We check the license for TX hardware timestamping capability.
The PTP probe will have enabled PTP sync events from the adapter. If
later, at TX queue init, it turns out we do not have the license, we
don't need the sync events either.

We can now transmit SKBs in 2 ways:
1. Via the MC (for the 7XXX series and earlier), using
efx_ptp_xmit_skb_mc().
2. Via the TX queues on the dedicated PTP channel (8XXX series and later),
using efx_ptp_xmit_skb_queue().
The PTP worker thread uses the method set up at probe time. It never
checked the return code from the old efx_ptp_xmit_skb(), so it now
returns void.
We increment the TX dropped counter of the device if the transmit fails.
As a result of the probe per channel the remove gets called multiple times.
Clean up efx->ptp_data properly to avoid the 2nd call blowing up.

Before this work, TX timestamping is done by sending each SKB to the MC.
On the 8000 series (Medford1) we have high speed timestamping via the
MAC, which means we can use normal TX queues for this without a
significant drop in bandwidth. On the X2000 series (Medford2) support
for transmitting via the MC is removed, so the new way must be used.
This patch enables timestamping on a TX queue, if requested.
It also enhances TX event handling to process the extra completion events,
and puts the time in the SKB.

While the Linux driver doesn't use CTPIO ('cut-through programmed I/O'),
other drivers on the same port might, so if we're responsible for
reporting per-port stats we need to include the CTPIO stats.

Medford2 NICs support more than MC_CMD_MAC_NSTATS stats, and report the new
count in a field of MC_CMD_GET_CAPABILITIES_V4. This also means that the
end generation count moves (it is, as before, the last 64 bits of the DMA
buffer, but that is no longer MC_CMD_MAC_GENERATION_END).
So read num_mac_stats from the GET_CAPABILITIES response, if present;
otherwise assume MC_CMD_MAC_NSTATS; and always use num_mac_stats - 1 rather
than MC_CMD_MAC_GENERATION_END.

The timer mode register now has a separate field for the reload value.
Since we always use this timer with the reload (for interrupt moderation)
we set this to the same as the initial value.
Previous hardware ignores this field, so we can safely set these bits
on all hardware that uses this register.

The RX_L4_CLASS field has shrunk from 3 bits to 2 bits. The upper
bit was never used in previous hardware, so we can use the new
definition throughout.
The TSO OUTER_IPID field was previously spelt differently from the
external definitions.

Medford2 can also have 16k or 64k VI stride. This is reported by MCDI in
GET_CAPABILITIES, which fortunately is called before the driver does
anything sensitive to the VI stride (such as accessing or even allocating
VIs past the zeroth).

Ethernet FCS inclusion (rx-fcs) is supported on EF10 NICs, conditional on
a firmware capability bit (MC_CMD_GET_CAPABILITIES_OUT_RX_INCLUDE_FCS).
To receive frames with bad FCS (rx-all) we just don't return the discard
flag EFX_RX_PKT_DISCARD from efx_ef10_handle_rx_event_errors() or
efx_farch_handle_rx_not_ok().

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining
ACCESS_ONCE() instances to {READ,WRITE}_ONCE(), using the following
coccinelle script:

If we have more than 32 unicast MAC addresses assigned to an interface
we will read beyond the end of the address table in the driver when
adding filters. The next 256 entries store multicast addresses, so we
will end up attempting to insert duplicate filters, which is mostly
harmless. If we add more than 288 unicast addresses we will then read
past the multicast address table, which is likely to be more exciting.

When filter insertion fails with no rollback, we were trying to convert
EFX_EF10_FILTER_ID_INVALID to an id to store in 'ids' (which is either
vlan->uc or vlan->mc). This would WARN_ON_ONCE and then record a bogus
filter ID of 0x1fff, neither of which is a good thing.

The 8000 series adapters uses catch-all filters for encapsulated traffic
to support filtering VXLAN, NVGRE and GENEVE traffic.
This new filter functionality requires a longer MCDI command.
This patch increases the size of buffers on stack that were missed, which
fixes a kernel panic from the stack protector.

Include HWTSTAMP_FILTER_NTP_ALL in net_hwtstamp_validate() as a valid
filter and update drivers which can timestamp all packets, or which
explicitly list unsupported filters instead of using a default case, to
handle the filter.

If the mc_list is longer than 256 addresses, we enter mc_promisc mode.
If we're in mc_promisc mode and the firmware doesn't support cascaded
multicast, normally we also insert our mc_list, to prevent stealing by
another VI. However, if the mc_list was too long, this isn't really
helpful - the MC groups that didn't fit in the list can still get
stolen, and having only some of them stealable will probably cause
more confusing behaviour than having them all stealable. Since
inserting 256 multicast filters takes a long time and can lead to MCDI
state machine timeouts, just skip the mc_list insert in this overflow
condition.

EFX_POPULATE_QWORD_* takes values in host byte order (which it then
converts to little-endian, as MCDI is little-endian).

efx_start_all can return without initialising queues as a reset is pending.
This means that when netif_device_attach is called, the kernel can start
sending traffic without having an initialised TX queue to send to.
This patch avoids this by not calling netif_device_attach if there is a
pending reset.

On EF10, hardware filter IDs are 13 bits, but in some places we store
32-bit "full filter IDs" in which higher order bits encode the filter
match-priority. This could cause a filter to have a full filter ID of
0xffff, which is also the value EFX_EF10_FILTER_ID_INVALID which we use
in 16-bit "short" filter IDs (without match-priority bits). This would
occur if the hardware filter ID was 0x1fff and the match-priority was 7.
Unfortunately, some code that checks for EFX_EF10_FILTER_ID_INVALID can
be called on full filter IDs, and will WARN_ON if this ever happens.
So, since we have plenty of spare bits in the full filter ID, this patch
shifts the priority bits left one bit when constructing the full filter
IDs, ensuring that the 0x2000 bit of a full filter ID will always be 0
and thus no full filter ID can ever equal EFX_EF10_FILTER_ID_INVALID.
This patch also replaces open-coded full<->short filter ID conversions
with calls to functions, thus keeping the definition of the full filter
ID format in one place.

Implement ndo_udp_tunnel_{add,del} to update the NIC's list of VXLAN and
GENEVE UDP ports. Also reset the port list to empty on driver load and
on driver unload, with appropriate flag set on the unload case.
These port numbers are used for RX inner checksum offload, and in future
will also be used for TX inner checksum offload and encapsulated TSO.

Add support for RX checksum offload of encapsulated packets. This
essentially just means paying attention to the inner checksum flags
in the RX event, and if *either* checksum flag indicates a fail then
don't tell the kernel that checksum offload was successful.
Also, count these checksum errors and export the counts to ethtool -S.
Test the most common "good" case of RX events with a single bitmask
instead of a series of ifs. Move the more specific error checking
in to a separate function for clarity, and don't use unlikely() there
since we know at least one of the bits is bad.

8000 series adapters support filtering VXLAN, NVGRE and GENEVE traffic
based on inner fields, and when the NIC recognises such traffic, it
does not match unencapsulated traffic filters any more. So add catch-
all filters for encapsulated traffic on supporting platforms.
Although recognition of VXLAN and GENEVE is based on UDP ports, and thus
will not occur until the driver (on the primary PF) notifies the
firmware of UDP ports to use, NVGRE will always be recognised, hence
without this patch 8000 series adapters will drop all NVGRE traffic.

If the NIC is switched from full-featured to low-latency, encapsulated
filters are no longer available, and this causes errors. This patch
removes those filters from the filter table on restore.
Also, if filters which are removed by the above, or which we fail to
insert when restoring filters, were UC, MC or broadcast default
filters, invalidate the corresponding vlan->default_filters entry.

PIO buffer allocation can fail for two valid reasons:
- we've run out of them (results in -ENOSPC)
- the NIC configuration doesn't support them (results in -EPERM)
Since both these failures are expected netif_err is excessive.

The 8000 series SFC NICs have 4K PIO buffers, rather than the 2K of
the 7000 series. Rather than having a hard-coded PIO buffer size
(ER_DZ_TX_PIOBUF_SIZE), read it from the GET_CAPABILITIES_V2 MCDI
response.

If an option descriptor has been sent on a queue but not followed by a
packet, there will have been no completion event, so the read and write
counts won't match and we'll think we can't do PIO. This combines with
the fact that we have two TX queues (for en/disable checksum offload),
and that both must be empty for PIO to happen.
This patch adds a separate "packet_write_count" that tracks the most
recent write_count we expect to see a completion event for; this excludes
option descriptors but _includes_ PIO descriptors (even though they look
like option descriptors). This is then used, rather than write_count,
in efx_nic_tx_is_empty().
We only bother to maintain packet_write_count on EF10, since on Siena
(a) there are no option descriptors and it always equals write_count, and
(b) there's no PIO, so we don't need it anyway.

Setting dev_port changes the device names allocated by systemd. Any devices
with a dev_port >0 will (in default distro configurations) have a suffix of
"d<port-number>" appended.
This is not something done by other drivers, and causes confusion for users.

If we failed to set up RSS on EF10 (e.g. because firmware declared
RX_RSS_LIMITED), ethtool --show-nfc $dev rx-flow-hash ... should report
no fields, rather than confusingly reporting what fields we _would_ be
hashing on if RSS was working.

Logically, EFX_BUG_ON_PARANOID can never be correct. For, BUG_ON should
only be used if it is not possible to continue without potential harm;
and since the non-DEBUG driver will continue regardless (as the BUG_ON is
compiled out), clearly the BUG_ON cannot be needed in the DEBUG driver.
So, replace every EFX_BUG_ON_PARANOID with either an EFX_WARN_ON_PARANOID
or the newly defined EFX_WARN_ON_ONCE_PARANOID.

It gives no advantage over GSO now that xmit_more exists. If we find
ourselves unable to handle a TSO skb (because our TXQ doesn't have a
TSOv2 context and the NIC doesn't support TSOv1), hand it back to GSO.
Also do that if the TSO handler fails with EINVAL for any other reason.
As Falcon-architecture NICs don't support any firmware-assisted TSO,
they no longer advertise TSO feature flags at all.

Add support for FATSOv2 to the driver. FATSOv2 offloads far more of the task
of TCP segmentation to the firmware, such that we now just pass a single
super-packet to the NIC. This means TSO has a great deal in common with a
normal DMA transmit, apart from adding a couple of option descriptors.
NIC-specific checks have been moved off the fast path and in to
initialisation where possible.
This also moves FATSOv1/SWTSO to a new file (tx_tso.c). The end of transmit
and some error handling is now outside TSO, since it is common with other
code.

MC_CMD_TRIGGER_INTERRUPT does not work on the SFC9140, as used in the
sfn7x42q and sfn7x24f.
Check for this using the MCDI workaround mechanism.
The command is only used during self test. If it's not supported, skip
the interrupt test.

On SFN8000 series adapters the MC provides a method to get the timer
quantum and the maximum timer setting. We revert to the old values if the
new call is unavailable.

SFN8000-series NICs require a new method of setting interrupt moderation,
via MCDI. This is indicated by a workaround flag. This new MCDI command
takes an explicit time value rather than a number of ticks. It therefore
makes sense to also store the moderation values in terms of time, since
that is what the ethtool interface is interested in.

If it is not supported we simply disable the feature.
For the feature to work we need firmware filter support for
OUTER_VID + LOC_MAC and for OUTER_VID + LOC_MAC_IG.
The low-latency firmware can match on OUTER_VID + LOC_MAC but not on
OUTER_VID + LOC_MAC_IG.
For the capture packet firmware it is the other way around.
Only the full-feature variant can match on both combinations.

Filter match flags are not unique criteria to be mapped to priority
because of both unknown unicast and unknown multicast are mapped to
LOC_MAC_IG. So, local MAC is required to map filter to priority.

Nearly every time we call efx_ef10_filter_remove_unsafe, we first check
for EFX_EF10_FILTER_ID_INVALID, in which case we do nothing. So move
that check into the function, simplifying all the call sites.
Also, change the return type to void, since none of the callers check it.

This comes from function efx_ef10_filter_vlan_sync_rx_mode.
To solve the bug we ensure the mac_lock is taken before calling
efx_ef10_filter_add_vlan. But to avoid a priority inversion mac_lock must
be taken before filter_sem.
To satisfy these requirements we end up taking mac_lock in
efx_ef10_vport_set_mac_address, efx_ef10_set_mac_address,
efx_ef10_sriov_set_vf_vlan and efx_probe_filters.

When certain firmware variants are selected (via the sfboot utility) the
SFC7000 and SFC8000 series NICs don't support RSS. The driver still
tries (and fails) to insert filters with the RSS flag, and the NIC fails
to pass traffic.
When the firmware reports RSS_LIMITED suppress allocating a default RSS
context. The absence of an RSS context is picked up in filter insertion
and RSS flags are discarded.

There are situations - mostly reset related - where our view of the
filter table differs from the hardware. In this case we may try and
remove filters that aren't actually installed. This isn't that
interesting in most situations, so downgrade the logging.

A change in MCFW behaviour means that the net driver must update its record
of the warm_boot_count by reading it from the ER_DZ_BIU_MC_SFT_STATUS
register.
On v4.6.x MCFW the global boot count was incremented when some functions
needed to be reset to enable multicast chaining, so all functions saw the
same value. In that case, the driver needed to increment its
warm_boot_count when other functions were reset, to avoid noticing it later
and then trying to reset itself to recover unnecessarily.
With v4.7+ MCFW, the boot count in firmware doesn't change as that is
unnecessary since the PFs that have been reset will each receive an MC
reboot notification. In that case, the driver re-reads the unchanged
value.

The Solarflare 8000 series NIC will use a new TSO scheme. The current
driver refuses to load if the current TSO scheme is not found. Remove
that check and instead make the TSO version a per-queue parameter.

When the IP stack passes SKBs the sfc driver puts them in 2 different TX
queues (called partners), one for checksummed and one for not checksummed.
If the SKB has xmit_more set the driver will delay pushing the work to the
NIC.
When later it does decide to push the buffers this patch ensures it also
pushes the partner queue, if that also has any delayed work. Before this
fix the work in the partner queue would be left for a long time and cause
a netdev watchdog.

On EF10, MC_CMD_VPORT_RECONFIGURE can cause a CODE_MC_REBOOT event
to be sent to a function without incrementing the (adapter-wide)
warm_boot_count. In this case, the reboot is not detected by the
loop on efx_mcdi_poll_reboot(), so prepare for recovery from an MC
reboot anyway. When this codepath is run, the MC has always just
rebooted, so this recovery is valid.
The loop on efx_mcdi_poll_reboot() is still required for other MC
reboot cases, so that actions in response to an MC reboot are
performed, such as clearing locally calculated statistics.
Siena NICs are unaffected by this change as the above scenario
does not apply.

Previously, the driver would refuse to load if it couldn't secure
enough VIs from the MC to fulfill its RSS requirements.
This was causing probe to fail on later functions in
configurations where we'd run out of VIs, such as having many
VFs.
This change allows the driver to load with fewer VIs, down to a
minimum of 2. A warning will be printed saying that RSS
requirements were not met, possibly affecting performance.
efx->max_tx_channels needs to be set to avoid going down the
failure path in efx_probe_nic() immediately in the loop after the
probe() NIC-type function.
Also, Set rc=ENOSPC when bombing out of efx_probe_nic due to lack
of VIs.

The policy in the net driver is to attempt MCDI commands and
then handle any EPERM error codes appropriately when returned
by unprivileged functions.
The ethtool selftest contains some tests which are useful on
an unprivileged function, such as the event queue interrupt
tests, but other tests cannot be performed as the function
does not have the required permissions.
If a test returns -EPERM, act as though the test was not run
and continue.

If the workaround to support cascaded multicast filters ("workaround_26807") is
enabled, the broadcast filter and individual multicast filters are not inserted
when in promiscuous or allmulti mode.
There is a race while inserting and removing filters when entering and leaving
promiscuous mode. When changing promiscuous state with cascaded multicast
filters, the old multicast filters are removed before inserting the new filters
to avoid duplicating packets; this can lead to dropped packets until all
filters have been inserted.
The efx_nic:mc_promisc flag is added to record the presence of a multicast
promiscuous filter; this gives a simple way to tell if the promiscuous state is
changing.

If a function is in promiscuous mode and another function has a broadcast or
multicast filter inserted, the function in promiscuous mode won't see that
broadcast or multicast traffic.
Most notably this breaks broadcast, which means ARP doesn't work. Less
show-stoppingly, a function listening on a multicast address that's also in
promiscuous mode will not see that multicast traffic if another function is
also listening on that multicast address.

When enabling the workaround for cascaded multicast filters, the MC
can reset other functions if they have already inserted filters.
In that case, the workaround has been enabled, but print an info
message in the log recording that other functions had to be reset.
As other functions were reset, the MC will have incremented its boot
count, so also increment the warm_boot_count on the function which
enabled the workaround, as that function won't have received an MC
reboot event and does not need to reset.

GET_WORKAROUNDS was only introduced in May 2014, not all firmware
will have it. So call sites need to handle ENOSYS.
In this case we're probing the bug26807 workaround, which is not
implemented in any firmware that doesn't have GET_WORKAROUNDS.
So interpret ENOSYS as 'false'.

After creating event queue 0, check to see if the workaround is enabled,
and enable it if necessary. This will be called during PCI probe and
also when coming back up after a reset. The nic_data->workaround_26807
will be used in the future to control the filter insertion behaviour
based on this workaround.
Only the primary PF can enable this workaround, so tolerate an EPERM
error and continue. Otherwise, if any step in the checking and enabling
of the workaround fails, the event queue must be removed.
We check that workaround is implemented before trying to enable it,
and store the current workaround setting before trying to change it.

Some versions of MCFW do not support the MC_CMD_VADAPTOR_SET_MAC
command, and ENOSYS will be returned.
If the PF created its own vport, the function's datapath must be
stopped and the vport can be reconfigured to reflect the new MAC
address.
If the MCFW created the vport for the PF (which is the case when
the nic_data->vport_mac is blank), nothing further needs to be
done as the vport is not under the control of the PF.
This only applies to PFs because the MCFW in question does not
support VFs.

If any VF is assigned as the PF is unloaded, do not attempt to
remove its vport or the vswitch. These will be removed if the
driver binds to the PF again, as an entity reset occurs during
probe.
A 'force' flag is added to efx_ef10_pci_sriov_disable() to
distinguish between disabling SR-IOV and driver unload.
SR-IOV cannot be disabled if VFs are assigned to guests.
If the PF driver is unloaded while VFs are assigned, the driver
may try to bind to the VF again at a later point if the driver
has been reloaded and the VF returns to the same domain as the PF.
In this case, the PF will not have a VF data structure, so the VF
can check this and drop out of probe early.
In this case, efx->vf_count will be zero but VFs will be present.
The user is advised to remove the VF and re-create it. The check
at the beginning of efx_ef10_pci_sriov_disable() that
efx->vf_count is non-zero is removed to allow SR-IOV to be
disabled in this case. Also, if the PF driver is unloaded, it
will disable SR-IOV to remove these unknown VFs.
By not disabling bus-mastering if VFs are still assigned, the VF
will continue to pass traffic after the PF has been removed.
When using the max_vfs module parameter, if VFs are already
present do not try to initialise any more.

The ifenslave command to set up a bond runs in an atomic
context, and it queries the stats on the devices that are
being enslaved. A VF needs to make an MCDI call to update
its stats, which is not allowed in atomic context.
The releasing of the stats_lock is moved to the beginning of
the VF stats update function so that in_interrupt() can be
used; it must be taken again before returning from this
function.

The raw_mask array is not initialised, so it needs to be
explicitly set to zero in the 'else' branch.
If the EVB capability is not present, a port cannot have multiple
functions so the per-port MAC stats are correct and should match
the corresponding vadaptor stats, so this redundancy can be
removed from the ethtool stats output.

MC_CMD_MAC_STATS can be called on a function before a
vadaptor has been created, as the kernel can call into this
through ndo_get_stats/ndo_get_stats64.
If MC_CMD_MAC_STATS is called before the DMA queues have been
setup, so that a vadaptor has not been created yet, firmware
will return ENOENT. This is expected, so suppress the MCDI
error message in this case.

The netdevice statistics (in /proc/net/dev) are per-function
stats so they must use the vadaptor stats. Change the use of
MAC stats to vadaptor stats, and remove any statistics that
can only be measured per-port. All stats that are removed
will be shown as zeroes when these statistics are displayed.

All interfaces will display vadaptor statistics, so set all the
relevant bits in the stats bitmask. Only functions with the
LINKCTRL flag will see other stats, including (per-port) MAC stats.

MCDI tracing is conditional on CONFIG_SFC_MCDI_LOGGING, which is enabled
by default.
Each MCDI command will produce a console line like
sfc dom🚌dev:fn ifname: MCDI RPC REQ: xxxxxxxx [yyyyyyyy...]
where xxxxxxxx etc. are the raw MCDI payload in 32-bit hex chunks.
The response will then produce a similar line with "RESP" instead of "REQ",
and containing the MCDI response payload (if any).

Add a set_mac_address() NIC-type function for EF10 only, and
use this to set the MAC address on the vadaptor. For Siena and
earlier, the MAC address continues to be set by MC_CMD_SET_MAC;
this is still called on EF10, and including a MAC address in
this command has no effect.
The sriov_mac_address_changed() NIC-type function is no longer
needed on EF10, but it is needed for Siena where it is used to
update the peer address of the PF for VFDI. Change this to use
the new set_mac_address function pointer.
efx_ef10_sriov_mac_address_changed() is no longer called, as VFs
will try to change the MAC address on their vadaptor rather than
trying to change to the context of the PF to alter the vport.
When a VF is running in direct passthrough mode with MAC spoofing
enabled, it will be able to change the MAC address on its vadaptor.
In this case, there is a link to the PF, so find the correct VF in
its ef10_vf array and update the MAC address.
ndo_set_mac_address() can be called during driver unload while
bonding, and in this case the device has already been stopped, so
don't call efx_net_open() to restart it after reconfiguration.
efx->port_enabled is set to false in efx_stop_port(), so it is
indicator of whether the device needs to be restarted.

Currently we do an entity reset when we detect an MC reboot.
This messes up SRIOV because it leaves VFs orphaned. The extra
reset is rather redundant anyway, since the MC reboot will have
basically reset everything.
This change replaces the entity reset after MC reboot with a
simpler datapath reset that reallocates resources but doesn't
perform the entity reset.

In order to avoid MC bugs the flags field needs to be set to 0.
Instead of explicitly clearing out the flags individually, a
better way to do this is to memset the MCDI_BUF to 0.

A VF's MAC address is set by its parent PF and added to its vport.
To get this MAC address, the VF must use MC_CMD_ VPORT_GET_MAC_ADDRESSES.
In the current scheme, a VF's vport should only have one MAC address,
so warn if this is not the case.

If MCDI timeouts are encountered during efx_ef10_filter_table_remove(),
an FLR will be queued, but efx->filter_state will still be kfree()d.
The queued FLR will then call efx_ef10_filter_table_restore(), which
will try to use efx->filter_state. This previously caused a panic.
This patch adds an rwsem to protect the existence of efx->filter_state,
separately from the spinlock protecting its contents. Users which can
race against efx_ef10_filter_table_remove() should down_read this rwsem.

vf->efx is needed in future patches to change the MAC address
of the VF via the parent PF, while the driver is bound to the
VF.

control function.

Add the device ID of the VF to the PCI device ID table.
Added a boolean flag is_vf in efx_nic_type to differentiate
between a VF and PF at probe time. This flag is useful in later
patches while setting MAC address specially in the
PCI-passthrough case.

Allow PFs to allocate shared RSS contexts if we exhaust our
exclusive RSS contexts. Make VFs use shared RSS contexts in
all cases.
Spruce up error handling so that the shadow copy of the RSS
table is updated after successful update, rather than in all
cases, so that we report the actual contents of the RSS table
after a failure to set it, rather than what we'd like it to be.
Populate context_size parameter when vacuously allocating RSS
context of size 1.

* Accept EPERM in some simple cases, the following cases are handled:
1) efx_mcdi_read_assertion()
Unprivileged PCI functions aren't allowed to GET_ASSERTS.
We return success as it's up to the primary PF to deal with asserts.
2) efx_mcdi_mon_probe() in efx_ef10_probe()
Unprivileged PCI functions aren't allowed to read sensor info, and
worrying about sensor data is the primary PF's job.
3) phy_op->reconfigure() in efx_init_port() and efx_reset_up()
Unprivileged functions aren't allowed to MC_CMD_SET_LINK, they just have
to accept the settings (including flow-control, which is what
efx_init_port() is worried about) they've been given.
4) Fallback to GET_WORKAROUNDS in efx_ef10_probe()
Unprivileged PCI functions aren't allowed to set workarounds. So if
efx_mcdi_set_workaround() fails EPERM, use efx_mcdi_get_workarounds()
to find out if workaround_35388 is enabled.
5) If DRV_ATTACH gets EPERM, try without specifying fw-variant
Unprivileged PCI functions have to use a FIRMWARE_ID of 0xffffffff
(MC_CMD_FW_DONT_CARE).
6) Don't try to exit_assertion unless one had fired
Previously we called efx_mcdi_exit_assertion even if
efx_mcdi_read_assertion had received MC_CMD_GET_ASSERTS_FLAGS_NO_FAILS.
This is unnecessary, and the resulting MC_CMD_REBOOT, even if the
AFTER_ASSERTION flag made it a no-op, would fail EPERM for unprivileged
PCI functions.
So make efx_mcdi_read_assertion return whether an assert happened, and only
call efx_mcdi_exit_assertion if it has.

To be able to use MC_CMD_VADAPTOR_SET_MAC, vadaptors must be
manually allocated and freed as automatic vadaptors will disappear
when their reference_count reaches zero, which must happen before
the MAC address is changed.
Vadaptors are allocated and freed in the vswitching_probe/remove
functions for PFs and VFs, and this means that vadaptors are restored
correctly following an MC reboot or other reset when required.

Adds functions to allocate and free vswitches and vports; vadaptors
are automatically allocated and freed when TX/RX queues are
initialised and finalised. This vswitching structure is only created
if the firmware supports it, so a check that full-featured firmware
is running is performed first.
If the MC resets, the vswitching infrastructure will need to be
recreated, so mark the "must_probe_vswitching" flag when an MC reboot
is detected.
Don't try to create a vswitch if vf-count=0
This allocation of vswitches and vports does not currently support
configuring VLAN tags, but that can be added in a future change.

The default port ID of EVB_PORT_ID_ASSIGNED is a "magic" number
for the MCFW to select the physical port of the PF. If other
vswitches and vports are created on top of the default firmware
configuration, the ID of the newly created vport is then required
when passed to MCDI commands. Currently, this doesn't happen so
the vport_id is never changed, but a subsequent patch will change
this behaviour so that other vswitches and vports are created.
The vport_id recorded in nic_data is only relevant for PFs.
VFs will have their vports created by their parent PF, and in
that case the parent PF will record the vport ID of each VF.
For a VF, nic_data->vport_id is expected to remain at the default
value.

The (future) code to add/remove vswitches and vports will be
dependent on the firmware variant.
To simplify the checking of the firmware variant, record
values for rx_dpcpu_fw_id and tx_dpcpu_fw_id in EF10 nic_data.
There was only one place where this was previously used:
efx_mcdi_print_fwver() in ethtool.c.
The MC_CMD_GET_CAPABILITIES can be replaced and the values from
nic_data used instead.
Note that the printing of "?" if the MC command fails or if the
outlength is incorrect no longer apply, because errors are returned
in efx_ef10_init_datapath_caps() in both of these cases.

By putting all the efx_{siena,ef10}_sriov_* declarations in
{siena,ef10}_sriov.h, ensure they cannot be called from nic-generic code.
Also fixes up an instance of this, where mcdi.c was calling
efx_siena_sriov_flr.
The single instance of netdev_ops should call general high level
functions that can then call something adapter specific in efx_nic_type.
We should only do adapter specialisation via efx_nic_type.
Removal of sriov functionality from the Falcon code means that tests
are needed for the presence of some callbacks.

efx_ef10_probe() was BUGging out if the BAR2 size was 0. This is
unnecessarily violent; instead we should just fail to probe the device.
Kept a WARN_ON as this problem indicates a broken or misconfigured NIC.

When an MCDI command times out (whether or not we find it
completed when we poll), call efx_mcdi_abandon(), which tells
all subsequent MCDI calls to fail-fast, and queues up an FLR.
Because an FLR doesn't lead to receiving any reboot even from
the MC (unlike most other types of reset), we have to call
efx_ef10_reset_mc_allocations.
In efx_start_all(), if a reset (of any kind) is pending, we
bail out.
Without this, attempts to reconfigure (e.g. change mtu) can
cause driver/mc state inconsistency if the first MCDI call
triggers an FLR.
For similar reasons, on EF10, in
efx_reset_down(method=RESET_TYPE_MCDI_TIMEOUT), set the number
of active queues to zero before calling efx_stop_all().
And, on farch, in efx_reset_up(method=RESET_TYPE_MCDI_TIMEOUT),
set active_queues and flushes pending & outstanding to zero.
efx_mcdi_mode_{poll,event}() should not take us out of fail-fast
mode. Instead, this is done by efx_mcdi_reset() after the FLR
completes.
The new FLR reset_type RESET_TYPE_MCDI_TIMEOUT doesn't really
fit into the hierarchy of reset 'scopes' whereby efx_reset()
decides some resets subsume others. Thus, it uses separate logic.
Also, fixed up some inconsistency around RESET_TYPE_MC_BIST,
which was in the wrong place in that hierarchy.

When using the "separate_tx_channels=1" module parameter, the TX queues are
initially numbered starting from the first TX-only channel number (after all the
RX-only channels). efx_set_channels() renumbers the queues so that they are
indexed from zero.
On EF10, the TX queues need to be relabelled in this way before calling the
dimension_resources NIC type operation, otherwise the TX queue PIO buffers can be
linked to the wrong VIs when using "separate_tx_channels=1".

Processing any incoming packets with a with a napi budget of 0
is incorrect driver behavior.
This matters as netpoll will shortly call drivers with a budget of 0
to avoid receive packet processing happening in hard irq context.

EF10 implements option descriptors to switch TX checksum offload
on and off between packets. We could therefore use a single
hardware TX queue per kernel TX queue, although we don't yet.

When we insert an filter, the firmware checks that the given RX queue
index is in range even if it will not be used. In case we're
inserting a drop filter, pass the value 0.
