fa2e0f8bbc nvmet-tcp: support secure channel concatenation
Evaluate the SC_C flag during DH-CHAP-HMAC negotiation to check if secure
concatenation as specified in the NVMe Base Specification v2.1, section
8.3.4.3: "Secure Channel Concatenationand" is requested. If requested the
generated PSK is inserted into the keyring once negotiation has finished
allowing for an encrypted connection once the admin queue is restarted.

43043c9b97 nvmet: Introduce nvmet_req_transfer_len()
Add the new function nvmet_req_transfer_len() to parse a request command
to extract the transfer length of the command. This function
implementation relies on multiple helper functions for parsing I/O
commands (nvmet_io_cmd_transfer_len()), admin commands
(nvmet_admin_cmd_data_len()) and fabrics connect commands
(nvmet_connect_cmd_data_len).

cd0c1b8e04 nvmet: always initialize cqe.result
The spec doesn't mandate that the first two double words (aka results)
for the command queue entry need to be set to 0 when they are not
used (not specified). Though, the target implemention returns 0 for TCP
and FC but not for RDMA.

Let's make RDMA behave the same and thus explicitly initializing the
result field. This prevents leaking any data from the stack.

44e3c25efa nvmet: return DHCHAP status codes from nvmet_setup_auth()
A failure in nvmet_setup_auth() does not mean that the NVMe
authentication command failed, so we should rather return a protocol
error with a 'failure1' response than an NVMe status.

Also update the type used for dhchap_step and dhchap_status to u8 to
avoid confusions with nvme status. Furthermore, split dhchap_status and
nvme status so we don't accidentally mix these return values.

6f66d046ea nvme-auth: always set valid seq_num in dhchap reply
Currently a seqnum of zero is sent during uni-directional
authentication. The zero value is reserved for the secure channel
feature which is not yet implemented.

Relevant extract from the spec:
The value 0h is used to indicate that bidirectional authentication
is not performed, but a challenge value C2 is carried in order to
generate a pre-shared key (PSK) for subsequent establishment of a
secure channel

f965b281fd nvmet-auth: complete a request only after freeing the dhchap pointers
It may happen that the work to destroy a queue
(for example nvmet_tcp_release_queue_work()) is started while
an auth-send or auth-receive command is still completing.

nvmet_sq_destroy() will block, waiting for all the references
to the sq to be dropped, the last reference is then
dropped when nvmet_req_complete() is called.

When this happens, both nvmet_sq_destroy() and
nvmet_execute_auth_send()/_receive() will free the dhchap pointers by
calling nvmet_auth_sq_free().
Since there isn't any lock, the two threads may race against each other,
causing double frees and memory corruptions, as reported by KASAN.

Reproduced by stress blktests nvme/041 nvme/042 nvme/043

Fix this bug by calling nvmet_req_complete() only after freeing the
pointers, so we will prevent the race by holding the sq reference.

1befd944e0 nvmet-auth: don't try to cancel a non-initialized work_struct
Currently blktests nvme/002 trips up debugobjects if CONFIG_NVME_AUTH is
enabled, but authentication is not on a queue. This is because
nvmet_auth_sq_free cancels sq->auth_expired_work unconditionaly, while
auth_expired_work is only ever initialized if authentication is enabled
for a given controller.

Fix this by calling most of what is nvmet_init_auth unconditionally
when initializing the SQ, and just do the setting of the result
field in the connect command handler.
