Instead of exposing the mips-optimized SHA-1 code via mips-specific
crypto_shash algorithms, instead just implement the sha1_blocks()
library function. This is much simpler, it makes the SHA-1 library
functions be mips-optimized, and it fixes the longstanding issue where
the mips-optimized SHA-1 code was disabled by default. SHA-1 still
remains available through crypto_shash, but individual architectures no
longer need to handle it.

Consolidate the CPU-based SHA-256 code into a single module, following
what I did with SHA-512:
- Each arch now provides a header file lib/crypto/$(SRCARCH)/sha256.h,
replacing lib/crypto/$(SRCARCH)/sha256.c. The header defines
sha256_blocks() and optionally sha256_mod_init_arch(). It is included
by lib/crypto/sha256.c, and thus the code gets built into the single
libsha256 module, with proper inlining and dead code elimination.
- sha256_blocks_generic() is moved from lib/crypto/sha256-generic.c into
lib/crypto/sha256.c. It's now a static function marked with
__maybe_unused, so the compiler automatically eliminates it in any
cases where it's not used.
- Whether arch-optimized SHA-256 is buildable is now controlled
centrally by lib/crypto/Kconfig instead of by
lib/crypto/$(SRCARCH)/Kconfig. The conditions for enabling it remain
the same as before, and it remains enabled by default.
- Any additional arch-specific translation units for the optimized
SHA-256 code (such as assembly files) are now compiled by
lib/crypto/Makefile instead of lib/crypto/$(SRCARCH)/Makefile.

irq_domain_add_*() interfaces are going away as being obsolete now.
Switch to the preferred irq_domain_create_*() ones. Those differ in the
node parameter: They take more generic struct fwnode_handle instead of
struct device_node. Therefore, of_fwnode_handle() is added around the
original parameter.
Note some of the users can likely use dev->fwnode directly instead of
indirect of_fwnode_handle(dev->of_node). But dev->fwnode is not
guaranteed to be set for all, so this has to be investigated on case to
case basis (by people who can actually test with the HW).

DWC3 as implemented in Cavium SoC is using UCTL bridge unit
between I/O interconnect and USB controller.
Currently there is no bond with dwc3 core code, so if anything goes
wrong in UCTL setup dwc3 is left in reset, which leads to bus error
while trying to read any device register. Thus any failure in UCTL
initialization ends with kernel panic.
To avoid this move Octeon DWC3 glue code from arch/mips and make it
proper glue driver which is used instead of dwc3-of-simple.

The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it was merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

Move all register access code into separate functions and
provide their no-op version for non Octeon platforms.
Later it might be possible to replace them with standard
Linux functions, however datasheets are not publicly available
and I have only one Octeon board to test, so lets stay on safe
side for now.

Switch to the CPU hotplug core state tracking and synchronization
mechanim. This unfortunately requires to add dead reporting to the non CPS
platforms as CPS is the only user, but it allows an overall consolidation
of this functionality.

this inconsistently follow this new, common convention, and fix all the fallout
that objtool can now detect statically.

Fundamentally semaphores are a counted primitive, but
DEFINE_SEMAPHORE() does not expose this and explicitly creates a
binary semaphore.
Change DEFINE_SEMAPHORE() to take a number argument and use that in the
few places that open-coded it using __SEMAPHORE_INITIALIZER().

Console output currently looks like USB clocks initialized succesfully
even in case of error. Fix that and use consistently dev_err for fatal
errors otherwise dev_warn.

Some of them were pointless because CONFIG_CAVIUM_RESERVE32 is now always
defined, some were not enough (Yu Zhao reported
"Failed to allocate CAVIUM_RESERVE32 memory area" error).
Removing the directives allows for compiler coverage of RESERVE32 code and
replacing one of [always-true] "ifdef" with a compiler conditional fixes
the [cosmetic] error message.
