Currently c-octeon relies on octeon's own smp function to flush
I-Cache. However this function is not available on generic platform.

bcache is not tied to CPU's cache interface. Just move those
declaration to cache.c so it can be avaialble to CPU with all

As nowadays DMA coherence is managed per device, it is possible
to have a system that is defaulted to coherent dma but still
have noncoherent device that needs to use those flush functions.
Just install them unconditionally.

Patch series "mm/sparse-vmemmap: Generalise helpers and enable for
LoongArch", v14.
This series is in order to enable sparse-vmemmap for LoongArch. But
LoongArch cannot use generic helpers directly because MIPS&LoongArch need
to call pgd_init()/pud_init()/pmd_init() when populating page tables. So
we adjust the prototypes of p?d_init() to make generic helpers can call
them, then enable sparse-vmemmap with generic helpers, and to be further,
generalise vmemmap_populate_hugepages() for ARM64, X86 and LoongArch.

We are preparing to add sparse vmemmap support to LoongArch. MIPS and
LoongArch need to call pgd_init()/pud_init()/pmd_init() when populating
page tables, so adjust their prototypes to make generic helpers can call
them.
NIOS2 declares pmd_init() but doesn't use, just remove it to avoid build
errors.

_PAGE_NO_EXEC can be '0' or '1 << _PAGE_NO_EXEC_SHIFT' depending on the
build and runtime configuration, which is what the negation operators
are trying to convey. To silence the warning, explicitly compare against
0 so the result of the '<<' operator is not implicitly converted to a
boolean.
According to its documentation, GCC enables -Wint-in-bool-context with
-Wall but this warning is not visible when building the same
configuration with GCC. It appears GCC only warns when compiling C++,
not C, although the documentation makes no note of this:

This enables ARCH_HAS_VM_GET_PAGE_PROT on the platform and exports
standard vm_get_page_prot() implementation via DECLARE_VM_GET_PAGE_PROT,
which looks up a private and static protection_map[] array. Subsequently
all __SXXX and __PXXX macros can be dropped which are no longer needed.

It is permissible for kernel code to call virt_to_phys() against virtual
addresses that are in KSEG0 or KSEG1 and we need to be dealing with both
types. Rewrite the test condition to ensure that the kernel virtual
addresses are above PAGE_OFFSET which they must be, and below KSEG2
where the non-linear mapping starts.
For EVA, there is not much that we can do given the linear address range
that is offered, so just return any virtual address as being valid.
Finally, when HIGHMEM is not enabled, all virtual addresses are assumed
to be valid as well.

I observed that for each of the shared file-backed page faults, we're very
likely to retry one more time for the 1st write fault upon no page. It's
because we'll need to release the mmap lock for dirty rate limit purpose
with balance_dirty_pages_ratelimited() (in fault_dirty_shared_page()).
Then after that throttling we return VM_FAULT_RETRY.
We did that probably because VM_FAULT_RETRY is the only way we can return
to the fault handler at that time telling it we've released the mmap lock.
However that's not ideal because it's very likely the fault does not need
to be retried at all since the pgtable was well installed before the
throttling, so the next continuous fault (including taking mmap read lock,
walk the pgtable, etc.) could be in most cases unnecessary.
It's not only slowing down page faults for shared file-backed, but also add
more mmap lock contention which is in most cases not needed at all.
To observe this, one could try to write to some shmem page and look at
"pgfault" value in /proc/vmstat, then we should expect 2 counts for each
shmem write simply because we retried, and vm event "pgfault" will capture
that.
To make it more efficient, add a new VM_FAULT_COMPLETED return code just to
show that we've completed the whole fault and released the lock. It's also
a hint that we should very possibly not need another fault immediately on
this page because we've just completed it.
This patch provides a ~12% perf boost on my aarch64 test VM with a simple
program sequentially dirtying 400MB shmem file being mmap()ed and these are
the time it needs:
Before: 650.980 ms (+-1.94%)
After: 569.396 ms (+-1.38%)
I believe it could help more than that.
We need some special care on GUP and the s390 pgfault handler (for gmap
code before returning from pgfault), the rest changes in the page fault
handlers should be relatively straightforward.
Another thing to mention is that mm_account_fault() does take this new
fault as a generic fault to be accounted, unlike VM_FAULT_RETRY.
I explicitly didn't touch hmm_vma_fault() and break_ksm() because they do
not handle VM_FAULT_RETRY even with existing code, so I'm literally keeping
them as-is.

If define CONFIG_KPROBES, __kprobes annotation forces the whole function
into the ".kprobes.text" section, NOKPROBE_SYMBOL() only stores the given
function address in the "_kprobe_blacklist" section which is introduced
to maintain kprobes blacklist.
Modify the related code to use NOKPROBE_SYMBOL() to protect functions from
kprobes instead of __kprobes annotation under arch/mips.
No obvious functional change in this patch, some more work needs to be done
to fix the kernel panic when execute the following testcase on mips:

Add pcpu_fc_cpu_to_node_fn_t and pass it into pcpu_fc_alloc_fn_t, pcpu
first chunk allocation will call it to alloc memblock on the
corresponding node by it, this is prepare for the next patch.

Since commit 4064b98270 ("mm: allow VM_FAULT_RETRY for multiple
times") allowed VM_FAULT_RETRY for multiple times, the
FAULT_FLAG_ALLOW_RETRY bit of fault_flag will not be changed in the page
fault path, so the following check is no longer needed:
flags & FAULT_FLAG_ALLOW_RETRY
So just remove it.

The MIPS cache flush logic needs to know whether the mapping was already
established to decide how to flush caches. This is done by checking the
valid bit in the PTE. The commit above breaks this logic by setting the
valid in the PTE in new mappings, which causes kernel crashes.

I am not sure why this is required, but if this is not enabled, reading
from a buffer in which data has been DMA'd may read incorrect values.
This used to happen for instance in mmc_app_send_scr()
(drivers/mmc/core/sd_ops.c), where data is DMA'd to a buffer then copied
by the CPU to a different location.

Patch series "hugetlb: Disable huge pmd unshare for uffd-wp", v4.
This series tries to disable huge pmd unshare of hugetlbfs backed memory
for uffd-wp. Although uffd-wp of hugetlbfs is still during rfc stage,
the idea of this series may be needed for multiple tasks (Axel's uffd
minor fault series, and Mike's soft dirty series), so I picked it out
from the larger series.

It is a preparation work to be able to behave differently in the per
architecture huge_pte_alloc() according to different VMA attributes.
Pass it deeper into huge_pmd_share() so that we can avoid the find_vma() call.

Provide hooks to intercept bad usages of virt_to_phys() and
__pa_symbol() throughout the kernel. To make this possible, we need to
rename the current implement of virt_to_phys() into
__virt_to_phys_nodebug() and wrap it around depending on
CONFIG_DEBUG_VIRTUAL.
A similar thing is needed for __pa_symbol() which is now aliased to
__phys_addr_symbol() whose implementation is either the direct return of
RELOC_HIDE or goes through the debug version.

MIPS page fault path(except huge page) takes 3 exceptions (1 TLB Miss + 2
TLB Invalid), butthe second TLB Invalid exception is just triggered by
__update_tlb from do_page_fault writing tlb without _PAGE_VALID set. With
this patch, user space mapping prot is made young by default (with both
_PAGE_VALID and _PAGE_YOUNG set), and it only take 1 TLB Miss + 1 TLB
Invalid exception
Remove pte_sw_mkyoung without polluting MM code and make page fault delay
of MIPS on par with other architecture

Function set_pmd_at is to set pmd entry, if tlb entry need to be flushed,
there exists pmdp_huge_clear_flush alike function before set_pmd_at is
called. So it is not necessary to call flush_tlb_all in this function.
In these scenarios, tlb for the pmd range needs to be flushed:
- privilege degrade such as wrprotect is set on the pmd entry
- pmd entry is cleared
- there is exception if set_pmd_at is issued by dup_mmap, since
flush_tlb_mm is called for parent process, it is not necessary to
flush tlb in function copy_huge_pmd.

Lift the dma_default_coherent variable from the mips architecture code
to the driver core. This allows an architecture to sdefault all device
to be DMA coherent at run time, even if the kernel is build with support
for DMA noncoherent device. By allowing device_initialize to set the
->dma_coherent field to this default the amount of arch hooks required
for this behavior can be greatly reduced.

When there's a fatal signal pending, MIPS's do_page_fault()
implementation returns. The intent is that we'll return to the
faulting userspace instruction, delivering the signal on the way.
However, if we take a fatal signal during fixing up a uaccess, this
results in a return to the faulting kernel instruction, which will be
instantly retried, resulting in the same fault being taken forever. As
the task never reaches userspace, the signal is not delivered, and the
task is left unkillable. While the task is stuck in this state, it can
inhibit the forward progress of the system.
To avoid this, we must ensure that when a fatal signal is pending, we
apply any necessary fixup for a faulting kernel instruction. Thus we
will return to an error path, and it is up to that code to make forward
progress towards delivering the fatal signal.

- Consolidate all kmap_atomic() internals into a generic implementation
which builds the base for the kmap_local() API and make the
kmap_atomic() interface wrappers which handle the disabling/enabling of
preemption and pagefaults.
- Switch the storage from per-CPU to per task and provide scheduler
support for clearing mapping when scheduling out and restoring them
when scheduling back in.
- Merge the migrate_disable/enable() code, which is also part of the
scheduler pull request. This was required to make the kmap_local()
interface available which does not disable preemption when a mapping
is established. It has to disable migration instead to guarantee that
the virtual address of the mapped slot is the same accross preemption.
- Provide better debug facilities: guard pages and enforced utilization
of the mapping mechanics on 64bit systems when the architecture allows
it.
- Provide the new kmap_local() API which can now be used to cleanup the
kmap_atomic() usage sites all over the place. Most of the usage sites
do not require the implicit disabling of preemption and pagefaults so
the penalty on 64bit and 32bit non-highmem systems is removed and quite
some of the code can be simplified. A wholesale conversion is not
possible because some usage depends on the implicit side effects and
some need to be cleaned up because they work around these side effects.
The migrate disable side effect is only effective on highmem systems
and when enforced debugging is enabled. On 64bit and 32bit non-highmem
systems the overhead is completely avoided.

guarantee that the virtual address of the mapped slot is the same

- Provide better debug facilities: guard pages and enforced
utilization of the mapping mechanics on 64bit systems when the
architecture allows it.
- Provide the new kmap_local() API which can now be used to cleanup
the kmap_atomic() usage sites all over the place. Most of the usage
sites do not require the implicit disabling of preemption and
pagefaults so the penalty on 64bit and 32bit non-highmem systems is
removed and quite some of the code can be simplified. A wholesale
conversion is not possible because some usage depends on the
implicit side effects and some need to be cleaned up because they
work around these side effects.
The migrate disable side effect is only effective on highmem
systems and when enforced debugging is enabled. On 64bit and 32bit
non-highmem systems the overhead is completely avoided"

As the user manual and code comment said, Loongson-3 has 4-scache banks,
while Loongson-2K has only 2 banks, so we should multiply the number of
scache banks, this multiply operation should be done by c->scache.sets
instead of scache_size, otherwise we will get the wrong scache size when
execute lscpu. For example, the scache size should be 8192K instead of
2048K on the Loongson 3A3000 and 3A4000 platform, we can see the related
info in the following boot message:

Protection map difference between RIXI and non RIXI cpus is _PAGE_NO_EXEC
and _PAGE_NO_READ usage. Both already take care of cpu_has_rixi while
setting up the page bits. So we just need one setup of protection map
and can drop the now unused (and broken for RIXI) PAGE_* defines.

Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

When transferring DMA ownership back to the CPU there should never
be any writeback from the cache, as the buffer was owned by the
device until now. Instead it should just be invalidated for the
mapping directions where the device could have written data.
Note that the changes rely on the fact that kmap_atomic is stubbed
out for the !HIGHMEM case to simplify the code a bit.

When the BMIPS generic cpu-feature-overrides.h file was introduced,
cpu_has_inclusive_caches/MIPS_CPU_INCLUSIVE_CACHES was not set for
BMIPS5000 CPUs. Correct this when we have initialized the MIPS secondary

Schaefer's report on an issue a week ago regarding to incorrect page fault
accountings for retried page fault after commit 4064b98270 ("mm: allow
VM_FAULT_RETRY for multiple times"):

What this series did:
- Correct page fault accounting: we do accounting for a page fault
(no matter whether it's from #PF handling, or gup, or anything else)
only with the one that completed the fault. For example, page fault
retries should not be counted in page fault counters. Same to the
perf events.
- Unify definition of PERF_COUNT_SW_PAGE_FAULTS: currently this perf
event is used in an adhoc way across different archs.
Case (1): for many archs it's done at the entry of a page fault
handler, so that it will also cover e.g. errornous faults.
Case (2): for some other archs, it is only accounted when the page
fault is resolved successfully.
Case (3): there're still quite some archs that have not enabled
this perf event.
Since this series will touch merely all the archs, we unify this
perf event to always follow case (1), which is the one that makes most
sense. And since we moved the accounting into handle_mm_fault, the
other two MAJ/MIN perf events are well taken care of naturally.
- Unify definition of "major faults": the definition of "major
fault" is slightly changed when used in accounting (not
VM_FAULT_MAJOR). More information in patch 1.
- Always account the page fault onto the one that triggered the page
fault. This does not matter much for #PF handlings, but mostly for
gup. More information on this in patch 25.
Patchset layout:
Patch 1: Introduced the accounting in handle_mm_fault(), not enabled.
Patch 2-23: Enable the new accounting for arch #PF handlers one by one.
Patch 24: Enable the new accounting for the rest outliers (gup, iommu, etc.)

This is a preparation patch to move page fault accountings into the
general code in handle_mm_fault(). This includes both the per task
flt_maj/flt_min counters, and the major/minor page fault perf events. To
do this, the pt_regs pointer is passed into handle_mm_fault().
PERF_COUNT_SW_PAGE_FAULTS should still be kept in per-arch page fault
handlers.
So far, all the pt_regs pointer that passed into handle_mm_fault() is
NULL, which means this patch should have no intented functional change.

Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.
In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.
Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:

To support kmap_atomic_prot(), all architectures need to support
protections passed to their kmap_atomic_high() function. Pass protections
into kmap_atomic_high() and change the name to kmap_atomic_high_prot() to
match.
Then define kmap_atomic_prot() as a core function which calls
kmap_atomic_high_prot() when needed.
Finally, redefine kmap_atomic() as a wrapper of kmap_atomic_prot() with
the default kmap_prot exported by the architectures.
