e500 supports many page sizes among which the following size are
implemented in the kernel at the time being: 4M, 16M, 64M, 256M, 1G.
On e500, TLB miss for hugepages is exclusively handled by SW even on e6500
which has HW assistance for 4k pages, so there are no constraints like on
the 8xx.
On e500/32, all are at PGD/PMD level and can be handled as cont-PMD.
On e500/64, smaller ones are on PMD while bigger ones are on PUD. Again,
they can easily be handled as cont-PMD and cont-PUD instead of hugepd.
On e500/32, use the pagesize bits in PTE to know if it is a PMD or a leaf
entry. This works because the pagesize bits are in the last 12 bits and
page tables are 4k aligned.
On e500/64, use highest bit which is always 1 on PxD (Because PxD contains
virtual address of a kernel memory) and always 0 on PTEs because not all
bits of RPN are used/possible.

There are two possibilities for book3e_htw_mode, PPC_HTW_E6500 or
PPC_HTW_NONE.
The TLB miss handlers are patched to use, respectively:
- exc_[data|indstruction]_tlb_miss_e6500_book3e
- exc_[data|indstruction]_tlb_miss_bolted_book3e
Which means the default handlers are never used. Remove those, and use
the bolted handlers (PPC_HTW_NONE) by default.

Patch series "Reimplement huge pages without hugepd on powerpc (8xx, e500,
book3s/64)", v7.
Unlike most architectures, powerpc 8xx HW requires a two-level pagetable
topology for all page sizes. So a leaf PMD-contig approach is not
feasible as such.
Possible sizes on 8xx are 4k, 16k, 512k and 8M.
First level (PGD/PMD) covers 4M per entry. For 8M pages, two PMD entries
must point to a single entry level-2 page table. Until now that was done
using hugepd. This series changes it to use standard page tables where
the entry is replicated 1024 times on each of the two pagetables refered
by the two associated PMD entries for that 8M page.
For e500 and book3s/64 there are less constraints because it is not tied
to the HW assisted tablewalk like on 8xx, so it is easier to use leaf PMDs
(and PUDs).
On e500 the supported page sizes are 4M, 16M, 64M, 256M and 1G. All at
PMD level on e500/32 (mpc85xx) and mix of PMD and PUD for e500/64. We
encode page size with 4 available bits in PTE entries. On e300/32 PGD
entries size is increases to 64 bits in order to allow leaf-PMD entries
because PTE are 64 bits on e500.
On book3s/64 only the hash-4k mode is concerned. It supports 16M pages as
cont-PMD and 16G pages as cont-PUD. In other modes (radix-4k, radix-6k
and hash-64k) the sizes match with PMD and PUD sizes so that's just leaf
entries. The hash processing make things a bit more complex. To ease
things, __hash_page_huge() is modified to bail out when DIRTY or ACCESSED
bits are missing, leaving it to mm core to fix it.

The nohash HTW_IBM (Hardware Table Walk) code is unused since support for
A2 was removed in commit fb5a515704 ("powerpc: Remove platforms/ wsp and
associated pieces") (2014).
The remaining supported CPUs use either no HTW (data_tlb_miss_bolted), or
the e6500 HTW (data_tlb_miss_e6500).

When using the LLVM integrated assembler (llvm-as), the book3e build
fails with:
arch/powerpc/mm/nohash/tlb_low_64e.S:354:2: error: invalid instruction
tlbilxva 0,%r15
^
tlbilxva is an extended mnemonic for tlbilx, but llvm-as also doesn't
support tlbilx, despite it being an e500mc instruction.
Fix it by using the existing PPC_TLBILX_VA macro. The resulting binary
is identical when building with binutils.

The interrupt entry code carefully saves a minimal number of registers,
so in some places the TOC is required, it is loaded into a different
register, so provide a macro that can supply an alternate TOC register.
This continues to use got addressing because TOC-relative results in
"got/toc optimization is not supported" messages by the linker. Having
r2 be one of the saved registers and using that for TOC addressing may
be the best way to avoid that and switch this to TOC addressing.

A later change stops the kernel using r2 and loads it with a poison

Today nohash/64 have linear memory based at 0xc000000000000000 and
virtual memory based at 0x8000000000000000.
In order to implement KASAN, we need to regroup both areas.
Move virtual memmory at 0xc000100000000000.
This complicates a bit TLB miss handlers. Until now, memory region
was easily identified with the 4 higher bits of address:
- 0 ==> User
- c ==> Linear Memory
- 8 ==> Virtual Memory
Now we need to rely on the 20 higher bits, with:
- 0xxxx ==> User
- c0000 ==> Linear Memory
- c0001 ==> Virtual Memory

Commit fb5a515704 ("powerpc: Remove platforms/wsp and associated
pieces") removed the last CPU having features MMU_FTRS_A2 and
commit cd68098bce ("powerpc: Clean up MMU_FTRS_A2 and
MMU_FTR_TYPE_3E") removed MMU_FTRS_A2 which was the last user of
MMU_FTR_USE_TLBRSRV and MMU_FTR_USE_PAIRED_MAS.
Remove all code that relies on MMU_FTR_USE_TLBRSRV and
MMU_FTR_USE_PAIRED_MAS.
With this change done, TLB miss can happen before the mmu feature
fixups.

With KUAP, the TLB miss handler bails out when an access to user
memory is performed with a nul TID.
But the normal TLB miss routine which is only used early during boot
does the check regardless for all memory areas, not only user memory.
By chance there is no early IO or vmalloc access, but when KASAN
come we will start having early TLB misses.
Fix it by creating a special branch for user accesses similar to the
one in the 'bolted' TLB miss handlers. Unfortunately SPRN_MAS1 is
now read too early and there are no registers available to preserve
it so it will be read a second time.

set_memory_x() calls pte_mkexec() which sets _PAGE_EXEC.
set_memory_nx() calls pte_exprotec() which clears _PAGE_EXEC.
Book3e has 2 bits, UX and SX, which defines the exec rights
resp. for user (PR=1) and for kernel (PR=0).
_PAGE_EXEC is defined as UX only.
An executable kernel page is set with either _PAGE_KERNEL_RWX
or _PAGE_KERNEL_ROX, which both have SX set and UX cleared.
So set_memory_nx() call for an executable kernel page does
nothing because UX is already cleared.
And set_memory_x() on a non-executable kernel page makes it
executable for the user and keeps it non-executable for kernel.
Also, pte_exec() always returns 'false' on kernel pages, because
it checks _PAGE_EXEC which doesn't include SX, so for instance
the W+X check doesn't work.
To fix this:
- change tlb_low_64e.S to use _PAGE_BAP_UX instead of _PAGE_USER
- sets both UX and SX in _PAGE_EXEC so that pte_exec() returns
true whenever one of the two bits is set and pte_exprotect()
clears both bits.
- Define a book3e specific version of pte_mkexec() which sets
either SX or UX based on UR.
