cavium-thunderx enables its PCI device with pcim_enable_device(). This,
implicitly, switches the function pci_request_regions() into managed
mode, where it becomes a devres function.
The PCI subsystem wants to remove this hybrid nature from its
interfaces. To do so, users of the aforementioned combination of
functions must be ported to non-hybrid functions.
Moreover, since both functions are already managed in this driver, the
calls to pci_release_regions() are unnecessary.
Remove the calls to pci_release_regions().
Replace the call to sometimes-managed pci_request_regions() with one to
the always-managed pcim_request_all_regions().

The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it as merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

In thunder_mmc_probe(), we should call of_node_put() when breaking
out of for_each_child_of_node() which has increased and decreased
the refcount during each iteration.

If the regulator probing is not yet finished this driver
might catch a -EPROBE_DEFER. Returning after this condition
did not remove the created platform device. On a repeated
call to the probe function the of_platform_device_create
fails.
Calling of_platform_device_destroy after EPROBE_DEFER resolves
this bug.
