
print any debug message for testing I added one in pci_release_dev().
This revealed that we are indeed leaking the PCI device on PCI
hotunplug. Further testing also revealed another missing pci_dev_put() in
disable_slot().
Fix this by adding the missing pci_dev_put() in disable_slot() and fix
zpci_remove_device() with the correct pci_dev_put() calls. Also instead
of calling pci_get_slot() in __zpci_event_availability() to determine if
a PCI device is registered and then doing the same again in
zpci_remove_device() do this once in zpci_remove_device() which makes
sure that the pdev in __zpci_event_availability() is only used for the
result of pci_scan_single_device() which does not need a reference count
decremnt as its ownership goes to the PCI bus.
Also move the check if zdev->zbus->bus is set into zpci_remove_device()
since it may be that we're removing a device with devfn != 0 which never
had a PCI bus. So we can still set the pdev->error_state to indicate
that the device is not usable anymore, add a flag to set the error state.

When a PCIe card is hot-removed, the Presence Detect State and Data Link
Layer Link Active bits often do not clear simultaneously. I've seen delays
of up to 244 msec between the two events with Thunderbolt.
After pciehp has brought down the slot in response to the first event, the
other bit may still be set. It's not discernible whether it's set because
a new card is already in the slot or if it will soon clear. So pciehp
tries to bring up the slot and in the latter case fails with a bunch of
messages, some of them at KERN_ERR severity. If the slot is no longer
occupied, the messages are false positives and annoy users.

Users are particularly irritated to see a bringup attempt even though the
slot was explicitly brought down via sysfs. In a perfect world, we could
avoid this by setting Link Disable on slot bringdown and re-enabling it
upon a Presence Detect State change. In reality however, there are broken
hotplug ports which hardwire Presence Detect to zero, see 80696f9914
("PCI: pciehp: Tolerate Presence Detect hardwired to zero"). Conversely,
PCIe r1.0 hotplug ports hardwire Link Active to zero because Link Active
Reporting wasn't specified before PCIe r1.1. On unplug, some ports first
clear Presence then Link (see Stuart Hayes' splat) whereas others use the
inverse order (see Dongdong Liu's splat). To top it off, there are hotplug
ports which flap the Presence and Link bits on slot bringup, see
6c35a1ac3d ("PCI: pciehp: Tolerate initially unstable link").
pciehp is designed to work with all of these variants. Surplus attempts at
slot bringup are a lesser evil than not being able to bring up slots at
all. Although we could try to perfect the behavior for specific hotplug
controllers, we'd risk breaking others or increasing code complexity.
But we can certainly minimize annoyance by emitting only a single message
with KERN_INFO severity if bringup is unsuccessful:
* Drop the "Timeout waiting for Presence Detect" message in
pcie_wait_for_presence(). The sole caller of that function,
pciehp_check_link_status(), ignores the timeout and carries on. It emits
error messages of its own and I don't think this particular message adds
much value.
* There's a single error condition in pciehp_check_link_status() which
does not emit a message. Adding one allows dropping the "Failed to check
link status" message emitted by board_added() if
pciehp_check_link_status() returns a non-zero integer.
* Tone down all messages in pciehp_check_link_status() to KERN_INFO
severity and rephrase them to look as innocuous as possible. To this
end, move the message emitted by pcie_wait_for_link_delay() to its
callers.
As a result, Stuart Hayes' splat becomes:

every removal of a zdev from the common PCI subsystem.
So in preparation re-introduce zpci_remove_device() and use that instead
of directly calling the common code functions. This was actually still
declared from earlier code but no longer implemented.

If context is not NULL in acpiphp_grab_context(), but the
is_going_away flag is set for the device's parent, the reference
counter of the context needs to be decremented before returning
NULL or the context will never be freed, so make that happen.

We allow multiple functions on a single bus.
We suppress the ZPCI_DEVFN definition and replace its
occurences with zpci->devfn.
We verify the number of device during the registration.
There can never be more domains in use than existing
devices, so we do not need to verify the count of domain
after having verified the count of devices.

The zPCI bus is in charge to handle common zPCI resources for
zPCI devices.
Creating the zPCI bus, the PCI bus, the zPCI devices and the
PCI devices and hotplug slots
done in a specific order:
- PCI hotplug slot creation needs a PCI bus
- PCI bus needs a PCI domain
which is reported by the pci_domain_nr() when setting up the
host bridge
- PCI domain is set from the zPCI with devfn 0
this is necessary to have a reproducible enumeration
Therefore we can not create devices or hotplug slots for any PCI
device associated with a zPCI device before having discovered
the function zero of the bus.
The discovery and initialization of devices can be done at several
points in the code:
- On Events, serialized in a thread context
- On initialization, in the kernel init thread context
- When powering on the hotplug slot, in a user thread context
The removal of devices and their parent bus may also be done on
events or for devices when powering down the slot.
To guarantee the existence of the bus and devices until they are
no more needed we use kref in zPCI bus and introduce a reference
count in the zPCI devices.
In this patch the zPCI bus still only accept a device with
a devfn 0.

Without this commit, a PCIe hotplug port can stop generating interrupts on
hotplug events, so device adds and removals will not be seen:
The pciehp interrupt handler pciehp_isr() reads the Slot Status register
and then writes back to it to clear the bits that caused the interrupt. If
a different interrupt event bit gets set between the read and the write,
pciehp_isr() returns without having cleared all of the interrupt event
bits. If this happens when the MSI isn't masked (which by default it isn't
in handle_edge_irq(), and which it will never be when MSI per-vector
masking is not supported), we won't get any more hotplug interrupts from
that device.
That is expected behavior, according to the PCIe Base Spec r5.0, section
6.7.3.4, "Software Notification of Hot-Plug Events".
Because the Presence Detect Changed and Data Link Layer State Changed event
bits can both get set at nearly the same time when a device is added or
removed, this is more likely to happen than it might seem. The issue was
found (and can be reproduced rather easily) by connecting and disconnecting
an NVMe storage device on at least one system model where the NVMe devices
were being connected to an AMD PCIe port (PCI device 0x1022/0x1483).
Fix the issue by modifying pciehp_isr() to loop back and re-read the Slot
Status register immediately after writing to it, until it sees that all of
the event status bits have been cleared.

David Hoyer reports that powering pciehp slots up or down via sysfs may
hang: The call to wait_event() in pciehp_sysfs_enable_slot() and
_disable_slot() does not return because ctrl->ist_running remains true.
This flag, which was introduced by commit 157c1062fc ("PCI: pciehp: Avoid
returning prematurely from sysfs requests"), signifies that the IRQ thread
pciehp_ist() is running. It is set to true at the top of pciehp_ist() and
reset to false at the end. However there are two additional return
statements in pciehp_ist() before which the commit neglected to reset the
flag to false and wake up waiters for the flag.
That omission opens up the following race when powering up the slot:
* pciehp_ist() runs because a PCI_EXP_SLTSTA_PDC event was requested
by pciehp_sysfs_enable_slot()
* pciehp_ist() turns on slot power via the following call stack:
pciehp_handle_presence_or_link_change() -> pciehp_enable_slot() ->
__pciehp_enable_slot() -> board_added() -> pciehp_power_on_slot()
* after slot power is turned on, the link comes up, resulting in a
PCI_EXP_SLTSTA_DLLSC event
* the IRQ handler pciehp_isr() stores the event in ctrl->pending_events

* after pciehp_ist() has successfully brought up the slot, it resets
ctrl->ist_running to false and wakes up the sysfs requester
* the genirq code re-runs pciehp_ist(), which sets ctrl->ist_running
to true but then returns with IRQ_NONE because ctrl->pending_events
is empty
* pciehp_sysfs_enable_slot() is finally woken but notices that
ctrl->ist_running is true, hence continues waiting
The only way to get the hung task going again is to trigger a hotplug
event which brings down the slot, e.g. by yanking out the card.
The same race exists when powering down the slot because remove_board()
likewise clears link or presence changes in ctrl->pending_events per commit
3943af9d01 ("PCI: pciehp: Ignore Link State Changes after powering off a
slot") and thereby may cause a re-run of pciehp_ist() which returns with
IRQ_NONE without resetting ctrl->ist_running to false.
Fix by adding a goto label before the teardown steps at the end of
pciehp_ist() and jumping to that label from the two return statements which
currently neglect to reset the ctrl->ist_running flag.

The eeh_ops->probe() function is called from two different contexts:
1. On pseries, where we set EEH_PROBE_MODE_DEVTREE, it's called in
eeh_add_device_early() which is supposed to run before we create
a pci_dev.
2. On PowerNV, where we set EEH_PROBE_MODE_DEV, it's called in
eeh_device_add_late() which is supposed to run *after* the
pci_dev is created.
The "early" probe is required because PAPR requires that we perform an RTAS
call to enable EEH support on a device before we start interacting with it
via config space or MMIO. This requirement doesn't exist on PowerNV and
shoehorning two completely separate initialisation paths into a common
interface just results in a convoluted code everywhere.
Additionally the early probe requires the probe function to take an pci_dn
rather than a pci_dev argument. We'd like to make pci_dn a pseries specific
data structure since there's no real requirement for them on PowerNV. To
help both goals move the early probe into the pseries containment zone
so the platform depedence is more explicit.

The pci hotplug helper (pci_hp_add_devices()) calls
eeh_add_device_tree_early() to scan the device-tree for new PCI devices and
do the early EEH probe before the device is scanned. This early probe is a
no-op in a lot of cases because:
a) The early init is only required to satisfy a PAPR requirement that EEH
be configured before we start doing config accesses. On PowerNV it is
a no-op.
b) It's a no-op for devices that have already had their eeh_dev
initialised.
There are four callers of pci_hp_add_devices():
1. arch/powerpc/kernel/eeh_driver.c
Here the hotplug helper is called when re-scanning pci_devs that
were removed during an EEH recovery pass. The EEH stat for each
removed device (the eeh_dev) is retained across a recovery pass
so the early init is a no-op in this case.
2. drivers/pci/hotplug/pnv_php.c
This is also a no-op since the PowerNV hotplug driver is, suprisingly,
PowerNV specific.
3. drivers/pci/hotplug/rpaphp_core.c
4. drivers/pci/hotplug/rpaphp_pci.c
In these two cases new devices have been hotplugged and FW has
provided new DT nodes for each. These are the only two cases where
the EEH we might have new PCI device nodes in the DT so these are
the only two cases where the early EEH probe needs to be done.
We can move the calls to eeh_add_device_tree_early() to the locations where
it's needed and remove it from the generic path. This is preparation for
making the early EEH probe pseries specific.

Some systems have in-band presence detection disabled for hot-plug PCI
slots but do not report this in the slot capabilities 2 (SLTCAP2) register.
On these systems, presence detect can become active well after the link is
reported to be active, which can cause the slots to be disabled after a
device is connected.
Add a DMI table to flag these systems as having in-band presence detect
disabled.

When in-band presence detect is disabled, PDS may come up at any time or
not at all. PDS being low may indicate that the card is still mating, and
we could expect contact bounce to bring down the link as well.
It is reasonable to assume that most cards will mate in a hotplug slot in
about a second. Thus, when we know PDS only reflects out-of-band presence
detect, it's worthwhile to wait the extra second or so to make sure the
card is properly mated before loading the driver and to prevent the hotplug
code from disabling a device if the presence detect change goes active
after the device is enabled.

The presence detect state (PDS) is normally a logical OR of in-band and
out-of-band (OOB) presence detect. As of PCIe 4.0, there is the option to
disable in-band presence so that the PDS bit always reflects the state of
the out-of-band presence.
The recommendation of the PCIe spec is to disable in-band presence whenever
supported (PCIe r5.0, appendix I implementation note):
Due to architectural issues, the in-band (Physical-Layer-based) portion
of the PD mechanism is deprecated for use with async hot-plug. One issue
is that in-band PD as architected does not detect adapter removal during
certain LTSSM states, notably the L1 and Disabled States. Another issue
is that when both in-band and OOB PD are being used together, the
Presence Detect State bit and its associated interrupt mechanism always
reflect the logical OR of the inband and OOB PD states, and with some
hot-plug hardware configurations, it is important for software to detect
and respond to in-band and OOB PD events independently. If OOB PD is
being used and the associated DSP supports In-Band PD Disable, it is
recommended that the In-Band PD Disable bit be Set, and the Presence
Detect State bit and its associated interrupt mechanism be used
exclusively for OOB PD. As a substitute for in-band PD with async
hot-plug, the reference model uses either the DPC or the DLL Link Active
mechanism.

An opencapi slot doesn't have an associated bridge device. It's not
needed for operation, but any warning is displayed through pci_warn()
which uses the pci_dev struct of the assocated bridge device. So wrap
those warning so that a different trace mechanism can be used if it's
an opencapi slot.

The driver only allows to disable a slot in the POPULATED
state. However, if an error occurs while enabling the slot, say
because the link couldn't be trained, then the POPULATED state may not
be reached, yet the power state of the slot is on. So allow to disable
a slot in the REGISTERED state. Removing the devices will do nothing
since it's not populated, and we'll set the power state of the slot
back to off.

When changing the slot state, if opal hits an error and tells as such
in the asynchronous reply, the warning "Wrong msg" is logged, which is
rather confusing. Instead we can reuse the better message which is
already used when we couldn't submit the asynchronous opal request
initially.

Valerio and others reported that commit 84c8b58ed3 ("ACPI / hotplug /
PCI: Don't scan bridges managed by native hotplug") prevents some recent
LG and HP laptops from booting with endless loop of:

What seems to happen is that during boot, after the initial PCI enumeration
when EC is enabled the platform triggers ACPI Notify() to one of the root
ports. The root port itself looks like this:

The BIOS has configured the root port so that it does not have I/O bridge
window.
Now when the ACPI Notify() is triggered ACPI hotplug handler calls
acpiphp_native_scan_bridge() for each non-hotplug bridge (as this system is
using native PCIe hotplug) and pci_assign_unassigned_bridge_resources() to
allocate resources.
The device connected to the root port is a PCIe switch (Thunderbolt
controller) with two hotplug downstream ports. Because of the hotplug ports
__pci_bus_size_bridges() tries to add "additional I/O" of 256 bytes to each
(DEFAULT_HOTPLUG_IO_SIZE). This gets further aligned to 4k as that's the
minimum I/O window size so each hotplug port gets 4k I/O window and the
same happens for the root port (which is also hotplug port). This means
3 * 4k = 12k I/O window.
Because of this pci_assign_unassigned_bridge_resources() ends up opening a
I/O bridge window for the root port at first available I/O address which
seems to be in range 0x1000 - 0x3fff. Normally this range is used for ACPI
stuff such as GPE bits (below is part of /proc/ioports):

1804-1805 : ACPI PM1a_CNT_BLK
1808-180b : ACPI PM_TMR
1810-1815 : ACPI CPU throttle
1850-1850 : ACPI PM2_CNT_BLK

However, when the ACPI Notify() happened this range was not yet reserved
for ACPI/PNP (that happens later) so PCI gets it. It then starts writing to
this range and accidentally stomps over GPE bits among other things causing
the endless stream of messages about missing GPE handler.
This problem does not happen if "pci=hpiosize=0" is passed in the kernel
command line. The reason is that then the kernel does not try to allocate
the additional 256 bytes for each hotplug port.
Fix this by allocating resources directly below the non-hotplug bridges
where a new device may appear as a result of ACPI Notify(). This avoids the
hotplug bridges and prevents opening the additional I/O window.

The newer ibm,drc-info property is a condensed description of the old
ibm,drc-* properties (ie. names, types, indexes, and power-domains).
When matching a drc-index to a drc-name we need to verify that the
index is within the start and last drc-index range and map it to a
drc-name using the drc-name-prefix and logical index.
Fix the mapping by checking that the index is within the range of the
current drc-info entry, and build the name from the drc-name-prefix
concatenated with the starting drc-name-suffix value and the sequential
index obtained by subtracting ibm,my-drc-index from this entries
drc-start-index.

The device tree is in big endian format and any properties directly
retrieved using OF helpers that don't explicitly byte swap should
be annotated. In particular there are several places where we grab
the opaque property value for the old ibm,drc-* properties and the
ibm,my-drc-index property.
Fix this for better static checking by annotating values we know to
explicitly big endian, and byte swap where appropriate.

In the event that the partition is migrated to a platform with older
firmware that doesn't support the ibm,drc-info property the device
tree is modified to remove the ibm,drc-info property and replace it
with the older style ibm,drc-* properties for types, names, indexes,
and power-domains. One of the requirements of the drc-info firmware
feature is that the client is able to handle both the new property,
and old style properties at runtime. Therefore we can't rely on the
firmware feature alone to dictate which property is currently
present in the device tree.
Fix this short coming by checking explicitly for the ibm,drc-info
property, and falling back to the older ibm,drc-* properties if it
doesn't exist.

The first entry of the ibm,drc-info property is an int encoded count
of the number of drc-info entries that follow. The "value" pointer
returned by of_prop_next_u32() is still pointing at the this value
when we call of_read_drc_info_cell(), but the helper function
expects that value to be pointing at the first element of an entry.
Fix up by incrementing the "value" pointer to point at the first
element of the first drc-info entry prior.

This addresses deadlocks in these common cases in hierarchies containing
two switches:
- All involved ports are runtime suspended and they are unplugged. This
can happen easily if the drivers involved automatically enable runtime
PM (xHCI for example does that).
- System is suspended (e.g., closing the lid on a laptop) with a dock +
something else connected, and the dock is unplugged while suspended.

What happens here is that the whole hierarchy is runtime resumed and the
parent PCIe downstream port, which got the hot-remove event, starts
removing devices below it, taking pci_lock_rescan_remove() lock. When the
child PCIe port is runtime resumed it calls pciehp_check_presence() which
ends up calling pciehp_card_present() and pciehp_check_link_active(). Both
of these use pcie_capability_read_word(), which notices that the underlying
device is already gone and returns PCIBIOS_DEVICE_NOT_FOUND with the
capability value set to 0. When pciehp gets this value it thinks that its
child device is also hot-removed and schedules its IRQ thread to handle the
event.
The deadlock happens when the child's IRQ thread runs and tries to acquire
pci_lock_rescan_remove() which is already taken by the parent and the
parent waits for the child's IRQ thread to finish.
Prevent this from happening by checking the return value of
pcie_capability_read_word() and if it is PCIBIOS_DEVICE_NOT_FOUND stop
performing any hot-removal activities.

We try to keep PCIe hotplug ports runtime suspended when entering system
suspend. Because the PCIe portdrv sets the DPM_FLAG_NEVER_SKIP flag, the PM
core always calls system suspend/resume hooks even if the device is left
runtime suspended. Since PCIe hotplug driver re-used the same function for
both runtime suspend and system suspend, it ended up disabling hotplug
interrupt twice and the second time following was printed:

Prevent this from happening by checking whether the device is already
runtime suspended when the system suspend hook is called.

Infinite timeout loops are hard to read. Refactor it to plausible 'do {}
while ()'.
Note, the supplied timeout can't be negative for current use, though if
it's not dividable to 10, we may go below 0, that's why type of the
parameter is int. And thus, we may move the check to the loop condition.

A sysfs request to enable or disable a PCIe hotplug slot should not
return before it has been carried out. That is sought to be achieved by
waiting until the controller's "pending_events" have been cleared.
However the IRQ thread pciehp_ist() clears the "pending_events" before
it acts on them. If pciehp_sysfs_enable_slot() / _disable_slot() happen
to check the "pending_events" after they have been cleared but while
pciehp_ist() is still running, the functions may return prematurely
with an incorrect return value.
Fix by introducing an "ist_running" flag which must be false before a sysfs
request is allowed to return.

Add pciehp_set_indicators() to set power and attention indicators with a
single register write.
This is a minor optimization because we frequently set both indicators and
this can do it with a single command. It also reduces the number of
interfaces related to the indicators and makes them more discoverable
because callers use the PCI_EXP_SLTCTL_ATTN_IND_* and
PCI_EXP_SLTCTL_PWR_IND_* definitions directly.

pnv_php is generally used with PCIe bridges which provide a native
interface for setting the attention and power indicator LEDs. Wire up
those interfaces even if firmware does not have support for them (yet...)

Currently we check that an IODA2 compatible PHB is upstream of this slot.
This is mainly to avoid pnv_php creating slots for the various "virtual
PHBs" that we create for NVLink. There's no real need for this restriction
so allow it on IODA3.

When performing EEH recovery of devices in a hotplug slot we need to use
the slot driver's ->reset_slot() callback to prevent spurious hotplug
events due to spurious DLActive and PresDet change interrupts. Add a
reset_slot() callback to pnv_php so we can handle recovery of devices
in pnv_php managed slots.

Previously pciehp debug messages were enabled by the pciehp_debug module
parameter, e.g., by booting with this kernel command line option:
pciehp.pciehp_debug=1
Convert this mechanism to use the generic dynamic debug (dyndbg) feature.
After this commit, pciehp debug messages are enabled by building the kernel
with CONFIG_DYNAMIC_DEBUG=y and booting with this command line option:
dyndbg="file pciehp* +p"
The dyndbg facility is much more flexible: messages can be enabled at boot-
or run-time based on the file name, function name, line number, message
test, etc. See Documentation/admin-guide/dynamic-debug-howto.rst for more
details.

When allocating the slot structure we store a pointer to the associated
device_node. We really should be incrementing the reference count, so add
an of_node_get() during slot alloc and an of_node_put() during slot
dealloc.

The find_dlpar_node() helper returns a device node with its reference
incremented. Both the add and remove paths use this helper for find the
appropriate node, but fail to release the reference when done.
Annotate the find_dlpar_node() helper with a comment about the incremented
reference count and call of_node_put() on the obtained device_node in the
add and remove paths. Also, fixup a reference leak in the find_vio_slot()
helper where we fail to call of_node_put() on the vdevice node after we
iterate over its children.

During a safe hot remove, the OS powers off the slot, which may cause a
Data Link Layer State Changed event. The slot has already been set to
OFF_STATE, so that event results in re-enabling the device, making it
impossible to safely remove it.
Clear out the Presence Detect Changed and Data Link Layer State Changed
events when the disabled slot has settled down.
It is still possible to re-enable the device if it remains in the slot
after pressing the Attention Button by pressing it again.
Fixes the problem that Micah reported below: an NVMe drive power button may
not actually turn off the drive.

Commit 0e157e5286 ("PCI/PME: Implement runtime PM callbacks") tried to
solve an issue where the hierarchy immediately wakes up when it is
transitioned into D3cold. However, it turns out to prevent PME
propagation on some systems that do not support D3cold.
I looked more closely at what might cause the immediate wakeup. It happens
when the ACPI power resource of the root port is turned off. The AML code
associated with the _OFF() method of the ACPI power resource starts a PCIe
L2/L3 Ready transition and waits for it to complete. Right after the L2/L3
Ready transition is started the root port receives a PME from the
downstream port.
The simplest hierarchy where this happens looks like this:

It seems that the PCIe link between the two switches, before
PME_Turn_Off/PME_TO_Ack is complete for the whole hierarchy, goes
inactive and triggers PME towards the root port bringing it back to D0.
The L2/L3 Ready sequence is described in PCIe r4.0 spec sections 5.2 and
5.3.3 but unfortunately they do not state what happens if DLLSCE is
enabled during the sequence.
Disabling Data Link Layer State Changed event (DLLSCE) seems to prevent
the issue and still allows the downstream hotplug port to notice when a
device is plugged/unplugged.

The sem_exit variable is conceptually a completion, so it should be called
that.
Similarly, the semOperations semaphore is a simple mutex, and can be
changed into that, respectively.
With both converted, the ibmphp_hpc_initvars() function is no longer used
and can be removed.

The issue was bisected down to commit 720d6a671a ("PCI: pciehp: Do not
handle events if interrupts are masked") and was further analyzed by the
reporter to be caused by the fact that pciehp first updates the hardware
and only then cache the ctrl->slot_ctrl in pcie_do_write_cmd(). If the
interrupt happens before we cache the value, pciehp_isr() reads value 0 and
decides that the interrupt was not meant for it causing the above timeout
to trigger.
Fix by moving ctrl->slot_ctrl assignment to happen before it is written to
the hardware.

Currently, eeh_pe_state_mark() marks a PE (and it's children) with a
state and then performs additional processing if that state included
EEH_PE_ISOLATED.
The state parameter is always a constant at the call site, so
rearrange eeh_pe_state_mark() into two functions and just call the
appropriate one at each site.

Basically we need to do the same thing when runtime suspending than with
system sleep so re-use those operations here. This makes sure hotplug
interrupt does not trigger immediately when the link goes down.

PCIe native hotplug shares MSI vector with native PME so the interrupt
handler might get called even the hotplug interrupt is masked. In that case
we should not handle any events because the interrupt was not meant for us.
Modify the PCIe hotplug interrupt handler to check this accordingly and
bail out if it finds out that the interrupt was not about hotplug.

When PCIe hotplug port is transitioned into D3hot, the link to the
downstream component will go down. If hotplug interrupt generation is
enabled when that happens, it will trigger immediately, waking up the
system and bringing the link back up.
To prevent this, disable hotplug interrupt generation when system suspend
is entered. This does not prevent wakeup from low power states according
to PCIe 4.0 spec section 6.7.3.4:
Software enables a hot-plug event to generate a wakeup event by
enabling software notification of the event as described in Section
6.7.3.1. Note that in order for software to disable interrupt generation
while keeping wakeup generation enabled, the Hot-Plug Interrupt Enable
bit must be cleared.
So as long as we have set the slot event mask accordingly, wakeup should
work even if slot interrupt is disabled. The port should trigger wake and
then send PME to the root port when the PCIe hierarchy is brought back up.
Limit this to systems using native PME mechanism to make sure older Apple
systems depending on commit e3354628c376 ("PCI: pciehp: Support interrupts
sent from D3hot") still continue working.

The spec has timing requirements when waiting for a link to become active
after a conventional reset. Implement those hard delays when waiting for
an active link so pciehp and dpc drivers don't need to duplicate this.
For devices that don't support data link layer active reporting, wait the
fixed time recommended by the PCIe spec.
