This patch removes kmem cache allocation and deallocation in favor of
having the ipoib_txreq in the ring.
The consumer is now the packet sending side allocating tx descriptors from
ring and the producer is the napi interrupt handling freeing tx
descriptors.
The locks are now eliminated because the napi tx lock insures a single
consumer and the napi handling insures a single producer.
The napi poll is converted to memory poll looking for items that have been
marked completed.

kmalloc_array() is called to allocate memory for tx->descp. If it fails,
the function __sdma_txclean() is called:
__sdma_txclean(dd, tx);
However, in the function __sdma_txclean(), tx-descp is dereferenced if
tx->num_desc is not zero:
sdma_unmap_desc(dd, &tx->descp[0]);
To fix this possible null-pointer dereference, assign the return value of
kmalloc_array() to a local variable descp, and then assign it to tx->descp
if it is not NULL. Otherwise, go to enomem.

It is possible for the primary IPoIB network device associated with any
RDMA device to fail to join certain multicast groups preventing IPv6
neighbor discovery and possibly other network ULPs from working
correctly. The IPv4 broadcast group is not affected as the IPoIB network
device handles joining that multicast group directly.
This is because the primary IPoIB network device uses the pkey at ndex 0
in the associated RDMA device's pkey table. Anytime the pkey value of
index 0 changes, the primary IPoIB network device automatically modifies
it's broadcast address (i.e. /sys/class/net/[ib0]/broadcast), since the
broadcast address includes the pkey value, and then bounces carrier. This
includes initial pkey assignment, such as when the pkey at index 0
transitions from the opa default of invalid (0x0000) to some value such as
the OPA default pkey for Virtual Fabric 0: 0x8001 or when the fabric
manager is restarted with a configuration change causing the pkey at index
0 to change. Many network ULPs are not sensitive to the carrier bounce and
are not expecting the broadcast address to change including the linux IPv6
stack. This problem does not affect IPoIB child network devices as their
pkey value is constant for all time.
To mitigate this issue, change the default pkey in at index 0 to 0x8001 to
cover the predominant case and avoid issues as ipoib comes up and the FM
sweeps.
At some point, ipoib multicast support should automatically fix
non-broadcast addresses as it does with the primary broadcast address.

try to remove it again in the future.
- tgid mapping fixed to handle more than PID_MAX_DEFAULT pids/tgids.
- New boot command line option "tp_printk_stop", as tp_printk causes trace
events to write to console. When user space starts, this can easily live
lock the system. Having a boot option to stop just after boot up is
useful to prevent that from happening.
- Have ftrace_dump_on_oops boot command line option take numbers that match
the numbers shown in /proc/sys/kernel/ftrace_dump_on_oops.
- Bootconfig clean ups, fixes and enhancements.
- New ktest script that tests bootconfig options.
- Add tracepoint_probe_register_may_exist() to register a tracepoint
without triggering a WARN*() if it already exists. BPF has a path from

The __assign_str macro has an unusual ending semicolon but the vast
majority of uses of the macro already have semicolon termination.

init_port was only being used to register sysfs attributes against the
port kobject. Now that all users are creating static attribute_group's we
can simply set the attribute_group list in the ops and the core code can
just handle it directly.
This makes all the sysfs management quite straightforward and prevents any
driver from abusing the naked port kobject in future because no driver
code can access it.

hfi1 should not be creating a mess of kobjects to attach to the port
kobject - this is all attributes. The proper API is to create an
attribute_group list and create it against the port's kobject.

This is being used to implement both the port and device global stats,
which is causing some confusion in the drivers. For instance EFA and i40iw
both seem to be misusing the device stats.
Split it into two ops so drivers that don't support one or the other can
leave the op NULL'd, making the calling code a little simpler to
understand.

The AIP and VNIC implementation turns that model inside out and used a
kfree() to free what appears to be a netdev struct when in reality, it is
a struct that enbodies the rx state as well as the dummy netdev used to
support napi_poll across disparate receive contexts. The relationship is
infered by the odd allocation:

The code currently assumes that the mmu_notifier struct
embedded in mmu_rb_handler only contains two fields.
There are now extra fields:

The completion ring for tx is using the wrong size to size the ring,
oversizing the ring by two orders of magniture.
Correct the allocation size and use kcalloc_node() to allocate the ring.
Fix mistaken GFP defines in similar allocations.

The current rdma_netdev handling in ipoib hooks the tx_timeout handler,
but prints out a totally useless message that prevents effective debugging
especially when multiple transmit queues are being used.
Add a tx_timeout rdma_netdev hook and implement the callback in the hfi1
to print additional information.
The existing non-helpful message is avoided when the driver has presented
a callback.

Current code uses many different types when dealing with a port of a RDMA
device: u8, unsigned int and u32. Switch to u32 to clean up the logic.
This allows us to make (at least) the core view consistent and use the
same type. Unfortunately not all places can be converted. Many uverbs
functions expect port to be u8 so keep those places in order not to break
UAPIs. HW/Spec defined values must also not be changed.
With the switch to u32 we now can support devices with more than 255
ports. U32_MAX is reserved to make control logic a bit easier to deal
with. As a device with U32_MAX ports probably isn't going to happen any
time soon this seems like a non issue.
When a device with more than 255 ports is created uverbs will report the
RDMA device as having 255 ports as this is the max currently supported.
The verbs interface is not changed yet because the IBTA spec limits the
port size in too many places to be u8 and all applications that relies in
verbs won't be able to cope with this change. At this stage, we are
extending the interfaces that are using vendor channel solely
Once the limitation is lifted mlx5 in switchdev mode will be able to have
thousands of SFs created by the device. As the only instance of an RDMA
device that reports more than 255 ports will be a representor device and
it exposes itself as a RAW Ethernet only device CM/MAD/IPoIB and other
ULPs aren't effected by this change and their sysfs/interfaces that are
exposes to userspace can remain unchanged.
While here cleanup some alignment issues and remove unneeded sanity
checks (mainly in rdmavt),

Two earlier bug fixes have created a security problem in the hfi1
driver. One fix aimed to solve an issue where current->mm was not valid
when closing the hfi1 cdev. It attempted to do this by saving a cached
value of the current->mm pointer at file open time. This is a problem if
another process with access to the FD calls in via write() or ioctl() to
pin pages via the hfi driver. The other fix tried to solve a use after
free by taking a reference on the mm.
To fix this correctly we use the existing cached value of the mm in the
mmu notifier. Now we can check in the insert, evict, etc. routines that
current->mm matched what the notifier was registered for. If not, then
don't allow access. The register of the mmu notifier will save the mm
pointer.
Since in do_exit() the exit_mm() is called before exit_files(), which
would call our close routine a reference is needed on the mm. We rely on
the mmgrab done by the registration of the notifier, whereas before it was
explicit. The mmu notifier deregistration happens when the user context is
torn down, the creation of which triggered the registration.
Also of note is we do not do any explicit work to protect the interval
tree notifier. It doesn't seem that this is going to be needed since we
aren't actually doing anything with current->mm. The interval tree
notifier stuff still has a FIXME noted from a previous commit that will be
addressed in a follow on patch.

Make changes to use sysfs_emit in the RDMA code as cocci scripts can not
be written to handle _all_ the possible variants of various sprintf family
uses in sysfs show functions.
While there, make the code more legible and update its style to be more
like the typical kernel styles.

The following message occurs when running an AI application with TID RDMA
enabled:

The issue happens when TID RDMA WRITE request is followed by an
IB_WR_RDMA_WRITE_WITH_IMM request, the latter could be completed first on
the responder side. As a result, no ACK packet for the latter could be
sent because the TID RDMA WRITE request is still being processed on the
responder side.
When the TID RDMA WRITE request is eventually completed, the requester
will wait for the IB_WR_RDMA_WRITE_WITH_IMM request to be acknowledged.
If the next request is another TID RDMA WRITE request, no TID RDMA WRITE
DATA packet could be sent because the preceding IB_WR_RDMA_WRITE_WITH_IMM
request is not completed yet.
Consequently the IB_WR_RDMA_WRITE_WITH_IMM will be retried but it will be
ignored on the responder side because the responder thinks it has already
been completed. Eventually the retry will be exhausted and the qp will be
put into error state on the requester side. On the responder side, the TID
resource timer will eventually expire because no TID RDMA WRITE DATA
packets will be received for the second TID RDMA WRITE request. There is
also risk of a write-after-write memory corruption due to the issue.
Fix by adding a requester side interlock to prevent any potential data
corruption and TID RDMA protocol error.

pcie_speeds() and restore_pci_variables() returns PCIBIOS_ error codes
from PCIe capability accessors.
PCIBIOS_ error codes have positive values. Passing on these values is
inconsistent with functions which return only a negative value on failure.
Before passing on the return value of PCIe capability accessors, call
pcibios_err_to_errno() to convert any positive PCIBIOS_ error codes to
negative generic error values.

When running iperf in a two host configuration the following trace can
occur:
[ 319.728730] NETDEV WATCHDOG: ib0 (hfi1): transmit queue 0 timed out
The issue happens because the current implementation relies on the netif
txq being stopped to control the flushing of the tx list.
There are two resources that the transmit logic can wait on and stop the
txq:
- SDMA descriptors
- Ring space to hold completions
The ring space is tested on the sending side and relieved when the ring is
consumed in the napi tx reaping.
Unfortunately, that reaping can run conncurrently with the workqueue
flushing of the txlist. If the txq is started just before the workitem
executes, the txlist will never be flushed, leading to the txq being
stuck.
Fix by:
- Adding sleep/wakeup wrappers
* Use an atomic to control the call to the netif routines inside the
wrappers
- Use another atomic to record ring space exhaustion
* Only wakeup when the a ring space exhaustion has happened and it
relieved
Add additional wrappers to clarify the ring space resource handling.

The current code mishandles -EBUSY in two ways:
- The flow change doesn't test the return from the flush and runs on to
process the current packet racing with the wakeup processing
- The -EBUSY handling for a single packet inserts the tx into the txlist
after the submit call, racing with the same wakeup processing
Fix the first by dropping the skb and returning NETDEV_TX_OK.
Fix the second by insuring the the list entry within the txreq is inited
when allocated. This enables the sleep routine to detect that the txreq
has used the non-list api and queue the packet to the txlist.
Both flaws can lead to having the flushing thread executing in causing two
threads to manipulate the txlist.

When the try_module_get calls were removed from opening and closing of the
i2c debugfs file, the corresponding module_put calls were missed. This
results in an inaccurate module use count that requires a power cycle to
fix.

The hfi1_vnic_up() function doesn't check whether hfi1_netdev_rx_init()
returns errors. In hfi1_vnic_init() we need to change the code to
preserve the error code instead of returning success.

As described in earlier patches, ipoib netdev will share receive
contexts with existing VNIC netdev through a dummy netdev. The
following changes are made to achieve that:
- Set up netdev receive contexts after user contexts. A function is
added to count the available netdev receive contexts.
- Add functions to set/get receive map table free index.
- Rename NUM_VNIC_MAP_ENTRIES as NUM_NETDEV_MAP_ENTRIES.
- Let the dummy netdev own the receive contexts instead of VNIC.
- Allocate the dummy netdev when the hfi1 device is added and free it
when the device is removed.
- Initialize AIP RSM rules when the IpoIb rxq is initialized and
remove the rules when it is de-initialized.
- Convert VNIC to use the dummy netdev.

Ipoib netdev will share receive contexts with existing VNIC netdev.
To achieve that, a dummy netdev is allocated with hfi1_devdata to
own the receive contexts, and ipoib and VNIC netdevs will be put
on top of it. Each receive context is associated with a single
NAPI object.
This patch adds the functions to receive incoming packets for
accelerated ipoib.

Currently the ipoib UD mtu is restricted to 4K bytes. Remove this
limitation so that the IPOIB module can potentially use an MTU (in UD
mode) that is bounded by the MTU of the underlying device. A field is
added to the ib_port_attr structure to indicate the maximum physical
MTU the underlying device supports.

This is implementation of RSM rule for AIP packets.
AIP rule will use rule RSM2 and will match standard
Infiniband packet containg BTH (LNH==BTH) and
having Dest QPN prefixed with value 0x81. Spread between
receive contexts will be done using source QPN bits.
VNIC and AIP will share receive contexts, so their rules
will point to the same RMT entries and their shared
code is moved to separate functions.
If any of the rules is active RMT mapping will be skipped
for latter.
Changed function hfi1_vnic_is_rsm_full to be more general
and moved it from main header to chip.c.
Changed the order of RSM rules because AIP rule as
more specific one is needed to be placed before more
general QOS rule. Rules are occupying two last RSM
registers.

Adds capability to create a qpn to be recognized as an accelerated
UD QP for ipoib.
This is accomplished by reserving 0x81 in byte[0] of the qpn as the
prefix for these qp types and reserving qpns between 0x810000 and
0x81ffff.
The hfi1 capability mask already contained a flag for the VNIC netdev.
This has been renamed and extended to include both VNIC and ipoib.
The rvt code to allocate qps now recognizes this flag and sets 0x81
into byte[0] of the qpn.
The code to allocate qpns is modified to reset the qpn numbering when it
is detected that a value is located in byte[0] for a UD QP and it is a
qpn being requested for net dev use. If it is a regular UD QP then it is
allowable to have bits set in byte[0] of the qpn and provide the
previously normal behavior.
The code to free the qpn now checks for the AIP prefix value of 0x81 and
removes it from the qpn before being freed so that the lower 16 bit
number can be reused.
This patch requires minor changes in the IB core and ipoib to facilitate
the creation of accelerated UP QPs.

The module parameter for KDETH qpns is being removed in favor
of always using the default value of 0x80 as the qpn prefix.

This implements the transmit side of the multiple transmit queue RDMA
netdev used to accelerate ipoib. The receive side remains the ipoib
internal implementation.
The init/unint/open/stop netdev operations are saved off and called by the
versions within the hfi1 netdev in order to initialize the connected mode
resources present in ipoib thus allowing us to switch modes between
datagram and connected.
The datagram queue pair instantiated by the ipoib ulp is used by this
implementation for its queue pair number and to register with multicast.
The above queue pair is not used on transmit other than its qpn as the
verbs layer is skipped and packets are directly submitted to the sdma
engines.

This patch implements the mechanism to accelerate the transmit side of
a multiple transmit queue RDMA netdev by submitting the packets to
the SDMA engine directly instead of sending through the verbs layer.
This patch also changes the UD/SEND_ONLY op to output the entropy value
in byte 0 of deth[1]. UD/SEND_ONLY_WITH_IMMEDIATE uses the previous
behavior with no entropy value being output.
The code in the ipoib rdma netdev which submits tx requests upon
successful submission will call trace_sdma_output_ibhdr to output
the ibhdr to the trace buffer.

The commit noted below fixed a case where a pq is left on the sdma wait
list.
It however missed another case.
user_sdma_send_pkts() has two calls from hfi1_user_sdma_process_request().
If the first one fails as indicated by -EBUSY, the pq will be placed on
the waitlist as by design.
If the second call then succeeds, the pq is still on the waitlist setting
up a race with the interrupt handler if a subsequent request uses a
different SDMA engine
Fix by deleting the first call.
The use of pcount and the intent to send a short burst of packets followed
by the larger balance of packets was never correctly implemented, because
the two calls always send pcount packets no matter what. A subsequent
patch will correct that issue.

When kobject_init_and_add() returns an error in the function
hfi1_create_port_files(), the function kobject_put() is not called for the
corresponding kobject, which potentially leads to memory leak.
This patch fixes the issue by calling kobject_put() even if
kobject_init_and_add() fails.

The issue happens when wait_event_interruptible_timeout() returns a value
<= 0.
In that case, the pq is left on the list. The code continues sending
packets and potentially can complete the current request with the pq still
on the dmawait list provided no descriptor shortage is seen.
If the pq is torn down in that state, the sdma interrupt handler could
find the now freed pq on the list with list corruption or memory
corruption resulting.
Fix by adding a flush routine to ensure that the pq is never on a list
after processing a request.

The seqlock use for sdma will then be converted to a spin lock since the

The packet handling function, specifically the iteration of the qp list
for mad packet processing misses locking RCU before running through the
list. Not only is this incorrect, but the list_for_each_entry_rcu() call
can not be called with a conditional check for lock dependency. Remedy
this by invoking the rcu lock and unlock around the critical section.
This brings MAD packet processing in line with what is done for non-MAD
packets.

The current codebase makes use of the zero-length array language
extension to the C90 standard, but the preferred mechanism to declare
variable-length types such as these ones is a flexible array member[1][2],
introduced in C99:

By making use of the mechanism above, we will get a compiler warning
in case the flexible array does not occur last in the structure, which
will help us prevent some kind of undefined behavior bugs from being
inadvertently introduced[3] to the codebase from now on.
Also, notice that, dynamic memory allocations won't be affected by
this change:
"Flexible array members have incomplete type, and so the sizeof operator
may not be applied. As a quirk of the original implementation of
zero-length arrays, sizeof evaluates to zero."[1]
This issue was found with the help of Coccinelle.

Each user context is allocated a certain number of RcvArray (TID)
entries and these entries are managed through TID groups. These groups
are put into one of three lists in each user context: tid_group_list,
tid_used_list, and tid_full_list, depending on the number of used TID
entries within each group. When TID packets are expected, one or more
TID groups will be allocated. After the packets are received, the TID
groups will be freed. Since multiple user threads may access the TID
groups simultaneously, a mutex exp_mutex is used to synchronize the
access. However, when the user file is closed, it tries to release
all TID groups without acquiring the mutex first, which risks a race
condition with another thread that may be releasing its TID groups,
leading to data corruption.
This patch addresses the issue by acquiring the mutex first before
releasing the TID groups when the file is closed.

Convert infiniband to use the new pin_user_pages*() calls.
Also, revert earlier changes to Infiniband ODP that had it using
put_user_page(). ODP is "Case 3" in
Documentation/core-api/pin_user_pages.rst, which is to say, normal
get_user_pages() and put_page() is the API to use there.
The new pin_user_pages*() calls replace corresponding get_user_pages*()
calls, and set the FOLL_PIN flag. The FOLL_PIN flag requires that the
caller must return the pages via put_user_page*() calls, but infiniband
was already doing that as part of an earlier commit.

IRQ name was connected to IRQ type, this is not sufficient and it would be
better to use name as argument to msix_request_irq instead of assigning it
to variables when function is called.
Index argument was required to generate name and now it can be removed.
To generate name correctly helpers function were added and updated.

This patch eliminate special cases by adding a fast_handler member to the
receive context and changes to the fast handler as specified in the new
variable. Initialize the variable as soon as the setting for dma tail is
known when the context is created.
Setting fast path is called every time when any context has entered slow
path. Add function to check if contexts is using fast path and do not set
fast path when it is already done to improve RCD fastpath setting.

Move routines and defines associated with hdrq size validation to a chip
specific routine since the limits are specific to the device.
Fix incorrect value for min size 2 -> 32
CSR writes should also be in chip.c.
Create a chip routine to write the hdrq specific CSRs and call as
appropriate.

When a TID RDMA ACK to RESYNC request is received, the flow PSNs for
pending TID RDMA WRITE segments will be adjusted with the next flow
generation number, based on the resync_psn value extracted from the flow
PSN of the TID RDMA ACK packet. The resync_psn value indicates the last
flow PSN for which a TID RDMA WRITE DATA packet has been received by the
responder and the requester should resend TID RDMA WRITE DATA packets,
starting from the next flow PSN.
However, if resync_psn points to the last flow PSN for a segment and the
next segment flow PSN starts with a new generation number, use of the old
resync_psn to adjust the flow PSN for the next segment will lead to
miscalculation, resulting in WARN_ON and sge rewinding errors:

The current debugfs output for receive contexts (rcds), stops after the
kernel receive contexts have been displayed. This is not enough
information to fully diagnose packet drops.
Display all of the receive contexts.
Augment the output with some more context information.
Limit the ring buffer header output to 5 entries to avoid overextending
the sequential file output.

In the iowait structure, two iowait_work entries were included to queue a
given object: one for normal IB operations, and the other for TID RDMA
operations. For non-TID RDMA operations, the iowait_work structure for TID
RDMA is initialized to contain a NULL function (not used). When the QP is
reset, the function iowait_cancel_work will be called to cancel any
pending work. The problem is that this function will call
cancel_work_sync() for both iowait_work entries, even though the one for
TID RDMA is not used at all. Eventually, the call cascades to
__flush_work(), wherein a WARN_ON will be triggered due to the fact that
work->func is NULL.
The WARN_ON was introduced in commit 4d43d395fe ("workqueue: Try to
catch flush_work() without INIT_WORK().")
This patch fixes the issue by making sure that a work function is present
for TID RDMA before calling cancel_work_sync in iowait_cancel_work.

the driver pattern to use mmu notifiers to monitor a VA range. This code
is lifted out of many drivers and hmm_mirror directly into the
mmu_notifier core and written using the best ideas from all the driver

Rework event_create_dir() to use an array of static data instead of
function pointers where possible.
The problem is that it would call the function pointer on module load
before parse_args(), possibly even before jump_labels were initialized.
Luckily the generated functions don't use jump_labels but it still seems
fragile. It also gets in the way of changing when we make the module map
executable.
The generated function are basically calling trace_define_field() with a
bunch of static arguments. So instead of a function, capture these
arguments in a static array, avoiding the function call.
Now there are a number of cases where the fields are dynamic (syscall
arguments, kprobes and uprobes), in which case a static array does not
work, for these we preserve the function call. Luckily all these cases
are not related to modules and so we can retain the function call for
them.
Also fix up all broken tracepoint definitions that now generate a
compile error.

Normal RDMA WRITE request never returns IB_WC_RNR_RETRY_EXC_ERR to ULPs
because it does not need post receive buffer on the responder side.
Consequently, as an enhancement to normal RDMA WRITE request inside the
hfi1 driver, TID RDMA WRITE request should not return such an error status
to ULPs, although it does receive RNR NAKs from the responder when TID
resources are not available. This behavior is violated when
qp->s_rnr_retry_cnt is set in current hfi1 implementation.
This patch enforces these semantics by avoiding any reaction to the updates
of the RNR QP attributes.

For a TID RDMA WRITE request, a QP on the responder side could be put into
a queue when a hardware flow is not available. A RNR NAK will be returned
to the requester with a RNR timeout value based on the position of the QP
in the queue. The tid_rdma_flow_wt variable is used to calculate the
timeout value and is determined by using a MTU of 4096 at the module
loading time. This could reduce the timeout value by half from the desired
value, leading to excessive RNR retries.
This patch fixes the issue by calculating the flow weight with the real
MTU assigned to the QP.

If an hfi1 card is inserted in a Gen4 systems, the driver will avoid the
gen3 speed bump and the card will operate at half speed.
This is because the driver avoids the gen3 speed bump when the parent bus
speed isn't identical to gen3, 8.0GT/s. This is not compatible with gen4
and newer speeds.
Fix by relaxing the test to explicitly look for the lower capability
speeds which inherently allows for gen4 and all future speeds.

A TID RDMA READ request could be retried under one of the following
conditions:
- The RC retry timer expires;
- A later TID RDMA READ RESP packet is received before the next
expected one.
For the latter, under normal conditions, the PSN in IB space is used
for comparison. More specifically, the IB PSN in the incoming TID RDMA
READ RESP packet is compared with the last IB PSN of a given TID RDMA
READ request to determine if the request should be retried. This is
similar to the retry logic for noraml RDMA READ request.
However, if a TID RDMA READ RESP packet is lost due to congestion,
header suppresion will be disabled and each incoming packet will raise
an interrupt until the hardware flow is reloaded. Under this condition,
each packet KDETH PSN will be checked by software against r_next_psn
and a retry will be requested if the packet KDETH PSN is later than
r_next_psn. Since each TID RDMA READ segment could have up to 64
packets and each TID RDMA READ request could have many segments, we
could make far more retries under such conditions, and thus leading to
RETRY_EXC_ERR status.
This patch fixes the issue by removing the retry when the incoming
packet KDETH PSN is later than r_next_psn. Instead, it resorts to
RC timer and normal IB PSN comparison for any request retry.

1. Lots of call sites become simpler if a bool is passed into
put_user_page*(), instead of making the call site choose which
put_user_page*() variant to call.
2. Christoph Hellwig's observation that set_page_dirty_lock() is
usually correct, and set_page_dirty() is usually a bug, or at least
questionable, within a put_user_page*() calling chain.
This leads to the following API choices:
* put_user_pages_dirty_lock(page, npages, make_dirty)
* There is no put_user_pages_dirty(). You have to
hand code that, in the rare case that it's
required.

In a congested fabric with adaptive routing enabled, traces show that
packets could be delivered out of order. A stale TID RDMA data packet
could lead to TidErr if the TID entries have been released by duplicate
data packets generated from retries, and subsequently erroneously force
the qp into error state in the current implementation.
Since the payload has already been dropped by hardware, the packet can
be simply dropped and it is no longer necessary to put the qp into
error state.

When processing a TID RDMA READ RESP packet that causes KDETH EFLAGS
errors, the packet's IB PSN is checked against qp->s_last_psn and
qp->s_psn without the protection of qp->s_lock, which is not safe.
This patch fixes the issue by acquiring qp->s_lock first.

In a congested fabric with adaptive routing enabled, traces show that
the sender could receive stale TID RDMA NAK packets that contain newer
KDETH PSNs and older Verbs PSNs. If not dropped, these packets could
cause the incorrect rewinding of the software flows and the incorrect
completion of TID RDMA WRITE requests, and eventually leading to memory
corruption and kernel crash.
The current code drops stale TID RDMA ACK/NAK packets solely based
on KDETH PSNs, which may lead to erroneous processing. This patch
fixes the issue by also checking the Verbs PSN. Addition checks are
added before rewinding the TID RDMA WRITE DATA packets.

sl is controlled by user-space, hence leading to a potential
exploitation of the Spectre variant 1 vulnerability.
Fix this by sanitizing sl before using it to index ibp->sl_to_sc.
Notice that given that speculation windows are large, the policy is
to kill the speculation on the first load and not worry if it can be
completed with a dependent load/store [1].
