drm/vkms: Remove index parameter from init_vkms_output
VKMS currently supports only one CRTC, so it make no sense to have this
index configurable. To avoid issues, replace this hardcoded index by
drm_crtc_mask when applicable.

There is no need to manually set a crtc mask on primary and cursor plane
as it is automatically set by drmm_crtc_alloc_with_planes.

In addition, this will remove the use of an uninitialized structure in
vkms_add_overlay_plane. This currently works by chance because two things:
- vkms_plane_init always set a possible_crtcs value, so the problematic
  branch is never used;
- drm_crtc_mask on an kzalloc'd drm_crtc returns BIT(0), and the VKMS CRTC
  always have this id.

drm/vkms: Remove <drm/drm_simple_kms_helper.h> include
The driver doesn't use simple-KMS helpers to set a simple display pipeline
but only the drm_simple_encoder_init() function to initialize an encoder.

That helper is just a wrapper of drm_encoder_init(), but passing a struct

Since the <drm/drm_simple_kms_helper.h> header is only included for this
helper and because the connector is initialized with drm_connector_init()
as well, do the same for the encoder and drop the header include.

1ae752bf39 drm/vkms: Remove useless call to drm_connector_register/unregister()
The function vkms_output_init() is invoked during the module
initialization, and it handles the creation/configuration of the vkms
essential elements (e.g., connectors, encoder, etc). Among the
initializations, this function tries to initialize a connector and
register it by calling drm_connector_register(). However, inside the
drm_connector_register(), at the beginning of this function there is the
following validation:

 if (!connector->dev->registered)
   return 0;

In this sense, invoke drm_connector_register() after initializing the
connector has no effect because the register field is false. The
connector register happens when drm_dev_register() is invoked; the same
issue exists with drm_connector_unregister(). Therefore, this commit
removes the unnecessary call to drm_connector_register() and
drm_connector_unregister().

Changes since v2:
* Remove unnecessary call to drm_connector_unregister()
* Remove unused label

fcd70cd36b drm: Split out drm_probe_helper.h
Having the probe helper stuff (which pretty much everyone needs) in
the drm_crtc_helper.h file (which atomic drivers should never need) is
confusing. Split them out.

To make sure I actually achieved the goal here I went through all
drivers. And indeed, all atomic drivers are now free of
drm_crtc_helper.h includes.

v2: Make it compile. There was so much compile fail on arm drivers
that I figured I'll better not include any of the acks on v1.

v3: Massive rebase because i915 has lost a lot of drmP.h includes, but
not all: Through drm_crtc_helper.h > drm_modeset_helper.h -> drmP.h
there was still one, which this patch largely removes. Which means
rolling out lots more includes all over.

This will also conflict with ongoing drmP.h cleanup by others I
expect.

v3: Rebase on top of atomic bochs.

v4: Review from Laurent for bridge/rcar/omap/shmob/core bits:
- (re)move some of the added includes, use the better include files in
  other places (all suggested from Laurent adopted unchanged).
- sort alphabetically

v5: Actually try to sort them, and while at it, sort all the ones I
touch.

v6: Rebase onto i915 changes.

v7: Rebase once more.

854502fa0a drm/vkms: Add basic CRTC initialization
This commit adds the essential infrastructure for around CRTCs which
is composed of: a new data struct for output data information, a
function for creating planes, and a simple encoder attached to the
connector. Finally, due to the introduction of a new initialization
function, connectors were moved from vkms_drv.c to vkms_display.c.
