017bbc922a clk: samsung: Drop unused clk.h and of.h headers
<clk.h> header is for clock consumers, so drop its include from the
Samsung clock controller drivers which do not use the consumer API
(there are few which do, so leave it there).

Drop including of <of.h> and <of_address.h> headers for all drivers
which do not use anything from generic OF API or of_iomap().

a4c78367f6 clk: samsung: Set dev in samsung_clk_init()
Some drivers set dev to context in order to implement PM. Make that part
of samsung_clk_init() instead of assigning `ctx->dev = dev' separately.

No functional change.

65bf1fbe78 clk: samsung: Don't pass reg_base to samsung_clk_register_pll()
Base address can be derived from context structure. Remove `base'
argument from samsung_clk_register_pll() and use `ctx->reg_base'
instead, as it's done in other clock registering functions.

No functional change.

015e70585b clk: samsung: remove __clk_lookup() usage
__clk_lookup() interface is obsolete, so remove it from the Samsung clock
drivers. This has been achieved by getting rid of custom _get_rate()
helper and replacing it with clk_hw_get_rate().

Almost no functional change, the only difference is in handling

ba30011577 clk: s5pv210: add missing call to samsung_clk_of_add_provider()
Commit d5e136a21b ("clk: samsung: Register
clk provider only after registering its all clocks", merged to v3.17-rc1)
modified a way that driver registers registers to core framework. This
change has not been applied to s5pv210 clocks driver, which has been
merged in parallel to that commit. This patch adds a missing call to
samsung_clk_of_add_provider(), so the driver is operational again.

6f1ed07a14 clk: samsung: Properly include clk.h and clkdev.h
Clock provider drivers generally shouldn't include clk.h because
it's the consumer API. Only include clk.h in files that are
using it. The clkdev.h header isn't always used either, so remove
it and add in slab.h where files were relying on it to include
slab for them.

692d8328e8 clk: don't use __initconst for non-const arrays
The statement

	static const char *name[];

defines a modifiable array of pointers to constant chars. That is

	*name[0] = 'f';

is forbidden, but

	name[0] = "f";

is not. So marking an array that is defined as above with __initconst is
wrong. Either an additional const must be added such that the whole
definition reads:

	static const char *const name[] __initconst;

or where this is not possible __initdata must be used.
