The macro p_to_snd_ivtv_card() is just a variant for container_of(),
not actually used inside the code. So, drop it.
This address a clang warning.

MODULE_SUPPORTED_DEVICE was added in pre-git era and never was
implemented. We can safely remove it, because the kernel has grown
to have many more reliable mechanisms to determine if device is
supported or not.

ivtv-alsa-mixer functions was introduced in commit 269c11fbac
("[media] ivtv, ivtv-alsa: Add initial ivtv-alsa interface driver for ivtv")
But according to commit message, ivtv-alsa-mixer.c was already dead code.
5 years after, we should remove it.

Move the pr_fmt() macro to ivtv_driver.h and ensure that it
will be the first file to be included on all ivtv files.
While here, put the includes inside ivtv-driver.h on
alphabetic order.

Due to the 80-cols restrictions, and latter due to checkpatch
warnings, several strings were broken into multiple lines. This
is not considered a good practice anymore, as it makes harder
to grep for strings at the source code.
As we're right now fixing other drivers due to KERN_CONT, we need
to be able to identify what printk strings don't end with a "\n".
It is a way easier to detect those if we don't break long lines.
So, join those continuation lines.
The patch was generated via the script below, and manually

When using multiple capture cards, it might be necessary to identify a
specific device with an ALSA one. If not, the order of the ALSA devices
might have no relation to the id of the radio or video device.

Commits 5e6e81b289 (cx18) and
2aebbf6737 (ivtv) added an __init
annotation to the cx18-alsa-load and ivtv-alsa-load functions. However,
these functions are called *after* initialization by the main cx18/ivtv
driver. By that time the memory containing those functions is already
freed and your machine goes BOOM.

This is a cut-and-paste port of the cx18-alsa driver to
create an ivtv-alsa interface module for the ivtv driver.
It is not actually hooked-up to the PCM stream DMA buffers
from the ivtv driver yet. That will be done in a coming change,
since that portion is so very different from the cx18 driver.
This code has all or more of the bugs and shortcomings of the
cx18-alsa interface driver: inconsistent use of itvsc->slock,
ivtv-alsa-mixer.c is dead code, assumes 48 ksps regardless
of the actual setting of the audio capture, problems with
proper struct ivtv and struct ivtv_stream housekeeping,
struct ivtv_open_id.v4l2_fh abuse, and $DIETY knows what else.
