fd98693cb0 ibmvnic: Allow queueing resets during probe
We currently don't allow queuing resets when adapter is in VNIC_PROBING
state - instead we throw away the reset and return EBUSY. The reasoning
is probably that during ibmvnic_probe() the ibmvnic_adapter itself is
being initialized so performing a reset during this time can lead us to
accessing fields in the ibmvnic_adapter that are not fully initialized.
A review of the code shows that all the adapter state neede to process a
reset is initialized before registering the CRQ so that should no longer
be a concern.

Further the expectation is that if we do get a reset (transport event)
during probe, the do..while() loop in ibmvnic_probe() will handle this
by reinitializing the CRQ.

While that is true to some extent, it is possible that the reset might
occur _after_ the CRQ is registered and CRQ_INIT message was exchanged
but _before_ the adapter state is set to VNIC_PROBED. As mentioned above,
such a reset will be thrown away. While the client assumes that the
adapter is functional, the vnic server will wait for the client to reinit
the adapter. This disconnect between the two leaves the adapter down
needing manual intervention.

Because ibmvnic_probe() has other work to do after initializing the CRQ
(such as registering the netdev at a minimum) and because the reset event
can occur at any instant after the CRQ is initialized, there will always
be a window between initializing the CRQ and considering the adapter
ready for resets (ie state == PROBED).

So rather than discarding resets during this window, allow queueing them
- but only process them after the adapter is fully initialized.

To do this, introduce a new completion state ->probe_done and have the
reset worker thread wait on this before processing resets.

This change brings up two new situations in or just after ibmvnic_probe().
First after one or more resets were queued, we encounter an error and
decide to retry the initialization.  At that point the queued resets are
no longer relevant since we could be talking to a new vnic server. So we
must purge/flush the queued resets before restarting the initialization.
As a side note, since we are still in the probing stage and we have not
registered the netdev, it will not be CHANGE_PARAM reset.

Second this change opens up a potential race between the worker thread
in __ibmvnic_reset(), the tasklet and the ibmvnic_open() due to the
following sequence of events:

	1. Register CRQ
	2. Get transport event before CRQ_INIT completes.
	3. Tasklet schedules reset:
		a) add rwi to list
		b) schedule_work() to start worker thread which runs
		   and waits for ->probe_done.
	4. ibmvnic_probe() decides to retry, purges rwi_list
	5. Re-register crq and this time rest of probe succeeds - register
	   netdev and complete(->probe_done).
	6. Worker thread resumes in __ibmvnic_reset() from 3b.
	7. Worker thread sets ->resetting bit
	8. ibmvnic_open() comes in, notices ->resetting bit, sets state
	   to IBMVNIC_OPEN and returns early expecting worker thread to
	   finish the open.
	9. Worker thread finds rwi_list empty and returns without
	   opening the interface.

If this happens, the ->ndo_open() call is effectively lost and the
interface remains down. To address this, ensure that ->rwi_list is
not empty before setting the ->resetting  bit. See also comments in
__ibmvnic_reset().

f628ad531b ibmvnic: clear fop when retrying probe
Clear ->failover_pending flag that may have been set in the previous
pass of registering CRQ. If we don't clear, a subsequent ibmvnic_open()
call would be misled into thinking a failover is pending and assuming
that the reset worker thread would open the adapter. If this pass of
registering the CRQ succeeds (i.e there is no transport event), there
wouldn't be a reset worker thread.

This would leave the adapter unconfigured and require manual intervention
to bring it up during boot.

ae16bf1537 ibmvnic: init init_done_rc earlier
We currently initialize the ->init_done completion/return code fields
before issuing a CRQ_INIT command. But if we get a transport event soon
after registering the CRQ the taskslet may already have recorded the
completion and error code. If we initialize here, we might overwrite/
lose that and end up issuing the CRQ_INIT only to timeout later.

If that timeout happens during probe, we will leave the adapter in the
DOWN state rather than retrying to register/init the CRQ.

Initialize the completion before registering the CRQ so we don't lose
the notification.

61772b0908 ibmvnic: don't release napi in __ibmvnic_open()
If __ibmvnic_open() encounters an error such as when setting link state,
it calls release_resources() which frees the napi structures needlessly.
Instead, have __ibmvnic_open() only clean up the work it did so far (i.e.
disable napi and irqs) and leave the rest to the callers.

If caller of __ibmvnic_open() is ibmvnic_open(), it should release the
resources immediately. If the caller is do_reset() or do_hard_reset(),
they will release the resources on the next reset.

This fixes following crash that occurred when running the drmgr command
several times to add/remove a vnic interface:

48079e7fdd ibmvnic: don't spin in tasklet
ibmvnic_tasklet() continuously spins waiting for responses to all
capability requests. It does this to avoid encountering an error
during initialization of the vnic. However if there is a bug in the
VIOS and we do not receive a response to one or more queries the
tasklet ends up spinning continuously leading to hard lock ups.

If we fail to receive a message from the VIOS it is reasonable to
timeout the login attempt rather than spin indefinitely in the tasklet.

151b6a5c06 ibmvnic: init ->running_cap_crqs early
We use ->running_cap_crqs to determine when the ibmvnic_tasklet() should
send out the next protocol message type. i.e when we get back responses
to all our QUERY_CAPABILITY CRQs we send out REQUEST_CAPABILITY crqs.
Similiary, when we get responses to all the REQUEST_CAPABILITY crqs, we
send out the QUERY_IP_OFFLOAD CRQ.

We currently increment ->running_cap_crqs as we send out each CRQ and
have the ibmvnic_tasklet() send out the next message type, when this
running_cap_crqs count drops to 0.

This assumes that all the CRQs of the current type were sent out before
the count drops to 0. However it is possible that we send out say 6 CRQs,
get preempted and receive all the 6 responses before we send out the
remaining CRQs. This can result in ->running_cap_crqs count dropping to
zero before all messages of the current type were sent and we end up
sending the next protocol message too early.

Instead initialize the ->running_cap_crqs upfront so the tasklet will
only send the next protocol message after all responses are received.

Use the cap_reqs local variable to also detect any discrepancy (either
now or in future) in the number of capability requests we actually send.

Currently only send_query_cap() is affected by this behavior (of sending
next message early) since it is called from the worker thread (during
reset) and from application thread (during ->ndo_open()) and they can be
preempted. send_request_cap() is only called from the tasklet  which
processes CRQ responses sequentially, is not be affected.  But to
maintain the existing symmtery with send_query_capability() we update
send_request_capability() also.

db9f0e8bf7 ibmvnic: Allow extra failures before disabling
If auto-priority-failover (APF) is enabled and there are at least two
backing devices of different priorities, some resets like fail-over,
change-param etc can cause at least two back to back failovers. (Failover
from high priority backing device to lower priority one and then back
to the higher priority one if that is still functional).

Depending on the timimg of the two failovers it is possible to trigger
a "hard" reset and for the hard reset to fail due to failovers. When this
occurs, the driver assumes that the network is unstable and disables the
VNIC for a 60-second "settling time". This in turn can cause the ethtool
command to fail with "No such device" while the vnic automatically recovers
a little while later.

Given that it's possible to have two back to back failures, allow for extra
failures before disabling the vnic for the settling time.

When trying to decide whether or not reuse existing rx/tx pools
we tried to allow a range of values for the pool parameters rather
than exact matches. This was intended to reuse the resources for
instance when switching between two VIO servers with different
default parameters.

But this optimization is incomplete and breaks when we try to
change the number of queues for instance. The optimization needs
to be updated, so drop it for now and simplify the code.

6b278c0cb3 ibmvnic: delay complete()
If we get CRQ_INIT, we set errno to -EIO and first call complete() to
notify the waiter. Then we try to schedule a FAILOVER reset. If this
occurs while adapter is in PROBING state, ibmvnic_reset() changes the
error code to EAGAIN and returns without scheduling the FAILOVER. The
purpose of setting error code to EAGAIN is to ask the waiter to retry.

But due to the earlier complete() call, the waiter may already have seen
the -EIO response and decided not to retry. This can cause intermittent
failures when bringing up ibmvnic adapters during boot, specially in
in kexec/kdump kernels.

Defer the complete() call until after scheduling the reset.

Also streamline the error code to EAGAIN. Don't see why we need EIO
sometimes. All 3 callers of ibmvnic_reset_init() can handle EAGAIN.

6e20d00158 ibmvnic: Process crqs after enabling interrupts
Soon after registering a CRQ it is possible that we get a fail over or
maybe a CRQ_INIT from the VIOS while interrupts were disabled.

Look for any such CRQs after enabling interrupts.

Otherwise we can intermittently fail to bring up ibmvnic adapters during
boot, specially in kexec/kdump kernels.

8878e46fcf ibmvnic: don't stop queue in xmit
If adapter's resetting bit is on, discard the packet but don't stop the
transmit queue - instead leave that to the reset code. With this change,
it is possible that we may get several calls to ibmvnic_xmit() that simply
discard packets and return.

But if we stop the queue here, we might end up doing so just after
__ibmvnic_open() started the queues (during a hard/soft reset) and before
the ->resetting bit was cleared. If that happens, there will be no one to
restart queue and transmissions will be blocked indefinitely.

This can cause a TIMEOUT reset and with auto priority failover enabled,
an unnecessary FAILOVER reset to less favored backing device and then a
FAILOVER back to the most favored backing device. If we hit the window

them on open, reuse the tx pools unless the pool parameters (number
of pools, size of each pool or size of each buffer in a pool) have
changed.

If the pool parameters changed, then release the old pools (if
any) and allocate new ones.

Specifically release tx pools, if:
	- adapter is removed,
	- pool parameters change during reset,
	- we encounter an error when opening the adapter in response
	  to a user request (in ibmvnic_open()).

and don't release them:
	- in __ibmvnic_close() or
	- on errors in __ibmvnic_open()

in the hope that we can reuse them during this or next reset.

With these changes reset_tx_pools() can be dropped because its
optimization is now included in init_tx_pools() itself.

cleanup_tx_pools() releases all the skbs associated with the pool and
is called from ibmvnic_cleanup(), which is called on every reset. Since
we want to reuse skbs across resets, move cleanup_tx_pools() out of
ibmvnic_cleanup() and call it only when user closes the adapter.

Add two new adapter fields, ->prev_mtu, ->prev_tx_pool_size to track the
previous values and use them to decide whether to reuse or realloc the
pools.

129854f061 ibmvnic: Use bitmap for LTB map_ids
In a follow-on patch, we will reuse long term buffers when possible.
When doing so we have to be careful to properly assign map ids. We
can no longer assign them sequentially because a lower map id may be
available and we could wrap at 255 and collide with an in-use map id.

Instead, use a bitmap to track active map ids and to find a free map id.
Don't need to take locks here since the map_id only changes during reset
and at that time only the reset worker thread should be using the adapter.

Noticed this when analyzing an error Dany Madden ran into with the
patch set.

Basically we have a set of pools, num_pools. Each pool has a set of
buffers, pool_size and each buffer is of size buff_size.

pool_size is a bit ambiguous (whether size in bytes or buffers). Add

bb55362bd6 ibmvnic: Remove the proper scrq flush
Commit 65d6470d13 ("ibmvnic: clean pending indirect buffs during reset")
intended to remove the call to ibmvnic_tx_scrq_flush() when the
->resetting flag is true and was tested that way. But during the final
rebase to net-next, the hunk got applied to a block few lines below
(which happened to have the same diff context) and the wrong call to
ibmvnic_tx_scrq_flush() got removed.

Fix that by removing the correct ibmvnic_tx_scrq_flush() and restoring
the one that was incorrectly removed.

4f408e1fa6 ibmvnic: retry reset if there are no other resets
Normally, if a reset fails due to failover or other communication error
there is another reset (eg: FAILOVER) in the queue and we would process
that reset. But if we are unable to communicate with PHYP or VIOS after
H_FREE_CRQ, there would be no other resets in the queue and the adapter
would be in an undefined state even though it was in the OPEN state
earlier. While starting the reset we set the carrier to off state so
we won't even get the timeout resets.

If the last queued reset fails, retry it as a hard reset (after the
usual 60 second settling time).

65d6470d13 ibmvnic: clean pending indirect buffs during reset
We batch subordinate command response queue (scrq) descriptors that we
need to send to the VIOS using an "indirect" buffer. If after we queue
one or more scrqs in the indirect buffer encounter an error (say fail
to allocate an skb), we leave the queued scrq descriptors in the
indirect buffer until the next call to ibmvnic_xmit().

On the next call to ibmvnic_xmit(), it is possible that the adapter is
going through a reset and it is possible that the long term  buffers
have been unmapped on the VIOS side. If we proceed to flush (send) the
packets that are in the indirect buffer, we will end up using the old
map ids and this can cause the VIOS to trigger an unnecessary FATAL
error reset.

Instead of flushing packets remaining on the indirect_buff, discard
(clean) them instead.

0ec13aff05 Revert "ibmvnic: simplify reset_long_term_buff function"
This reverts commit 1c7d45e7b2.

We tried to optimize the number of hcalls we send and skipped sending
the REQUEST_MAP calls for some maps. However during resets, we need to
resend all the maps to the VIOS since the VIOS does not remember the
old values. In fact we may have failed over to a new VIOS which will
not have any of the mappings.

When we send packets with map ids the VIOS does not know about, it
triggers a FATAL reset. While the client does recover from the FATAL
error reset, we are seeing a large number of such resets. Handling
FATAL resets is lot more unnecessary work than issuing a few more
hcalls so revert the commit and resend the maps to the VIOS.

53f8b1b254 ibmvnic: Allow device probe if the device is not ready at boot
Allow the device to be initialized at a later time if
it is not available at boot. The device will be allowed to probe but
will be given a "down" state. After completing device probe and
registering the net device, the driver will await an interrupt signal
from its partner device, indicating that it is ready for boot. The
driver will schedule a work event to perform the necessary procedure
and begin operation.

d3a6abccbd ibmvnic: remove duplicate napi_schedule call in do_reset function
During adapter reset, do_reset/do_hard_reset calls ibmvnic_open(),
which will calls napi_schedule if previous state is VNIC_CLOSED
(i.e, the reset case, and "ifconfig down" case). So there is no need
for do_reset to call napi_schedule again at the end of the function
though napi_schedule will neglect the request if napi is already
scheduled.

0775ebc4cf ibmvnic: avoid calling napi_disable() twice
__ibmvnic_open calls napi_disable without checking whether NAPI polling
has already been disabled or not. This could cause napi_disable
being called twice, which could generate deadlock. For example,
the first napi_disable will spin until NAPI_STATE_SCHED is cleared
by napi_complete_done, then set it again.
When napi_disable is called the second time, it will loop infinitely
because no dev->poll will be running to clear NAPI_STATE_SCHED.

To prevent above scenario from happening, call ibmvnic_napi_disable()
which checks if napi is disabled or not before calling napi_disable.

870e04ae45 ibmvnic: queue reset work in system_long_wq
The reset process for ibmvnic commonly takes multiple seconds, clearly
making it inappropriate for schedule_work/system_wq. The reason to make
this change is that ibmvnic's use of the default system-wide workqueue
for a relatively long-running work item can negatively affect other
workqueue users. So, queue the relatively slow reset job to the
system_long_wq.

334c424147 ibmvnic: improve failover sysfs entry
The current implementation relies on H_IOCTL call to issue a
H_SESSION_ERR_DETECTED command to let the hypervisor to send a failover
signal. However, it may not work if there is no backup device or if
the vnic is already in error state,
e.g., "ibmvnic 30000003 env3: rx buffer returned with rc 6".
Add a last resort, that is to schedule a failover reset via CRQ command.

ibmvnic_remove locks multiple spinlocks while disabling interrupts:

As reported by coccinelle, the second _irqsave() overwrites the value
saved in 'flags' by the first _irqsave(),   therefore when the second
_irqrestore() comes,the value in 'flags' is not valid,the value saved
by the first _irqsave() has been lost.
This likely leads to IRQs remaining disabled. So remove the second
_irqsave():

67eb211487 ibmvnic: always store valid MAC address
The last change to ibmvnic_set_mac(), 8fc3672a8a, meant to prevent
users from setting an invalid MAC address on an ibmvnic interface
that has not been brought up yet. The change also prevented the
requested MAC address from being stored by the adapter object for an
ibmvnic interface when the state of the ibmvnic interface is
VNIC_PROBED - that is after probing has finished but before the
ibmvnic interface is brought up. The MAC address stored by the
adapter object is used and sent to the hypervisor for checking when
an ibmvnic interface is brought up.

The ibmvnic driver ignoring the requested MAC address when in
VNIC_PROBED state caused LACP bonds (bonds in 802.3ad mode) with more
than one slave to malfunction. The bonding code must be able to
change the MAC address of its slaves before they are brought up
during enslaving. The inability of kernels with 8fc3672a8a to set
the MAC addresses of bonding slaves is observable in the output of
"ip address show". The MAC addresses of the slaves are the same as
the MAC address of the bond on a working system whereas the slaves
retain their original MAC addresses on a system with a malfunctioning
LACP bond.

386a966f5c vio: make remove callback return void
The driver core ignores the return value of struct bus_type::remove()
because there is only little that can be done. To simplify the quest to
make this function return void, let struct vio_driver::remove() return
void, too. All users already unconditionally return 0, this commit makes
it obvious that returning an error code is a bad idea.

Note there are two nominally different implementations for a vio bus:
one in arch/sparc/kernel/vio.c and the other in
arch/powerpc/platforms/pseries/vio.c. This patch only adapts the powerpc
one.

Before this patch for a device that was bound to a driver without a
remove callback vio_cmo_bus_remove(viodev) wasn't called. As the device
core still considers the device unbound after vio_bus_remove() returns
calling this unconditionally is the consistent behaviour which is
implemented here.

8f1c0fd2c8 ibmvnic: fix a race between open and reset
__ibmvnic_reset() currently reads the adapter->state before getting the
rtnl and saves that state as the "target state" for the reset. If this
read occurs when adapter is in PROBED state, the target state would be
PROBED.

Just after the target state is saved, and before the actual reset process
is started (i.e before rtnl is acquired) if we get an ibmvnic_open() call
we would move the adapter to OPEN state.

But when the reset is processed (after ibmvnic_open()) drops the rtnl),
it will leave the adapter in PROBED state even though we already moved
it to OPEN.

To fix this, use the RTNL to improve serialization when reading/updating
the adapter state. i.e determine the target state of a reset only after
getting the RTNL. And if a reset is in progress during an open, simply
set the target state of the adapter and let the reset code finish the
open (like we currently do if failover is pending).

One twist to this serialization is if the adapter state changes when we
drop the RTNL to update the link state. Account for this by checking if
there was an intervening open and update the target state for the reset
accordingly (see new comments in the code). Note that only the reset
functions and ibmvnic_open() can set the adapter to OPEN state and this
must happen under rtnl.

4a41c421f3 ibmvnic: serialize access to work queue on remove
The work queue is used to queue reset requests like CHANGE-PARAM or
FAILOVER resets for the worker thread. When the adapter is being removed
the adapter state is set to VNIC_REMOVING and the work queue is flushed
so no new work is added. However the check for adapter being removed is
racy in that the adapter can go into REMOVING state just after we check
and we might end up adding work just as it is being flushed (or after).

The ->rwi_lock is already being used to serialize queue/dequeue work.
Extend its usage ensure there is no race when scheduling/flushing work.

Timeout reset will trigger the VIOS to unmap it automatically,
similarly as FAILVOER and MOBILITY events. If we unmap it
in the linux side, we will see errors like

So, don't call send_request_unmap for timeout reset.

buffer to zero. After doing that, it is not necessary to

ef66a1eace ibmvnic: Clear failover_pending if unable to schedule
Normally we clear the failover_pending flag when processing the reset.
But if we are unable to schedule a failover reset we must clear the
flag ourselves. We could fail to schedule the reset if we are in PROBING
state (eg: when booting via kexec) or because we could not allocate memory.

Thanks to Cris Forno for helping isolate the problem and for testing.

5e9eff5dfa ibmvnic: device remove has higher precedence over reset
Returning -EBUSY in ibmvnic_remove() does not actually hold the
removal procedure since driver core doesn't care for the return
value (see __device_release_driver() in drivers/base/dd.c
calling dev->bus->remove()) though vio_bus_remove
(in arch/powerpc/platforms/pseries/vio.c) records the
return value and passes it on. [1]

During the device removal precedure, checking for resetting
bit is dropped so that we can continue executing all the
cleanup calls in the rest of the remove function. Otherwise,
it can cause latent memory leaks and kernel crashes.

Ensure that received Command-Response Queue (CRQ) entries are
properly read in order by the driver. dma_rmb barrier has
been added before accessing the CRQ descriptor to ensure
the entire descriptor is read before processing.

3f5ec374ae ibmvnic: merge do_change_param_reset into do_reset
Commit b27507bb59 ("net/ibmvnic: unlock rtnl_lock in reset so
linkwatch_event can run") introduced do_change_param_reset function to
solve the rtnl lock issue. Majority of the code in do_change_param_reset
duplicates do_reset. Also, we can handle the rtnl lock issue in do_reset
itself. Hence merge do_change_param_reset back into do_reset to clean up
the code.
