Inside ublk_abort_requests(), gendisk is grabbed for aborting all
inflight requests. And ublk_abort_requests() is called when exiting
the uring context or handling timeout.
If add_disk() fails, the gendisk may have been freed when calling
ublk_abort_requests(), so use-after-free can be caused when getting
disk's reference in ublk_abort_requests().
Fixes the bug by detaching gendisk from ublk device if add_disk() fails.

BLK_MQ_F_SHOULD_MERGE is set for all tag_sets except those that purely
process passthrough commands (bsg-lib, ufs tmf, various nvme admin
queues) and thus don't even check the flag. Remove it to simplify the
driver interface.

max_zone_append_sectors differs from all other queue limits in that the
final value used is not stored in the queue_limits but needs to be
obtained using queue_limits_max_zone_append_sectors helper. This not
only adds (tiny) extra overhead to the I/O path, but also can be easily
forgotten in file system code.
Add a new max_hw_zone_append_sectors value to queue_limits which is
set by the driver, and calculate max_zone_append_sectors from that and
the other inputs in blk_validate_zoned_limits, similar to how
max_sectors is calculated to fix this.

In ublk_ch_mmap(), queue id is calculated in the following way:
(vma->vm_pgoff << PAGE_SHIFT) / `max_cmd_buf_size`
'max_cmd_buf_size' is equal to
`UBLK_MAX_QUEUE_DEPTH * sizeof(struct ublksrv_io_desc)`
and UBLK_MAX_QUEUE_DEPTH is 4096 and part of UAPI, so 'max_cmd_buf_size'
is always page aligned in 4K page size kernel. However, it isn't true in
64K page size kernel.
Fixes the issue by always rounding up 'max_cmd_buf_size' with PAGE_SIZE.

behaviors using flags specified at device creation time, namely:
default behavior: A + 1
UBLK_F_USER_RECOVERY: B + 2
UBLK_F_USER_RECOVERY|UBLK_F_USER_RECOVERY_REISSUE: C + 2
The behavior A + 2 is currently unsupported. Add support for this
behavior under the new flag combination
UBLK_F_USER_RECOVERY|UBLK_F_USER_RECOVERY_FAIL_IO.

Setting UBLK_F_USER_RECOVERY_REISSUE without also setting
UBLK_F_USER_RECOVERY is currently silently equivalent to not setting any
recovery flags at all, even though that's obviously not intended. Check
for this case and fail add_dev (with a paranoid warning to aid debugging
any program which might rely on the old behavior) with EINVAL if it is
detected.

UBLK_F_USER_COPY requires userspace to call write() on ublk char
device for filling request buffer, and unprivileged device can't

When two UBLK_CMD_START_USER_RECOVERY commands are submitted, the
first one sets 'ubq->ubq_daemon' to NULL, and the second one triggers
WARN in ublk_queue_reinit() and subsequently a NULL pointer dereference
issue.
Fix it by adding the check in ublk_ctrl_start_recovery() and return
immediately in case of zero 'ub->nr_queues_ready'.

ublk zoned takes 16 bytes in each request pdu just for handling REPORT_ZONE
operation, this way does waste memory since request pdu is allocated
statically.
Store the transient zone report data into one global xarray, and remove
it after the report zone request is completed. This way is reasonable
since report zone is run in slow code path.

Now that device mapper can handle resetting all zones of a mapped zoned
device using REQ_OP_ZONE_RESET_ALL, all zoned block device drivers
support this operation. With this, the request queue feature
BLK_FEAT_ZONE_RESETALL is not necessary and the emulation code in
blk-zone.c can be removed.

Move the nonrot flag into the queue_limits feature field so that it can
be set atomically with the queue frozen.
Use the chance to switch to defaulting to non-rotational and require
the driver to opt into rotational, which matches the polarity of the
sysfs interface.
For the z2ram, ps3vram, 2x memstick, ubiblock and dcssblk the new
rotational flag is not set as they clearly are not rotational despite
this being a behavior change. There are some other drivers that
unconditionally set the rotational flag to keep the existing behavior
as they arguably can be used on rotational devices even if that is
probably not their main use today (e.g. virtio_blk and drbd).
The flag is automatically inherited in blk_stack_limits matching the
existing behavior in dm and md.

Move the cache control settings into the queue_limits so that the flags
can be set atomically with the device queue frozen.
Add new features and flags field for the driver set flags, and internal
(usually sysfs-controlled) flags in the block layer. Note that we'll
eventually remove enough field from queue_limits to bring it back to the
previous size.
The disable flag is inverted compared to the previous meaning, which
means it now survives a rescan, similar to the max_sectors and
max_discard_sectors user limits.
The FLUSH and FUA flags are now inherited by blk_stack_limits, which
simplified the code in dm a lot, but also causes a slight behavior
change in that dm-switch and dm-unstripe now advertise a write cache
despite setting num_flush_bios to 0. The I/O path will handle this
gracefully, but as far as I can tell the lack of num_flush_bios
and thus flush support is a pre-existing data integrity bug in those
targets that really needs fixing, after which a non-zero num_flush_bios
should be required in dm for targets that map to underlying devices.

By default, this will be 511, as that's the block layer default. But
drivers these days can support memory alignments that aren't tied to
the sector sizes, instead just being limited by what the DMA engine
supports. An example is NVMe, where it's generally set to a 32-bit or
64-bit boundary. As ublk itself doesn't really care, just set it low
enough that we don't run into issues with NVMe where the required
O_DIRECT memory alignment is now more restrictive on ublk than it is
on the underlying device.
This was triggered by spurious -EINVAL returns on O_DIRECT IO on a
setup with ublk managing NVMe devices, which previously worked just
fine on the NVMe device itself. With the alignment relaxed, the test
works fine.

ublk_drv currently creates block devices with the default max_segments
and max_segment_size limits of BLK_MAX_SEGMENTS (128) and
BLK_MAX_SEGMENT_SIZE (65536) respectively. These defaults can
artificially constrain the I/O size seen by the ublk server - for
example, suppose that the ublk server has configured itself to accept
I/Os up to 1M and the application is also issuing 1M sized I/Os. If the
I/O buffer used by the application is backed by 4K pages, the buffer
could consist of up to 1M / 4K = 256 physically discontiguous segments
(even if the buffer is virtually contiguous). As such, the I/O could
exceed the default max_segments limit and get split. This can cause
unnecessary performance issues if the ublk server is optimized to handle
1M I/Os. The block layer's segment count/size limits exist to model
hardware constraints which don't exist in ublk_drv's case, so just
remove those limits for the block devices created by ublk_drv.

The only user of blk_revalidate_disk_zones() second argument was the
SCSI disk driver (sd). Now that this driver does not require this
update_driver_data argument, remove it to simplify the interface of
blk_revalidate_disk_zones(). Also update the function kdoc comment to
be more accurate (i.e. there is no gendisk ->revalidate method).

With zone write plugging enabled at the block layer level, any zone
device can only ever see at most a single write operation per zone.
There is thus no need to request a block scheduler with strick per-zone
sequential write ordering control through the ELEVATOR_F_ZBD_SEQ_WRITE
feature. Removing this allows using a zoned ublk device with any
scheduler, including "none".

The current command UBLK_CMD_DEL_DEV won't return until the device is
released, this way looks more reliable, but makes userspace more
difficult to implement, especially about orders: unmap command
buffer(which holds one ublkc reference), ublkc close,
io_uring_file_unregister, ublkb close.
Add UBLK_CMD_DEL_DEV_ASYNC so that device deletion won't wait release,
then userspace needn't worry about the above order. Actually both loop
and nbd is deleted in this async way.

When zones were first added the SCSI and ATA specs, two different
models were supported (in addition to the drive managed one that
is invisible to the host):
- host managed where non-conventional zones there is strict requirement
to write at the write pointer, or else an error is returned
- host aware where a write point is maintained if writes always happen
at it, otherwise it is left in an under-defined state and the
sequential write preferred zones behave like conventional zones
(probably very badly performing ones, though)
Not surprisingly this lukewarm model didn't prove to be very useful and
was finally removed from the ZBC and SBC specs (NVMe never implemented
it). Due to to the easily disappearing write pointer host software
could never rely on the write pointer to actually be useful for say
recovery.
Fortunately only a few HDD prototypes shipped using this model which
never made it to mass production. Drop the support before it is too
late. Note that any such host aware prototype HDD can still be used
with Linux as we'll now treat it as a conventional HDD.

linux/io_uring.h is slowly becoming a rubbish bin where we put
anything exposed to other subsystems. For instance, the task exit
hooks and io_uring cmd infra are completely orthogonal and don't need
each other's definitions. Start cleaning it up by splitting out all
command bits into a new header file.

Monitor work actually introduces one extra context for handling abort, this
way is easy to cause race, and also introduce extra delay when handling
aborting.
Now we start to support cancelable uring_cmd, so use it instead:
1) this cancel callback is either run from the uring cmd submission task
context or called after the io_uring context is exit, so the callback is
run exclusively with ublk_ch_uring_cmd() and __ublk_rq_task_work().
2) the previous patch freezes request queue when calling ublk_abort_queue(),
which is now completely exclusive with ublk_queue_rq() and
ublk_ch_uring_cmd()/__ublk_rq_task_work().
3) in timeout handler, if all IOs are in-flight, then all uring commands
are completed, uring command canceling can't help us to provide forward
progress any more, so call ublk_abort_requests() in timeout handler.
This way simplifies aborting queue, and is helpful for adding new feature,
such as, relax the limit of using single task for handling one queue.

So far aborting queue ends request when the ubq daemon is exiting, and
it can be run concurrently with ublk_queue_rq(), this way is fragile and
we depend on the tricky usage of UBLK_IO_FLAG_ABORTED for avoiding such
race.
Quiesce queue when aborting queue, and the two code paths can be run
completely exclusively, then it becomes easier to add new ublk feature,
such as relaxing single same task limit for each queue.

ublk_cancel_dev() just calls ublk_cancel_queue() to cancel all pending
io commands after ublk request queue is idle. The only protection is just
the read & write of ubq->nr_io_ready and avoid duplicated command cancel,
so add one per-queue lock with cancel flag for providing this protection,
meantime move ublk_cancel_dev() out of ub->mutex.
Then we needn't to call io_uring_cmd_complete_in_task() to cancel
pending command. And the same cancel logic will be re-used for
cancelable uring command.
This patch basically reverts commit ac5902f84b ("ublk: fix AB-BA lockdep warning").

In well-done ublk server implementation, ublk io command won't be
linked into any link chain. Meantime they are always handled in no-wait
style, so basically io cmd is always handled in submitter task context.
However, the server may set IOSQE_ASYNC, or io command is linked to one
chain mistakenly, then we may still run into io-wq context and
ctx->uring_lock isn't held.
So in case of IO_URING_F_UNLOCKED, schedule this command by
io_uring_cmd_complete_in_task to force running it in submitter task. Then
ublk_ch_uring_cmd_local() is guaranteed to run with context uring_lock held,
and we needn't to worry about sync among submission code path any more.

The dev_id/ub_number is used for the ublk dev's char device's minor
number so it has to fit into MINORMASK. This patch adds checks to prevent
userspace from passing a number that's too large and limits what can be
allocated by the ublk_index_idr for the case where userspace has the
kernel allocate the dev_id/ub_number.

There isn't any reason to not support REQ_OP_ZONE_RESET_ALL given everything
is actually handled in userspace, not mention it is pretty easy to support
RESET_ALL.
So enable REQ_OP_ZONE_RESET_ALL and let userspace handle it.
Verified by 'tools/zbc_reset_zone -all /dev/ublkb0' in libzbc[1] with
libublk-rs based ublk-zoned target prototype[2], follows command line
for creating ublk-zoned:

The added check of 'req_op(req) == REQ_OP_ZONE_APPEND' should have been
done after the request is confirmed as valid.
Actually here, the request should always been true, so add one
WARN_ON_ONCE(!req), meantime move the zone_append check after

Add zoned storage support to ublk: report_zones and operations:
- REQ_OP_ZONE_OPEN
- REQ_OP_ZONE_CLOSE
- REQ_OP_ZONE_FINISH
- REQ_OP_ZONE_RESET
- REQ_OP_ZONE_APPEND
The zone append feature uses the `addr` field of `struct ublksrv_io_cmd` to
communicate ALBA back to the kernel. Therefore ublk must be used with the
user copy feature (UBLK_F_USER_COPY) for zoned storage support to be
available. Without this feature, ublk will not allow zoned storage support.

Now that the driver core allows for struct class to be in read-only

The only overlap between the block open flags mapped into the fmode_t and
other uses of fmode_t are FMODE_READ and FMODE_WRITE. Define a new
blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and
->ioctl and stop abusing fmode_t.

Add control command of UBLK_U_CMD_GET_FEATURES for returning driver's
feature set or capability.
This way can simplify userspace for maintaining compatibility because
userspace doesn't need to send command to one device for querying driver
feature set any more. Such as, with the queried feature set, userspace
can choose to use:

Currently copy between io request buffer(pages) and userspace buffer is
done inside ublk_map_io() or ublk_unmap_io(). This way performs very
well in case of pre-allocated userspace io buffer.
For dynamically allocated or external userspace backend io buffer,
UBLK_F_NEED_GET_DATA is added for ublk server to provide buffer by one
extra command communication for WRITE request. For READ, userspace
simply provides buffer, but can't know when the buffer is done[1].
Add UBLK_F_USER_COPY by moving io data copy out of kernel by providing
read()/write() on /dev/ublkcN, and simply let ublk server do the io
data copy. This way makes both side cleaner, the cost is that one extra
syscall for copy io data between request and backend buffer.
With UBLK_F_USER_COPY, it actually becomes possible to run per-io zero
copy now, such as, only do zero copy for big size IO, so it can be
thought as one prep patch for supporting zero copy. Meantime zero copy
still needs to expose read()/write() buffer for some corner case, such
as passthrough IO.

Support pread()/pwrite() on ublk char device for reading/writing request
io buffer, so data copy between io request buffer and userspace buffer
can be moved to ublk server from ublk driver. Then UBLK_F_NEED_GET_DATA
becomes not necessary, so ublk server can allocate buffer without one
extra round uring command communication for userspace to provide buffer.
IO buffer can be located by iocb->ki_pos which encodes buffer offset, io
tag and queue id info, and type of iocb->ki_pos is u64, so it is big
enough for holding reasonable queue depth, nr_queues and max io buffer
size.

Add one reference counter into request pdu data, and hold this reference
in the request's lifetime.
Prepare for supporting to move request data copy into userspace, which
needs to copy request data by read()/write() on /dev/ublkcN, so we have
to guarantee that read()/write() is done on one valid/active request,
and that will be enhanced by holding the io request reference in
read()/write().

task_work_add() is used from early ublk development stage for handling
request in batch. However, since commit 7d4a93176e ("ublk_drv: don't
forward io commands in reserve order"), we can get similar batch
processing with io_uring_cmd_complete_in_task(), and similar performance
data is observed between task_work_add() and
io_uring_cmd_complete_in_task().
Meantime we can kill one fast code path, which is actually seldom used
given it is common to build ublk driver as module.

When handling UBLK_IO_FETCH_REQ, ctx->uring_lock is grabbed first, then
ub->mutex is acquired.
When handling UBLK_CMD_STOP_DEV or UBLK_CMD_DEL_DEV, ub->mutex is
grabbed first, then calling io_uring_cmd_done() for canceling uring
command, in which ctx->uring_lock may be required.
Real deadlock only happens when all the above commands are issued from
same uring context, and in reality different uring contexts are often used
for handing control command and IO command.
Fix the issue by using io_uring_cmd_complete_in_task() to cancel command
in ublk_cancel_dev(ublk_cancel_queue).

Currently uring CMD operation relies on having large SQEs, but future
operations might want to use normal SQE.
The io_uring_cmd currently only saves the payload (cmd) part of the SQE,
but, for commands that use normal SQE size, it might be necessary to
access the initial SQE fields outside of the payload/cmd block. So,
saves the whole SQE other than just the pdu.
This changes slightly how the io_uring_cmd works, since the cmd
structures and callbacks are not opaque to io_uring anymore. I.e, the
callbacks can look at the SQE entries, not only, in the cmd structure.
The main advantage is that we don't need to create custom structures for
simple commands.
Creates io_uring_sqe_cmd() that returns the cmd private data as a null
pointer and avoids casting in the callee side.
Also, make most of ublk_drv's sqe->cmd priv structure into const, and use
io_uring_sqe_cmd() to get the private structure, removing the unwanted
cast. (There is one case where the cast is still needed since the
header->{len,addr} is updated in the private structure)

Add timeout handler, so that we can provide forward progress guarantee for
unprivileged ublk, which can't be trusted.
One thing is that sync() calls sync_bdevs(wait) for all block devices after
running sync_bdevs(no_wait), and if one device can't move on, the sync() won't
return any more.
Add timeout for unprivileged ublk to avoid such affect for other users which call
sync() syscall.
Meantime clear UBLK_F_USER_RECOVERY_REISSUE for unprivileged ublk since
that feature may cause IO hang too.

Here is the large set of driver core changes for 6.4-rc1.
Once again, a busy development cycle, with lots of changes happening in
the driver core in the quest to be able to move "struct bus" and "struct
class" into read-only memory, a task now complete with these changes.
This will make the future rust interactions with the driver core more
"provably correct" as well as providing more obvious lifetime rules for
all busses and classes in the kernel.
The changes required for this did touch many individual classes and
busses as many callbacks were changed to take const * parameters
instead. All of these changes have been submitted to the various
subsystem maintainers, giving them plenty of time to review, and most of
them actually did so.
Other than those changes, included in here are a small set of other
things:
- kobject logging improvements
- cacheinfo improvements and updates
- obligatory fw_devlink updates and fixes
- documentation updates
- device property cleanups and const * changes
- firwmare loader dependency fixes.
All of these have been in linux-next for a while with no reported

All ublk commands(control, IO) should have taken ioctl command encoding
from the beginning, because ioctl command encoding defines each code
uniquely, so driver can figure out wrong command sent from userspace
easily; 2) it might help security subsystem for audit uring cmd[1].
Unfortunately we didn't do that way, and it could be one lesson for
ublk driver.
So switch to ioctl command encoding now, we still support commands encoded
in old way, but they become legacy definition. Any new command should take
ioctl encoding.

could panic kernel easily.
Make sure that block size is set correctly.
Meantime if ublk_validate_params() fails, clear ub->params so that disk
is prevented from being added.

Since SQE memory is shared with userspace, we should only be reading it
once. We cannot read it multiple times, particularly when it's read once
for validation and then read again for the actual use.
ublk_ch_uring_cmd() is safe when called as a retry operation, as the
memory backing is stable at that point. But for normal issue, we want
to ensure that we only read ublksrv_io_cmd once. Wrap the function in
a helper that reads the value into an on-stack copy of the struct.

for copying data between userspace buffer and request pages.
Here what matters is userspace buffer address/len and 'struct request',
so replace ->io field with user buffer address, and rename max_bytes
as len.
Meantime remove 'ubq' field from ublk_map_data, since it isn't used
any more.
Then code becomes more readable.

io_uring_cmd_done() currently assumes that the uring_lock is held
when invoked, and while it generally is, this is not guaranteed.
Pass in the issue_flags associated with it, so that we have
IO_URING_F_UNLOCKED available to be able to lock the CQ ring
appropriately when completing events.

IO can be started before add_disk() returns, such as reading parititon table,
then the monitor work should work for making forward progress.

Currently, uring_cmd with UBLK_IO_FETCH_REQ or
UBLK_IO_COMMIT_AND_FETCH_REQ is always checked whether
userspace server has provided IO buffer even flag
UBLK_F_NEED_GET_DATA is configured.
This is a excessive check. If UBLK_F_NEED_GET_DATA is
configured, FETCH_RQ doesn't need to provide IO buffer;
COMMIT_AND_FETCH_REQ also doesn't need to do that if
the IO type is not READ.
Check ub_cmd->addr together with ublk_need_get_data()
and IO type in ublk_ch_uring_cmd().
With this fix, userspace server doesn't need to preserve
buffers for every ublk_io when flag UBLK_F_NEED_GET_DATA
is configured, in order to save memory.

Inside ublk_ctrl_del_dev(), when the device is removed, we wait
until the device number is freed with holding global lock of
ublk_ctl_mutex, this way isn't friendly from user viewpoint:
1) if device is in-use, the current delete command hangs in
ublk_ctrl_del_dev(), and user can't break from the handling
because wait_event() is used
2) global lock is held, so any new device can't be added and
other old devices can't be removed.
Improve the deleting handling by the following way, suggested by
Nadav:
1) wait without holding the global lock
2) replace wait_event() with wait_event_interruptible()

Owner of one unprivileged ublk device could be one evil user, which
can grant this disk's privilege to other users deliberately, and
this way could be like making one trap and waiting for other users
to be caught.
So only owner to open unprivileged disk even though the owner
grants disk privilege to other user. This way is reasonable too
given anyone can create ublk disk, and no need other's grant.

When validating drafted SPDK ublk target, in a case that
assigning large queue depth to multiqueue ublk device,
ublk target would run into a weird incorrect state. During
rounds of review and debug, An overflow bug was found
in ublk driver.
In ublk_cmd.h, UBLK_MAX_QUEUE_DEPTH is 4096 which means
each ublk queue depth can be set as large as 4096. But
when setting qd for a ublk device,
sizeof(struct ublk_queue) + depth * sizeof(struct ublk_io)
will be larger than 65535 if qd is larger than 2728.
Then queue_size is overflowed, and ublk_get_queue()
references a wrong pointer position. The wrong content of
ublk_queue elements will lead to out-of-bounds memory
access.
Extend queue_size in ublk_device as "unsigned int".

unprivileged ublk device is helpful for container use case, such
as: ublk device created in one unprivileged container can be controlled
and accessed by this container only.
Implement this feature by adding flag of UBLK_F_UNPRIVILEGED_DEV, and if
this flag isn't set, any control command has been run from privileged
user. Otherwise, any control command can be sent from any unprivileged
user, but the user has to be permitted to access the ublk char device
to be controlled.
In case of UBLK_F_UNPRIVILEGED_DEV:
1) for command UBLK_CMD_ADD_DEV, it is always allowed, and user needs
to provide owner's uid/gid in this command, so that udev can set correct
ownership for the created ublk device, since the device owner uid/gid
can be queried via command of UBLK_CMD_GET_DEV_INFO.
2) for other control commands, they can only be run successfully if the
current user is allowed to access the specified ublk char device, for
running the permission check, path of the ublk char device has to be
provided by these commands.
Also add one control of command UBLK_CMD_GET_DEV_INFO2 which always
include the char dev path in payload since userspace may not have
knowledge if this device is created in unprivileged mode.
For applying this mechanism, system administrator needs to take
the following policies:
1) chmod 0666 /dev/ublk-control
2) change ownership of ublkcN & ublkbN
- chown owner_uid:owner_gid /dev/ublkcN
- chown owner_uid:owner_gid /dev/ublkbN
Both can be done via one simple udev rule.

Userspace side only knows device ID, but the associated path of ublkc* and
ublkb* could be changed by udev, and that depends on userspace's policy, so
add parameter of UBLK_PARAM_TYPE_DEVT for retrieving major/minor of the
ublkc* and ublkb*, then user may figure out major/minor of the ublk disks
he/she owns. With major/minor, it is easy to find the device node path.

Either ublk_can_use_task_work() is true or not, io commands are
forwarded to ublk server in reverse order, since llist_add() is
always to add one element to the head of the list.
Even though block layer doesn't guarantee request dispatch order,
requests should be sent to hardware in the sequence order generated
from io scheduler, which usually considers the request's LBA, and
order is often important for HDD.
So forward io commands in the sequence made from io scheduler by
aligning task work with current io_uring command's batch handling,
and it has been observed that both can get similar performance data
if IORING_SETUP_COOP_TASKRUN is set from ublk server.

io_uring cmd is supposed to be used in ubq daemon context mainly,
and we should try to avoid to touch it in ublk io submission context,
otherwise this data could become shared between the two contexts,
and performance is hurt.
So link request into one per-queue list, and use same batching policy
of io_uring command, just avoid to touch ucmd in blk-mq io context.

START_USER_RECOVERY and END_USER_RECOVERY are two new control commands
to support user recovery feature.
After a crash, user should send START_USER_RECOVERY, it will:
(1) check if (a)current ublk_device is UBLK_S_DEV_QUIESCED which was
set by quiesce_work and (b)chardev is released
(2) reinit all ubqs, including:
(a) put the task_struct and reset ->ubq_daemon to NULL.
(b) reset all ublk_io.
(3) reset ub->mm to NULL.
Then, user should start a new process and send FETCH_REQ on each
ubq_daemon.
Finally, user should send END_USER_RECOVERY, it will:
(1) wait for all new ubq_daemons getting ready.
(2) update ublksrv_pid
(3) unquiesce the request queue and expect incoming ublk_queue_rq()
(4) convert ub's state to UBLK_S_DEV_LIVE
Note: we can handle STOP_DEV between START_USER_RECOVERY and
END_USER_RECOVERY. This is helpful to users who cannot start new process
after sending START_USER_RECOVERY ctrl-cmd.

With a dying ubq_daemon, ublk_drv let monitor_work requeues rq issued to
userspace(ublksrv) before the ubq_daemon is dying.
UBLK_F_USER_RECOVERY_REISSUE is designed for backends which:
(1) tolerate double-write since ublk_drv may issue the same rq
twice.
(2) does not let frontend users get I/O error, such as read-only FS
and VM backend.

With USER_RECOVERY feature enabled, the monitor_work schedules
quiesce_work after finding a dying ubq_daemon. The monitor_work
should also abort all rqs issued to userspace before the ubq_daemon is
dying. The quiesce_work's job is to:
(1) quiesce request queue.
(2) check if there is any INFLIGHT rq. If so, we retry until all these
rqs are requeued and become IDLE. These rqs should be requeued by
ublk_queue_rq(), task work, io_uring fallback wq or monitor_work.
(3) complete all ioucmds by calling io_uring_cmd_done(). We are safe to
do so because no ioucmd can be referenced now.
(5) set ub's state to UBLK_S_DEV_QUIESCED, which means we are ready for
recovery. This state is exposed to userspace by GET_DEV_INFO.
The driver can always handle STOP_DEV and cleanup everything no matter
ub's state is LIVE or QUIESCED. After ub's state is UBLK_S_DEV_QUIESCED,
user can recover with new process.
Note: we do not change the default behavior with reocvery feature
disabled. monitor_work still schedules stop_work and abort inflight
rqs. And finally ublk_device is released.

With recovery feature enabled, in ublk_queue_rq or task work
(in exit_task_work or fallback wq), we requeue rqs instead of
ending(aborting) them. Besides, No matter recovery feature is enabled
or disabled, we schedule monitor_work immediately.

Define some macros for recovery feature.
UBLK_S_DEV_QUIESCED implies that ublk_device is quiesced
and is ready for recovery. This state can be observed by userspace.
UBLK_F_USER_RECOVERY implies that:
(1) ublk_drv enables recovery feature. It won't let monitor_work to
automatically abort rqs and release the device.
(2) With a dying ubq_daemon, ublk_drv ends(aborts) rqs issued to
userspace(ublksrv) before crash.
(3) With a dying ubq_daemon, in task work and ublk_queue_rq(),
ublk_drv requeues rqs.

This check is not atomic. So with recovery feature, ubq_daemon may be
modified simultaneously by recovery task. Instead, check 'current' is
safe here because 'current' never changes.
Also add comment explaining this check, which is really important for
understanding recovery feature.

In ublk_queue_rq(), Assume current request is a re-issued request aborted
previously in monitor_work because the ubq_daemon(ioucmd's task) is
PF_EXITING. For this request, we cannot call
io_uring_cmd_complete_in_task() anymore because at that moment io_uring
context may be freed in case that no inflight ioucmd exists. Otherwise,
we may cause null-deref in ctx->fallback_work.
Add a check on UBLK_IO_FLAG_ABORTED to prevent the above situation. This
check is safe and makes sense.
Note: monitor_work sets UBLK_IO_FLAG_ABORTED and ends this request
(releasing the tag). Then the request is restarted(allocating the tag)
and we are here. Since releasing/allocating a tag implies smp_mb(),
finding UBLK_IO_FLAG_ABORTED guarantees that here is a re-issued request
aborted previously.

If ublksrv sends UBLK_IO_NEED_GET_DATA with new allocated io buffer, we
have to update iod->addr in task_work before calling io_uring_cmd_done().
Then usersapce target can handle (write)io request with the new io
buffer reading from updated iod.
Without this change, userspace target may touch a wrong io buffer!

UBLK_IO_NEED_GET_DATA is one ublk IO command. It is designed for a user
application who wants to allocate IO buffer and set IO buffer address
only after it receives an IO request from ublksrv. This is a reasonable
scenario because these users may use a RPC framework as one IO backend
to handle IO requests passed from ublksrv. And a RPC framework may
allocate its own buffer(or memory pool).
This new feature (UBLK_F_NEED_GET_DATA) is optional for ublk users.
Related userspace code has been added in ublksrv[1] as one pull request.
Test cases for this feature are added in ublksrv and all the tests pass.
The performance result shows that this new feature does bring additional
latency because one IO is issued back to ublk_drv once again to copy data
from bio vectors to user-provided data buffer. UBLK_IO_NEED_GET_DATA is
suitable for bigger block size such as 512B or 1MB.

Remove all block device related info from ublksrv_ctrl_dev_info,
meantime reduce its size into 64 bytes because:
1) ublksrv_ctrl_dev_info becomes cleaner without including any
block related info
2) generic set/get parameter command can be used to set block
related setting easily and cleanly
3) generic set/get parameter command can be used for extending
ublk without needing more info in ublksrv_ctrl_dev_info

Add two commands to set/get parameters generically.
One important goal of ublk is to provide generic framework for making
block device by userspace flexibly.
As one generic block device, there are still lots of block parameters,
such as max_sectors, write_cache/fua, discard related limits,
zoned parameters, ...., so this patch starts to add generic mechanism
for set/get device parameters.
Both generic block parameters(all kinds of queue settings) and ublk
feature parameters can be covered with this way, then it becomes quite
easy to extend in future.
Add two parameter types are used so far: basic(covers basic queue setting
and misc settings which can't be grouped easily) and discard, basic type
must be set, and discard type becomes optional now
This way provides mechanism to simulate any kind of generic block device
from userspace easily, from both block queue setting viewpoint or ublk
feature viewpoint.
The style of putting all parameters together is suggested by Christoph.

Each ublk queue is started before adding disk, we have to cancel queues in
ublk_stop_dev() so that ubq daemon can be exited, otherwise DEL_DEV command
may hang forever.
Also avoid to cancel queues two times by checking if queue is ready,
otherwise use-after-free on io_uring may be triggered because ublk_stop_dev
is called by ublk_remove() too.

The test/clear_bit() functions take a bit number, but this code is
passing as shifted value. It's the equivalent of saying BIT(BIT(0))
instead of just BIT(0).
This doesn't affect runtime because numbers are small and it's done
consistently.

Userspace may support more features or new added flags, but the driver
side can be old, so make sure correct flags(features) returned to
userpsace, then userspace can work as expected.
Also mark the 2nd flags as reversed, just use the 1st one. When we run
out of flags, the reserved one can be handled at that time.

__ublk_destroy_dev() is called for handling error in ublk_add_dev(),
but either tagset isn't allocated or mutex isn't initialized.
So fix the issue by letting replacing ublk_add_dev with a
ublk_add_tag_set function that is much more limited in scope and
instead unwind every single step directly in ublk_ctrl_add_dev.
To allow for this refactor the device freeing so that there is
a helper for freeing the device number instead of coupling that
with freeing the mutex and the memory.
Note that this now copies the dev_info to userspace before adding
the character device. This not only simplifies the erro handling
in ublk_ctrl_add_dev, but also means that the character device
can only be seen by userspace if the device addition succeeded.

Defer allocating the gendisk and request_queue until UBLK_CMD_START_DEV
is called. This avoids funky life times where a disk is allocated
and then can be added and removed multiple times, which has never been
supported by the block layer.

Looking at the hctxs and cpumap is not safe without at very last a RCU
reference. It also requires the queue to be set up before starting the
device, which leads to rather awkward life time rules.
Instead rewrite ublk_ctrl_get_queue_affinity to just build the cpumask
directly from the mq_map in the tag set, similar to hctx->cpumask is
built.

Move all per-command work into the per-command ublk_ctrl_* helpers
instead of being split over those, ublk_ctrl_cmd_validate, and the main
ublk_ctrl_uring_cmd handler. To facilitate that, the old
ublk_ctrl_stop_dev function that just contained two function calls is
folded into both callers.

fops->open and fops->release are always paired. Use simple atomic bit
ops ot indicate if the device is opened instead of a count that can
only be 0 and 1 and a useless cmpxchg loop in ublk_ch_release.
Also don't bother clearing file->private_data is the file is about to
be freed anyway.

metadata, so don't define this flag, which requires that an integrity
payload is attached to a bio.

Call blk_cleanup_queue() in release code path for fixing request
queue leak.
Also for-5.20/block has cleaned up blk_cleanup_queue(), which is
basically merged to del_gendisk() if blk_mq_alloc_disk() is used
for allocating disk and queue.
However, ublk may not add disk in case of starting device failure, then
del_gendisk() won't be called when removing ublk device, so blk_mq_exit_queue
will not be callsed, and it can be bit hard to deal with this kind of
merge conflict.
Turns out ublk's queue/disk use model is very similar with scsi, so switch
to scsi's model by allocating disk and queue independently, then it can be
quite easy to handle v5.20 merge conflict by replacing blk_cleanup_queue
with blk_mq_destroy_queue.

Use task_work_add if it is available, since task_work_add can bring
up better performance, especially batching signaling ->ubq_daemon can
be done.
It is observed that task_work_add() can boost iops by +4% on random
4k io test. Also except for completing io command, all other code
paths are same with completing io command via
io_uring_cmd_complete_in_task.
Meantime add one flag of UBLK_F_URING_CMD_COMP_IN_TASK for comparing
the mode easily.
